[Prostate neoplasm prevalence in Talca, VII Region of Chile].

BACKGROUND: In 1998, there were 1,218 deaths in Chile caused by prostate cancer. This figure results in a death rate of 16.6 per 100,000 males for this disease. AIM: To assess the prevalence of prostate cancer in the Seventh Region of Chile. MATERIAL AND METHODS: A probabilistic sample of 327 males aged 40 to 59 years old was studied. In all, a codified questionnaire was applied, a digital rectal examination was performed and a blood sample was drawn to measure prostate specific antigen. All digital rectal examinations were performed by the same observer. Patients with an abnormal rectal examination or prostate specific antigen were subjected to a prostatic biopsy under ultrasound guidance. RESULTS: In 14 subjects, the digital rectal examination was considered abnormal and in seven, prostate specific antigen was over 4 ng/ml. All subjects with elevated prostate specific antigen had an abnormal rectal examination. In three of the 14 subjects, the biopsy showed a well differentiated adenocarcinoma. All three were aged over 50 years old. The resulting calculated prevalence of prostate cancer was 9.2 per 1,000 males (CI 4.2-14.1). CONCLUSIONS: The cost effectiveness of screening for early diagnosis of prostate cancer must be calculated, to decide its incorporation in preventive medical examinations.

[Parameters of normality in multifrequency tympanometry].

Multifrequency tympanometry studies consist of tympanography using probe tone frequencies ranging from 200 to 2000 Hz, improving the study of acoustic transmission through the tympano-ossicular system because then two components of admittance, conductance and susceptance, can be separated. The resonance frequency is the frequency at which mass and spring elements of the middle ear cancel each other out, leaving only the friction component. This measurement has been found to be more sensitive to the presence of pathologies that affect the tympano-ossicular system, such as otosclerosis and rheumatoid arthritis. It is necessary to know normal pattents of tympanometric parameters to improve the study of these diseases. Multifrequency tympanometry performed on 136 patients, 91 women and 45 men, age range 11-78 years. The mean resonant frequency of the middle ear was 1132.33 Hz, mean static admittance 0.76 dapa, and mean tympanometric amplitude 94.31 mmhos ac. Age showed no systematic effect of age on any of these measures in this population, and no significant association was found between static admittance or tympanometric amplitude and resonance frequency.

[Oral nutritional supplements. Are they useful?].

Oral nutritional supplements are products included in enteral nutrition, preferably used in hospital settings, although their use in the community is gradually increasing and in the United Kingdom has doubled in the last seven years, with prescriptions covering a wide range of the population from children to the elderly, and different pathologies affecting their nutritional status. There is, however, no consensus on the usefulness of oral nutritional supplements among these patients, so we do not have any recommendations for use. In this paper we have reviewed the various studies available in the literature in order to clarify the usefulness of these supplements in different contexts or pathologies.

The V3 loop based multi-epitope polypeptide TAB9 adjuvated with montanide ISA720 is highly immunogenic in nonhuman primates and induces neutralizing antibodies against five HIV-1 isolates.

In a previous work we selected montanide ISA720 (M-ISA720) among different adjuvants for the vaccination with a V3 loop based multi-epitope polypeptide TAB9. In this paper we present the evaluation of the toxicity and immunogenicity of this formulation in non-human primates. TAB9 in M-ISA720 was highly immunogenic in macaques (Macaca fascicularis) inducing antibodies against TAB9 in all animals after one inoculation and a strong anamnestic response after booster injections. Furthermore 97% of the V3 peptides included were recognized by TAB9 sera. No differences between doses of 200 microg and 1 mg of TAB9 in M-ISA720 were observed after four immunizations. Neutralizing antibodies against five HIV-1 isolates were detected in most animals. Animals remain healthy throughout the study and did not show lesions at the inoculation site.

Effect of cation and anion of an electrolyte on apparent molar volume, isentropic compressibility and refractive index of glycine in aqueous solutions.

Experiments at 298.15 K have been performed to measure the density, velocity of sound and refractive index in three water+glycine+electrolyte systems. The electrolytes studied were KCl, KNO3 and NaNO3. The values of apparent molar volume and isentropic compressibility of glycine in aqueous electrolyte solutions were calculated from the measured data. The results obtained in this study and those reported previously for water+glycine+NaCl system have been comparatively studied. The results show that the nature of both the cation and the anion of an electrolyte influence the behaviour of glycine in aqueous solutions. For all four electrolytes studied, the comparison shows a positive volume transfer for glycine from an electrolyte solution to a more concentrated solution of the same electrolyte. The results also show a negative apparent isentropic compressibility for glycine in the presence of the electrolytes studied. These effects indicate that the volume of a glycine molecule is larger in solutions with higher electrolyte concentration and the water molecules around the glycine molecules are less compressible than the water molecules in the bulk solution. These effects were attributed to the doubly charged behaviour of glycine and to the formation of physically bonded ion-pairs between the charged groups of glycine and the cation and the anion of the electrolyte.

Rifampin reduces early mortality in experimental Streptococcus pneumoniae meningitis.

Compared with beta-lactam antibiotics, rifampin releases smaller quantities of proinflammatory cell wall products from Streptococcus pneumoniae in vitro. Mice infected intracerebrally with S. pneumoniae were treated subcutaneously with 2-mg doses of rifampin or ceftriaxone (n=43 each) every 12 h for 3 days and then observed for another 3 days. Rifampin reduced overall mortality from 49% to 26% (P=.04). Kaplan-Meyer analysis revealed a substantial reduction of mortality during the first 24 h in mice receiving rifampin (difference in survival time: P=.007). Eight h after receiving a single 2-mg dose of rifampin or ceftriaxone, rifampin-treated mice had lower serum and cerebrospinal fluid concentrations of lipoteichoic and teichoic acids than did ceftriaxone-treated mice (median serum level: <0.5 vs. 27.0 ng/mL, P=.02; median cerebrospinal fluid level of pooled specimens: 97.5 vs. 206.0 ng/mL). Thus, the use of rifampin appears promising for reducing the release of proinflammatory bacterial components and decreasing early mortality in bacterial meningitis.

Apoptosis of neurons in the dentate gyrus in humans suffering from bacterial meningitis.

Apoptosis of granular cells in the dentate gyrus frequently occurs in animal models of bacterial meningitis. In 37 autopsy cases of bacterial meningitis we evaluated, by light microscopy and in-situ tailing, whether this pattern of neuronal damage is of relevance in humans. Neuronal apoptoses in the dentate gyrus (density 1 to 19/mm2) were observed in 26 cases of bacterial meningitis and in none of 10 aged-matched control cases dying from non-neurological diseases. The density of apoptotic neurons depended on the interval between the onset of symptoms of meningitis and death (death within the first 2 days: 1.8+/-2.8 apoptoses/mm2; later than 18 days: 1.8+/-1.7/mm2; compared to death between days 3 and 18: 7.4+/-6.6 apoptoses/mm2, p = 0.007 and 0.004, respectively). Neuronal apoptosis in the dentate gyrus was not linked to neuronal damage in other parts of the brain or previous treatment with corticosteroids. Since learning deficits are frequently observed in survivors of bacterial meningitis, strategies to reduce the density of apoptotic neurons in the dentate gyrus may decrease the frequency of neurological sequela in patients surviving bacterial meningitis.

Heterologous expression of a plant small heat-shock protein enhances Escherichia coli viability under heat and cold stress.

A small heat-shock protein (sHSP) that shows molecular chaperone activity in vitro was recently purified from mature chestnut (Castanea sativa) cotyledons. This protein, renamed here as CsHSP17. 5, belongs to cytosolic class I, as revealed by cDNA sequencing and immunoelectron microscopy. Recombinant CsHSP17.5 was overexpressed in Escherichia coli to study its possible function under stress conditions. Upon transfer from 37 degrees C to 50 degrees C, a temperature known to cause cell autolysis, those cells that accumulated CsHSP17.5 showed improved viability compared with control cultures. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis analysis of cell lysates suggested that such a protective effect in vivo is due to the ability of recombinant sHSP to maintain soluble cytosolic proteins in their native conformation, with little substrate specificity. To test the recent hypothesis that sHSPs may be involved in protection against cold stress, we also studied the viability of recombinant cells at 4 degrees C. Unlike the major heat-induced chaperone, GroEL/ES, the chestnut sHSP significantly enhanced cell survivability at this temperature. CsHSP17.5 thus represents an example of a HSP capable of protecting cells against both thermal extremes. Consistent with these findings, high-level induction of homologous transcripts was observed in vegetative tissues of chestnut plantlets exposed to either type of thermal stress but not salt stress.

Experimental data and modelling of apparent molar volumes, isentropic compressibilities and refractive indices in aqueous solutions of glycine + NaCl.

Experiments have been performed at 298.15 K to measure the density, sound velocity and refractive index of glycine in aqueous solutions of NaCl over a wide range of both glycine and NaCl concentrations. The values of apparent molar volume and isentropic compressibility of glycine were calculated from the measured data. The results show a positive transfer volume of glycine from an NaCl solution to a more concentrated NaCl solution. This indicates that the size of a glycine molecule is larger in a solution with higher NaCl concentration. The negative values of apparent isentropic compressibility imply that the water molecules around the glycine molecules are less compressible than the water molecules in the bulk solution. These effects are attributed to the doubly charged behaviour of glycine and to the formation of physically bonded ion-pairs between the charged groups of glycine and sodium and chloride ions. The formation of ion-pairs, whose extents of binding reactions depend on the concentrations of both NaCl and glycine, alter the hydration number of glycine. This also explains the reason for the increase in the size of glycine with an increase in the NaCl concentration. A model based on the Pitzer formalism has been developed to correlate the activity coefficient, apparent molar volume and isentropic compressibility of glycine in aqueous solutions of NaCl. The results show that the model can accurately correlate the interactions in aqueous solutions of glycine and NaCl.

Effect of the cation and the anion of an electrolyte on the solubility of DL-aminobutyric acid in aqueous solutions: measurement and modelling.

The solubilities at 298.2 K of dl-aminobutyric acid in aqueous solutions of NaCl, KCl, NaNO(3) and KNO(3) were measured. The solubility of DL-aminobutyric acid was found to be influenced by the concentration and by the nature of both the cation and the anion of the electrolyte. Comparison of the results obtained in this study and those for other amino acids reported in the literature, indicates that the structure of the hydrocarbon backbone of an amino acid plays an important role in the interactions of an amino acid with an electrolyte. A thermodynamic model has been used to correlate the solubilities of DL-aminobutyric acid in aqueous electrolyte solutions. The activity coefficients of the amino acid in the electrolyte solutions, were represented by a model proposed by Khoshkbarchi and Vera [M.K. Khoshkbarchi, J.H. Vera, AIChE J. 42 (1996) 2354; M.K. Khoshkbarchi, J.H. Vera, Ind. Eng. Chem. Res. 35 (1996) 4755]. This model, which considers a combination of both long- and short-range interactions, contains only two adjustable parameters. All other parameters are available in the literature. The model can accurately correlate the solubility of dl-aminobutyric acid in aqueous solutions of electrolytes.

Cytokine release and mitogenic activity in the viridans streptococcal shock syndrome.

Viridans streptococci are a heterogeneous group of Gram-positive bacteria that are normal inhabitants of the mouth, upper gastrointestinal tract and oropharynx. These organisms are typically thought of as of low virulence, classically as the cause of infective endocarditis, although recently they have been implicated in serious infections in other settings. In particular, viridans group streptococci have been described as responsible for the alpha-streptococcal shock syndrome in neutropenic patients. The mechanism by which viridans streptococci cause bacteraemia associated with adult respiratory distress syndrome (ARDS) in these patients has not been elucidated. Using enzyme-linked immunosorbent assays, we compared the ability of cell-free bacterial supernatants derived from commensal and clinical strains of viridans streptococci to induce the pro-inflammatory cytokines tumour necrosis factor alpha (TNF-alpha), tumour necrosis factor beta (TNF-beta) and interleukin 8 (IL-8) from human peripheral blood mononuclear cells (PBMC) in vitro. Supernatants of clinical isolates induced significantly more TNF-beta (P < 0.002) and IL-8 (P < 0.001) than did supernatants from commensal strains. The increased production of IL-8 by the clinical strains may be of importance in view of the role of IL-8 in the pathogenesis of the acute respiratory distress syndrome (ARDS), one of the principal clinical features of the alpha-streptococcal shock syndrome.

Effect of particulation on the immunogenic and protective properties of the recombinant Bm86 antigen expressed in Pichia pastoris.

The recombinant Bm86 tick antigen expressed in Pichia pastoris is obtained in a highly particulated form, as a distinguish feature of this expression system. This particulated protein, the active principle of the recombinant vaccine Gavac against the cattle tick, have shown high immunogenic and protective properties, probably associated with its own characteristics. To evaluate the effects of particulation on the properties of Bm86, three groups of calves were immunized with particulated or non-particulated recombinant Bm86 and the anti-Bm86 antibody response determined. Animals were challenged with a controlled tick infestation and the protective capacities of both proteins assessed. Humoral immune response and protection in cattle vaccinated with the particulated antigen were higher. These experiments suggested that particulation of the Bm86 expressed in P. pastoris is an important feature for the protective properties of the antigen in vaccine preparations.

Susceptibility to new beta-lactams of enterobacterial extended-spectrum beta-lactamase (ESBL) producers and penicillin-resistant Streptococcus pneumoniae in Mexico.

The activities of several beta-lactam antimicrobial agents, aminoglycosides and ciprofloxacin, were determined against 62 clinical isolates of enterobacteria resistant to oxyimino cephalosporins (extended-spectrum beta-lactamase producers), collected during 1991 to 1993, and 16 penicillin-resistant invasive isolates of Streptococcus pneumoniae collected during 1994-1996. The numbers and percentages of susceptible enterobacterial strains to tested antibiotics were: imipenem 60 (97%), ciprofloxacin 57 (92%), cefepime 56 (90%), cefpirome 34 (55%), aztreonam 13 (21%), cefotaxime 7 (11%), ceftazidime 0 (0%), amikacin 11 (18%) and gentamicin 16 (26%). Despite the fact that these strains had never been exposed previously to cefepime or cefpirome, the susceptibility was 90% and 55%, respectively. No penicillin-resistant strains of S. pneumoniae were susceptible to cefotaxime, imipenem or cefepime. Only one strain was susceptible to ceftazidime and 4 (25%) were susceptible to cefpirome. Erythromycin showed the greatest activity with 12 (75%) susceptible strains.

Bioequivalence evaluation of 2 different oral formulations of ciprofloxacin in healthy volunteers.

A bioequivalence study of 2 oral formulations of 500 mg tablets of ciprofloxacin was carried out in 24 healthy volunteers according to a single dose, two-sequence, crossover randomized design. Blood samples were taken prior to each administration and at 13 points within 32 hours after the dose, and plasma concentrations of ciprofloxacin were determined by HPLC. The pharmacokinetic parameters Cmax and tmax were obtained directly from plasma data, ke was estimated by log-linear regression, and AUC was calculated by the trapezoidal rule. The pharmacokinetic parameters AUC and Cmax were tested for bioequivalence after log-transformation of data, differences of tmax were evaluated nonparametrically. The 90% standard confidence intervals of the mean values for the test/reference ratios were 0.87-0.97 for AUC and 0.91-1.05 for Cmax, within the bioequivalence acceptable range of 0.80-1.25 limits. So, we conclude that both formulations were found bioequivalent and, therefore, interchangeable.

Expression of small RNAs by Bacillus sp. strain PS3 and B. subtilis cells during sporulation.

A small RNA sequence identified in an rRNA-tRNA cluster from the thermophilic Bacillus sp. strain PS3 was examined. An oligonucleotide probe specific for the RNA bound to multiple restriction fragments in Bacillus sp. strain PS3 DNA, thus several copies of this sequence occur in its genome. Similar findings were observed using DNA from B. subtilis, B. stearothermophilus, Escherichia coli, Staphylococcus aureus, Haemophilus influenzae and Thermus thermophilus. This sequence apparently is widespread in the eubacteria. Northern analysis of RNA from sporulating Bacillus sp. strain PS3 and B. subtilis cells revealed RNA species homologous to the probe in both bacteria. Expression of the small RNA in B. subtilis depended on sigma H.

Large-scale production in Pichia pastoris of the recombinant vaccine Gavac against cattle tick.

A gene coding for the Bm86 tick protein was recently cloned, expressed in Pichia pastoris and shown to induce an inmunological response in cattle against ticks. Moreover, the Gavac vaccine (Heber Biotec S.A., Havana, Cuba), which contains this recombinant protein, has proved to control the Boophilus microplus populations under field conditions. This paper reviews the development and large-scale production of this vaccine, the efficacy of the resulting product and the strategy followed in designing its production plant. The production plant fulfills biosafety requirements and GMP.
