Paralysis and unilateral arthritis: is the association established?

A patient with long-standing upper limb lower motor neurone paresis more recently developed homolateral upper motor neurone hemiplegia. Subsequent primary generalised osteoarthritis spared only the paralysed upper limb. Detailed review of reported cases suggest that the "protective" effect of paralysis against subsequent development of arthritis is not as clearly established as generally believed.

Severe vascular spasm due to erythromycin-ergotamine interaction.

We report a case of ergotism due to the interaction of erythromycin with a small dose of ergotamine tartrate (2 mg). Besides severe peripheral vasospasm with dysaesthesiae there was transient renal ischaemia. Recovery occurred after several weeks without sodium nitroprusside therapy. Cases previously reported in the literature are reviewed.

The neck-tongue syndrome: occurrence with cervical arthritis as well as normals.

The syndrome of unilateral occipital pain with simultaneous ipsilateral tongue dysesthesia on sharp turning of the neck, recently reported in 4 otherwise normal young adults, is now described in 3 additional instances; one without disease and one each having degenerative spondylosis and ankylosing spondylitis. The originally proposed anatomical explanation for the syndrome, interconnections between the lingual and hypoglossal nerves and the C2, C3 nerve roots, is consistent with this syndrome complex arising from arthritis affecting the C1-C2-C3 articulation.

[Hematopoietic stem cell transplantations in treatment of autoimmune diseases].

Autologous and allogeneic blood or marrow transplants are currently under intensive discussion as therapeutic option for patients with severe autoimmune disorders. Preclinical data as well as case reports from patients with transplants for other indications and concommitant autoimmune disorders support such concepts. In order to advance in a coordinated way the European League against Rheumatism EULAR and the European Group for Blood and Marrow Transplantation EBMT prepared common guide lines on how to proceed. They are presented and summarised. Clear clinical data are still missing. Preliminary data are promising and protocols for prospective studies are being prepared. They should allow a better evaluation of the procedures in a few years from now.

[Human parvovirus B19 (HPV-B19), a clinical overview with emphasis of rheumatologic manifestations].

The human parvovirus B19 (HPV-B19) causes a spectrum of diseases: Erythema infectiosum (fifth disease), aplastic crises in patients with chronic hemolytic anemias, chronic bone marrow failure in patients with congenital or acquired immune deficiency, vascular purpura, hydrops fetalis, abortion, intrauterine fetal death and HPV-19-arthropathy. In this review we will discuss the clinical pattern, the diagnostic questions and therapeutic possibilities particularly relating to rheumatic manifestations.
