Direct screening and confirmation of priority volatile organic pollutants in drinking water.

A screening tool was proposed for the rapid detection of eight priority volatile organic pollutants according to European standards in drinking water. The method is based on the direct coupling of a headspace sampler with a mass spectrometer, using a chromatographic column heated to 175 degrees C as an interface. The water sample was subjected to the headspace extraction process and the volatile fraction was introduced directly into the mass spectrometer, without prior chromatographic separation, achieving low detection limits (0.6-1.2 ng/ml) for all compounds. The mass spectrum resulting from the simultaneous ionization and fragmentation of the mixture of molecules constitutes the volatile profile of each sample. An appropriate chemometric treatment of these signals permitted them to be classified, on the basis of their volatile composition, as contaminated or uncontaminated with respect to the legally established concentration levels for these compounds in drinking water, and providing no false negatives. A conventional confirmation method was carried out to analyze positive water samples by using the same instrumental setup as in the screening method, but using an appropriate temperature program in the chromatographic column to separate, identify and quantify each analyte.

Sensitive headspace gas chromatography-mass spectrometry determination of trihalomethanes in urine.

A sensitive and straightforward method for the determination of trihalomethanes (THMs) in urine by using headspace extraction technique has been developed. Chemical and instrumental variables were studied in order to optimize the method for sensitivity: an excess of KCl (4 g per 12 ml of urine), an oven temperature of 85 degrees C and an equilibration time of 30 min were selected. The use of the mass spectrometer in selected ion monitoring mode allows achieving linear ranges between 10 and 5000 ng/l and detection limits from 3 to 10 ng/l, for 12 ml of urine. The stability of the urine sample during storage at 4 and -20 degrees C was also evaluated: THMs remained stable for up to 2 days and 2 months, respectively. Finally, the method was successfully applied to study the THM uptake from swimmers of an indoor swimming pool, as well as non-swimmers. This study revealed that the concentrations of THMs in urine increased approximately three times for chloroform and bromodichloromethane after swimming activity. In addition, THMs in unchanged form were mainly excreted within 2-3h after the end of exposure.

Sorption study of 25 volatile organic compounds in several Mediterranean soils using headspace-gas chromatography-mass spectrometry.

A sorption study of 25 volatile organic compounds (VOCs) in different agricultural soils was carried out by using headspace-gas chromatography-mass spectrometry. The extraction of the VOCs from soil samples was carried out following the EPA method with some differences such as addition of potassium chloride and different instrumental conditions which provide higher sample throughput. In addition, a complementary study on several procedures for soil fortification with VOCs was also assayed, fortification with minimal sample handling was selected in order to minimise evaporation losses of the VOCs. The effect of clay minerals (7.0-69.7%) and organic carbon (0.2-3.5%) contents on acid and alkaline (pH 5.3-8.8) soils were evaluated. Based on the results, all compounds assayed were more sorbed in alkaline soils than acid ones; chlorobenzenes interact more strongly with agricultural soils than do alkylbenzenes. The organic carbon content affects the sorption of 25 VOCs in alkaline soils (the highest sorption was found for the most organic soil), while in acid soils VOC sorption increases as the organic carbon content decreases. The clay mineral fraction plays an important role in the sorption of VOCs in acid soil owing to pi-/n-electron interactions, this effect being more marked for chlorobenzenes.

[Giant adrenal myelolipoma: hypertension, renal failure and spontaneous rupture].

Adrenal myelolipoma is a rare, benign, slow-growing tumor composed of adipose tissue and hematopoietic elements. It is usually diagnosed incidentally, although there are reports of patients with symptoms and descriptions of retroperitoneal hemorrhage due to rupture of large tumors. The condition has been associated with obesity, high blood pressure and adrenal dysfunction. We present a patient with retroperitoneal hemorrhage due to spontaneous rupture of a myelolipoma, hypertension, and renal failure secondary to nephroangiosclerosis.

Continuous microwave-assisted extraction coupled on-line with liquid-liquid extraction: determination of aliphatic hydrocarbons in soil and sediments.

This paper describes a new extraction method for the determination of aliphatic hydrocarbons (AHs) in soil and sediment samples, using continuous microwave-assisted extraction (MAE) combined with liquid-liquid extraction, for clean-up purposes. Analytical determinations were carried out by gas chromatography coupled with impact ionization mass spectrometry. The influence of the experimental conditions was tested using an agricultural soil spiked with standards (stored at 4 degrees C for 1 month) as reference soil. Maximum extraction efficiencies (80-90%) were achieved using 0.1-1.0g of sample, 60microl of water and 3ml of n-hexane (extractant) and 5min of extraction time; less than 70% of the most volatile hydrocarbons (C(9)-C(12)) were recovered since many evaporated during the drying step of the sample. MAE was compared with a conventional extraction method such as Soxhlet and a good agreement in the results was obtained (average recovery percentage value of 105% by comparing MAE against Soxhlet). Quality parameters such as linear range (0.5-800microg/g), limits of detection (LODs) (0.1-0.2microg/g) and precision (RSD, 4-6%) were determined using spiked soil samples. This method was successfully applied to the analysis of aliphatic hydrocarbons (C(9)-C(27) including pristane and phytane) in contaminated real samples.

Monitoring the Prestige oil spill impacts on some key species of the Northern Iberian shelf.

Selected key components of the continental shelf benthic and demersal communities were monitored for the two years following the Prestige oil spill (POS) in order to identify the possible ecological effects of the oil. This work includes the first results regarding changes in abundance, distribution and food habits of hake (Merluccius merluccius), four-spot megrim (Lepidorhombus boscii), Norway lobster (Nephrops norvegicus) and Pandalid shrimp (Plesionika heterocarpus) populations of Galician and Cantabrian Sea shelves following the POS. Significant reductions in the abundance of Norway lobster, Plesionika heterocarpus and four-spot megrim were detected in the POS maximum impact area, located over the Galician shelf. Noteworthy recoveries were observed in the 2004 abundance indices of four-spot megrim and Plesionika. On the other hand, no significant effects were detected in the abundance or distribution of hake juveniles even though the tar aggregates were bound by the same oceanographic drift events as the hake recruits were during the winter of 2003 (Navidad current) in different water column layers of the Cantabrian Sea. Feeding patterns of the four species analysed did not present apparent modifications that can be related to the POS.

Effects of cinnarizine, a calcium antagonist that produces human parkinsonism, in parkin knock out mice.

Cinnarizine, a calcium antagonist that produces parkinsonism in humans, induces behavioural changes such as alopecia, buco-lingual dyskinesia and reduction of motor activity in female parkin knock out (PK-KO) mice but not in wild-type (WT) controls. PK-KO mice have high striatal dopamine levels and increased dopamine metabolism in spite of low reduced tyrosine hydroxylase protein. Cinnarizine, which blocks dopamine receptors and increases dopamine release, further increased dopamine metabolism. PK-KO mice increased GSH levels as a compensatory mechanism against enhanced free radical production related to acceleration of dopamine turnover. Neuronal markers, such as beta-tubulin slightly increased in PK-KO and furthermore with cinnarizine. Astroglial markers were decreased in PK-KO mice, and this effect was potentiated by cinnarizine, suggesting abnormal glia in these animals. Microglia was hyperactivated in PK-KO midbrain, suggesting inflammation in these animals. Proapoptotic proteins were increased by cinnarizine and, to a lesser extent, in PK-KO mice. Our data indicate that mutation of parkin is a risk factor for drug-induced parkinsonism.

Dendritic cell immunizations alone or combined with low doses of interleukin-2 induce specific immune responses in melanoma patients.

Dendritic cell (DC)-based therapy has proved to be effective in patients with a variety of malignancies. However, an optimal immunization protocol using DCs and the best means for delivering antigens has not yet been described. In this study, 20 patients with malignant melanoma in stages III or IV were vaccinated with autologous DCs pulsed with a melanoma cell lysate, alone (n = 13) or in combination with low doses of subcutaneous (s.c.) interleukin (IL)-2 injections (n = 7), to assess toxicity, immunological and clinical responses. Monocyte-derived DCs were morphological, phenotypic and functionally characterized in vitro. Peripheral blood mononuclear cells (PBMC), harvested from patients either prior to and after the treatment, were analysed using enzyme-linked immunosorbent spot (ELISPOT). After vaccination, 50% of the patients tested (seven of 13) from the first group and (three of seven) from the second, showed an increase in interferon (IFN)-gamma production in response to allogeneic melanoma cell lines but not to controls. Four of five tested human leucocyte antigen (HLA)-A2(+) patients with anti-melanoma activity also showed specific T cell responses against peptides derived from melanoma-associated antigens. Delayed type IV hypersensitivity reaction (DTH) against melanoma cell lysate was observed in six of 13 patients from the group treated with DC vaccines only and four of seven from the group treated with the combination of DCs and IL-2. Significant correlations were found between DTH-positive responses against tumour lysate and both disease stability and post-vaccination survival on the stage IV patients. There were no toxicities associated with the vaccines or evidence of autoimmunity including vitiligo. Furthermore, no significant enhancement was observed as a result of combining DC vaccination with IL-2. Our data suggest that autologous DCs pulsed with tumour lysate may provide a standardized and widely applicable source of melanoma specific antigens for clinical use. It is safe and causes no significant side effects and has been demonstrated to be partially efficient at triggering effective anti-melanoma immunity.

[Left renal vein hypertension syndrome].

INTRODUCTION: [corrected] Left renal vein compression, causing retrograde hypertension, determines a syndrome characterized by the presence of hematuria, gonadal vein dilatation and varicocele. Being a rare cause of hematuria its aetiology is diverse but of precise characteristics. Diagnosis is not easy and treatment requires to rule out its precise aetiology and consider the intensity of the compression phenomenon because of interventionist attitudes have important implications and are not risk free. MATERIAL AND METHODS: We report nine children, aged 8-15 years, presenting with hematuria of urologic characteristics. Diagnosis of left renal vein hypertension or nutcracker phenomenon was established through ultrasound (regular and doppler-ultrasound) and helical-TC performed in all, and left renal venography performed in six of them. RESULTS: Retroaortic renal vein was present in two cases, a varicose accessorial renal vein due to anomalous drainage in one and compression at the aortomesenteric space in six. Because of the symptoms severity therapeutic attitude was interventionist in two cases, performing an auto-transplantation in one, and endovascular stent placement al the level of the left renal vein compression in other. CONCLUSIONS: Diagnosis of this syndrome is not always easy because less invasive explorations usually show parameters that are difficult to distinguish from normality. Severity of clinical manifestations and therapeutic attitude are related with the presence of renal vein hyperpressure and collateral varicose circulation indicating the lack of compensation of a difficult drainage.

Direct screening and confirmation of benzene, toluene, ethylbenzene and xylenes in water.

A novel, straightforward sample screening method for benzene, toluene, ethylbenzene and xylene isomers (BTEX) in water is proposed. The method is based on the direct coupling of a headspace (HS) sampler with a mass spectrometer by using a chromatographic column heated to 200 degrees C as an interface. Samples are acidified and subjected to the headspace generation process, the resulting volatile fraction being directly introduced into the source of the mass spectrometer. The large number of samples to be analyzed and the wide range of m/z ratios scanned (75-110) suggest the use of chemometric approaches based on pattern recognition techniques (PRT). For sample classification purposes, the detection limit of the method (overall response 4.0 ng/ml BTEX) was selected as the cut-off level. The method proved highly reliable as no false negatives were obtained at the legally established concentration levels. Positive water samples were confirmed by using the same instrumental setup as in the screening method, but by heating the chromatographic column at 40-200 degrees C to separate the analytes.

Efficacy of directly observed treatment of HIV infection: experience in AIDS welfare homes.

With the objective of analyzing the efficacy of directly observed treatment (DOT) of HIV infection in the management of severely immunodepressed patients, this method was examined in individuals cared for in two social welfare facilities for HIV-infected persons and compared to self-administered therapy in outpatients. Forty-seven patients with registered HIV infection, stage C, were assigned to DOT for 9 months, the majority of whom had previously received antiretroviral therapy. A group of 51 HIV-infected outpatients, who attended day clinics attached to the reference hospitals, served as a comparison group. Together with increases in weight (9.2+/-7.5 kg) and Karnofsky scores (16.9+/-12.2) in the DOT group, a significant improvement of surrogate markers, such as CD4+ T-cell counts (increase in DOT group, 113.4+/-151.0 cells/microl; control group, -2.8+/-114.1 cells/microl; P<0.001) and HIV load (decrease in DOT group, -1.7+/-2.3 log10 copies/ml; control group, -0.4+/-1.5 log10 copies/ml; P<0.01) was detected in the DOT group. Morbidity and mortality were similar in both groups. The results indicate that such welfare facilities provide a useful framework not only for social objectives but also for healthcare purposes.

Biogenic amines in restructured beef steaks as affected by added walnuts and cold storage.

This article evaluates changes in biogenic amines and how these relate to microbiological growth in chilled, fresh restructured beef steaks containing transglutaminase as a cold binding agent and different amounts of walnut. Added walnut and chilling favored higher total and lactic acid bacteria counts during storage, whereas Enterobacteriaceae were not affected. The highest initial biogenic amine concentrations were identified as spermidine, spermine, and tyramine. Both added walnut and cold storage generally favored the formation of amines (tyramine, histamine, putrescine, and cadaverine), which was more obviously apparent by the end of the storage period. Agmatine, on the other hand, was not generally affected by the walnut.

Neoadjuvant radiation and chemotherapy in rectal cancer does not increase postoperative complications.

BACKGROUND AND AIMS: Neoadjuvant radiation and chemotherapy in rectal cancer reduces local recurrences and increases the rate of conservative sphincter surgery. However, an increase in postoperative morbidity and mortality has also been observed. This study analyzed the operative difficulty and postoperative complications in patients with this treatment. PATIENTS AND METHODS: Retrospective review of 103 patients with rectal cancer, divided into two groups: group A, 53 patients undergoing preoperative radiotherapy with 45 Gy combined with chemotherapy, and group B, 50 patients with rectal cancer who received surgery after diagnosis. Both groups were homogeneous. The two groups were compared for both technical difficulty, using intraoperative data and rate of complications. RESULTS: There were no statistically significant differences between the two groups with regard to intraoperative or postoperative data. In group A there were 20 complications in 17 patients (32%) and in group B 22 complications in 19 patients (38%). The rates of perineal wound infection were similar. The percentage of anastomotic leaks was higher in group A. A greater number of anterior resections was performed in group A. CONCLUSION: Preoperative radiation and chemotherapy in rectal cancer does not increase postoperative complications and increases the rate of sphincter-preserving surgery.

Primary eosinophilic esophagitis.

Eosinophilic esophagitis is an uncommon pathology that generally affects children with a history of allergies and intrinsic asthma. We present a clinical case of eosinophilic esophagitis in a 16-year-old boy with upper dysphagia for solids since childhood. The analytical study showed only a repeat serum eosinophilia. Barium transit disclosed a reduction in caliber of the whole esophagus. Functional esophageal tests with pH monitoring and manometry were normal. Endoscopy showed a small-diameter esophagus and fibrosis with a very friable mucosa. The histological study of the esophageal biopsies revealed a full thickness major eosinophil infiltration of the esophagus. These findings suggest a differential diagnosis with a great variety of pathologies that can cause similar lesions in the esophagus, especially between primary eosinophilic esophagitis and eosinophilic esophagitis secondary to gastro-esophageal reflux disease (GERD). We implemented medical treatment with oral corticoids and total suppression of allergens from the diet, and the patient was asymptomatic.

Oral N-acetylcysteine attenuates the rat pulmonary inflammatory response to antigen.

Oxidative stress is involved in the pathophysiology of inflammatory airway diseases including asthma; therefore, antioxidants might be of clinical benefit in asthma treatment. In the present study, the effects of N-acetylcysteine on sensitised brown Norway rats were examined. N-Acetylcysteine (3 mmol kg body weight(-1) administered orally) was given daily for 1 week before challenge and various antigen-induced pulmonary responses were studied. Antigen exposure increased lipid peroxidation in bronchoalveolar lavage fluid (BALF) and oxidised glutathione levels in lung tissue 2 h after challenge. Lung nuclear transcription factor-KB-binding activity was increased 2 h after challenge, and BALF tumour necrosis factor-alpha and inducible nitric oxide synthase expression in lungs peaked 4 h after challenge. Expression of intercellular adhesion molecule-1 and mucin MUC5AC was also increased 4 h after challenge. These changes in oxidant status, transcription factor activation, and inflammatory cytokine and gene expression were reduced by N-acetylcysteine. This thiol did not affect the immediate bronchospasm reaction to antigen in anaesthetised rats but inhibited airways hyperresponsiveness to 5-hydroxytryptamine and the augmented eosinophil numbers in BALF, which appear 24 h after exposure of conscious rats to antigen aerosol, and abolished antigen-induced extravasation of Evans blue into BALF. These results indicate that oral N-acetylcysteine exerts an antioxidant protective effect and attenuates pulmonary inflammation in experimental asthma.
