Effect of denervation on loss of ions in chick skeletal muscle.

Leaching behaviour of chick gastrocnemius muscle has been studied with respect to the loss of Na+, K+ and Ca2+ ions in a donor-solvent (muscle-water) system under normal as well as denervated conditions. An attempt has been made to explain the alterations in the rate of electrolytic loss in terms of membrane dysfunctions induced as a result of loss of neural control.

Endoscopic and histopathological study of duodenitis.

Fifty patients who presented with symptoms suggestive of disease related to peptic acidity having endoscopic evidence of duodenitis were analysed. Endoscopic duodenal biopsy was done in all these patients. Histologically duodenitis was labelled if there was infiltration of epithelial layer. Male to female ratio was found to be 3:2. Duration of symptoms in the patients ranged from less than one month to more than 5 years. Possible aetiological factors like drugs, alcohol and smoking were established in 26 patients (52%), drugs being the commonest (26%). Stool examination showed cyst or ova of parasites in 8 patients (16%). Endoscopically multiple hyperaemic patches were seen in 15 (30%), erosions in 19 (38%), coarse mucosal folds in 5 (10%), nodules in 2 (4%) and multiple lesions in 9 (18%) patients. Histological and endoscopical correlationship was established in 40 (80%) cases.

Jaundice in Plasmodium falciparum.

Thirty-two patients of smear positive Plasmodium falciparum malaria having jaundice were analysed retrospectively. Majority of the cases were in the age group of 31-40 years. Serum bilirubin levels ranged from 2 mg to 40 mg%. Fourteen (42.6%) had serum bilirubin above 10 mg%. Conjugated bilirubinaemia was found in twenty one patients (65.5%), unconjugated in 4 (12.5%) while 7 patients (21.8%) had a mixed pattern. Serum transaminases were high in 21.8% patients. Twenty five patients (78%) had associated azotaemia, 11(34.3%) had intravascular haemolysis and 3(9.3%) had possible cerebral malaria. Hepatosplenomegaly was seen in all the 32 patients. Mortality was 21% but none died of hepatic encephalopathy. Histologically the most consistent finding in liver biopsies was reticulo-endothelial cell hyperplasia. Pigmentation in kupffer cells, fatty change, sinusoidal and portal infiltration and cholestasis were the other features seen.

Changes in leaching properties of chick skeletal muscles on denervation.

The leaching of electrolytes in normal and denervated chick gastrocnemius muscle has been studied by recording electroconductivity changes in donor-solvent (muscle-water) system at 11 intervals leading to a total immersion of 5 hr 15 min, at days 1,5,10,21 and 28, post-hatching. The results show age related changes in the permeability properties of normal muscle membrane system. The loss of neural control induces a blocking of electrolytes 10 days post-denervation. The possible nature of blocking mechanism has been related to the non-availability of neuro-trophic factors in the tissue.

Hypolipidemic activity of trimethylamine-carbomethoxyborane and related derivatives in rodents.

A number of substituted amine-carboxyborane esters were examined for their hypolipidemic activity in rodents. Trimethylamine-carbomethoxyborane, Me3NBH2C(O)OMe, was observed to be the most effective hypolipidemic agent in the rodent at the low dose of 8 mg/kg/d. This agent effectively increased the high density lipoprotein (HDL) cholesterol content and lowered the low density lipoprotein (LDL) cholesterol and triglyceride contents of rat lipoproteins. Furthermore, the agent accelerated cholesterol excretion from the body of the rat and reduced the activity of the rate-limiting enzyme involved in the de novo synthesis of lipids in the liver. The agent has a safe therapeutic index, and appeared to have more promise as a therapeutic agent than other borane esters.

Gene expression: chemical synthesis of E. coli ribosome binding sites and their use in directing the expression of mammalian proteins in bacteria.

Mammalian genes, when inserted into bacterial plasmid or phage DNAs, will not be expressed into the corresponding specific proteins in E. coli unless proper initiations signals required for recognition by E. coli ribosomes are provided. We have studied these signals and chemically synthesized two DNA duplexes each containing different initiation signals. These have been inserted in front of the Simian virus 40 (SV40) small tumor antigen gene (SV40 t gene) at varying distances from the ATG initiation codon prior to its cloning into pBR322 plasmid DNA. Plasmid containing clones carrying either of these two synthetic ribosome binding sites (RBS) at varying distances from the SV40 t gene all produced a 17K protein identical to authentic t antigen by immunologic, electrophoretic and proteolytic digestion analyses. This provides a novel method to ensure the specific expression of any contiguous mammalian gene to be cloned to bacteria, and also a unique in vivo method for studying the structure-function (efficiency) relationship of RBS with specific base changes.

Observations on the vitamin D state of pregnant Asian women in London.

Vitamin D nutrition in pregnancy was investigated in 115 Asian women living in London, and in 50 of their newborn infants. Mean serum 25-hydroxy cholecalciferal (25-OHD) concentration at the beginning of the last trimester was 20.2 nmol/l, and fell to 16.0 nmol/1 post partum. Thirty-six per cent of the women post partum and 32 per cent of the infants had undetectable 25-OHD concentrations (less than 3 nmol/l). Such low values are commonly associated with osteomalacia and rickets. The bone isoenzyme of alkaline phosphatase was elevated in 20 per cent of the women post partum, and in 50 per cent of the infants, indicating the presence of sub-clinical bone disease. Five infants had symptomatic hypocalcaemia. Vitamin D deficiency was most likely to occur in Pakistanis, Hindu Indians and East African Asians, and in vegetarians. All British Asians should receive supplementary Vitamin D during pregnancy.

Vitamin D binding globulin levels and affinity in various clinical conditions.

The approximate association constants of the plasma vitamin D binding globulin (Gc-globulin) for 25-hydroxycholecalciferol (25(OH)D3) and the plasma 25(OH)D3 binding capacities were measured in samples from 123 patients with a variety of disorders. No gross differences in binding affinities were observed between different groups of patients and controls. Many patients, however, had moderately reduced, and several had grossly reduced, plasma binding capacities. The changes in Gc-globulin relative to some other proteins are also described in detail in three patients during the course of their illness. Gc-globulin concentration and hence plasma vitamin D binding capacity can undergo rapid and marked changes during illness.

Vitamin D binding globulin phenotypes in liver disease.

The binding properties towards vitamin D metabolites of plasma from individuals with the three common Gc-globulin phenotypes, Gc-1, Gc-2 and Gc-2-1, have been found to be identical. In patients with liver disease there is a good correlation between the levels of Gc-globulin andalbumin in plasma. In addition the Gc-globulin levels correlate well with the ability of plasma to bind 25-hydroxycholecalciferol. Patients with the Gc-2-1 phenotype showed a significantly smaller depression in plasma Gc-globulin than those with the Gc-2 and Gc-1 phenotypes. The relation of these findings to the pathogenesis of disorders of calcium metabolism in liver disease is discussed.

Acute dosage with dexrazoxane, but not doxorubicin, is associated with increased rates of hepatic protein synthesis in vivo.

An investigation was carried out into the effects of dexrazoxane and doxorubicin on hepatic protein synthesis in vivo. The protocol included 8 groups of rats and involved a pretreatment stage of 30 min followed by a treatment stage of either 2.5 or 24 h. Male Wistar rats (=0.15-0.20 kg) were pretreated with either dexrazoxane (100 mg/kg; 5 ml/kg) or saline (0.15 mol/l NaCl; 5 ml/kg). At 30 min after the pretreatment, rats were again injected with either doxorubicin (5 mg/kg; 10 ml/kg) or saline (0.15 mol/l NaCl; 10 ml/kg) in the treatment phase. Rats were sacrificed at either 2.5 or 24 h after the last doxorubicin or saline injection. Rate of protein synthesis were measured 10 min prior to sacrificing rats, with a flooding dose of L-[4-3H]phenylalanine. Liver was analyzed for the protein synthetic capacity (Cs, mg RNA/g protein), the fractional rate of protein synthesis (k(s), %/d), and the RNA activity (kRNA mg protein/d/mg RNA). Complementary analysis included plasma albumin, total protein and activities of alkaline phosphatase, and aspartate aminotransferase. In the 2.5-h study, doxorubicin alone had no effect on any of the above variables. Dexrazoxane alone increased Cs, k(s) and kRNA at 2.5 h. Combined dexrazoxane + doxorubicin increased hepatic Cs and k(s) with concomitant reductions in total plasma protein. In the 24-h study, doxorubicin alone had no effect on any of the variables. Dexrazoxane alone had no effect on either Cs, k(s), or kRNA but raised plasma activities of alkaline phosphatase and aspartate aminotransferase. Combined dexrazoxane + doxorubicin increased Cs and k(s) and decreased total plasma protein and increased plasma aspartate aminotransferase activities at 24 h. In conclusion, there is no evidence that acutely doxorubicin per se has measurable effects on hepatic protein synthesis in vivo in an acute period. However, acutely dexrazoxane increases hepatic protein synthesis, which may represent its putative cytotoxic effects, as indicated by raised serum activities of liver enzymes. A combination of both dexrazoxane + doxorubicin appears to have a greater effect in increasing liver protein synthesis than dexrazoxane alone.

Preparation and characterization of microencapsulated gelospheres for controlled oral theophylline delivery.

Microencapsulated gelospheres were prepared using the water swellable polymers, poly(vinyl alcohol) and polyacrylamide in which the drug was embedded. Polymeric coating was formed by interfacial polymerization using 1,6 hexamine and sebacoyl chloride. The size, shape, in vitro release, kinetics and in vivo efficiency of the formulation were determined. The shape was found to be spherical with a size range below 100 microm. The per cent drug loaded was found to be higher in the case of gelospheres prepared with polyacrylamide than those prepared with poly(vinyl alcohol). The release rate was found to be near zero order. The AUC was found to be higher in the case of polyacrylamide and polyvinyl alcohol gelospheres as compared to plain drug solution.

Effect of chronic morphine administration on glucose tolerance and insulin binding to isolated rat adipocytes.

Morphine addiction was induced in six male Wistar rats. Improved glucose tolerance (peak value less by 28%, p<0.01) was observed in chronically morphinized rats as compared to the control rats, injected with saline. An increase in the maximal specific binding of 125I-labeled insulin to unit membrane area of adipocytes was observed in the experimental group (p < 0.01). The changes in insulin receptor number could be responsible for the improved glucose tolerance observed during morphine addiction.

Ultrasound guided percutaneous nephrostomy for obstructive uropathy in benign and malignant diseases.

OBJECTIVE: Analyze the success rate, complications and overall benefit of ultrasound guided percutaneous nephrostomy (PCN) for the relief of obstructive uropathy in benign and malignant diseases. MATERIALS AND METHODS: PCN was performed in 50 kidneys of 32 patients. It was performed in emergency rooms totally under ultrasound guidance by general surgeons. Seldinger technique was used in all cases. Changes in renal function after the procedure were analyzed using paired t-test. RESULTS: The procedure was successfully completed in 42 out of 50 kidneys (84%). There has been no major complication and 28% minor complications. The renal function improved significantly when PCN was performed for benign conditions (mean creatinine 3.52 mg/dL before and 2.18 mg/dL after PCN), however in malignancy there has been no significant improvement in renal function (before PCN mean creatinine 6.39 mg/dL and after PCN 5.41 mg/dL). CONCLUSION: We conclude that PCN can be effectively performed under ultrasound guidance and should be the initial procedure in acutely obstructed kidneys with pyonephrosis and poor renal function. In malignant cases, however, improvement in renal function is possible only if the procedure is carried out at an early stage.

Role of azathioprine in severe ulcerative colitis: one-year, placebo-controlled, randomized trial.

OBJECTIVE: To investigate the efficacy of azathioprine in treating patients with severe ulcerative colitis. DESIGN: One-year, randomized, placebo-controlled trial. SUBJECTS: 83 patients with severe ulcerative colitis were enrolled. Fifty patients who relapsed within two months on corticosteroid withdrawal were randomized into two groups. The azathioprine group received oral sulfasalazine (6-8 g/day), oral prednisolone (1 mg/Kg/day) and oral azathioprine (2 mg/Kg/day). The placebo group received oral sulfasalazine (6-8 g/day), oral prednisolone (1 mg/Kg/day) and placebo. Corticosteroids were tapered over 12-16 weeks. RESULTS: Five patients (2 in azathioprine group, 3 in placebo group) dropped out of the study. Three patients in the azathioprine group had side effects. The number of patients going into complete remission and partial remission was not significantly different in the two groups. The proportion of relapses in the azathioprine group was lower than in the placebo group (p < 0.05). CONCLUSIONS: In patients with ulcerative colitis, azathioprine had no effect in achieving remission, when given in combination with prednisolone; however, it lowers the proportion of relapses. Side effects like pancreatitis and hepatitis are mild and respond promptly to drug withdrawal.

Hereditary pancreatitis.

Hereditary pancreatitis is a rare cause of chronic pancreatitis and has been mainly reported in western literature. It is inherited in an autosomal dominant manner. In this report, we present our data on two affected members in a family from Punjab, North India.
