Use of gastroepiploic and internal mammary arteries for myocardial revascularization.

Triple coronary artery bypass grafting utilizing the right gastroepiploic artery and both internal mammary arteries was performed successfully in a 37-year-old male, achieving primary myocardial revascularization. All grafts were in situ. The right gastroepiploic artery was anastomosed to the posterior descending artery. The right and left internal mammary arteries were anastomosed to the left anterior descending artery and the posterolateral branch, respectively. The postoperative angiogram showed good patency of all grafts. The patient recovered uneventfully with resolution of his angina pectoris.

Magnetic field of atrial depolarization.

The isomagnetic maps of normal subjects and patients with right and left atrial overloading were recorded to determine the characteristic features of the magnetic field of atrial depolarization. The isomagnetic maps examined in this study indicated the instantaneous current source, which specifically localizes the current sources due to the right and left atria, respectively. The magnetic field recorded with a second derivative gradiometer clearly detected the cardiac current source from the right atrium, which is located close to the anterior chest wall, thus this method improved the diagnostic sensitivity for right atrial overloading. In patients with left atrial overloading, the isomagnetic map showed multiple dipoles due to the right and left atria, respectively, which are difficult to be detected by the electrocardiogram or isopotential map. These results suggest that the magnetocardiogram provides useful information on the current source to supplement information obtained by the conventional electrocardiogram.

Chronic effect of left ventricular assist pumping on right ventricular function.

We evaluated right ventricular (RV) function during left ventricular assist device (LVAD) pumping during acute and chronic experiments in both normal hearts and hearts with left ventricular failure. An LVAD was implanted between the left atrium and the descending aorta of goats. The LVAD was pumped in a counterpulsation mode and at maximum bypass flow for 4-6 weeks in chronic experiments. The following results were obtained: 1) LVAD pumping itself did not affect RV function, even after 6 weeks of pumping, 2) the dimensions of the RV free-wall segments measured by sonomicrometers showed a gradual and small increase during the early postoperative days, which was due to an increase in volume returned to the RV and the deformity of the RV cavity derived from reduction in LV volume, and 3) the same tendencies were seen even with LV failure hearts. These results suggested that 1) LVADs can assist not only the LV but also the RV by reducing RV afterload, 2) excessive and prolonged assistance by an LVAD seems to dilate the RV free-wall, and 3) preexisting RV dysfunction is the main cause of signs of RV failure during LV pumping.

Non-phagocytic beta-glucuronidase release in Behçet's disease.

Non-phagocytic beta-glucuronidase release from neutrophils obtained from patients with Behçet's disease was assessed using chemotactic peptide as an enzyme inducer. Neutrophils from the patients with active Behçet's disease released much more beta-glucuronidase than neutrophils from the patients with inactive Behçet's disease or from normal healthy people. Beta-glucuronidase release was parallel to the activity of the disease and can be used as an indicator for the activity of Behçet's disease.

Radiographic abnormalities in patients with Behçet's disease.

It was believed that there were no abnormal radiographic changes in patients with Behcet's disease, but in this study, we examined X-ray films of the hands of 20 patients with Behcet's disease who had had a history of arthritis in the hands for more than 5 years. As a result, juxta-articular demineralization was seen in 11 patients, carpal rotation was seen in 9, narrowing of the joint space was observed in 3, and asymmetrical bone destruction was seen in 2. From our study, radiological abnormalities of the joints appear more common in Behcet's disease than believed.

Antinuclear antibody-negative systemic lupus erythematosus: loss in body fluids.

Repeated determination for antinuclear antibody (ANA) were negative in a patient with systemic lupus erythematosus (SLE) and severe multisystem involvement including diffuse proliferative glomerulonephritis and the nephrotic syndrome. However, concentration of urinary protein, of pleural fluid and subsequently of serum revealed ANA. This case demonstrates another cause of a false negative ANA in SLE--a low concentration of the antibody in serum and loss in body fluids.

Myocarditis in Behcet's disease--a case report and review of the literature.

A 33-year-old male with Behçet's disease developed congestive heart failure without apparent etiology. Although an electrocardiogram and thallium-201 scintigraphy showed myocardial ischemic damage, there were no abnormalities detected in the coronary arteries. Echocardiographic studies and a left ventriculogram were compatible with congestive cardiomyopathy. These findings suggested that the patient had myocarditis, an unusual association with Behçet's disease.

The mechanism of hyperchemotaxis in Behçet's disease.

We previously reported that the chemotactic response of granulocytes from the patients with Behcet's disease is higher than normal neutrophils. In this study we examined the mechanism by which neutrophils from the patients with Behcet's disease were hyperchemotactic. Normal and patients' leukocytes were disrupted in hypertonic sucrose, sonicated and the supernate was examined. The extracts from the disrupted Behcet's neutrophils increased the normal granulocyte response to the chemotactic stimulus more strongly than extracts from normal cells. This active substance was localized mainly in the cytoplasmic fraction of leukocytes. The fact that Behcet's neutrophils have a higher concentration of the cytoplasmic activator might play an important role in the pathogenesis of hyperchemotaxis.

Ubiquitous interleukin-1 alpha in fetal bovine serum may mislead the experimental results in vitro.

Fetal bovine sera (FBS) from several commercial suppliers were fractionated by gel filtration. Interleukin-1 (IL-1) activity was bioassayed using the IL-1-specific murine T cell line D10(N4)M. All the sera examined contained IL-1-like activity, with molecular weights (M(r)) of 30 kDa and 15-10 kDa. Under isoelectric focusing (IEF), the majority of IL-1 activity in either 30 kDa or 15-10 kDa fractions was focused into a position of pl 5. The activity recovered from either IEF or gel filtration was inhibited by either recombinant human IL-1 receptor antagonist (rhlL-1ra) or by the antibody against human IL-1 alpha. These biological and physicochemical properties strongly suggest that the active molecules were bovine IL-1 alpha and its precursor. There was no correlation between the amount of endotoxin and IL-1 activity. Quantification of the fractionated IL-1 indicated its presence in concentrations of 200-5000 pg/ml equivalent to human IL-1. However, high levels of IL-1 were not apparent in unfractionated FBS. Proliferation of T cells in the presence of FBS absorbed with protein A-Sepharose was greater than that of cells in original FBS. Therefore, the activity in FBS as a whole appeared to result from the balance between IL-1 and the inhibitory molecule(s). These results suggest that data obtained from experiments conducted in the presence of FBS may be influenced by the effect of bovine IL-1.
