Role of fibronectin in diagnosis of malignant ascites.

The aim of the study was to evaluate the role of ascitic fluid fibronectin in the diagnosis of malignant ascites. Fibronectin is a glycoprotein which plays an important role in regulating the organisation of the cell cytoskeleton and cell morphology. Ascitic fluid samples from 35 patients, 20 with portal hypertension (Group-I) and 15 with malignant ascites (Group-II) were analysed for proteins, cell counts, fibronectin levels and malignant cell cytology. Mean ascitic fluid fibronectin level was found to be significantly higher in malignant ascites as compared to portal hypertension (p < 0.001). At a cut off value of 94.67 microg/ml, the sensitivity, specificity, positive accuracy, negative accuracy and overall diagnostic accuracy was found to be 100%, 95%, 93.8%, 100% and 97.1% respectively.

Some approaches to improve bioavailability of peptides and proteins through oral and other mucosal routes.

In this article, an attempt is made to review methods of protein and peptidal delivery through oral and non-oral mucosal routes. Strategies that have been or are being employed to improve the bioavailability of peptides and proteins include alteration of physiological factors by absorption enhancers and enzyme inhibitors, modification of native proteins by chemical synthesis of prodrugs and analogues and dosage form modifications, e.g. lipid vesicles and emulsions, particulate carriers or mucoadhesive polymer systems.

p126 (CDw101), a costimulatory molecule preferentially expressed on mucosal T lymphocytes.

Intestinal mucosal lymphocytes are defined by their anatomic location within the epithelium (intraepithelial lymphocytes), the interstitium between the epithelial basement membrane and the underlying muscularis mucosa (lamina propria lymphocytes), or in organized lymphoid tissues (Peyer's patches). Although intestinal intraepithelial lymphocytes have a distinct localization, their function has not been determined. To define cell surface proteins that are involved in intestinal intraepithelial lymphocyte localization or function, cultured human mucosal lymphocytes were used as immunogens to develop mAbs that react predominantly with this cell population in an immunohistochemical screening assay. Three mAbs were selected that subsequently were found by biochemical analysis to identify a 200-kDa homodimeric polypeptide on 88 to 98% of CD3+ mucosal lymphocytes but only 18 +/- 13% of PBLs. Expression on granulocytes and monocytes was also observed. This polypeptide has been termed p126 based on its SDS-PAGE-determined M(r) under reducing conditions. Cleveland digest maps demonstrated similarity between the p126 and CDw101 polypeptides. Determined amino acid sequence analysis of the purified p126 polypeptide revealed that it is the protein product of the recently identified V7 gene, which has structural similarities to members of the Ig gene superfamily. Two of the anti-p126 mAbs were costimulatory with suboptimal concentrations of anti-CD3 mAb inducing proliferation of cultured intestinal intraepithelial lymphocytes. Thus, we conclude that p126 is CDw101 encoded by a gene that predicts a seven-Ig domain chain-like structure. It has restricted expression predominantly on mucosal T lymphocytes and appears to have a costimulatory function of special relevance for CD28- T cells and for mucosal lymphocytes.

An abundance of p53 null mutations in ovarian carcinoma.

Ovarian cancers from 64 midwestern US women were screened for p53 dysfunction both by immunohistochemical staining (IHCS) and single strand conformation polymorphism (SSCP) analysis of the entire open reading frame (ORF). Forty SSCP abnormalities in 39 tumors included nine deletion, one insertion, two splice junction, two nonsense, one silent and 25 missense mutations were confirmed by direct genomic sequencing. Eight of the insertion/deletion defects may have occurred due to slippage during the course of DNA replication. This observation suggests that genomic instability may play an important role in ovarian carcinogenesis. Fifteen percent of the mutations encountered were located outside exons 5-9 and four of these were null. The sensitivity of IHCS was 96% for missense mutations but only 14% for null mutations. This contrasted with 100% sensitivity of the SSCP screening methodology. The 21% overall incidence of null mutations in the present study far exceeds the reported 6.8% incidence in the world literature (P=0.0003). Explanations for this difference include: (1) our complete analysis of the entire ORF of the p53 gene; (2) the tendency of others to rely upon IHCS to screen tumors prior to mutation analysis; and (3) environmental or endogenous genetic influences.

Tissue-specific regulation of malic enzyme by thyroid hormone in the neonatal rat.

Two recent studies have claimed that thyroid hormone administration accelerates malic enzyme gene expression in the neonatal brain in contrast to the well-documented lack of effect of triiodothyronine on malic enzyme gene expression in the adult brain. Since these observations conflict with earlier observations in our laboratory, we reinvestigated the effect of thyroid hormone status on the ontogeny of malic enzyme gene expression in the neonatal rat. Neither hypothyroidism nor hyperthyroidism influenced the ontogenesis of malic enzyme activity in neonatal brain whereas the patterns of gene expression and enzyme activity in liver were markedly affected. Our results suggest that tissue-specific factors in brain block thyroid hormone-induced gene expression by thyroid hormone.

Epithelial ovarian carcinoma metastatic to the central nervous system: a report on two cases with review of literature.

Metastasis to the central nervous system (CNS) from ovarian malignancy is rare. We describe two such patients who developed CNS metastasis in the form of multiple deposits in brain parenchyma after an interval of 27 and 16 months from the diagnosis of ovarian carcinoma. Both patients received cranial radiotherapy but survived only 2 weeks and 5 months, respectively. Data on 99 patients collected from the literature (Medline, National Library of Medicine, Bethesda, MD) are reviewed and an attempt is made to suggest therapeutic guidelines depending on the extent of the intracranial and extracranial disease and performance status of the patients.

Role of botulinum toxin in treatment of achalasia cardia.

Pneumatic dilatation and surgery are equally effective in the treatment of achalasia cardia but carry some risk of complications. We report our initial experience with the use of botulinum toxin in the treatment of achalasia. Six of seven patients treated responded, with improvement lasting till the end of follow-up at 3 months. No adverse effect was observed.

Primary tuberculosis of esophagus.

Primary tuberculosis of the esophagus is rare. We report a young lady with odynophagia who had esophageal tuberculosis without evidence of the disease elsewhere in the body.

Myosin light chains in regenerating graft in chick gastrocnemii.

Myosin light chains in normal and minced autografted chick gastrocnemii have been studied by disc gel electrophoresis during 70 days postgrafting period. As a result of induced mechanical trauma, the quantitative relationships between myosin light chain fractions are disturbed affecting the muscle functions.

Giant cavernous hemangiomas of liver mimicking metastasis.

Most hepatic hemangiomas are small and symptomless. These are now being increasingly diagnosed with the greater use of scanning procedures. Hemangiomas can occasionally grow to a large size and become manifest to the patient and the clinician. Giant hemangiomas can produce symptoms including awareness of abdominal mass, pain due to thrombosis, and very rarely, rupture. Though ultrasound is known to be quite suggestive of the diagnosis, large hemangiomas may be mistaken for liver metastases due to their enormous size and variegated picture on the scanning procedure. Dynamic CT scan and at times MRI may be required for confirmation of the diagnosis. Needle biopsy is contraindicated if the diagnosis is suspected.

N7-cyanoborane-2'-deoxyguanosine 5'-triphosphate is a good substrate for DNA polymerase.

The 5'-triphosphate of the boronated nucleoside analog N7-cyanoborane-2'-deoxyguanosine (7bdGTP) was synthesized, and a series of experiments was initiated to assess the potential of the compound to serve as a substrate for DNA polymerases. We show here that 7bdGTP can be incorporated into DNA by Sequenase. The resulting hemiboronated extension products are resistant to cleavage by treatment with either DMS and heat or a number of restriction enzymes. Further, in the polymerase chain reaction, 7bdGTP can be utilized as a substrate for Taq polymerase. Finally, by kinetic analysis, we have found that 7bdGTP is a more efficient substrate for exonuclease-free Klenow than normal dGTP. Thus, the introduction of a cyanoborane moiety to the N7 position of dGTP results in a nucleotide that is accepted in lieu of normal dGTP by a number of DNA polymerases.

Anti-inflammatory activity of amine-carboxyboranes in rodents.

Amine-carboxyboranes are potent anti-inflammatory agents reducing induced edema and pleural effusion at 8 mg/Kg, i.p. They protect against LPS (Salmonella) induced septic shock from 2-8 mg/Kg/day and are effective in blocking pain mediated both locally and centrally. The mode of action of these agents is by blocking release of cytokines from macrophages, thus reducing lysosomal hydrolytic and proteolytic enzyme activities of affected cells. The agents also reduce prostaglandin and leukotriene synthesis by blocking the activities of regulatory enzymes of the respective pathways.

The disposition, tissue distribution, and cellular transport of N-[[(trimethylamino)boryl]carbonyl]-L-phenylalanine methyl ester in CF1 mice.

The disposition and tissue distribution of 14C-labeled N-[[(trimethylamino)boryl]carbonyl]-L-phenylalanine methyl esther (1) determined in CF1 mice following i.v., p.o., and i.p. administration. Compound 1 was found to undergo rapid and extensive metabolism, and the majority of the radioactivity was found in the skin and carcass regardless of the route of administration. Approximately 55% of the radioactivity was recovered in urine and feces after 78 h; however, excretion via these routes was not complete. Degradation of compound 1 occurred at the amide bond at low pH (0.8) but at the ester bond a physiological pH. This difference in degradation was reflected in elevation of the blood radioactivity levels after p.o. administration compared to i.p. and i.v. administration.

The effect of nicotine on in vitro sperm motion characteristics.

UNLABELLED: PURPOSE AND STUDY PLAN: Men who are habitual smokers tend to have poor semen quality. We studied the effect of nicotine on sperm motility in vitro. Spermatozoa from 13 normal fertile nonsmoking donors, washed free of seminal plasma, were treated with medium alone (control); and, 10 mM, 5 mM, 1 mM and 0.1 mM nicotine (concentrations estimated to approximate residual concentrations of nicotine in the testes of heavy smokers). Computerized sperm motion analysis (CASA) was done at 2, 4, 6 and 24 h after incubation. RESULTS: Sperm motility, beat/cross frequency, linearity and maximum anterior lateral head displacement (ALH max.) were significantly decreased across nominal dosages (P < 0.001 by repeated measures analysis of variance). Of the concentrations tested, 0.1 mM had no effect; 1 mM significantly decreased sperm motility (P = 0.003) and linearity (P = 0.02); 5 mM decreased the beat frequency (P = 0.001) and linearity (0.02); and 10 mM markedly decreased motility (P = 0.0001), beat frequency (P = 0.0002), linearity (P = 0.02) and ALH max. (P = 0.02). The interactions between dose and time were insignificant. CONCLUSION: Nicotine at concentrations of > or = 1 mM significantly decreased sperm motion characteristics after different periods of incubation, whereas 0.1 mM concentration had the least effect.
