Evaluation of left atrial thrombus with computed tomography.

Left atrial thrombi were evaluated with computed tomography in 23 patients with mitral valvular diseases. In three of the patients, left atrial thrombi were delineated with computed tomography and were confirmed by cardiac surgery or autopsy. The minimum size of the thrombi detected tomographically was 3.5 gm. There were no false-positive or false-negative results with computed tomography in 13 patients who subsequently underwent cardiac surgery. Computed tomography is essentially noninvasive and appears to be one of the best methods to evaluate left atrial thrombus.

Septicemia during compromised host treated in a protected isolation unit.

Bacteriological, hematological and immunological data were evaluated in patients with septicemia in a protected isolation unit. Patients were randomly given antimicrobial prophylaxis. Three out of four episodes of septicemia occurred 2 to 4 days after discontinuing oral nonabsorbed antibiotics at the stage of leukopenia and immunodeficiency induced by chemotherapy. Death occurred within 10 days in two patients whose peripheral white blood counts did not recover. In the other patients sepricemia was suppressed and complete remission was achieved in one of them. Parentheral administration of antibiotics is necessary after discontinuing oral prophylactic antibiotics in a protected isolation unit.

Clinical application of cardiovascular computed tomography.

The application of computed tomography to cardiovascular diagnosis was attempted. One hundred and four consecutive patients with various cardiovascular diseases were studied with a third-generation computed tomographic three-second whole body scanner. Identification and evaluation of abnormalities of the great vessels, atria, ventricles and interventricular septum were possible by cardiac computed tomography. Sizes and locations of intracardiac thrombi were accurately assessed by computed tomography. Computed tomography was valuable for the evaluation of pericardial effusion because distribution of the effusion as well as the gross nature of the fluid could be estimated by the CT number.

A case of persistent atrial standstill.

A case of persistent atrial standstill is described. The criteria for diagnosing atrial standstill are separated into two categories: the lack of electrical atrial activity and the lack of mechanical atrial activity. Echocardiography of the anterior mitral leaflet is to be used as one of the noninvasive methods for the diagnosis of the lack of mechanical atrial activity.
