[Examination and surgical treatment of patients with combination of solid and liquid lesions of the kidneys].

The experience in examination and surgical treatment of 41 patients with a combination of solid and liquid lesions of the kidneys is analyzed. Complex examination (including USE, CT and MRI) diagnosed a combination of typical tumor and uncomplicated cyst of the kidney in 39 (95.12%) cases. Combination of a solid node and liquid lesion with renal cell carcinoma structure was seen in 2 (4.88%) cases. Surgical policy was individual for each patient. Patients with liquid lesions of a solitary kidney who have undergone earlier nephrectomy for renal tumor should be followed up because of risk of metachronous cancer.

[Differential diagnosis of renal colic].

Potential of modern examinations in differential diagnosis of renal colic and other emergency states is considered. Renal ultrasonography permits in the majority of cases to make diagnosis based on dilatation of caliceal-pelvic system, sometimes with detection of obstruction cause. It is necessary to differentiate parapelvical cysts from dilatation of caliceal-pelvic system, therefore pharmacultrasonic examination of the kidneys with lasix is recommended. Color dopplerography of the urinary bladder with detection of urine ejection from ureters ostia is analogue of conventional invasive diagnostic method--chromocystoscopy. Magnetic-resonance urography is updated non-invasive, non-contrast and non-radiation method of diagnosis of urinary tract dilatation.

[Pharmacoechography].

Ultrasonic scanning of the kidney was conducted in the presence of drug-induced polyuria in order to detect upper urinary urodynamic disorders and termed "pharmaco-echography". Pharmaco-echography with furosemide enables an indirect assessment of functional capacity and reserve potentials of the kidneys and the upper urinary tract, and detect a latent functional insufficiency. Detection of upper-urinary urodynamic disorders, using this method, makes possible timely treatment, thus preventing the development of disease. Pharmaco-echography with furosemide, combined with roentgenologic investigation in urologic patients provides an assessment of the events occurring in the upper urinary tract as well as the latter's performance and reserve potentials.

[Ultrasonic methods of functional diagnosis in urology].

The results of ultrasonic investigations (n = 15,822) and treatment of 7778 patients treated in the urological clinic of 1. M. Sechenov Moscow Medical Academy in 1985-1998. Functional ultrasonic diagnosis serves the basis for a new line in urology--application of ultrasonic methods with various pharmacological tests to improve the diagnosis and assess functional status of the kidneys and urinary tracts. Comparison of the findings of ultrasonic and other examinations in patients with urinary tract diseases with the operative and morphological findings characterizes diagnostic value of ultrasonic functional techniques in some diseases.

[A phytogenic drug kanefron H in patients with chronic cystitis and urolithiasis].

To evaluate efficacy of kanefron H (KH) in combined therapy of chronic cystitis and urolithiasis (after extracorporeal shock-wave lithotripsy-ESWL), we examined 48 women suffering from chronic cystitis. The patients were divided into two groups by the presence of pyuria: 20 patients of group 1 had pyuria, 28 patients of group 2 had no pyuria. Each group was subdivided into two groups in relation to KH. Subgroup 1a received phosphomycin as monotherapy, subgroup 1b--phosphomycin with KH (2 pellets 3 times a day for 30 days). Subgroup 2a was initially treated with anti-inflammatory drugs, local medication physiotherapy, circulation improving drugs for 10 days. Then the patients were followed up for a months without any treatment. Subgroup 2b received the same initial course but it was followed for 30 days with KH. 79 patients with urolithiasis (uroliths and ureteroliths) have undergone ESWL. 45 entered KH group (2 pellets 3 times a day), 34--the control group (spasmolytic and anti-inflammatory therapy). KH in combined treatment of chronic cystitis raises efficacy of the initial therapy (antibacterial or combined, made in the absence of pyuria), promotes achievement of longer disease remission, elimination of concrement fragments from the urinary tract. Long-term administration of KH induce no side effects. Thus, KH can be recommended in chronic cystitis and urolithiasis in patients exposed to ESWL as an effective and safe drug.

[The treatment of certain forms of chronic prostatitis].

Based on 653 ultrasound investigations of patients with chronic prostatitis the author concluded that among various forms of parenchymatous inflammations there were some that could be diagnosed as being cystic lesions. Purulent or seropurulent contents of the cysts were identified. The cyst-affected prostate in the patients with chronic prostatitis was featured by a definite echographic picture confirmed in 38 cases. Scanning revealed either small-sized cysts with occasionally rough inner surface or round-shaped echo-negative formations with fine inner structures. The failure of all therapeutic routines employed inspired the author to seek for newer approaches to the problem. As a result he suggested that the patient should be subjected to transperineal needle (in parallel with a rectal transducer) aspiration of cystic contents under the control of ultrasonic technique, then the cavity of the cyst should be cleansed with antiseptic solution and irrigated with tannins and, at last, the cavity of the cyst should be evacuated. Indications to the method proposed should be carefully considered. If employed, the method would prove its high efficiency. Skilled puncturing is required to avoid any associated complications.
