[Specific features of war injuries of the eyes in peace time].

The authors analyze clinical features of injuries to the organ of vision under extreme conditions of a local war conflict and present their recommendations on rehabilitation of the victims. The status of 203 patients with eyeball wounds inflicted under such conditions is analyzed. To optimize the outcomes of war traumas to the eye inflicted in peace time the authors give the following recommendations based on the specific features of such injuries and the objective conditions in foci of regional conflicts: (1) Rapid evacuation of wounded subjects to the nearest ophthalmological institutions; (2) Maximally early primary ophthalmological care with obligatory primary surgical debridement of the wound; (3) No attempts at removal of a foreign body during primary surgical debridement at local medical institutions; (4) the earliest possible evacuation of the victims after wound debridement to specialized ophthalmo-traumatological institutions for further rehabilitation.

[A system for the mass combined vaccination of the adult population against influenza, viral hepatitis, typhoid, meningitis and diphtheria].

A safe, moderately reactogenic and immunologically effective scheme of complex (combined) immunization against meningitis A, diphtheria, typhoid fever, viral hepatitis A and influenza has been developed as the result of experimental and clinico-immunological studies. Depending on the epidemiological situation, the newly developed scheme can be used in two variants. According to the first variant of this scheme, the following preparations are injected subcutaneously into three different sites: a mixture of group A meningococcal vaccine and diphtheria toxoid, typhoid vaccine and influenza vaccine. The second variant of the scheme differs from its first variant in using intramuscular injection of normal human immunoglobulin instead of injection of influenza vaccine. Moreover, in practical realization these variants may be altered by excluding vaccines, unnecessary under present conditions. The newly developed scheme of vaccinal prophylaxis is recommended for practical use.

[The activity of natural killers and K cells and their interferon sensitivity in subjects inoculated against diphtheria and meningococcal infection].

The work deals with the time course of changes in the activity of natural killers, K-cells, their sensitivity to interferon, which reflects, in our opinion, the reserve possibilities of this killer system simultaneously with the development of the specific sensitization of lymphocytes. Significant changes in the cytotoxicity of natural killers, K-cells, have been detected in persons immunized against meningococcal infection, especially in those immunized with meningococcal vaccine introduced in combination with diphtheria toxoid. In this latter group of volunteers even more pronounced sensitization of peripheral blood lymphocytes has been observed than in persons immunized with monopreparations. The results obtained in this investigation indicate that the determination of these cell reactions may be of importance in the evaluation of the effectiveness of immunization.

[The protective properties of the outer membranes of Francisella tularensis in an experimental infection in guinea pigs].

Subcutaneous immunization, made in a single injection, with outer membrane preparations obtained from F.tularensis vaccine strain 15 and virulent strain A'Cole results in intensive immunity to tularemia in guinea pigs, ensuring the protection of 60-100% of the animals within a month after challenge with F.tularensis virulent strain 503 in a dose of 1,000 DCL. The development of protective effect induced by F.tularensis outer membranes can be observed during the first 24 hours and reaches its maximum by days 15-21 after immunization.

[Clinical-functional state of the retina and optic nerve in contusions of the eyeball].

The function of the retina and optic nerve was studied in 79 patients with contusions of the eyeball of various severity. A complex electrophysiological method was used, consisting of recording of visual evoked potentials and of the general and macular electroretinograms, assessment of the threshold electrical sensitivity and lability. Changes were detected in all the layers of the retina and optic nerve, the degree of these changes augmenting with the severity of injury. Changes in the functional parameters of the paired intact eyes, similar in the trend, were observed.

[A new approach to the prevention and treatment of lesions caused by bacterial toxins: prospects for using inhibitors of ADP ribosylation].

The damaging action of a number of bacterial toxins is determined by their capacity for blocking the specific functions of regulatory proteins of eukaryotic cells by ADP-ribosylation. Experiments, made with the use of type B botulinic toxin and 3,N-butyrylaminobenzamide as an example, have demonstrated that specific ADP-ribosylation inhibitors are capable of making up a new group of highly active antagonists of microbial toxins.

[Characterization of B-type bacteriophages adsorbed on Pseudomonas aeruginosa pili].

Pilus-dependent B-morphotype bacteriophages isolated from various sources were studied. The adsorption of phages on Pseudomonas aeruginosa pili was proven. Electron-microscopic examination of the morphology of phages was carried out, and the adsorption properties were partially described. It was suggested that adsorption apparatuses of different pilus-dependent B-phages are alike with respect to structure and function.

[Surgery of post-contusion dislocation intraocular syndrome].

The author discusses clinical variants of grave contusions of the eyeball involving the iridolenticular diaphragm and characterized by dislocation of the iris, lens, and vitreous body and subsequent development of secondary glaucoma. This condition is proposed to be singled out as the postcontusion dislocation intraocular syndrome. Sixty-four patients were treated. Variants of surgical repair are proposed, aimed at elimination of the dislocation and maximal repair of the anatomical ratios, including removal of the lens, vitrectomy, and iridoplasty with a circular pupil suture, depending on the severity of posttraumatic changes. Such treatment helped remove the retention disorders and prevent the irreversible changes; high ocular function was preserved in 41.7% patients and a good cosmetic effect attained in the rest cases. Remote results were followed up for up to 10 years. In only 8.3% patients they may be considered unsatisfactory.

[Initial stages of interaction of temperate phage 04 with Pseudomonas aeruginosa cells].

The first steps of the interaction of the temperate pilus-dependent phage 04 with susceptible and 04-lysogenic cells of Pseudomonas aeruginosa were investigated. The 04-lysogenic cells retained their ability to adsorb viral particles on the pili. However, after the translocation to the cell surface, the bacteriophage failed to infect the immune microorganism. It is assumed that was modified the surface of 04-immune cells due to lysogenic conversion, and this modification presented the terminal tail fibers of viral particles interacting from with the outer membrane of P. aeruginosa cells.

[Laser surgery for some forms of secondary posttraumatic glaucoma].

Zeiss Visulas YAG I Nd:YAG laser (Opton, Germany) was used in the treatment of 70 patients with three forms of secondary posttraumatic glaucoma (adhesive glaucoma with anterior-chamber adhesions and goniosynechias, adhesive glaucoma with aphakia-aniridia symptom complex, and aphakic malignant glaucoma). Laser synechiolysis was highly effective in patients with the first of the listed forms of glaucoma if the operation was performed as early as possible. The efficacy of hyaloidotomy, particularly peripheral, is confirmed; this modality in combination with drug therapy helps normalize intraocular pressure. The place of transpupillary laser hyaloidotomy in the treatment of aphakic malignant glaucoma is defined.

[Efficacy and indications for YAG laser optico-reconstructive surgery on the anterior eye segment in children].

Efficacy of YAG laser surgery is analyzed in 486 children treated at the Helmholtz Institute of Ocular Diseases. Indications and contraindications for YAG laser correcting opticoreconstructive surgery on the anterior segment of the eye are defined for children with secondary cataracts, some congenital, traumatic, and complicated cataracts, anterior and posterior synechias, goniosynechias, ectopic and pinhole pupil, pupil block, anterior chamber cysts, vitreous adhesions, and some consequences of intraocular lens implantation. Optimal terms of laser intervention are defined, improving its efficacy and decreasing the risk of complications.
