Evidence of glutamatergic deficiency in schizophrenia.

Studies of amino acid release were carried out using frozen sections from brains of schizophrenics and controls. Synaptosomes were prepared via differential centrifugation in Ficoll allowing the veratridine-induced release of aspartate, glutamate, glycine, and GABA to be measured. The release of glutamate and gamma-aminobutyric acid (GABA) was reduced in the synaptosomes from schizophrenics. This decrease could be reversed partially by pre-incubation of the synaptosomes with haloperidol. Additionally, the activity of glutamate decarboxylase was decreased and partially restored by haloperidol pre-incubation. These data are consistent with the hypothesis of a glutamatergic/GABAergic deficit in schizophrenia.

Upper gastrointestinal bleeding: causes and treatment.

Upper gastrointestinal bleeding occurs above the ligament of Treitz as a result of duodenal or gastric ulcers, gastric or esophageal varices caused by cirrhosis of the liver, or Mallory-Weiss syndrome. Bleeding that requires more than three units of blood in 24 hours usually demands operative measures.

Human histocompatibility antigens and organ transplantation.

The success of organ transplantation depends not only upon the degree of skill employed in the execution of the technical procedure, but equally upon the proper antigenic matching of the donor and the recipient. Although there does not exist at the present time a perfect test for performing tissue typing-as in the case of blood transfusions-the most promising tests are those of leukocyte typing. This is possible because the histocompatibility antigens that determine the success or failure of tissue allografts are also found on the leukocytes. The problem concerns itself with developing an exact, specific, monosera test-simple and uncumbersome-that can be used to type the leukocytes and thus select a donor who corresponds antigenically to the recipient.

Early operation in the treatment of small bowel obstruction.

Small bowel obstruction, one of the most common causes of acute surgical abdomen in adults, constitutes approximately 20 percent of all cases of acute surgical abdomen. With improper management, strangulation of the bowel may occur and result in an increased probability of mortality. The author reviews the literature and discusses diagnosis, management, and complications of small bowel obstruction.

Breast cancer: pros and cons of conservative vs aggressive management.

Cancer of the breast is the leading type of cancer in women in the United States (28 percent), and the leading cause of death from cancer in women (20 percent). More than half of the cases of breast cancer can be anticipated to exhibit recurrent disease. Most deaths occur within five years of discovery. There has been a minimum reduction in the mortality rate in the past 35 years. This failure is due to microemboli that spread via both the lymphatic and vascular systems in the early predetection state of the primary lesion. In Stage I and Stage II disease, modified radical mastectomy is the treatment of choice. In Stage III disease, radical mastectomy gives a higher incidence of local control of the disease without an increased chance of survival. Adjuvant chemotherapy offers a definite improvement in survival rates.

Surgery and malpractice.

The malpractice problem affects medical practice throughout this country. The problem may be partially alleviated by physicians devoting more time and effort to health care delivery, medical education, research, and medical administration. This paper discusses the history of the present malpractice problem, analyzes its causes, and offers solutions.
