The patient was a 66-year-old man who complained of goiter. A diagnosis of functioning thyroid tumor was made because of high uptake of 131I after T3 suppression test and no response to thyrotropin releasing hormone. The tumor was removed and histologically proven to be follicular carcinoma. Twelve cases of thyroid carcinoma in autonomously functioning thyroid nodules have been reported, and the relation between carcinoma and an autonomously functioning nodule is reviewed from the literature.
The possibility that platelet aggregation is altered during pregnancy is controversial. We tested for alterations of alpha-adrenergic receptor-induced platelet function during pregnancy compared to that in the early phase of the menstrual cycle. We found no change in alpha-adrenergic receptor concentration or in affinity of the receptor for epinephrine. The potency and sensitivity of epinephrine to inhibit adenylate cyclase, a response mediated by alpha 2-adrenergic receptors, was not different in platelets from the two groups of subjects. The ability of epinephrine to potentiate aggregation by adenosine diphosphate was significantly increased during pregnancy; however, this was an artifact introduced by lower hematocrits during pregnancy that resulted in an increased citrate concentration in the preparations of platelet-rich plasma from these women. The difference was eliminated by normalization of citrate concentration. Thus effects of alpha-adrenergic receptors on platelet function are not different during pregnancy. It is mandatory to recognize artifactual effects of differing hematocrits on platelet aggregation studied in vitro.

A family diagnosed as cancer family syndrome because endometrial cancer was found in a member of this colorectal cancer-prone family was reported. The fact that uterine (endometrial) cancer was found in a family member during the serial follow-up of this family shows that there is no more than a difference of time between familial colorectal cancer and cancer family syndrome. The clinical significance of the familial cancer aggregation would be an important clue in the early detection of another new cancer in the family.

A 34-year-old postpartum woman with Burkitt's lymphoma is reported. Lymphoma involved the bilateral breasts chiefly, as well as the brain, ileocecal region and supraclavicular lymph nodes. Fourteen days after bilateral mastectomy, the patient suddenly went into a coma due to rapid progress of brain involvement. However, the coma improved the day after VEPA chemotherapy and radiation were undertaken, and the tumor-involved brain decreased significantly in size following chemoradiotherapy. Chemoradiotherapy produced the almost complete disappearance of the remaining tumor. The patient is now being treated by a maintenance dose of Endoxan and is doing well. Burkitt's lymphoma of the breast in the present case had the clinical characteristics of bilateral involvement with rapid growth arising during lactation and pregnancy.

Time sequence and specificity of thyroid hormones and biochemical parameters were investigated in patients following major surgery. Serum concentration of triiodothyronine decreases significantly following operation, with a biphasic regression. There is a reciprocal change in serum reverse triiodothyronine levels, but serum thyroxine levels show no significant change after operation. The significant decrease in serum concentration of alpha-2-macroglobulin and antithrombin III during and after surgery is the result of consumption of these inhibitors because the reciprocal change in serum concentration of elastase-like protease has been recognized. According to the change of curvilinear regression of serum triiodothyronine levels, 14 patients were grouped into 3. The patients for whom the curvilinear regression resembled a polynomial of degree 3 and 2 had a good prognosis, but the remaining 4 with no significant curvilinear regression had major complications and 2 died. It is meaningful that the postoperative change of triiodothyronine levels relates to the clinical outcome, to some degree.
A case of acute idiopathic myopericarditis with transient severe swelling and akinesis of the interventricular septum on echocardiograms was reported. A 35-year-old man was admitted to our CCU because of cardiogenic shock five days after the onset of "common cold". Electrocardiograms revealed low voltage and ST-segment elevation in all leads except for a VR and a VL. Serum CPK, GOT and LDH were slightly elevated. Echocardiograms showed severe asymmetrical septal hypertrophy (ASH, 20 mm), akinesis of the interventricular septum and moderate pericardial effusion. With improvement of the clinical course and of the hemodynamic data, the thickness and wall motion of the interventricular septum became normal. Serum antiviral antibodies were not elevated throughout the course. Transvenous percutaneous right ventricular endomyocardial biopsy revealed degeneration and fragmentation of the myofibrils with interstitial cellular infiltration and edema in the subacute phase (8th hospital day), but showed only focal fibrosis in the chronic phase. Coronary arteriograms and left ventriculograms in the chronic phase (50th hospital day) were normal. From these findings we can conclude that the severe transient ASH is due to myocardial inflammatory swelling.

A 61-year-old man with impending myocardial infarction was admitted and treated by percutaneous transluminal coronary recanalization (PTCR) therapy using Urokinase. Patient's symptoms subsided and his general condition maintained stable until the fifth hospital day, when he developed the signs of cardiac rupture. He was operated upon immediately, and the left ventricular rupture was confirmed and repaired successfully. Patient recovered from the surgery well. This report describes the rare successful surgical case of cardiac rupture secondary to acute myocardial infarction in Japan.
Diagnosis and treatment of 8 patients with malignant bladder tumors were performed with intravenous administration of hematoporphyrin derivative, followed by krypton-ion laser violet light photoradiation and argon-dye laser red light photoradiation therapy. Fluorescence permitting localization and evaluation of the lesion was recognized in all 8 patients. Photoradiation with red argon-dye laser light obtained complete remission in 6 patients, with followup from 6 to 18 months. To our knowledge this is the first report describing total regression of bladder tumors obtained by photoradiation. Swelling and edema were noted within 20 minutes after argon-dye laser photoradiation therapy of bladder tumors. Necrosis and partial covering of the tumor by a whitish necrotic mass were noted after 3 days. The whitish mass covered the entire tumor at 7 to 14 days. Gradual exfoliation of the mass was followed by disappearance of the tumor and regeneration of normal mucosa at 9 to 10 weeks.

A case of five metachronous carcinomas is reported. The patient was a 57-year-old male with carcinomas of the stomach, colon, rectum, and lip. A new malignancy was detected in the remnant stomach. This case suggests that frequent follow-ups and cancer screening are required for cancer patient, considering the possibility of subsequent malignancy.
