[Effect of bemithyl (bemactor) on the bronchial epithelium regeneration and the main protector mechanisms of the respiratory system in chronic inflammation].

To improve the efficiency of the therapy of respiratory diseases, the influence of a drug belonging to the group of actoprotectors--bemithyl (bemactor) on the main protector mechanisms of respiratory organs was evaluated, using functional-morphological studies of mucocilliary apparatus and local bronchial immunity. Bemithyl was found to produce no direct influence on these mechanisms, but the use of bemithyl by special technology is perspective for these purposes.

[The role of radio-inhalation scintigraphy in diagnosing tumors of the main bronchi].

The distribution of inhaled radio preparation has been studied in 7 cases of main bronchus neoplasms and 25 healthy subjects. Patterns of radio aerosols deposition have been followed versus regime of inhalation. Application of said procedure serves to improve the existing methods of radio inhalation scintigraphy as well as diagnostic procedures for tumors of main bronchi which X-ray examination and functional assessment of external respiration fail to detect.

[Topical diagnostic methods in primary hyperparathyroidism].

The efficiency of USI of the neck, scintigraphy of the parathyroid glands (PTG) with selen-methionine and selective catheterization of veins (SCV) of the neck and mediastinum for preoperative determination of the localization of the source of hyperproduction of the parathyroid hormone was analyzed on the basis of results of examination of 149 patients including 51 patients with primary hyperparathyroidism confirmed during the operation. It was noted that none of the methods of topical diagnostics used in the work is thought to be universal for different clinical situations. A method of their complex use is proposed which foresee USI of the neck as the first step. In case of insufficient information of the results of USI the authors propose SCV of the neck and mediastinum as a method of choice which allows the size and level of the altered PTG to become more exact and which makes further search with the help of other methods of topical diagnostics more purposeful and effective.

[Hepatic scintigraphy using 123I-rose bengal].

The paper is devoted to the experience in diagnostic investigations using rose bengal, labeled with half-life 123I (123I-RB). The drug was synthesized in the B. P. Konstantinov Leningrad Institute of Nuclear Physics, USSR Academy of Sciences. Analysis of the results of investigations of 201 patients with diseases of the liver and biliary tracts revealed the advantages of 123I-RB over standard RPs of similar diagnostic purpose. Owing to low radiation exposure one could use higher activity (up to 120 MBq and more) of the drug. It made it possible to combine a study of the functional parameters of the hepatobiliary system and obtaining scintigraphic data on the structure of the liver and biliary tracts. 123I-RB is the best radiodiagnostic agent for a study of function and topography of the liver in patients with an increased blood level of bilirubin.

[Radionuclide research on the bones using 99mTc-phosphonium in ear diseases].

The state of the skeleton was studied in 62 patients with otosclerosis to investigate diagnostic potentialities of 99mTc-phosphonium, of them in 34 patients the diagnosis was confirmed at operation. 38 patients suffered from acute and chronic otitis media, 18 from cochlear neuritis, 4 from Meniere's disease. Scintigraphic imaging was processed quantitatively. When comparing skeleton images of the patients and persons of the control group (25 persons) it was established that radionuclide investigation of the bones with 99mTc-phosphonium of patients with hypoacusis was a highly informative method for determination of a degree of disorder of the temporal bone. Systemic bone affection was observed in otosclerosis. Considerable RP fixation was found in patients with an active course of the otosclerotic process. A group of patients in whom hypoacusis was combined with disorder of the osseous capsule, was detected among the patients with acoustic neuritis. Scintigraphy with 99mTc-phosphonium was recommended as an obligatory diagnostic procedure for patients with hypoacusis.

[Radionuclide study of calcium homeostasis during combined treatment of lymphogranulomatosis].

The paper is concerned with a study of the levels of total and ionized Ca and calcitropic hormones in the peripheral blood of primary patients with Hodgkin's disease and at different stages of treatment. Altogether 27 primary patients, 18 patients after radiotherapy and 30 patients after chemotherapy were examined. The absorption of 131I-glycerin trioleate in the intestine was determined in 15 patients during chemotherapy. Twenty healthy persons entered into the control group. No differences in the levels of total, ionized Ca, calcitonin and parathyroid hormone in the blood were noted, on an average, either in the patients or in the controls. The absorption of glycerin trioleate was unchanged in the patients as compared to the controls. An increased evacuation of the gastric content into the intestine was observed in the patients. The results obtained were suggestive of the absence of the ectopic production of calcitonin and parathyroid hormone in Hodgkin's disease. Modern intensive care of these patients did not result in Ca homeostatic disturbances in them.

[Diagnostic value of radionuclide visualization of the reticuloendothelial system of macrophages in multiple sclerosis].

Scintigraphic examination of the liver, spleen, bone marrow and retroperitoneal lymph nodes with the help of colloidal radiopharmaceutical 113mIn was conducted in 55 patients with multiple sclerosis (MS) and in 13 patients with other diseases of the nervous system. The authors ascertained dysfunction of cells of the mononuclear phagocytic system (MPS) in the MS patients. Clinical manifestations of the disease were found to correlate with the findings of radionuclide imaging of the parenchymatous organs and lymph nodes. A conclusion was drawn about the total disruption of the functional activity of MPS cells in MS and the inevitable involvement of the liver in the pathogenesis of the disease.

[Radionuclide visualization of the thoracic lymphatic duct in patients with stomach cancer].

Lymphoductoscintigraphy with colloid 113mIn was carried out in 24 gastric cancer patients prior to catheterization of the thoracic duct. The procedure provided perfect scintigraphic images of the duct in 15 cases, failed completely in 7 cases and terminal part of the duct was not visualized in 2 cases. During surgery, clinically undetectable involvement of lymph nodes in the left supraclavicular region was found in patients of the latter groups (4 and 1 cases, respectively). The results of radionuclide examination were confirmed by surgery in 23 out of 24 patients (95.8%).

[Value of radionuclide examination of the thoracic duct for the determination of the operability of esophageal cancer].

The radionuclide visualization of the thoracic lymphatic duct was fulfilled in 37 patients suspected for carcinoma of the esophagus. Operations were performed in 31 of them. Results of lymphoductoscintigraphy and of morphological examination of the operation material were compared. The authors recommend to use radionuclide investigation of the thoracic lymphatic duct in addition to other obligatory methods of examination of patients with carcinoma of the esophagus.

[Clinical application of 113mIn-chloride for indirect lower radionuclide lymphography in cancer patients].

Carrier-free applications of 113mIn-chloride for indirect lower radionuclide lymphography were studied in 90 cancer patients. The period of radiolabel semiexcretion from site of injection (initial interdigital spaces of foot) ranged 25-74 min. 131mIn-chloride application provided a distinct visualization of retroperitoneal lymph nodes. The results of a radionuclide lymphographic study of 43 patients with uterine carcinoma were confirmed by postoperative histological examination of resected lymph nodes in 88%. The informative value of the pictures of retroperitoneal lymph nodes obtained by gamma chambers and scanners using 131mIn-chloride was essentially identical.

[Aseptic necrosis of the femur head as a complication of intensive cytostatic, glucocorticoid and radiation therapy of lymphogranulomatosis].

Aseptic necrosis of femoral head was observed in 12 cases of Hodgkin's disease. The available data suggest that aseptic necrosis was induced by the treatment of Hodgkin's disease. Patients who have received a course of combined therapy (polychemotherapy including glucocorticoids+radiation of hip joints) should be referred to the group at risk for development of this complication.

[Radionuclide study of the thymus gland in myasthenia patients].

Using 75Se methionine the authors performed a scintigraphic examination of the thymus in 65 patients, 15 of whom were suffering from myasthenia and 48 (control) from various neoplastic pathologies of the thyroid and the larynx. The examination has made it possible to discover parts of the thymus active tissue including those remaining after thymectomy. The method enables one to control the results of radiation and surgical treatment of myasthenic patients.
