[Results of surgical treatment of ulcer].

More than 2,000 patients with peptic duodenal ulcer (PDU) and approximately 500 patients with peptic gastric ulcer (PGU) have been operated on in the clinic since 1968. Selective proximal vagotomy (SPV) with or without a draining operation was carried out for duodenal ulcer 1,453 times; trunk vagotomy (TV) with pyloroplasty 208 times, and vagotomy in combination with economical resection of the stomach 380 times. PGU was treated by Billroth I operation in 172 patients, by Billroth II operation in 61, by TV with economical gastric resection in 90, by SPV in 27, and by TV with pyloroplasty in 218 patients. Most patients underwent complex examination in the immediate and late-term postoperative periods, on the grounds of which the advantages and shortcomings of the various operations could be determined and the optimal method of treatment chosen. SPV proved to be the method of choice in surgery for PDU, and SPV with a draining operation in the existence of a stenosis. It produces the best functional results and the incidence of a recurrent ulcer is no higher than 8.6%. TV with excision of the ulcer and pyloroplasty was the best operation in PGU. Repeated biopsy in gastroscopy and histological study of the excised ulcer during the operation is an obligatory condition for performing this operation. Only such tactics excludes malignant degeneration. With the performance of organ preserving operations in PDU and PGU the late-term results are significantly improved, the incidence of postoperative complications and invalidism is reduced, and the patients' mode of life is improved.

[Economic effectiveness of conservative and surgical methods of treatment of duodenal ulcer in young patients].

The work evaluates the results of treatment of duodenal ulcer in 152 young patients. The state's financial losses connected with their treatment by various methods are counted up. Prolonged nonoperative treatment was applied in 52 patients (group I), organ-preserving operations were carried out in 100 patients (group II). Group I patients felt much worse, most of them suffered from annual recurrences of the disease, 21% underwent various operative interventions later due to the development of complications. Good and excellent results were recorded in 86% of group II patients. It was established that not only are the clinical results better in group II patients, but the economical losses in timely performance of organ-preserving operations are half those in prolonged nonoperative therapy.

[Neuroendocrine tumors of pancreas (clinicomorphological, immunohistochemical and electron microscopic study)].

Neuroendocrine tumors of the pancreas were studied by histology, immunohistochemistry and electron microscopy. Tumors associated with organic hyperinsulinism showed heterogeneity in their ultrastructure, functional and proliferation activity. More than half of these tumors are multihormonal and 15% of them developed from polypotent cells which are capable for several types of differentiation.

[Determination of the primary and fine structures of galactomannan from seeds of Gleditsia triacanthos f. intermis L].

This was the first study to isolate galactomannan, a 660-kDa polysaccharide, from the seeds of Gleditsia triacanthos f. inermis L. (yield 15.4%). Its aqueous solutions were optically active ([alpha] D = +31.0 degrees) and highly viscous ([eta] = 578 ml/g). The analysis of this heteropolysaccharide using chemical, enzymatic, and chromatographic procedures, as well as IR- and 13C-NMR spectroscopy, showed that is consists of D-mannopyranose and D-galactopyranose residues (molar ratio 2.42:1). Its main chain is comprised of 1,4-beta-D-mannopyranose residues, 41% of which is substituted at C-6 with single residues of alpha-D-galactopyranose. The probability of occurrence of differently substituted mannobiose units in the chain, determined experimentally, was 0.16 for the unit Man-Man, 0.50 for the units Gal(Man-Man) and (Man-Man)Gal, and 0.34 for the dissubstitued unit Gal(Man-Man)Gal.

[Water-soluble galactomannan from the seeds of Lotus corniculatus L.: structure and properties].

Galactomannan, a water-soluble heteropolysaccharide, was isolated from the seed of Far-Eastern population of ground honeysuckle Lotus corniculatus L. (yield, 1.65%). Analysis of this galactomannan showed that is consists of D-mannose and D-galactose residues (molar ratio, 1.22:1). Its aqueous solutions were characterized by specific rotation [alpha]D = +84.10 and characteristic viscosity [eta] = 559 ml/g. Analysis of this heteropolysaccharide using chemical and enzymatic procedures, as well as IR- and 13C-NMR spectroscopy, showed that its main chain comprises 1,4-beta-D-mannopyranose residues, 95.5% of which are substituted at C-6 with single residues of alpha-D-galactopyranose.

[Composition and structure of galactomannan from the seed of Gleditsia ferox Desf].

Galactomannan, a heteropolysaccharide with a molecular weight of 1660 kDa, was isolated form the seed of Gleditsia ferox Desf., introduced in Russia, with a yield of 18.9%. Its aqueous solutions were optically active ([alpha]D = +30.5 degrees) and highly viscous ([eta] = 1430 ml/g). Analysis of the heteropolysaccharide using chemical, enzymatic, and chromatographic procedures showed that it consists of D-mannopyranose and D-galactopyranose residues (molar ratio, 2.54:1). The main chain of this galactomannan consists of 1,4-beta-D-mannopyranose residues, 39.2% of which are substituted at C6 with single residues of alpha-D-galactopyranose. The probability of occurrence of mannobiose units differentially substituted with galactose was determined by 13C-NMR data and equaled, respectively, 0.37, 0.47, and 0.16 for non-substituted Man-Man units, monosubstituted Gal(Man-Man) and (Man-Man)Gal units taken together, and for the disubstituted Gal(Man-Man)Gal units.

[Study of galactoglucomannans from the seed of Cercis canadensis L].

Galactoglucomannans were isolated by selective precipitation from aqueous and alkaline extracts of endosperm and hulls of Cercis canadensis, a member of the family Fabaceae. Their monosaccharide composition (Man: Gal: Glu) was as follows: 10.4: 0.9: 1 (polysaccharide from the endosperm) and 4.5: 0.9: 1 (polysaccharide from the hulls). The identity of IR spectra was indicative of the commonness of their structure. Analysis of the structure of the galactoglucomannan from endosperm by 13C NMR spectroscopy showed that its main chain consisted of 1,4-beta-D-manno- and 1,4-beta-D-glucopyranose. Part of mannose residues in the chain were substituted at C6 with single residues of alpha-D-galactopyranose. Galactoglucomannans are located in different part of the seed and implement different functions.
