Simultaneous measurement of changes in the membrane potential and the intracellular Ca2+ concentration caused by somatostatin in human GH-producing pituitary tumor cells.

Changes in the membrane potential and the intracellular Ca2+ concentration ([Ca2+]i) caused by somatostatin (SRIF) were simultaneously measured in human GH-producing pituitary tumor cells, by means of the nystatin-perforated whole cell clamp technique and Fura-2 AM. An application of 10(-8) M SRIF hyperpolarized the membrane and arrested Ca(2+)-dependent spontaneous action potentials. [Ca2+]i concurrently decreased during membrane hyperpolarization. When the membrane potential was clamped below the threshold for voltage-gated Ca2+ channels, [Ca2+]i decreased and SRIF did not further reduce [Ca2+]i. In cells which did not show spontaneous action potentials, SRIF hyperpolarized the membrane but it affected [Ca2+]i little. From these results it was concluded that the reduction in [Ca2+]i caused by SRIF was ascribed to the decrease in Ca2+ influx through voltage-gated channels during membrane hyperpolarization. The effect of SRIF on the voltage-gated Ca2+ channel current was also examined under the perforated whole cell clamp. SRIF (10(-8) M) inhibited the Ca2+ channel current to 80.8 +/- 15.4% (n = 5) of the control. Because SRIF-induced inhibition of the voltage-gated Ca2+ channel current was not prominent, it was considered that membrane hyperpolarization is the major cause of the reduction in [Ca2+]i in human GH-producing cells.

[Immunohistochemical and clinicoendocrinological studies of gonadotropin producing pituitary adenomas].

In order to study the secretion of gonadotropins in clinically non-functioning pituitary adenomas, 83 cases were investigated by immunohistochemistry. Expression of one or more of gonadotropin subunits (alpha-subunit, follicle-stimulating hormone (FSH) beta, luteinizing hormone (LH) beta) was found in 38 (45.8%) of all adenomas studied. alpha subunit and FSH beta were positive in 28 (33.7%) and 27 (32.5%) cases respectively, whereas LH beta was detected in seven (8.4%) adenomas. The presence of both alpha subunit and FSH beta was found in 17 cases, while alpha subunit was singly positive in 11 cases and FSH beta, in 10 cases. LH beta was not detected alone, but was always accompanied by alpha subunit or FSH beta. By the double staining method, alpha subunit and FSH beta were not always colocalized in the same cells. Some cells were found to contain both alpha subunit and FSH beta, but others contained either alpha subunit or FSH beta. Clinical characteristics of gonadotropin positive adenomas (38 cases) were studied in comparison with null cell adenomas (37 cases), which were negative for all anterior pituitary hormones. The former (male 27, female 11) ranged in age from 21-74 years old (mean, 46.5 yr.), and the latter (male 16, female 21) from 28-68 yr (mean, 49.5 yr.). Gonadotropin positive adenomas tended to occur in middle-aged men. All 38 adenomas were macroadenomas, and 29 patients complained of visual failure. Clinical symptoms accompanied by hypersecretion of FSH was infrequent. Hypogonadism (amenorrhea, galactorrhea, loss of libido) were observed in 8 (9.6%) cases.(ABSTRACT TRUNCATED AT 250 WORDS)

Inhibition of growth hormone secretion by activin A in human growth hormone-secreting tumour cells.

Effect of activin A on growth hormone secretion was studied in primary culture of 8 human GH-secreting adenomas, which were responsive to TRH in vivo. When studied in vitro, basal GH secretion was reduced in all cases when cells were pre-incubated for 48 h with activin A at a concentration of 5 X 10(-9) mol/l or greater. Pretreatment of GH-secreting cells with 1 X 10(-9) mol/l activin A did not affect either basal secretion or cellular content of GH. These tumour cells also responded to TRH in vitro and the GH response to TRH was completely blocked in cells pretreated with activin A. Activin A slightly reduced the increase in cytoplasmic free calcium concentration induced by TRH. Furthermore, pretreatment of the cells with activin A attenuated GH secretion induced by A23187 or 12-O-tetradecanoyl phorbol-4-acetate, agents which bypass receptor-mediated generation of second messengers. These results indicate that activin A inhibits GH secretion by directly acting on human GH-secreting cells and that activin A inhibits the action of TRH by acting on multiple steps in the messenger system.

[Three dimensional CT reconstruction system on a personal computer].

A new computer system to produce three dimensional surface image from CT scan has been invented. Although many similar systems have been already developed and reported, they are too expensive to be set up in routine clinical services because most of these systems are based on high power mini-computer systems. According to the opinion that a practical 3D-CT system should be used in daily clinical activities using only a personal computer, we have transplanted the 3D program into a personal computer working in MS-DOS (16-bit, 12 MHz). We added to the program a routine which simulates surgical dissection on the surface image. The time required to produce the surface image ranges from 40 to 90 seconds. To facilitate the simulation, we connected a 3D system with the neuronavigator. The navigator gives the position of the surgical simulation when the surgeon places the navigator tip on the patient's head thus simulating the surgical excision before the real dissection.

Solvent effects on the cooperative order-disorder transition of aqueous solutions of schizophyllan, a triple-helical polysaccharide.

A triple helical polysaccharide schizophyllan in aqueous solution exhibited a highly cooperative transition between ordered and disordered states associated with the conformation of its side chains and nearby water molecules. The transition was followed by optical rotation and calorimetry using water containing additives such as NaOH and DMSO as solvents. The ordered state was stabilized or destabilized depending on the kind and amount of the additive employed; in particular, the addition of DMSO had a remarkable stabilizing effect. This effect was analyzed by means of a statistical mechanical theory of linear cooperative transitions, where DMSO was assumed to interact favorably with the ordered side chains. A small amount of NaOH in a solvent mixture stabilized the ordered state and made the transition curve very gradual. No molecular mechanism was elucidated to account for the role of NaOH.

Bioactivity of human growth hormone in urine from acromegalic patients.

Bioactivity of hGH in urine from five acromegalic patients was determined by Nb2 bioassay and IM-9 receptor modulation assay (RMA). Urine samples were concentrated by immunoadsorbent chromatography, dialysis and lyophilization. The ratio of the bioactivity by Im-9 RMA and the immunoactivity by RIA was between 0.81 and 1.24 (1.01 +/- 0.19, mean +/- SD). The ratio of the bioactivity by Nb2 bioassay and the immunoactivity ranged from 0.45 to 0.60 (1.37 +/- 0.85). Gel chromatography of hGH in urine samples measured by sensitive sandwich enzyme immunoassay showed that more than 97% of hGH activity was the 22K form. Urinary hGH from acromegalic patients was bioactive in Nb2 bioassay and IM-9 RMA and the bioactivity showed a close correlation with the immunoactivity. The major immunoactive form of hGH (22K) seems to correspond to the bioactivity.

Role of prolactin in age-related change in serum dehydroepiandrosterone sulphate concentrations.

Serum dehydroepiandrosterone sulphate concentrations were measured in 70 patients with prolactinoma and in 54 patients with acromegaly with normal adrenocortical function. Compared with values in normal subjects of corresponding age, serum dehydroepiandrosterone sulphate levels were increased in 22 patients with prolactinoma (31%) and in 5 patients with acromegaly (9%). The four acromegalic patients who had elevated serum dehydroepiandrosterone sulphate levels had hyperprolactinemia. The mean serum dehydroepiandrosterone sulphate concentrations in patients with prolactinoma in each decade decreased with advancing age. There was a significant negative correlation between serum dehydroepiandrosterone sulphate concentrations and ages of the patients with prolactinoma. In all 8 women with prolactinoma as in 6 normal women, serum dehydroepiandrosterone sulphate levels declined definitely during the 9 years of follow-up despite persistent hyperprolactinemia. These results indicate that serum dehydroepiandrosterone sulphate levels are increased in a substantial number of patients with hyperprolactinemia, however, PRL per se may not play a significant role in the age-related change in serum dehydroepiandrosterone sulphate levels.

Atrial natriuretic peptide in human cerebrospinal fluid.

We measured immunoreactive atrial natriuretic peptide (ANP) levels in cerebrospinal fluid (CSF) collected from patients with various neurologic disorders requiring diagnostic lumbar puncture. ANP was present in all of the CSF samples from 45 patients (1.7 +/- 0.6 pmol/L, mean +/- SD). CSF ANP levels were not related to the underlying central nervous diseases of the patients, to the presence or the absence of consciousness disturbance, or to CSF osmolalities in individual patients. In 35 patients, the mean ANP concentration in CSF corresponded to 27% of that in plasma, and there was no significant correlation between ANP concentrations in each paired sample. When ANP in pooled CSF was extracted by anti-ANP-agarose and analyzed by reverse-phase high performance liquid chromatography (HPLC), multiple peaks of ANP were found. One of the major ANP peaks was identified as ANP-(103-126) on the basis of its retention time on HPLC and the specificity of the antiserum used in the radioimmunoassay; however, none of other peaks coeluted with ANP-(99-126), ANP-(101-126), ANP-(102-126), ANP-(103-125), or ANP-(105-126). We conclude from these results that ANP is present in human CSF that is differently processed from ANP in the cardiac atrium.

[A study of the secretion capacity of TSH-beta in patients with pituitary disorders].

We studied the secretion capacity of TSH-beta in 63 patients with various pituitary disorders, carrying out the observation of TSH-beta changes after TRH administration. Serum TSH-beta concentrations were measured by radioimmunoassay according to the modified method of Kourides. Serum TSH concentrations were measured by immunoradiometric assay, and serum concentrations of free thyroid hormones were measured by radioimmunoassay. Basal TSH-beta concentrations were below 0.39 ng/ml in 17 patients with Acromegaly, below 0.56 ng/ml in 5 patients with Prolactinoma, below 0.68 ng/ml in 4 patients with Cushing's disease, below 0.48 ng/ml in 12 patients with non-functioning tumor, below 6.4 ng/ml in 16 patients with SITSH, and below 0.45 ng/ml in 9 patients with other pituitary diseases. TSH-beta changes after TRH administration differed from TSH changes in 4 patients (25%) with Acromegaly, in 2 patients (67%) with Prolactinoma, in 5 patients (71%) with non-functioning tumor, in one patient (33%) with Cushing's disease, in 4 patients (100%) with Rathke's cleft cyst, in one patient with suprasellar meningioma, and in one patient with suprasellar arachnoid cyst. 2 patients (67%) with Prolactinoma, 3 patients (43%) with non-functioning tumor, all 6 patients with non-neoplastic SITSH, and one patient with Rathke's cleft cyst showed exaggerated TSH-beta changes after TRH administration. In patients with some pituitary disorders, we thought the secretion and synthesis of TSH and TSH-beta differed from that of normal subjects. We concluded that it was necessary to investigate the mechanism of secretion of TSH-beta in patients with pituitary disorders.

[Bioactivity and heterogeneity of human growth hormone in urine from acromegalic patients].

Bioactivity of hGH in urine from five acromegalic patients was determined by Nb2 rat lymphoma bioassay (Nb2BA) and IM-9 receptor modulation assay (RMA). One urine sample (case 1) was concentrated by dialysis and lyophilization. Bioactivity by IM-9 RMA showed close correlations with immunoactivity by RIA in serial dilutions (RMA/RIA; 0.66-1.77). The estimates of bioactivity by Nb2BA were higher in all diluted samples except undiluted one than those of immunoactivity. Four samples were concentrated by affinity chromatography, dialysis and lyophilization. The ratio of RMA and RIA estimates was between 0.81-1.24, and that of Nb2BA and RIA estimates was between 0.46-2.62. Heterogeneity of urinary hGH was analyzed by gelfiltration using TSK-G-3000SW column by high performance liquid chromatography (HPLC) and by sensitive sandwich enzyme immunoassay. In addition to the main peak of 22K molecular weight (more than 97%), very small peaks of 40-60K (big hGH) and 150K (big-big hGH) were detected in chromatographic profile. Urinary hGH from acromegalic patients was bioactive in IM-9 RMA and Nb2BA. The predominant form of urinary hGH from acromegalic patients was little hGH, but big and big-big hGH were detected by sensitive sandwich enzyme immunoassay.

Two types of calcium channels and hormone release in human pituitary tumor cells.

Voltage-gated Ca2+ channel currents were examined in human growth hormone-producing cells using the whole cell variation of patch electrode voltage clamp. In 20 mM Ba2+, Na+-free medium, the inward current was composed of a fast-inactivating component (transient type) and a steady component (long-lasting type). These two types had different properties. The transient type inactivated with the depolarizing prepulse but the long-lasting type did not. The transient type was activated at more negative potential levels than the long-lasting type. Deactivation kinetics of the transient type were slower than that of the long-lasting type. Nitrendipine inhibited and Bay K 8644 enhanced Ca2+ channel currents, and the effects of these agents were more prominent on the long-lasting type than on the transient type. Hormone release was inhibited by nitrendipine and enhanced by Bay K 8644, indicating involvement of Ca2+ influx through voltage-gated channels in the release process.

[Two autopsy cases of primary pituitary carcinoma].

We studied two autopsy cases of primary pituitary carcinoma. Case-1. A 45 year old female was admitted on Oct. 4 1978, with a complaint of right homonymous hemianopsia. And diagnosis was pituitary adenoma. Partial removal of pituitary tumor was performed on Oct. 23 1978. She died on Dec. 5 1978 due to bleeding of gastrointestinal tract. Autopsy disclosed a pituitary carcinoma invading the left hypothalamus, mamillary body, optic and V cranial nerves, and mid brain as well as sphenoid bone. No extracranial metastasis was noted. Case-2. A 44 year old female with a history of acromegaly for 6 years was admitted with a complaint of headache on May 8 1976. She was diagnosed as having pituitary adenoma. The subtotal removal of pituitary tumor was performed on May 21 1976 and followed by 4500 rad irradiation. At this time, pathological diagnosis was eosinophilic adenoma. Seven years later, she complained of progressive right hearing disturbance, dysarthria and ataxic gait 1983. The second subtotal removal of pituitary tumor was performed with a diagnosis of recurrence of pituitary adenoma on Oct. 7 1983. After the operation, she complicated sepsis and died on Jan. 14 1984. An autopsy disclosed a pituitary carcinoma from residual pituitary gland, continuously extending to the subarachnoid space of the pons, and invading right cerebello-pontine angle and cerebellum. The histological examination revealed pituitary carcinoma with high pleomorphism and glioblastoma multiform-like feature were within the tumor.(ABSTRACT TRUNCATED AT 250 WORDS)

[Reevaluation of serum TSH determination in various pituitary disorders using highly sensitive immunoradiometric assay].

In thirty-one patients with various pituitary disorders, their serum TSH levels were reevaluated using two kinds of highly sensitive immunoradiometric assays (IRMAs). TSH levels were measured by radioimmunoassay (RIA) supplied by Daiichi Radioisotope Laboratory and the IRMAs, which were RIA-gnost TSH Ultrasensitive and SUCROSEP TSH IRMA using two types of monoclonal antibodies. The sensitivities of them were 0.08 and 0.1 microU/ml, respectively. Free thyroxine and free triiodothyronine levels were measured by RIAs (Amerlex kits). There was a significant correlation between basal TSH levels and the maximum increase in serum TSH above baseline using the RIA and the IRMAs when responses to TRH (500 micrograms i.v.) was determined, but no correlation between basal TSH and thyroid free hormone levels. In their TRH tests, three patients showed absent responses to TRH which were measured by the RIA, although by the IRMAs slight responses were observed. In two patients, exaggerated responses to TRH were observed and the increase of TSH levels measured by the IRMAs showed 143 approximately 210% higher than that of the RIA. The IRMAs were able to handle their peak levels time in all the patients with responses, however, in some patients the RIA was not sensitive enough to distinguish it. These results indicated that measurement of TSH levels by the IRMAs might be useful in the evaluation of the TSH secretion capacity in patients with various pituitary disorders.

Secretion of glycoprotein hormone alpha-subunit by pituitary tumors.

Serum glycoprotein hormone alpha-subunit levels were determined in 165 patients with pituitary adenomas. Elevated serum alpha-subunit levels were found in 17 patients (acromegaly, 5 of 58; prolactinoma, 6 of 56; nonfunctioning adenoma, 5 of 32; and ACTH-producing adenoma, 1 of 19), most of whom had normal serum TSH and gonadotropin levels. When TRH (0.5 mg) was injected iv in the 6 prolactinoma patients with elevated serum alpha-subunit levels, serum PRL and alpha-subunit levels increased in only 1 patient. Four acromegalic patients with high serum alpha-subunit levels received TRH; serum GH and alpha-subunit increased in 1 patient and did not change in 2, and only serum GH increased in the remaining patient. Oral administration of bromocriptine (5 mg), on the other hand, consistently decreased serum alpha-subunit and PRL levels in 2 patients with prolactinoma and alpha-subunit and GH levels in 1 acromegalic patient. When serum from 3 patients was subjected to Sephadex G-100 gel filtration, immunoreactive alpha-subunit eluted in a single peak, which emerged in fractions corresponding to [125I]TSH alpha. Concanavalin A (Con A) affinity chromatography revealed that the major portion of immunoreactive alpha-subunit was retained to Con A. A pituitary adenoma removed at surgery from a patient with acromegaly was studied in monolayer cell culture. Secretion of both alpha-subunit and GH from cultured adenoma cells was stimulated by TRH and suppressed by dopamine in a dose-dependent manner. Immunohistochemistry of the pituitary adenomas removed from patients with prolactinoma and acromegaly who had high serum alpha-subunit levels demonstrated alpha-subunit-containing cells as well as PRL- or GH-containing cells. These results suggest that elaboration of glycoprotein hormone alpha-subunit occurs without concurrent production of glycoprotein hormones in a substantial number of patients with pituitary adenomas and that alpha-subunit responses to stimuli in such adenomas are generally parallel with those of the concomitantly produced hormones.

Effect of GnRH-associated peptide on prolactin secretion from human lactotrope adenoma cells in culture.

The effect of GnRH-associated peptide on PRL secretion by human pituitary lactotropes in culture was studied. Pituitary adenomas obtained at selective transsphenoidal adenomectomy from a patient with prolactinoma, and two patients with mixed GH- and PRL-secreting pituitary adenomas were cultured in monolayer. When cells were incubated with dopamine (10 nmol/l), a significant inhibition in PRL secretion was observed in all the experiments, which was blocked by co-incubation with haloperidol. In mixed GH- and PRL-secreting adenoma cells, dopamine likewise decreased GH secretion. Incubation of cells with synthetic GnRH-associated peptide at concentrations up to 100 nmol/l, on the other hand, failed to affect both PRL and GH secretion. These results suggest that synthetic GnRH-associated peptide has no inhibitory effect on PRL secretion in human pituitary lactotropes.

Ultrastructural localization of prolactin in the human pituitary prolactinomas and its changes by bromocriptine treatment.

Fourteen human prolactinomas, four of which received bromocriptine treatment, were studied by immunoelectron microscopy to observe the intracellular localization of prolactin (PRL). In the untreated cases two different patterns of PRL localization were noted. In the first type (Type I) PRL was seen in Golgi saccules, single stacks of rough endoplasmic reticulum (RER), and a few secretory granules and in the second type (Type II) it was present in the predominant parallel arrays of RER. Type II cells were observed more frequently in the cases with higher serum prolactin levels. In the treated cases with normalized serum prolactin levels, the tumor cells showed accumulation of secretory granules. This finding suggested secretory inhibition. RER and Golgi saccules were inconspicuous in these cells and this might indicate decreased production of PRL. In a case which was resistant to the bromocriptine treatment, most tumor cells were similar in ultrastructural localization of PRL to those cells in the untreated cases and this might suggest the lack of dopamine receptor.
