[Therapeutic approach to pulmonary hamartoma patients].

Hamartoma is one of the most common benign tumour of the lung. The authors report their results of the surgical treatment of 17 patients with lung hamartoma. Twelve of them underwent open thoracotomy and 5 video-assisted thoracoscopy. Lobectomy was performed in 6 patients, segmentectomy in 3 and patients atypical lung resection in 8. All of the patients did not have complications.

[Lung surfactant changes in acute destructive pancreatitis].

Severe acute pancreatitis (SAP), with mortality rate ranging from 15 to 40 per cent, continues to be a serious challenge to emergency surgeons. Not infrequently, in such cases lesions to the respiratory system develop, with the changes in pulmonary surfactant (PS) occurring during SAP considered as one of the major factors implicated. Alterations in structural phospholipids of PS (lecithin and sphyngomyelin) are assessed under experimental conditions in 26 dogs with modulated SAP at 1, 3, 6, 12 and 24 hours, and the obtained results compared to the ones prior to pancreatitis triggering. The animals are divided up into two groups--untreated and given Sandostatin treatment. In either group a reduction of PS fractions is documented, with a statistically significant lesser reduction of the indicators under study being established in the Sandostatin-treated group by comparison with the untreated one. Modulated SAP in dogs accounts for a significant reduction of the surfactant phospholipid values--lecithin and sphyngomyelin--in bronchoalveolar lavage (BAL).

[The diagnostic value of changes in the kallikrein-kinin system indices in patients with acute pancreatitis].

It is the purpose of the work to assay retrospectively the diagnostic value of KKS indicators--PK, KG, alpha 1-AT; alpha 2-MG, KOP N and plasminogen in the blood--studied in dynamics at 1, 3, 5, 7, 9 and 14 days after admission. For the purpose the percentage of patients showing abnormal values of the total number of cases being examined, with certain diagnosis acute pancreatitis and severity of the clinical form, is calculated. The highest is the rate of abnormal values for PK, next ranking KG, alpha 1-AT, KOP N and plasminogen the least. Presumably, PK and KG are adequate markers accurately mirroring the essence and dynamic pattern of the pathological process in acute pancreatitis patients.

Ethosuximide inducted changes in the gastro-intestinal tract.

Ethosuximide is an alternative medicament that is used for coupling of petit mal, especially in childhood. Some clinical observations show that it has secondary effects on the gastro intestinal tract (GIT). The present research tries to define the characteristics of Ethosuximide--the inducted secondary effects on the GIT, and to explain some of the possible mechanisms that cause them. The changes that occur in the GIT of patients cured with Ethosuximide are registered roentgenologically. The main change is the atony of the stomach and intestines and the reduced peristaltic activity. The influence of Ethosuximide is examined on smooth muscular samples of human stomach, taken in vitro using stomach resection. The medicament authoritatively reduce the spontaneous bioelectrical activity of the smooth muscular tissue, influences mainly it's components that have Ca+ nature. Together with that is indicated relaxation of the smooth muscular samples. In that research is expressed the thesis that this Ethosuximide reduction of the Ca(+)-influx in the smooth muscular cells and the related relaxation probably are one of the main reasons of the secondary effects on the GIT.
