[Characteristics of blood coagulation and of the red blood in patients with an arteriosclerotic lesion of the abdominal aorta].

The values of blood coagulation and red blood were studied in 74 patients with various forms of atherosclerotic affection of the abdominal aorta and its branches. Tendency towards hypercoagulation both before operation and in various periods after it was revealed in these patients. The development of thrombotic complications is preceded by a marked increase in the fibrinogen level and in the blood platelet count. High hematocrit with a relatively small erythrocyte count and hemoglobin concentration was revealed, which was linked with the spheric shape of the erythrocytes. It is concluded that a complex of antithrombotic therapy is necessary; it should be begun before the operation and continued particularly actively in the first two postoperative weeks.

[Value of puncture biopsy of the kidneys on the side of the stenosis in renovascular hypertension. I].

The morphological picture of bioptic renal tissue was studied in 23 patients with different forms of vasorenal hypertension, among whom 12 had fibromuscular dysplasia of the renal arteries, 8 had atherosclerotic stenosis, and 3 had panarteritis of the aorta and its branches with involvement of the renal arteries. The syndrome of malignant hypertension was encountered in 13 patients and stable arterial hypertension with high blood pressure levels in 10 patients. Analysis of the bioptic material allowed it to be divided into two groups: the 1st was marked by a picture of "Selye's kidney" and the 2nd by a combination of stenosis of the renal arteries and parenchymatous diseases of the kidneys. The significance of examining the morphological picture of renal bioptates, both with regard to the study of the pathogenesis of vasorenal hypertension and the diagnosis of concomitant parenchymatous process is shown.

Cardiac lesions in nonspecific aorto-arteritis.

Cardiac conditions were examined in 48 patients with nonspecific aorto-arteritis. The authors assume that nonspecific aorto-arteritis may be accompanied by lesions, although usually moderate, of the valvular apparatus. Such defects, even moderate ones, are accessible to intravital diagnosis based on auscultation findings. Additional study is required for resolving whether such valvular defects are manifestations of the primary disease or sequelae of nonspecific aorto-arteritis combined with a rheumatic affliction. The most frequent form of cardiac lesion, however, is myocardial hypertrophy resulting from systemic hypertension. Signs of coronary insufficiency may signalize coronaritis and result from relative coronary insufficiency. Besides, there also occur symptoms of myocardial dystrophy, which in some patients may be caused by thyrotoxicosis, occasionally encountered in patients with nonspecific aorto-arteritis.

[Surgical treatment of aneurysm of the abdominal aorta and its branches].

A total of 168 patients with aneurysms of the abdominal aorta and of its branches were examined, 117 of them were operated on. The leading method of surgery for abdominal aorta aneurysms consists in a resection of the aneurysm and its subsequent prosthetic repair. The choice of the operative technique for the aneurysms of the visceral branches and renal arteries depends on the localization and size of the aneurysm, on the importance of the diseased vessel in the blood supply to the digestive organs, as well as the organ which blood supply is impaired. The improvement of the operative technique and of the postoperative care of patients with abdominal aorta aneurysms helped to decrease the number of postoperative complications and the mortality in the recent years. Further improvement of the results of surgery for aneurysms of the abdominal aorta and its branches requires timely hospitalization of such patients in specialized vascular departments and further perfection of the operative technique.
