Serological response of mice to Fusobacterium necrophorum.

Mice immunised with killed or living Fusobacterium necrophorum, by five different regimens, almost invariably failed to produce antibodies demonstrable by a passive haemagglutination test. An enzyme-linked immunosorbent assay (ELISA), however, usually demonstrated a serum antibody response. This suggested that F necrophorum was not in fact immunosuppressive in mice--a possibility that had been entertained to explain the great difficulty in protecting mice immunologically against challenge with F necrophorum.

Candidosis of the genital tract in non-pregnant women.

Of 300 non-pregnant women seen in an STD clinic, 94 (31.3%) harboured C. albicans or C. glabrata in the genital tract; 84 of these women (89.4%) had at least one moderate or marked symptom or clinical sign. Of the 94 women with Candida, 50 had another genital infection; 38% of these 50 women had at least one moderate or marked symptom or clinical sign, compared with 75% of the 44 women with no other infection. This difference is significant. Of the 146 women taking oral contraceptives, 51 (34.9%) had Candida, compared with 43 (27.9%) of the 154 women who were not. The incidence of trichomoniasis, anaerobic vaginitis and non-specific genital infection was lower among women with Candida than among the others.

The weak immunogenicity of Fusobacterium necrophorum.

Three of four extreme methods of immunization completely failed to protect mice against challenge with the homologous strain of Fusobacterium necrophorum. Unsuccessful vaccines included (1) broth culture killed by mild heat and emulsified with Freund's complete adjuvant, and (2) a homogenate of heavily infected mouse brains, inactivated by mild heat and given in two doses. Also unsuccessful as a method of immunization was the production of a severe subcutaneous infection with F. necrophorum, followed by curative treatment with metronidazole. Slight but significant protection against subcutaneous challenge resulted, however, from two such infections given in rapid succession. It would appear that the main virulence factors of F. necrophorum are only weakly immunogenic, and the experiments give little encouragement to the prospect of an effective necrobacillosis vaccine.

Hand function in children with myelomeningocele.

Hand function in 33 patients with myelomeningocele, aged from 4 to 17 years, was investigated by clinical examination and formal tests of co-ordination and dexterity. The average score for hand function was 59% of normal and only two children had clinically normal upper limbs. Twenty-eight patients (85%) had cerebellar ataxia, either alone or combined with other abnormal signs, most commonly upper motor neuron lesions. Hand function was significantly better in children of 11 years and older, in children under 11 with milder degrees of hydrocephalus, and in the younger children who were right-handed. Function was not shown to be better in those with lesions lower in the spine, nor in those with stable spines as has been previously reported. When co-ordination of the upper limb, manual dexterity and simultaneous movements of both arms were examined separately, these three were affected equally. Some abnormalities of hand function could be overcome by encouraging affected children to sit up and to practise skillful use of their hands as early in life as possible.

Serum and sputum concentrations of ceftazidime in patients with cystic fibrosis.

The pharmacokinetic parameters of ceftazidime were assessed in six cystic fibrotic patients during eight courses administered for acute exacerbations of pulmonary infection. In order to assess the initial and steady state concentrations of ceftazidime at a dose of 35 mg/kg and to determine the levels found after a higher dose, each ten day course of ceftazidime consisted of eight doses of 35 mg/kg followed by 22 doses of 50 mg/kg, all administered at 8-hourly intervals. Samples of blood and sputum were collected following the 1st, 8th and 9th doses for ceftazidime assay. The mean peak serum concentrations of ceftazidime were 97, 110 and 147 mg/l respectively with corresponding mean concentrations in sputum of 2.7, 2.6 and 1.6 mg/l. The mean clearance rate was calculated on a two compartment model to be 197 ml/min/50 kg lean body mass, the mean volume of distribution was 281/50 kg lean body mass and the mean half life was 1.57 h. There were large inter-patient differences in the pharmacokinetic parameters, however, which suggests that the clinical condition of the patient affects the pharmacokinetics of ceftazidime in cystic fibrosis. In all the patients, even after the doses of 50 mg/kg, the trough serum concentrations were low. Samples of blood were also collected for adverse effects before, during and after each course; there was no evidence of renal, hepatic or haematological changes in response to the drug.

The radiological evaluation of multiple pin fixation of the femoral head.

Following pin fixation of the femoral neck, extrusion of a pin tip into the joint space cannot be excluded with conventional bi-plane radiography. A per-cervical pin was allowed to extrude from the head of a dried femur. Anteroposterior and lateral photographs and roentgenograms of this preparation, simulating corresponding intra-operative radiology, demonstrated misleadingly that the pin tip was wholly within the head. Careful intra-operative and post-operative screening is considered important to detect unwanted joint penetration and so avoid avascular necrosis and late degenerative change.

Charnley low-friction arthroplasty for the treatment of hips with late complications of femoral neck fractures.

Two hundred five Charnley low-friction arthroplasties (LFA) were performed in 205 patients for late complications of femoral neck fractures. LFA was an excellent procedure for relieving pain and restoring function with a good range of movement. The deep infection rate was 3.9%. A detailed analysis of the patients with infection showed a high incidence of previous superficial wound infection or positive bacteriologic culture swabs obtained from the wound at the time of LFA.

Computer-assisted rapid enzyme-linked immunosorbent assay (ELISA) in the serological diagnosis of aspergillosis.

A computer-assisted method for the positive/negative discrimination of ELISA data is described. This method was applied to a rapid ELISA procedure for IgG class antibodies to Aspergillus fumigatus in which the performance time of the test was reduced to 1 h. The method gives results which compare well with those by agar gel double diffusion (AGDD). The computer-assisted reading, calculation and tabulation of ELISA results from one microtitre plate is performed in less than 2 min and permits analysis of large numbers of specimens.

Comparative in vitro activity of semisynthetic penicillins against Proteeae.

The in vitro susceptibilities of 181 isolates of precisely identified Proteeae species to five semisynthetic penicillins were determined with low and high inocula. Significant differences in susceptibility patterns among various Proteeae species to the penicillins examined were demonstrated. Providencia stuartii was clearly distinguished from Providencia alcalifaciens by its greater resistance to the antibiotics tested.

Computer-assisted determination of antibiotic susceptibility in photometer-read microtiter plates.

A method for the computer-assisted determination of microdilution minimal inhibitory concentrations by using a Dynatech MR580 through-the-plate reading photometer is presented. The method was evaluated by comparison with manual determination of the minimal inhibitory concentrations of 12 antibiotics for 115 bacterial isolates. The two methods agreed within one dilution in 98.3% of the cases.

Ectopic ACTH-producing lung cancer presenting with prostatic metastasis.

The case is described of a patient presenting with prostatic symptoms and a biopsy proven neoplasm in the prostate gland. Radical radiotherapy initially given to the pelvis produced satisfactory symptomatic improvement with tumour regression, but the patient subsequently developed endocrine complications which led to the site of the primary carcinoma being reconsidered. The case illustrates the ubiquitous nature of aggressive metastatic cancer.

Vocational rehabilitation for industrially disabled workers.

The role of vocational rehabilitation services, which help to re-employ injured workers, is not well understood in Australia. The rehabilitation counsellor must be able to coordinate a wide variety of resources and to find the best possible socio-economic solution for the worker.
