Infantile autism and Duchenne muscular dystrophy.

We report a boy with autism and Duchenne muscular dystrophy. Myopathy was noted after 2 years of age and has since progressed slowly. At present this autistic child, 11 years 4 months old, has shown no signs of deterioration.

Dectection of spinal cord lesion using skin electrode recordings of spinal evoked potential.

The multi-channel recording of somatosensory spinal evoked potential (SSEP) was carried out on 30 patients with various neurological disorders. In some cases, SSEP changes corresponded well with the lesioned level. In general, the lesions of thoraco-cervical level were difficult to detect, because the appearance rate of SSEP peaks are reduced over the thoraco-cervical spine even in normal controls. In cases with spinal type of MS, SSEP revealed marked changes and subclinical sensory demyelinating lesions were detectable. Chronological SSEP recording in patients with acute transverse myelitis revealed functional improvements corresponding with clinical recovery. Surface and non-invasive SSEP recording is useful for detecting lesions in the spinal cord or peripheral nerves. It also offers valuable information about chronological modification of the lesions, and may reveal subclinical changes in certain neurological disorders.

Periodic EEG patterns observed in two cases with partial seizures.

Two cases with partial seizure and periodic EEG pattern were reported. Case 1. A 74-year-old woman reveal typical periodic lateralized epileptiform discharges (PLED's) on the right hemisphere. The patient was semicomatous with mild jaundice and epilepsia partialis continua in the left lower limb. Postmortem examination revealed a main metastatic carcinoma of pancreas head origin in the right parietooccipital region. In the right hemisphere, the cortical structures were relatively preserved, but the white matter including the frontal lobe was swollen and its demyelinating changes were observed diffusely. The PLED's might result from an anatomical or functional severance of the cerebral cortex from normal connections with deeper structures. Case 2. A 61-year-old woman, with idiopathic hypoparathyroidism showed partial complex seizure. The EEG revealed an anterior temporal spike focus and slowing in the right hemisphere, corresponding with repetitive seizures of about one minute duration and with several minutes interval. An interictal periodic EEG pattern appeared in the right anterior and mid-temporal region. No cerebral abnormalities were found with other neurological examinations including brain scanning, carotisangiogram, and echoencephalogram. The periodic pattern was assumed as subclinical focal seizure discharges from the right anterior temporal deep structures.

Clinical application of somatosensory cerebral evoked response for the localization and the level diagnosis of neuronal lesions.

A new SER recording system using the bilateral simultaneous peripheral nerve stimulation on cervical, thoracic and lumbar segments was found to be helpful as a diagnostic aid for the localization and the level diagnosis of sensory neuronal lesions including peripheral nerves. SER abnormalities were observed in 32 (76.2%) of the 42 cases with cerebrovascular disease, in 11 (64.7%) of the 17 cases with spinal cord lesions and 12 (75.0%) of the 16 cases with peripheral nerve lesions. As an interesting finding, marked SER abnormalities corresponding with the level and the distribution of the lesions were observed in cases with spinal cord tumor, myelopathy and polyneuropathy. In a case with recurrent myelitis, lesions of different levels of spinal cord were suggested from the SER changes. The recording system gives also valuable information about chronological modifications of the lesions, and may reveal subclinical neuronal damage in a certain neurological disorder.
