Transmigration of fluid rapidly infused into dogs with renal blood vessels ligation and increased pulmonary capillary permeability.

To study the transmigration of fluid rapidly infused intravenously, lactated-Ringer's solution was infused at the rate of 90 ml/kg/hr for 30 min into generally anesthetized dogs. Four groups were made; group C with no fluid infusion as the control, group F with rapid fluid infusion only, group LF with renal blood vessels ligation and rapid fluid infusion, and group OF with increased pulmonary capillary permeability experimentally induced by oleic acid and rapid fluid infusion. The extravascular lung water volume (ELWV) was measured by the thermal-sodium double indicator dilution method, and the amounts of ascites and urine, and other clinical parameters were monitored before and after fluid infusion. The results show that in the dogs without increased pulmonary capillary permeability excessive fluid is mainly excreted as urine (group F) or transudates into the abdominal cavity (group LF). But in the dogs with increased pulmonary capillary permeability (group OF) excessive fluid easily and rapidly transudated into the pulmonary interstitium, resulting in pulmonary edema.

Comparison of extravascular lung water volume with radiographic findings in dogs with experimentally increased permeability pulmonary edema.

The relationship between extravascular lung water volume (ELWV) and chest radiographical findings was studied in general-anesthetized beagles. The dogs were experimentally injected with oleic acid to increase pulmonary vascular permeability. When the ELWV value in the dogs increased more than approximately 37% from the control value, their chest radiographs began to show signs of pulmonary edema. At this time, the chest X-ray density increased to 10% above the control level. PaO2 decreased, and PaCO2 increased after the administration of oleic acid. This clearly showed that the pulmonary gas exchange function was reduced following increasing ELWV. This comparison showed that probably the thermal-sodium double indicator dilution measurement of ELWV can detect slight hyperpermeability pulmonary edema that does not show on chest radiographs. The chest radiograph was therefore not suitable for the detection of slight pulmonary edema, because it did not show any changes in the early stages in hyperpermeability pulmonary edema.

[Tubulo-interstitial lesions in human renal allograft].

Tubulo-interstitial lesions developed in renal allografts are divided to two major categories, i.e., primary group specific for renal allografts consisting of rejection, cyclosporine nephrotoxicity and post-transplant acute tubular necrosis, and secondary group accompanied with nephron atrophy caused by any kinds of glomerular, interstitial and/or vascular diseases. An international classification for renal allograft biopsy called Banff criteria was proposed and a comparative study using this criteria would be possible in any place of the world hereafter. After a clinical application of potent immunosuppressive agent cyclosporine or taclorimus, acute and chronic rejection related tubulo-interstitial lesions changed to mild and less specific findings for rejection required a new specific diagnostic criteria for chronic rejection. A concomitant lesion with rejection, glomerulonephritis and cyclosporine nephrotoxicity was an ordinary diagnosis in cyclosporine era. New diagnostic methods using immunohistochemistry and electron-microscopy will contribute to a new and sophisticated pathologic classification for tubulo-interstitial lesions of renal allografts.

[Experience in intra-hepatic-arterial chemotherapy and hepatic resection for metastatic colorectal cancer].

Out of 213 consecutive cases of colorectal cancer patients, 39 patients had liver metastasis (31 synchronous metastases and 8 metachronous metastases). One-and 3-year survivals of these patients with liver metastases were 46.5% and 30.1%, respectively. Twenty-four patients who were treated with intra-hepatic-arterial chemotherapy had better survival rates of 54.7% at 1 year, and 36.5% at 3 years than non-treated patients. Fifteen patients who underwent curative hepatic resection had better survival rates of 83.1% at 1 year, and 48.5% at 3 years than non-resected patients. Thirteen patients who underwent curative hepatic resection followed by intra-hepatic-arterial chemotherapy had good survival rates of 80.8% at 1 year, and 53.9% at 3 years. Seven patients who had multiple metastases to both lobes, were treated with intra-hepatic-arterial chemotherapy. These patients had a better survival rate of 22.9% at 1 year than non-treated patients.

Long-term corticosteroid and dipyridamole treatment of membranoproliferative glomerulonephritis type I in children.

Eighteen patients with biopsy-proven membranoproliferative glomerulonephritis (MPGN) type I (16 diffuse, 2 focal) and a minimum of three years of follow-up (median 6.0 years, range 3 to 15.5 years) were treated with corticosteroids combined with dipyridamole. Initial therapy consisted of intravenous methylprednisolone pulses and/or daily oral prednisolone at the dose of 1 mg/kg combined with dipyridamole at the dose of 5 mg/kg, depending on the disease severity. Corticosteroid dosage was then reduced according to clinical improvement. At the start of therapy, 12 patients had microscopic hematuria or mild proteinuria, three patients had heavy proteinuria and three patients had insufficient renal function. At a median of 53 months of treatment, 89% of the children had normal or mildly abnormal urinary findings with normal renal function. Follow-up biopsy revealed improved active glomerular injury in all children. Although one case progressed to end-stage renal failure, the renal survival of our patients was 94% at 4 to 10 years after clinical onset. No patient suffered a relapse during corticosteroid dosage adjustment, nor did any unacceptable complications arise during the regimen. Eleven patients stopped the regimen because their renal function and urinalysis had normalized, and they remained in remission for a median of 32 months. These results suggest that long-term, tapering and limited use of corticosteroids combined with dipyridamole is useful and safe for children with MPGN type I.

[Myocardial structural proteins in cor pulmonale. Analysis by two-dimensional electrophoresis].

Changes in cardiac structural proteins caused by cor pulmonale were studied with two-dimensional electrophoresis. Sprague-Dawley rats were exposed to an oxygen-poor gas mixture (10% O2/90% N2) for three weeks. These rats (the hypoxic group) were compared to a control group that was kept in room air. Hemoglobin, hematocrit, and right ventricular systolic pressure were greater in the hypoxic group than in the control group. After the third week, desmin content of both ventricular muscles had increased in the hypoxic group, and was significantly greater in the hypoxic group than in the control group. The contents of other cardiac structural proteins did not change. These data suggest that increases in desmin are caused by adaptation of the myocardium to pressure overload associated with pulmonary hypertension. However, increases in the desmin content of the left ventricular muscle might be caused by volume overload, humoral factors, or might be a direct effect of hypoxemia.

[Disturbance of hypothalamic-pituitary hormone secretion in familial chorea-acanthocytosis].

An endocrinological study was performed dopaminergic regulation of the hypothalamic-pituitary axis of 3 patients with familial chorea-acanthocytosis (females, 38 to 47 years of age). All 3 patients exhibited low basal levels of triiodo-thyronine (T3), and 2 patients had a slightly elevated baseline plasma prolactin (PRL) level. The patients had a delayed plasma thyroid stimulating hormone (TSH) response and plasma PRL excessive response to thyrotropin releasing hormone (TRH), and a low plasma growth hormone releasing hormone (GRF) response to L-dopa. These TSH, PRL and GRF responses represent a secretion pattern due to a hypothalamic disorder, suggesting impaired regulation of hormone secretion by the dopaminergic system, primarily in the hypothalamus. In addition, an increased growth hormone (GH) response secretion following TRH (paradoxical response) was observed in 2 patients, suggesting that the pituitary was also involved. An oral glucose tolerance test (75g-OGTT) revealed a diabetic pattern in all 3 patients, indicating frequent association with impaired glucose tolerance.

[Progressive systemic sclerosis (PSS) and cancer--increasing coincidence rate of cancer in 67 PSS patients].

The coincidence rate of cancer and PSS has been increasing according to reports from Nippon Byori Boken Shuho (Annual of Pathological Autopsy Cases in Japan), reaching 12.3% in the most recent report. Therefore we reviewed the histories of 67 PSS patients seen at our division over an 18-year period between 1974 and 1992, and found a high coincidence rate (14.6%) of cancer, reflecting the increasing tendency reported in the Nippon Byori Boken Shuho. The most frequent type of cancer was gastrointestinal cancer, including gastric and colon cancer and duodenal carcinoid. There were no significant differences in the clinical and laboratory findings between PSS patients with cancer and those without. Twenty-six of the 67 PSS patients died. Cancer was the cause of death in four, ranking second behind respiratory failure. The reason for the increasing coincidence rate of cancer and PSS is unclear at present. However, it is very important to discover cancer in PSS patients as early as possible, since it has a marked effect on prognosis.

Ultrastructural localization of the C-terminus of the 43-kd dystrophin-associated glycoprotein and its relation to dystrophin in normal murine skeletal myofiber.

We used single and double immunogold labeling electron microscopy to investigate ultrastructural localization of the C terminus of the 43-kd dystrophin-associated glycoprotein (43-DAG) and its relationship to dystrophin in normal murine skeletal myofibers. Single immunolabeling localized the antibody against the C terminus of 43-DAG to the inside surface of the muscle plasma membrane and the sarcoplasmic side of plasma membrane invaginations. Double immunolabeling co-localized antibodies against dystrophin and the C terminus of 43-DAG to the same site noted in the single immunolabeling localization of 43-DAG. In particular, dystrophin and the C-terminal 43-DAG antibody signals were often observed as doublets separated by less than 30 nm. We compared these results with those obtained from double immunogold labeling with anti-dystrophin and anti-beta-spectrin, as well as anti-C-terminal 43-DAG and anti-beta-spectrin antibodies. The antibodies against dystrophin and beta-spectrin, or beta-spectrin and 43-DAG, also co-localized to similar sites in skeletal muscle fibers. Signals of doublet formations were noted but their frequency was significantly lower than the doublet frequency of antidystrophin and anti-43-DAG antibodies. The results support the presence of dystrophin and 43-DAG linkage at the inside surface of the murine skeletal muscle plasma membrane.

Quantification of antiribosomal P0 protein antibodies by ELISA with recombinant P0 fusion protein and their association with central nervous system disease in systemic lupus erythematosus.

OBJECTIVE: Using solid phase ELISA with recombinant P0 fusion protein as the antigen for detecting antiribosomal P0 protein antibody, we analyzed the association of this antibody and anticardiolipin antibody (aCL) with central nervous system (CNS) disease in patients with active systemic lupus erythematosus (SLE). METHODS: Sera from 70 randomly selected Japanese patients with active SLE were assayed for IgG and IgM antiribosomal P0 protein antibody titers and IgG aCL. RESULTS: IgG and IgM antiribosomal P0 protein antibodies were present in 29 and 12 (41.4 and 17.1%) of the 70 patients, respectively. The incidence of CNS disease, excluding lupus psychosis, was significantly higher in patients with IgG and IgM antiribosomal P0 protein antibodies than in those who lacked them (IgG antiribosomal P0 protein antibody 11/29 vs 3/41; IgM antiribosomal P0 protein antibody 7/12 vs 7/58). In addition, both IgG and IgM antiribosomal P0 protein antibody titers were significantly higher in patients with CNS disease, excluding lupus psychosis, than those without. No significant association was observed between antiribosomal P0 protein antibodies and lupus psychosis. No significant association was observed between IgG aCL and CNS disease. Serial studies of antiribosomal P0 protein antibodies and aCL in patients with transverse myelopathy also showed that IgG and IgM antiribosomal P0 protein antibodies, but not IgG aCL, were associated with CNS disease, excluding lupus psychosis. CONCLUSION: These data suggest a strong association of IgG and IgM antiribosomal P0 protein antibodies with CNS disease, excluding lupus psychosis, in SLE.

[Immunological study of tissue-infiltrating lymphocytes in Sjögren's syndrome: virus specific T cell response].

The possible role of Epstein-Barr virus (EBV) in the pathogenesis of Sjögren's syndrome (SS) has been suggested. However, the evidence has been circumstantial and it is not known at all whether cellular immune responses to EBV are linked to the mechanism of the disease. Although the oligoclonal expansion of T lymphocytes in salivary glands of SS was indicated by some analyses of the T cell receptors, the corresponding ligands are unknown. The key question may be of whether this disorder involves virus specific immune attacks on the salivary glands being a site for EBV latency. We introduce here our functional study of the salivary gland-infiltrating T lymphocytes in patients with SS for detecting EBV specific memory T cells. The potential association of the state of EBV-specific CTL in the lesion with the reactivation of EBV will be discussed.

[Clinical outcome of radical prostatectomy and pelvic lymph node dissection].

Between September, 1987 and September, 1993, a total of 44 consecutive patients had undergone radical retropubic prostatectomy and pelvic lymphadenectomy for the treatment of prostate cancer. The patients were between 56 and 77 years (mean, 68 years). Eleven patients had clinical state A2 disease, 21 had stage B disease, and 12 had stage C disease. Fourteen of the 44 patients (32%) had positive lymph node metastases. The 5-year survival rate for patients with pT1, pT2 and pT3 was 100%, 71% and 87%, respectively. It was 77% in patients with positive node disease and 90% in patients with negative node disease. The 5-year disease-free survival rate for patients with pT1, pT2 and pT3 was 82%, and 78%, respectively. It was 54% in patients with positive node disease and 88% in patients with negative node disease. In 14 positive node patients, metastases were located in obturater nodes in 8 patients (57%), hypogastric nodes, in 6 patients (43%), external iliac nodes in 6 patients (43%), common iliac nodes in 4 patients (29%) and presacral nodes in 2 patients (14%). We confirmed that radical retropubic prostatectomy is effective treatment for locally confined prostate cancer and removal of obturater, hypogastric lymph nodes and the internal chain of external iliac lymph nodes is important in detecting metastases.

[Progressive cerebellar ataxia and distal amyotrophy of Charcot-Marie-Tooth type with hyperglutamataemia:two sibling cases].

We reported two sibling cases of progressive cerebellar ataxia accompanied with muscular atrophy of Charcot-Marie-Tooth (CMT) type. Autosomal recessive inheritance was suggested because of the parental consanguinity and other family history. The first symptom was ataxic gait in their teens, and speech disturbance appeared later. Subsequently weakness and muscular atrophy developed in the four limbs in their thirties or forties. These symptoms slowly progressed. Neurological examinations revealed weakness, muscular atrophy, and disturbance of superficial and deep sensation in the distal parts of all limbs. Deep tendon reflexes were absent in the four limbs. There were no pyramidal tract signs, nor dementia. Sural nerve biopsy demonstrated the axonal degeneration without any findings suggesting hypertrophic neuritis. MRI study revealed marked cerebellar atrophy. Although plasma amino acid analysis showed elevated glutamate levels in both cases, activities of glutamate dehydrogenase in leukocytes was not reduced. Here, we propose a new disease entity of hereditary cerebellar ataxia and sensorimotor neuropathy associated with elevated plasma glutamate levels. Abnormal glutamate metabolism may be related to the pathogenesis of this disease.

Increased expression of heme oxygenase mRNA in rat brain following transient forebrain ischemia.

Heme oxygenase is a rate-limiting enzyme in heme catabolism, the end of which include iron, carbon monoxide and bilirubin. Expression of the inducible form of heme oxygenase (HO-1) was investigated in rat brain following 20 min of forebrain ischemia by Northern blot and in situ hybridization analyses. The level of HO-1 mRNA was undetectable in the cerebral cortex of sham control, but increased following ischemic insult, reached the maximum after 12 h of reperfusion, and then decreased. In sham control brain, HO-1 mRNA was detectable only in the scattered neuron-like cells within the dentate gyrus hilus. At 12 h of reperfusion, the remarkable increase in HO-1 mRNA levels was observed in both neuronal and glia-like cells distributed in the neocortex, hippocampus and thalamus.

Localization in rat brain of the trace metals, zinc and manganese, after intracerebroventricular injection.

Autoradiographic studies of rat brain, after 65ZnCl2 or 54MnCl2 injection into the lateral ventricle, revealed that 65Zn and 54Mn were transferred freely through the cerebrospinal and extracellular fluid compartments; both tracers appeared in all the ventricles 1 h after injection. At 6 days after injection, 65Zn was concentrated in the hippocampal formation and hypothalamic nuclei but not in the cerebral cortex and superior colliculus of the ipsilateral hemisphere, with a lower uptake into contralateral hemisphere except for the hypothalamic nuclei. 54Mn was concentrated in some brainstem nuclei, such as the red and pontine reticular nuclei, to about the same extent in both hemispheres. These results suggest that both metals were taken up gradually into brain mainly via the cerebrospinal fluid. Relatively high uptake of 65Zn into the cerebral cortex and superior colliculus after intravenous injection suggests uptake in those regions is through two blood/brain barriers.

Alterations in CD45 glycosylation pattern accompanying different cell proliferation states.

CD45 is a leukocyte-specific transmembrane glycoprotein whose intracellular domain exhibits protein tyrosine phosphatase activity and plays a critical role in signal transduction. CD45 derived from stationary lymphocytes migrated faster in SDS-PAGE than that derived from exponentially growing cells. A change in N-linked saccharide structure other than the neuraminidase-sensitive terminal sialic acid portion was found to be responsible for the molecular size change in CD45. The differential glycosylation appeared to occur during late-stage posttranslational processing of CD45. We speculate that the N-glycosylation difference affects the interaction between CD45 and other factors involved in signal transduction leading to modulation of leukocyte proliferation.
