To determine the influence of intermittent positive pressure breathing (IPPB), the level of pulmonary capillary wedge pressure (PCWP) was compared during IPPB and after a short period off the respirator in 68 occasions on 42 patients with an acute respiratory failure (ARF) of various etiologies. During IPPB, the average PCWP was in the normal range in patients with toxic or neurologic comas and in cases of increased pulmonary capillary permeability edema (IPCPE), PCWP slightly increased within chronic obstructive pulmonary disease (COPD) complicated with ARF and in hemodynamic acute pulmonary edema (HAPE). During the weaning stage, PCWP decreased in the groups of coma, COPD, and IPCPE, but increased in HAPE. The weaning test demonstrates that IPPB influenced PCWP in all patients. Therefore, PCWP cannot be assumed to represent the left ventricle filling pressure. The weaning test allows differentiation of IPCPE from HAPE. In the event of over-infusion or hypovolemia, PCWP measured under IPPB can lead to misinterpretation if not followed up by a second measurement off the respirator.

In 85 patinets withstatus asthmaticus, the authors have studied the acid-base balance, the blood gas tensions and various humoral parameters. The values were classified into two groups according to the PaCO2 level: below or equal to 44 torr (Group I), higher than 44 torr (Group II). In the 58 cases of Group II, there was a very close positive correlation between PaCO2 and H + ions, practically the same as that established by BRACKETT et al. [3] in experimental acute hypercapnia in man. On the contrary, the correlation derived from cases of status asthmaticus in the literature showed, in some cases, a metabolic component in acidosis. In the present work, the mean value of lactates was close to normal; there was a slow increase in protein content and hematocrit, in the two groups. The prognosis of the status asthmaticus depends on the degrees of hypercapnia: when it reaches 70 torr, mechanical ventilation is urgently needed and is the main part of the treatment; the use of additional drugs remains a matter of specific case.
This study points out that in acute respiratory distress syndrome, the positive end-expiratory pressure (PEEP) had in every case the same action on functional residual capacity and static lung compliance. However its results on PaO2, PaCO2 and circulation are often different from patient to patient according also to the different levels of PEEP. The level of optimal PEEP is that which opens the largest number of alveoli; it is better defined by the value of PaO2 on 100% oxygen than by static compliance.
We defined a new optimal positive end expiratory pressure (PEEP) in the adult respiratory distress syndrome (ARDS). The optimal PEEP is the one which allows to obtain a PaO2 greater than or equal to 400 mmHg and/or an intrapulmonary shunt less than or equal to 15 p. cent, the cardiac output being held constant. 14 cases of ARDS have been treated by this method with encouraging results. The earlier optimal PEEP was applied, the more effective it was.

A 56-year-old woman developed diffuse interstitial pulmonary amyloidosis and died two years after the onset of dyspneic symptoms. Lung biopsy confirmed the amyloid nature of the reticulonodular lesions, which were at the origin of the mixed restrictive and obstructive ventilatory deficiency. Investigations showed an obvious hypogamma globulinemia but the etiology of the affection remained unknown. Pathological examination revealed massive infiltration of amyloid material in the lungs, no involvement of the heart, what was unexpected, and minimal deposits in the mediastinal lymph nodes kidneys, and spleen. An immunofluorescence study of the pulmonary amyloid deposits was performed. The authors emphasize the rarity of such diffuse interstitial pulmonary amyloid lesions which differ greatly from nodular amyloidosis and upper respiratory tract amyloid infections.
