Fifty patients with chronic resistant cervicobrachialgia were randomly divided into two groups. Twenty-five patients (group A) were treated with cervical epidural steroid/lidocaine injections and 17 patients (group B) were treated with steroid/lidocaine injections into the posterior neck muscles. Another eight patients from group B were excluded from the study because they had started the process of litigation of insurance claims and their subjective analysis of pain relief might therefore not be trustworthy. One to three injections were administered at 2-week intervals according to the clinical response. All patients continued their various pre-study treatments: non-steroidal anti-inflammatory drugs, non-opioid analgesics and physiotherapy. Pain relief was evaluated by the visual analogue scale 1 week after the last injection and then 1 year later. One week after the last injection we rated pain relief as very good and good in 76% of the patients in group A, as compared to 35.5% of the patients in group B. One year after the treatment 68% of the group A patients still had very good and good pain relief, whereas only 11.8% of group B patients reported this degree of pain relief. These differences were statistically significant. We failed to achieve significant improvement of tendon reflexes or of sensory loss in both groups, but the increase in the range of motion, the fraction of patients who were able to decrease their daily dose of analgesics, and recovery of the capacity for work were significantly better in group A. A case report of epithelioid adrenal angiosarcoma is presented. Tumor cells showed expression of cytokeratin, Factor VIII-related antigen, Ulex europaeus agglutinin-I, and vimentin. The patient also was found to have mesenteric fibromatosis (abdominal desmoid tumor) and an elevated serum level of estradiol. The authors discuss the unique appearance of these rare tumors, their relationship to hyperestrinism, and review the recent data in the literature showing cytokeratin expression by malignant epithelioid vascular tumors.
Metoclopramide is one of many drugs that have been recommended for the treatment of intractable hiccup. Methohexital may produce hiccup during induction of general anesthesia. 211 women received methohexital for induction and maintenance of general anesthesia for short gynaecological procedures. All the patients were premedicated with fentanyl, diazepam and atropine. 109 patients were randomly selected to receive metoclopramide before induction of anesthesia; the remaining 102 patients served as a control group, and were anesthetized without metoclopramide premedication. The frequency of hiccup was compared between the two groups. 7 patients had hiccup in the metoclopramide premedicated group, as compared to 17 patients in the control group. This difference was statistically significant. We conclude that metoclopramide reduces the frequency of methohexital induced hiccup, and recommend that metoclopramide be routinely used for the premedication of methohexital injection.

Hemodynamic aspects of obliterative processes in peripheral blood vessels were studied on a mechanical model built of distensible tubing, with a fixed peripheral resistance, through which citrated blood was circulated by pulsatile flow. Hemodynamics of progressive focal stenosis, elongated soft stenosis, and elongated rigid stenosis were assessed. By the use of a hydrodynamic model and a series of in vitro experiments, we have measured the pressure and flow characteristics, and calculated the pressure and energy losses for the various stenotic sites. The critical stricture was found to be larger for a rigid stenosis than a soft stenosis. The length of the stenosis was also an important factor. Increasing the length of a rigid stenosis, for example, by 50 percent resulted in an increase of 25 percent in the flow through the stenosis. The energy dissipation was determined as a preferred indication for several parameters such as: pressure drop, pulsed flow, pulse rate, and the geometry and mechanical properties of the stenosis.
A 2.5 kg female neonate, born after a normal 39-week pregnancy to Arab parents (1st degree cousins), developed unremitting vomiting of gastric contents, not bile-stained. Barium study demonstrated a grossly distended stomach with complete obstruction of the outlet. At laparotomy, pyloric atresia and a heterotopic pancreas located subserosally on the antrum of the stomach were diagnosed. A 3-cm gap was measured between the occluded antrum and the duodenum. Transmesocolic gastrojejunostomy was performed, and oral feeding was started 5 days later. Pyloric atresia and heterotopic pancreas are both rare congenital malformations. To the best of our knowledge, the combination of pyloric atresia and heterotopic pancreas has not previously been reported.

The relatively simple cytogenetic findings in an aggressive metastatic Merkel cell carcinoma are reported. Deletion 2p was found in 100% of the cells. Nevertheless, this was considered a secondary (metastatic?) change because the same aberration has been found in several other kinds of malignancy. The involvement of chromosome 22 [del(22q) and -22] in 85% of the cells seemed more intriguing, considering the fact that the Merkel cell carcinoma followed a previous meningioma.
A method for immediate anatomic reconstruction of the pelvis using Marlex (nylon) mesh is described, with good results in 2 cases of chondrosarcoma subjected to partial resections of the hemipelvis.
A familial fragile 8q22 and an interferon-induced fragile 16q22 were found in two sisters. Eight years previously, both sisters developed an endometrial adenocarcinoma and now one of them presented with an adenocarcinoma of the colon. An 8q22 deletion was found in all the cells of the colonic tumor and seemed to be the primary initiating change. Other nonrandom and possibly promoting aberrations were also present, among others, a 16q22 deletion. The possibility exists that a familial fragile 8q22 may predispose to cancer and a fragile 16q22 may have promoting capacities.

We report on the cytogenetic findings in direct preparations of two tumors of the sigmoid colon. Respectively, they had a near-triploid and a near-tetraploid constitution and relative numerical and other inconstant chromosomal imbalances. The only constant chromosomal structural anomaly apparently was inversion of chromosome #16, inv(16)(p13q22), which was found in all the cells examined. This rearrangement has been found to be associated with a type of acute myelomonocytic leukemia type M4. We suggest that an etiologic clastogenic (and oncogenic) agent responsible for this rearrangement may eventually be the cause of various kinds of malignancy.

The mortality experience of radiologists compared to that of other physician specialists demonstrates an increased risk of cancer deaths as well as deaths from all causes among physicians practicing in the early years of this century. However, for the radiologists who joined specialty societies after 1940, the age pattern of deaths has changed. Whereas among early entrants, young radiologists had higher mortality rates than those of other specialists, among later entrants, the young radiologists have lower mortality. However, as these later-entrant radiologists age, their rates appear to exceed those of other specialists. Although the level of radiation exposure is unknown, physicians in more recent years usually have lower cumulative doses. Lower radiation exposure may be one of a number of possible explanatory factors for the cross-over from "protected" to "higher risk" status as these physicians age.
