[Urgent mitral valve replacement for acute mitral regurgitation due to ruptured chordae tendineae in systemic lupus erythematousus].

We reported a successful surgical treatment of mitral regurgitation (MR) due to ruptured chordae tendineae in a 44-year-old man with systemic lupus erythematosus (SLE) who had received the steroid therapy. He had signs of acute congestive heart failure with severe pulmonary hypertension due to MR, and underwent urgent mitral valve replacement. The postoperative course was uneventful. When replacing valve in SLE, a careful manipulation should be taken because of friability of cardiovascular tissue. Patients are usually administered steroid agents, and the agents ought to be discontinued in perioperative period, but it seems to be better to resume as soon as possible. We conclude that the surgical treatment for valvular diseases should be considered, even in the patient with SLE.

Inhibitory effect of tannic acid sulfate and related sulfates on infectivity, cytopathic effect, and giant cell formation of human immunodeficiency virus.

The acquired immune deficiency syndrome (AIDS) is thought to result from infection of T cells by a pathogenic human retrovirus, human immunodeficiency virus [HIV (HTLV-III/LAV)]. In this report, we synthesized sulfated plant polyphenols such as tannic acid sulfate, rutin sulfate, ellagic acid sulfate, (-)-epicatechin sulfate, and (-)-epigallocatechin 3-gallate sulfate, and examined the in vitro inhibitory effect on HIV infection using human T-cell lymphotropic virus type-I-carrying MT-4 cells, which are extremely susceptible to HIV infection. Of the compounds tested, tannic acid sulfate was the most effective and had low cytotoxicity. Tannic acid sulfate completely inhibited the cytopathic effect of HIV and the HIV-specific antigen expression in MT-4 cells at the concentration of 6 micrograms/ml. In addition, this sulfate inhibited giant cell formation in coculture at the concentration of 5 micrograms/ml.

Indications and timing of surgery for cholelithiasis associated with valvular heart disease.

Twenty patients with cholelithiasis associated with valvular heart disease were studied to assess the need and the optimal time for cholecystectomy. Twelve patients (11 symptomatic and 1 asymptomatic patients) underwent cholecystectomy. The remaining patients were asymptomatic. The levels of the total bilirubin in 9 patients, and of LDH in 15, were higher than normal. In most of the patients, the serum transaminase levels were higher than normal, but in few cases, the levels were higher than 200 IU/l. These abnormal values, however, were not consistently observed in these patients. No clear association between the type and form of valvular heart disease was demonstrated. The type of prostheses used for valve replacement in these patients were ball, tilting disc and leaflet. No significant differences in efficacy were observed among different types of prostheses. The incidence of silent stones is high in patients with valvular heart disease and heart surgery often causes deterioration in patients with cholelithiasis. The recovery of the patients who underwent cholecystectomy before valve replacement were better than those who underwent cholecystectomy after heart surgery. In conclusion, therefore, patients showing any abnormal results in liver function tests should be assessed in detail by abdominal echography and should receive surgical treatment of biliary tract before heart surgery if necessary.

Presence of the diurnal rhythms of monocyte count and macrophage activities in chicks.

The diurnal rhythms of monocyte count in the peripheral blood and the nonspecific activities of peritoneal macrophages were investigated in 6- to 12-wk-old White Leghorn chicks. It was evident that monocyte count and activities of peritoneal macrophages, i.e., phagocytosis and microbicidal activity, showed clear diurnal rhythms. The rhythmic pattern of monocyte count was biphasic, unlike those of humans or mice (which exhibit a monophasic fluctuation pattern). Diurnal rhythms of phagocytic activity and microbicidal activity also showed two peaks. However, the correspondence of fluctuation patterns between phagocytic activity and microbicidal activity that have been shown in chicken heterophils was not shown in chicken macrophages. This result suggests that the activities of both functions fluctuate independently in chicken macrophages. The patterns of macrophage-nonspecific activities were not in agreement with any rhythmic patterns concerning B cell and T cell activities reported previously. This result may indicate that the accessory functions of macrophage for helper T cell activation are not involved in the formation of the rhythmic patterns of B cell and T cell functions in chicks.

[Extraction efficacy on uptake of intraportally injected monoclonal antibody by human colorectal metastases in nude mice].

Route of administration is an important determinant in tumor uptake of monoclonal antibody (mAb). We studied the extraction efficacy of intraportal injection (IP) on tumor uptake over time in hepatic metastases of human colon carcinoma (HT-29LMM) in nude mice. H-15, a murine IgG1 mAb reactive with HT-29LMM was labeled with I-125, and injected at dose of 0.1 microgram and 1.0 microgram intraportally (IP) or intravenously (IV). More than 3 animals per group were sacrificed immediately, 1 h, 24 h, 72 h, and 120 h following injection. Hepatic metastases, normal liver tissue, and blood were weighted and counted for radioactivity. Compared to IV, IP injection resulted in higher (P less than 0.01) percent injection dose per gram (%ID/g) in metastases at all time points for both doses. Metastases: blood and metastases: liver uptake ratios were higher (P less than 0.05) on day 3 and 5 for both IP doses compared to IV doses. Significant improvement in tumor uptake was seen following IP injection of specific mAb; this has important implications on the design of clinical trials using mAb.

[Acute leukemia successfully treated with natural interferon-alpha].

Natural interferon-alpha (IFN-alpha) alone was administered to a 42-year-old man with acute leukemia, whose bone marrow revealed hypocellularity (NCC = 6 X 10(4)/microliters) and a 50% increase in blasts. Initial chemotherapy regimens, including BHAC-DMP or low dose Ara-C were ineffective. One month after starting nIFN-alpha therapy, the blasts in his bone marrow decreased below 3% and peripheral blood cell counts became normal. He has been in remission for at least 7 months.

The effects of pacing-induced left bundle branch block on left ventricular systolic and diastolic performances.

To assess the effects of pacing-induced left bundle branch block on left ventricular (LV) systolic and diastolic performance, we performed digital subtraction ventriculography while simultaneously measuring LV pressure with a catheter tip micromanometer. The subjects included 10 patients with a sinus rhythm, a normal QRS duration and PR interval within 0.22 sec. LV performance was assessed during both right atrial pacing (AP) and atrioventricular sequential pacing (AVP) at the same pacing rate. The atrioventricular pacing interval during AVP was adjusted to be the maximal interval that showed the QRS configuration seen during complete right ventricular pacing. LV end-diastolic pressure and volume during AVP did not differ from those during AP. Peak positive and negative dp/dt during AVP were significantly lower than those during AP. Time constants were also significantly longer during AVP. The QRS duration during AVP significantly correlated with end-systolic volume and time constants, and inversely correlated with ejection fraction and +dp/dt. These observations indicated that conduction disturbance per se, induced by AVP, could not only impair LV systolic performance but also diastolic performance, possibly due to asynchronous contraction and relaxation of the left ventricle.

Detection of human immunodeficiency virus (HIV) in cultures of lymphocytes from HIV-seropositive persons with special reference to an enzyme immunoassay.

Three different assay systems for monitoring the production of human immunodeficiency virus (HIV) in infected cells were compared. These were (1) a reverse transcriptase assay (RTA), (2) an indirect immunofluorescence assay (IFA), and (3) an enzyme immunoassay (EIA) for detecting HIV antigen in vitro. MOLT-4 subclone 1 cells were infected with serial dilutions of the HTLV-IIIB strain of HIV. Six days after infection, production of HIV was examined by the three methods. EIA was most sensitive showing 64-fold higher sensitivity than RTA, which in turn was fourfold more sensitive than IFA. We applied EIA for detecting HIV in the lymphocyte cultures derived from 18 HIV-seropositive persons. By means of the three systems HIV was isolated from lymphocyte cultures derived from one of nine (11%) asymptomatic persons and from five of nine (56%) symptomatic patients. Interestingly, it was noticed that four samples of lymphocytes did not allow the isolation of HIV but gave positive results by EIA only. Thus, the detection rate of HIV was increased by the application of EIA, to three of nine (33%) persons in the asymptomatic group and to seven of nine (78%) patients in the symptomatic group. Possible use of this assay system for following the development of acquired immunodeficiency syndrome is discussed.

Direct effect of verapamil on left ventricular diastolic filling in canine heart.

This study compared the hemodynamic effects of verapamil with other interventions that alter loading conditions in canines. The close correlation between peak filling velocity and systolic excursion indicated that peak filling velocity was dependent on systolic performance. However, verapamil caused a significantly greater increase in peak filling velocity than nitroprusside or volume loading despite a comparable increase in systolic excursion. Although both verapamil and volume loading induced increases in the peak filling velocity, the end-diastolic pressure and the peak pressure of the atrial v-wave, the pressure at the nadir of the atrial y-wave fell after verapamil but rose after volume loading. The rate of y-descent was also significantly greater after verapamil than after volume loading. These results indicate that, in addition to a reduction in afterload and an increase in preload, verapamil has a direct beneficial effect on ventricular filling.

[A case report of atypical coarctation of the aortic arch with thoracic aortic aneurysm].

A 30-year-old woman was admitted to our hospital because of the abnormal shadow on chest X ray film and paralysis of right fingers. The preoperative aortogram and DSA showed atypical coarctation of the aortic arch with thoracic aortic aneurysm. We successfully performed extra-anatomical bypass with a 14 mm Cooley low porosity Ducron graft between ascending aorta and discending aorta. Her postoperative course was uneventful and discharged one month after the operation.

Human immunodeficiency virus (HIV)-induced cell fusion: quantification and its application for the simple and rapid screening of anti-HIV substances in vitro.

A coculture system using Molt-4 and its HIV-producing cell, Molt-4/HIV(HTLV-IIIB) induced syncytia very efficiently. Among the cocultivations with various ratios of two types of cells, multinucleated giant cells were most clearly visible 20 hr after culture in a cell ratio of 5:5. The degree of cell fusion apparently correlated with the decrease in cell number during fusion reaction. The grade of the syncytia formation can be quantitatively expressed by the fusion index (FI). Using this system we evaluated various substances which are known to inhibit HIV replication and virus-induced cytopathogenicity in a cell-free viral infection. Glycyrrhizin sulfate, dextran sulfate, PSK, which is a protein-bound polysaccharide extracted from Basidiomycetes, and human plasma containing anti-HIV antibody gave about 15-fold reduction in FI when compared to that of control. At the same time no difference was seen between the FI given by two nucleoside analogs (3'-azido-2',3'-dideoxythymidine and 2',3'-didehydro-2',3'-dideoxythymidine) and that of control.

Rapid screening method with a cell multisizer for inhibitors of human immunodeficiency virus-induced cell fusion in vitro.

Cocultivation of MOLT-4 and MOLT-4/HIVHTLV-IIIB cells induces syncytium formation very efficiently and is an appropriate model for evaluation of various substances which might inhibit human immunodeficiency virus (HIV)-induced multinucleated giant cell formation in vitro. We attempted here to quantify the grade of the syncytium formation by using a cell multisizer. The size distribution pattern of the cocultivated cells in the presence of glycyrrhizin sulfate, polysaccharide Krestin, dextran sulfate, ribofuranan sulfate, and lentinan sulfate was indistinguishable from that of cocultured cells grown in the absence of inhibitors. However, the pattern of cocultured cells without an inhibitor was quite different from that of MOLT-4 or MOLT-4/HIVHTLV-IIIB cells alone. Moreover, the size distribution pattern of cocultured cells after treatment with two nucleoside analogs, 3'-azido-2',3'-dideoxythymidine and 2',3'-didehydro-2',3'-dideoxythymidine, which were known to inhibit cell-free but not cell-to-cell infection, was similar to that of cocultured cells without an inhibitor. These data are well correlated with the fusion index which was reported previously. Application of the cell multisizer is very quantitative for evaluating the syncytium formation induced by HIV and providing a simple and rapid screening for anti-HIV substances, especially for virus-induced cell fusion.
