Premature ventricular contractions during +Gz with and without pressure breathing and extended coverage anti-G suit.

BACKGROUND: High +Gz is known to provoke cardiac dysrhythmias. Pressure breathing during G (PBG) and extended coverage anti-G suits (ECGS) are used to enhance +Gz-endurance and reduce fatigue during high +Gz flying. It is not known whether PBG in combination with ECGS increases the risk for premature ventricular contractions (PVC). HYPOTHESIS: PBG in combination with ECGS increases the risk of PVCs during high +Gz-loads. METHODS: Retrospective data were obtained from 14 subjects exposed to three different simulated aerial combat sorties each, in the centrifuge with a standard anti-G suit ensemble or with PBG in combination with ECGS. Each sortie consisted of a gradual onset G-exposure (GOR) and three simulated aerial combat maneuvers (SACM) containing four cycles of a +4.5 to +7 GzSACM; four cycles of +4 to +9 Gz tactical aerial combat maneuver (TACM) with several rapid transitions from +4 or +5 Gz to +8 or +9 Gz; and four cycles of +5 to +9 Gz SACM (5-9 SACM) with four cycles. ECG was recorded during the +Gz exposures to reveal any cardiac dysrhythmias. RESULTS AND CONCLUSIONS: No PVCs occurred during the GORs. During 4.5-7 SACMs, TACMs, and 5-9 SACMs there were 83, 50, and 24 PVCs with standard equipment, respectively, and 63, 54, and 39 PVCs with PBG and ECGS, respectively. There was no statistically significant difference between the equipment in any of the different types of +Gz exposures. No episodes of supraventricular tachycardia or relative bradycardia were found with either equipment.

[Synthesis and ion recognition function of 7-methoxy-4-methene-coumarin-monoaza-18-crown-6 (I) synthesis and spectroscopic properties].

The title reagent MMC-MAC (O5) is synthesized and characterized. Its acidic dissociation constant at ground and excited state are determined based on the changes of the absorption and fluorescence spectra in the water. According to the wavelength shift of fluorescence spectra in different solvent, the dipole moments of ground and excited state are estimated based on the molecular mechanics model using a alchemy 2000 software and the solvachromic method using the Lippert equation,the value is 3.11D and 8.13D,respectively. It's also found that fluorescence intensity of MMC-MAC(O5) aqueous solution is increased marvelously about twenty times and accompanying with delay fluorescence emit when deoxygenation from the solution by H2SO3-N+ (CH3) 4OH-. The intensity of normal fluorescence and delay fluorescence are proportional to the concentration of MMC-MAC(O5) in the range of 4 x 10(-8)-2 x 10(-6) and 2 x 10(-7)-2 x 10(-6) mol x L(-1), and the detection limits are 2 x 10(-9) and 1.3 x 10(-8) mol x L(-1) respectively.

Prognostic value of K-ras mutations, ras oncoprotein, and c-erb B-2 oncoprotein expression in adenocarcinoma of the lung.

This trial was undertaken to determine the prognostic role of K-ras (p21), c-erb B-2 (p185) protein expression, and the presence or nonpresence of a K-ras gene mutation in patients with adenocarcinoma of the lung. This was a retrospective study of 103 patients with adeno- or large-cell carcinoma of the lung who had available paraffin-stored tumor material. The relation of several clinical variables to survival was analyzed. Immunohistochemical techniques were used to determine expression of p21 and p185. Polymerase chain reaction (PCR) and sequencing were used to determine K-ras mutation status. Tumor stage was the only nonmolecular clinical variable predictive of survival (p=0.0001). A combination of K-ras mutation and p 185 expression (p=0.0144), ras mutation and strong p21 expression (p=0.0137), and K-ras mutation and the combined expression of p21 and p185 were predictive of poor survival (p=0.0415) in univariate analysis of all patients. The sole presence of K-ras mutation was predictive of survival. Additionally, when combined with elevated p21 or p185 expression in a subset of patients with 4 or more years of follow-up, negative correlation with survival was observed.

Improved survival in patients with advanced colorectal carcinoma failing 5-fluorouracil who received irinotecan hydrochloride and have high intratumor C-fos expression.

This study determines the prognostic role of c-fos protein expression in patients with colon cancer who previously failed therapy with 5-fluorouracil (5-FU). Patients with advanced colorectal who were refractory to 5-FU therapy received irinotecan (CPT-11) by a 90-minute intravenous infusion at a dose of 125 mg/m2 weekly for four weeks followed by a 2-week rest period were eligible for oncogene assessment. C-fos protein expression was evaluated using archival formalin-fixed, paraffin-embedded tumor tissue, and an automated immunoperoxidase histochemical technique. Thirty-five patients were found to have > 25% positive c-fos activity. Nine patients had no detectable c-fos expression. Characteristics of patient subgroups were not different, however, the median survival of patients with elevated c-fos expression from the time of treatment with CPT-11 was 436 days, whereas patients with no detectable c-fos expression had a median survival of 365 days (p = 0.045). C-fos exhibits a casual role in the initiation of apoptosis and is implicated in differentiation and proliferation. It has been shown to correlate with poor survival in breast cancer, but improved survival in patients with astrocytic glioma. In this analysis, there is a suggestion that elevated c-fos expression is a good prognostic marker for patients with refractory colorectal carcinoma.

[Room temperature phosphorimetric determination of practolol with dansyl chloride labeling].

Dansyl Chloride (DNS-Cl) was used as a phosphorescent labeling agent for the determination of practolol. The derivative was formed by refluxing practolol-ethyl acetate solution with the solid DNS-Cl in the presence of potassium carbonate anhydride. Room-temperature phosphorescent (RTP) properties of the derivative were studied by a micelle-stabilized room-temperature phosphorescence (MS-RTP) method. Phosphorescence lifetimes of practolol-DNS and the DNS-Cl were found to be 0.791 and 0.408ms, respectively. By applying the derivative MS-RTP technique, practolol could be quantitatively determined without previous separation process. Detection limit of practolol with the proposed method was 0.19 microg/mL.

[Synthesis and molecular recognition study of 6-Cs-beta-CD].

The reagents 6-Cs-beta-CD with molecular recognition function were synthesized and confirmed. Both basic condition and enough illumination were necessary for obtaining stable and strong fluorescence. With 6-Cs-beta-CD as the host, cyclohexanol, cyclohexene, cyclohexane and bromocyclohexane as the guest, the recognition properties were studied. The affinity of 6-Cs-beta-CD and the guest is distinct with different hydrophobicity of the guest.

[Phenomena of supramolecular assembly fluid room temperature phosphorescence (II)--effects of alcohols on room temperature phosphorescence of cyclodextrin/4-iodo-4'-ethyl-biphenyl/bromocyclohexane system].

Beta-cyclodextrin( beta/-CD)can induce 4-iodo-4'-ethyl-biphenyl(IEBP)to emit room temperature phosphorescence(RTP) in the presence of bromocyclohexane (BCH). If butyl alcohol (BuOH) is presented as a fourth component at the same time, RTP intensity of the system is enhanced obviously. Effects of different alcohols, CDs and derivatives of beta-CD on RTP emission of the system are studied. The results show that a ternary host-guest inclusion complex is formed among beta-CD, IEBP and BCH, but the alcohol hydroxyl bonds to the rim of the CD cavities by hydrogen-bonding and the alkyl end of alcohol flips over the two tops of the CD due to hydrophobic interaction force. Accordingly, the alcohol acts as a lid for CD cup, thereby shielding IEBP from oxygen and decreasing the collision quenching of oxygen, and decreasing the micro polarity of the CD cavity and the formation constant of ternary inclusion complex increases. As a result, the RTP of the system is enhanced. As far as we can see that the system studied is a typical supramolecular assembly one and the BuOH acts as a molecular regulation for RTP emitting.

Improved anti-G protection boosts sortie generation ability.

BACKGROUND: There is a need for evaluation of new G protection equipment. HYPOTHESIS: There is no difference between the two anti-G ensembles on affecting subjects' ability to tolerate multiple simulated aerial combat sorties. METHODS: There were 15 subjects wearing the standard CSU-13 B/P anti-G ensemble (STD) or COMBAT EDGE/ATAGS (CE/ATAGS) ensemble who were exposed to 3 centrifuge-based simulated air combat sorties during a 2-h period. Each sortie consisted of four different G-profiles: 1) a gradual onset profile; 2) simulated air combat maneuver consisting of +4.5 to +7 Gz plateaus (4.5-7 SACM); 3) simulated air combat maneuver derived from actual fighter maneuvers with peaks up to +9 Gz (TACM); 4) simulated air combat maneuver consisting of +5.0 to +9.0 Gz plateaus (5-9 SACM). Each sortie was separated by a 20-min rest period. We measured heart rate, peripheral light loss (PLL), subjective effort level, subjective fatigue level, and reported recovery time. RESULTS: There were no incidents of unintended G-induced loss of consciousness (G-LOC) with CE/ATAGS. There were four incidents of unintended G-LOC with STD. At the end of the third sortie, with CE/ATAGS, mean heart rate was lower during the 4.5-7 SACM (p < 0.001) and the TACM (p < 0.001); PLL was less during all three rapid onset profiles (p < 0.01); subjects reported less effort during the 4.5-7 SACM (p < 0.001), the TACM and the 5-9 SACM (p < 0.01); reported fatigue was significantly lower (p < 0.001); and reported recovery times were nearly halved (p < 0.01). CONCLUSION: CE/ATAGS provided significantly greater G-protection than the standard anti-G ensemble. There was no G-LOC with CE/ATAGS. This greater protection should be of significant operational value in enhancing sortie generation capability by increasing fighter aircrew G-tolerance and decreasing aircrew fatigue.

Tactical vs. other simulated aerial combat maneuvers.

BACKGROUND: There is a need for a more operationally relevant +Gz profile for centrifuge-based research and evaluation. This article describes a simulated aerial combat maneuver (SACM) named the Tactical Aerial Combat Maneuver (TACM). HYPOTHESIS: A more representative centrifuge-based SACM can be devised for high-G acceleration research and evaluation. METHODS: TACM consists of 9G x 5 s, 5G x 1 s, 8G x 5 s, and 4G x 2 s. TACM was compared against the widely accepted 4.5-7G and 5-9G SACMs. There were 15 centrifuge subjects used in this study. RESULTS: TACM accounted for all four incidents of G-Induced Loss of Consciousness (G-LOC); and most incidents of blackouts (5 of 8) observed. CONCLUSION: TACM simulates the seesaw pattern of +Gz forces of aerial combat and retains the reproducibility needed for acceleration research and evaluation. TACM captures the high onset, high amplitude +Gz changes of aerial combat when G-LOC and blackouts are more likely to occur. TACM should be particularly useful for evaluation of G-protective equipment and maneuvers, as well as fighter aircrew medical evaluations.

[The synthesize of mono-( 6-S-DNS )- beta-CD and its molecular recognition ability].

Mono- ( (6-S-DNS) - beta -CD is synthesized and characterized. The interaction between the luminescent derivative of beta-cyclodextrin and guest (cyclohexanol, cyclohexene, Bromocyclohexane, and p-phthalic acid, m-phthalic acid 3, 5-Dinitrobenzoic acid, p-hydroxybenzoic acid, p-animobenzoic acid)has been slutied. The binding constants and sensitivety factors are obtained based on modified Hildbrand-Benesi equation. The results show that the guest-Host inclusion complex with 1:1 stoichiometry is formed and that the binding constants and sensitivity factors for molecular recognition is directly related to the molecular structure and hydrophobic character of guest.

[The medium effects on fluorescence of coumarin-1 complexes].

The fluorescence spectra of 4-methyl-7-diethylaminocoumarin (coumarin-1) in cyclohexane-alcohol, 1, 4-dioxane-water binary mixed solvents and cyclodextrin, surfactant aqueous solutions were studied in detail. The reasons that fluorescence spectrum of coumarin-1 is very sensitive to environmental polar change were discussed based on solvent-solute molecule hydrogen bond and its effects on formation of nonradiative twisted intramolecular charge transfer (TICT) state for the fluorescence probe. In the meanwhile, it' s inclusion constant with CD and bonding constant with surfactan micella were determined.

The effect of a 4-mg preoperative intravenous dose of ondansetron in preventing nausea and vomiting after maxillofacial surgery.

PURPOSE: The efficacy of a preoperative 4-mg dose of ondansetron given intravenously in preventing postoperative nausea and vomiting after maxillofacial surgery was evaluated in a double-blind randomized study. PATIENTS AND METHODS: Sixteen patients had intravenous ondansetron, and an equal number had normal saline as a placebo immediately before induction of anesthesia. After intubation, a nasogastric tube was inserted, and general anaesthesia was induced with thiopentone, suxamethonium, fentanyl, nitrous oxide, enflurane, and vecuronium. Anesthesia lasted more than 1 hour in all patients. Postoperatively, episodes of vomiting were recorded, and nausea was assessed on a visual analog scale at 1, 4, and 24 hours from the time of awakening. RESULTS: Eight patients in the ondansetron group, compared with six in the placebo group, had nausea, and six in the ondansetron group and three in the placebo group vomited. Of the six patients who vomited in the ondansetron group, five vomited more than 8 hours from the injection of ondansetron. No significant adverse effects could be directly attributable to ondansetron. CONCLUSION: Four milligrams ondansetron given intravenously preoperatively was ineffective in controlling postoperative nausea and vomiting after maxillofacial surgery. Possibly, a larger dose or a second dose given 8 hours after the first dose may be effective.

Acute and follow-up intravascular ultrasound findings after balloon dilation of coarctation of the aorta.

BACKGROUND: The study objective was to examine the vascular wall changes caused by balloon dilation of coarctation of the aorta (CoA) acutely and at short-term follow-up using intravascular ultrasound imaging. Intravascular ultrasound has been valuable in assessing the vessel wall changes in coronary and peripheral arteries after balloon dilation, often with more detail than angiography. METHODS AND RESULTS: Intravascular ultrasound imaging, using 4.8F, 20-MHz or 6.2F, 12.5-MHz catheters on either Diasonics or HP scanners, was performed in 17 patients during balloon angioplasty for native (n = 12) and recurrent (n = 5) CoAs. Nine patients were also studied at the time of follow-up cardiac catheterization 28.1 +/- 18.0 months after angioplasty. Immediately after dilation, the mean pressure gradient across the CoA decreased from 42.9 +/- 16.4 to 9.0 +/- 5.4 mm Hg (P < .001) and the mean diameter of the coarcted segment increased from 4.4 +/- 1.9 to 7.9 +/- 2.4 mm (P < .001). An intimal tear or flap was noted by ultrasound in 12 of the 12 native CoAs and 4 of the 5 recoarctations. In contrast, only 6 of the native CoAs and 2 of the recoarctations had an intimal flap or dissection detected by angiography. At follow-up, the residual pressure gradient did not significantly change from that measured immediately after dilation, but the CoA diameter increased from 7.8 +/- 1.5 to 9.9 +/- 2.3 mm (P < .01). No aneurysms were detected. Four of the 9 patients showed ultrasonic and angiographic evidence of healing and remodeling with diminution in size or disappearance of the intimal tears. CONCLUSIONS: There is a high incidence of intimal tears and dissections immediately after balloon angioplasty for native and recurrent CoAs. Intravascular ultrasound is more sensitive than angiography in detecting the vascular wall changes. Even significant intimal tears are not necessarily associated with aneurysm formation, and many decrease in size or disappear at short-term follow-up.

The effect of head and body position on +Gz acceleration tolerance.

It has been suggested there is a relationship between acceleration-induced loss of consciousness (G-LOC) and head/body position. A two-part investigation was conducted to determine whether head and body position affects acceleration tolerance. A retrospective analysis of high-G training data (N = 1,914) compared G-LOC occurrence during straight-ahead exposure to a "check-6" exposure [10 s at +9 Gz; 6 G/s onset rate; G-suit inflated; anti-G straining maneuver (AGSM) performed]. A prospective study (N = 12) was conducted with acceleration exposures using light loss criteria with subjects in straight-ahead, above, over-the-right shoulder, or over-the-left shoulder positions. Profiles consisted of 0.1 G/s onset-rate runs (no G-suit inflation; relaxed) to a maximum of +9 Gz and 0.5 G/s onset-rate runs (G-suit inflated; AGSM performed) to +9 Gz for up to 26 s. In the retrospective study, no significant difference existed between G-LOC occurrence during straight-ahead (22/1914) and check-6 (32/1914) positions. During the prospective study with AGSM runs, there was no significant difference in the time at maximum G among any of the positions. During the relaxed runs, several comparisons yielded significant differences in peak G attained. These results indicate there may be an underlying physiologic effect of head and body position on acceleration tolerance; however, the AGSM and the G-suit overcame this effect. Although task saturation and distraction may compromise performance of the AGSM and subsequently predispose acceleration-related hazards, a proper AGSM, combined with effective protective systems, remains essential components of a protection strategy.

Eating habits of elderly Vietnamese in the United States.

A sample of 62 elderly from the Vietnamese Senior Citizens Association responded to a 24-hour dietary recall interview regarding their eating habits and food preferences in order to examine their health status. Results indicated that one-fifth of those sampled had milk at breakfast. The majority of elderly still consumed rice at both lunch and supper, and 94 percent did not snack. Thirty-one percent took vitamin supplements. The health problems most frequently mentioned were anemia, arthritis, hypertension, and diabetes. Research on food habits and diet in relation to diseases of this ethnic group is much needed.

Fluorescent probes for cancer detection.

Several new fluorescent dyes, derivatives of pyrene and of coumarin, were synthesized that have excitation and emission wavelength maxima considerably red-shifted as compared to most pyrene and coumarin compounds. These new fluorescent compounds have high extinction coefficients and high quantum yields, and they also are very environmentally sensitive, which makes them good probes of biological systems. Several of these fluorescent compounds are preferentially taken up and retained by leukemic and other cancer tissue cell lines as compared to normals, particularly 1,3-dihydroxy 6,8-pyrenedisodiumsulfonate and 3-(carboxymethylester)-7-julolidinocoumarin. These compounds may have some usefulness in cancer detection.

Susceptibility of inbred mice to Leishmania major infection: genetic analysis of macrophage activation and innate resistance to disease in individual progeny of P/J (susceptible) and C3H/HeN (resistant) mice.

We tested the possibility that two phenotypic traits, defective activation of macrophage antileishmanial activities and susceptibility to infection with Leishmania major, were controlled by the same gene. We used P/J (susceptible) and C3H/HeN (resistant) mice to breed F1, backcross (Bx), and F2 mice that were tested individually for both traits, each of which is known to be controlled by a single autosomal gene. We found no correlation between the macrophage defect and cutaneous disease. There was a correlation between development of systemic disease and defective macrophage activation in Bx mice; this correlation, however, was not confirmed in the F2 population.
