[MR tomography in the control of chemonucleolytic therapy].

MR tomography can be used for the diagnosis of lumbar disc prolapse and provide information concerning the indications and the follow-up of injection treatment with chymopapain. It is able to distinguish various parts of the disc from their varying signal intensity. Examination of 26 patients showed the effect of chymopapain on intradiscal fluid shift based on shifts in signal intensity. The use of T2-weighted gradient echo sequences over a period of seven to ten minutes is sufficient to show significant changes in the disc following chemonucleolysis.

[Indications for preventive sclerosing of esophageal varices].

In 72 patients with oesophageal varices the intravascular oesophageal variceal pressure was measured, in 31 of them the portal pressure as well. The portal pressure was identical with the variceal pressure only at the cardia. There was a positive relationship between the size of varices and the variceal pressure and between the size of varices and the occurrence of bleeding from varices. Prophylactic endoscopic sclerotherapy is indicated in grade II and III varices.

[Digital subtraction angiography in traumatology].

The methods, indications and results of digital subtraction angiography in traumatology are presented, based on 56 examinations. The different use of intravenous or intraarterial DSA will be discussed with respect to expanding and localisation of traumatic vascular injury. DSA is recommended as the method of choice for follow-up after vascular reconstructive procedure.

[MR tomography of hemophilic arthropathy of the knee joint].

The value of MR tomography in the diagnosis and follow-up of haemophilic arthropathy of the knee was studied in 40 examinations, using special surface coils. Good spatial resolution and reconstruction in various planes provide an accurate demonstration of the lesion, particularly of the intra-articular structures. Abnormalities of the synovia and of hyaline cartilage can be demonstrated. MR tomography is an important imaging method which is superior to sonography or CT in demonstrating complications resulting from the bleeding, such as effusions and haemarthroses.

[Results of percutaneous transhepatic bile duct drainage].

Significant reduction in serum bilirubin levels was obtained in 90.7% of 75 patients on whom percutaneous transhepatic biliary drainage had been carried out, either immediately before operation or as a prolonged palliative procedure. The technical requirements, complications, mortality rate and length of stay in hospital are less than following palliative surgery.

Effect of the autoregulator from Streptomyces griseus JA 5142 on surface cultures of blocked mutant ZIMET 43682.

Zero time addition of the autoregulator (L-factor) from S. griseus (Lkm+Amy+) to surface cultures of its bald mutant ZIMET 43 682 (Amy-Lkm-) restored the capacity to form both anthracycline-type antibiotic leukaemomycin and aerial mycelium. The pertinent mycelia displayed the same growth rate and cellular levels of nucleic acids as the asporogeneous phenotype but the composition of fatty acids and phospholipids as well as the ratio of cytochromes b and c were altered. These differences indicate alterations in the cellular architecture of substrate and aerial hyphae. The results suggest that the autoregulator triggers the onset of a complex programme of differentiation at a very early stage.

[Cytochrome pattern of methylotropic acetic acid bacterium MB 58 as dependent on growth conditions].

In contrast to methylotrophic bacteria investigated up to now the facultative methylotrophic Bacterium MB 58 (Acetobactersp. MB 58) does not possess a cytochrome aa3-complex, but we could find out cytochrome, cytochrome cco, cytochrome a1, and moreover cytochrome d in dependence on the growth conditions. Cytochrome d was found only in stationary phase of heterotrophic growth. Under methylotrophic growth conditions cytochrome d could be demonstrated only by lowering of the aeration rate during the fermentation, by variation the pH-value of the growth medium from 4.0 to 6.5 and with low growth rates (low dilution rates) during continuous fermentation. The addition of cyanide to the oxidized suspension of bacteria during the registration of the cytochrome-redox-difference spectrum allowed the selective representation of cytochrome d under all conditions even if no identification was possible in the spectrum normally. The oxidation of cytochrome d of Acetobacter sp. MB 58 in the presence of cyanide is an indication of its cyanide insensitivity. The low level of b-type cytochromes could be represented by a special technique for registration of spectra. In this connection a unknown absorption peak at 570 nm was registered. The cyanide insensitivity of cytochrome d from Acetobacter sp. MB 58 and the occurrence of several terminal oxidases is appreciated as a hint for a branched respiratory chain.

Assay for cyclo-, seco- and pentobarbital by multiple ion detection: kinetics after a single dose.

A sensitive assay for the determination of four different barbiturates: butabarbital, cyclobarbital, pentobarbital and secobarbital is described. The barbiturates are determined quantitatively by gas chromatography/mass spectrometry (GC/MS) as their dimethylated derivatives. The sensitivity of this simple procedure is limited to 10 x 10(-6) g/l for cyclo- and secobarbital and 1 x 10(-6) g/l for pentobarbital in plasma. The assay has been used to measure the kinetics of pentobarbital over 6 days after a single 50 mg dose of the sodium salt, and over 3 days for one combined preparation of 25 mg cyclobarbital-calcium and 75 mg secobarbital-sodium. The corresponding barbiturate levels in saliva were determined by the same assay.

Regulative influence of o-aminobenzoic acid (OABA) on the biosynthesis of nourseothricin in cultures of Streptomyces noursei JA 3890b. V. Effect of OABA on cytochrome levels and amino acid transport.

o-Aminobenzoic acid (OABA) known by its stimulatory effect on streptothricin biosynthesis by Streptomyces noursei JA 3890b was found to suppress specifically the formation of cytochrome a-type terminal oxidase while the levels of the b- and c-type cytochromes apparently remained unaffected. This change was coupled to decreased capacity of transport of u-14C-L-alanine and u-14C-L-glutamic acid into the mycelium, giving rise to delay of amino acid catabolism and decreased production of nitrogen catabolites within the cell. The above results provide supporting evidence for conjectures recently published by us concerning the control by nitrogen catabolites of the secondary metabolism of this strain. As a summary a scheme is proposed showing several hypothetic mechanisms by which nitrogen catabolite regulation of nourseothricin production could be accomplished.

Pneumocystis carinii pleuropneumonia after aerosolized pentamidine prophylaxis.

We observed an atypical Pneumocystis infection with spontaneous pneumothorax, bronchopleural fistulae, an apical cyst and Pneumocystis pleuritis after aerosolized pentamidine prophylaxis in an AIDS patient. These findings suggest a failure of pentamidine aerosol in controlling active Pneumocystis infection in peripheral pulmonary areas. A relapse of Pneumocystis carinii pneumonia (PCP) must be suspected when pneumothorax occurs during secondary prophylaxis with aerosolized pentamidine. It should always be confirmed by bronchoalveolar lavage or transbronchial or open biopsy. Cases presenting as atypical Pneumocystis pneumonia may additionally reveal extrapulmonary dissemination of Pneumocystis infection.
