5-FU infusion in advanced colorectal cancer: a comparison of three dose schedules.

Ninety-four patients with advanced colorectal adenocarcinoma were treated by continuous iv 5-FU infusion on three different dose schedules. Thirty-three patients received a 72-hour infusion of 5-FU (30 mg/kg/24 hours) every 3 weeks (Group A); 31 received a 72-hour infusion of 5-FU (30 mg/kg/24 hours) every 2 weeks (Group B); and 30 received a 48-hour infusion of 5-FU (30 mg/kg/24 hours) every week (Group C). Although this was a sequential nonrandomized study of the dose schedules, the groups were comparable with respect to various prognostic factors. Response rates were as follows: Group A--three patients had minor response (9%) and 30 had no response (91%); Group B--five patients achieved partial response (16%), nine had minor response (29%), and 17 had no response (55%), and Group C--one patient achieved complete response (3%), eight achieved partial response (27%), five had minor response (17%), and 16 had no response (53%). The median survival time for Group A was 9 months, for Group B was 9.5 months, and for Group C was 14 months. Intensifying the dose schedule of 5-FU by increasing the frequency of administration has significantly improved response rates. A prolongation of the median survival time of patients treated with a 48-hour infusion at 1-week intervals was noted, although this was not statistically significant.

The management of Graves' disease by Indian thyroidologists.

BACKGROUND: Although effective modes for treating Graves' disease are available, there is controversy over their choice. We conducted a nation-wide survey to determine trends in the diagnosis and management of Graves' disease in India. METHODS: A proforma was mailed to 45 practising thyroidologists who were members of the Endocrine Society of India or the Thyroid Association of India. One index case and 5 variations were provided, and the members were asked to indicate the investigations of choice, therapeutic modality and details about its implementation. RESULTS: The overall response rate was 71% (32). For diagnosis, serum T3, T4, TSH, sensitive TSH, free T3, and free T4 were asked for by 24, 25, 6, 13, 5, and 10 responders respectively. Radioactive iodine uptake with or without a scan was obtained by 19 of the responders. Antithyroid drugs were the choice of the majority in most situations (index case: 40-year-old female with first episode of typical Graves' disease--23; 40-year-old male--20; 16-year-old male--32; 40-year-old female without goitre--25), except for the patient with relapse and the 65-year-old female where radioiodine was the choice of the majority (20 and 23 respectively). CONCLUSION: Antithyroid drugs are the mainstay of treatment of Graves' disease in India. The predominant use of these drugs was similar to European data, but different from the practice in the USA, where radioiodine is used much more liberally.

Extraovarian endocrine abnormalities in north Indian women with premature ovarian failure.

BACKGROUND: Premature ovarian failure is a rare syndrome characterized by cessation of menstruation before 35 years of age associated with an elevated gonadotropin level (serum follicle stimulating hormone > 40 IU/L) and oestrogen deficiency. Premature ovarian failure is often the result of an autoimmune process and involvement of other endocrine glands has been reported but available studies are hampered by a lack of uniformity in diagnostic criteria. METHODS: The endocrine profile of 37 north Indian women with premature ovarian failure was studied. The investigations included measurement of serum follicle stimulating hormone, luteinizing hormone, oestradiol, prolactin, total thyroxine, total triiodothyronine and thyroid stimulating hormone. To assess adrenocortical reserve, adrenocorticotropic hormone stimulated plasma cortisol levels were estimated. RESULTS: Eighteen (49%) patients had extraovarian endocrine abnormalities. Eight (22%) had abnormal thyroid function tests and 12 of 29 (41%) patients tested for adrenocortical reserve showed impaired response of plasma cortisol to adrenocorticotropic hormone stimulation. CONCLUSION: Extraovarian endocrine (especially adrenocortical) involvement is frequent in patients with premature ovarian failure. Because none of the patients had overt clinical evidence of thyroid or adrenocortical disease, we suggest that tests for thyroid and adrenal function be performed routinely in females presenting with premature ovarian failure.

A double-blind, placebo-controlled trial of sustained-release diltiazem in patients with angina. Sustained-Release Diltiazem Study Group.

The efficacy of a new sustained-release formulation of diltiazem was examined in a placebo-controlled, double-blind trial in patients with stable angina. Doses of 60 mg BID, 90 mg BID, 180 mg BID and 240 mg BID were compared with placebo in 208 patients at 3 hours and 12 hours after dosing. Diltiazem, in doses of 90 mg BID and 180 mg BID, was statistically superior to placebo with respect to increasing total exercise time. Treadmill exercise time at 3 hours postdose increased with placebo from 356.1 +/- 118.7 sec to 375.7 +/- 119.8 sec (NS); with 90 mg BID, 382.7 +/- 111.8 sec to 445.4 +/- 117.5 sec (P < 0.005); and with 180 mg BID, 386.8 +/- 145.9 sec to 467.2 +/- 166.2 sec (P < 0.0001). At 12 hours postdose, treadmill exercise time with placebo increased from 357.6 +/- 128.3 sec to 383.8 +/- 128.7 sec (NS); with 90 mg BID, 395.2 +/- 119.4 sec to 449.7 +/- 123.1 sec (P = 0.053); with 180 mg BID, 395.3 +/- 141.4 sec to 476.6 +/- 165.6 sec (P < 0.0001). Time to onset of angina was also increased by the 180-mg-BID dose both at 3 hours postdose (257.3 +/- 126.8 sec to 354.3 +/- 158.7 sec; P < 0.0001) and at 12 hours postdose (274.7 +/- 131.2 sec to 377.4 +/- 186.2 sec; P < 0.0001). Sustained-release diltiazem is effective and safe in treating patients with chronic stable angina.

Biological activity of 6,12-dihydro-1-benzopyrano[3,4-b][1,4]benzothiazin-6-ones.

Ten 6,12-dihydro-1-benzopyrano[3,4-b][1,4]benzothiazin-6-ones (Coumarins)[1-7] and related coumarins [8-10] were compared for their cytotoxic activity and radical intensity. Among these compounds, compound 7 showed highest cytotoxic activity against human promyelocytic leukemic HL-60 cells. Compound 7 produced radicals under alkaline conditions, and showed the lowest pi-spin density at S-atom of the molecule, suggesting the delocalization of pi-spin density. These data suggest the possible relation between radical intensity and biological activity.

6,12-dihydro-1-benzopyrano[3,4-b][1,4]benzothiazin-6-ones: synthesis and mdr reversal in tumor cells.

Six 6,12-dihydro-1-benzopyrano[3,4-b][1,4]benzothiazin-6-ones and three coumarins were systematically investigated for reversal of multidrug resistance of bacteria and cancer cells in model experiments. 7-Methylcoumarin was able to eliminate the E. coli plasmid significantly; however, the other derivatives were ineffective. Four of 6,12-dihydro-1-benzopyrano[3,4-b] [1,4]benzothiazin-6-ones had a moderate effect on the multidrug resistance efflux pump of mouse lymphoma cells in vitro. Despite of the similarity of resistance mechanisms of bacteria and tumor cells, the reversal of drug resistance in bacteria and in cancer cells is not uniform because the structure-activity requirements are apparently different.

Allergic bronchopulmonary aspergillosis.

Allergic bronchopulmonary aspergillosis (ABPA) is an immunologically mediated lung disease which occurs predominantly in patients with asthma, and is caused by hypersensitivity to colonized Aspergillus fumigatus. It is a chronic, relapsing disorder which can clinically range from mild asthma to fibrotic lung disease. The immunopathogenesis of the disease is not clearly understood. Early diagnosis and aggressive therapy with oral corticosteroids can prevent the development of fibrotic lung disease and are therefore, the cornerstone of management.
