A histochemical and immunohistochemical study of certain defense mechanisms in the human lacrimal sac epithelium.

The mucosal surface of the human lacrimal sac represents an area exposed to exogenous agents including potentially harmful microorganisms. The human lacrimal sac was examined histochemically to identify glycoproteins, and immunohistochemically to identify secretory IgA. Neutral and acid glycoconjugates were detected mainly in the cytoplasm of the surface cells of the columnar stratified epithelial lining. The same reactions were recognized in occasional clusters of secretory cells forming intraepithelial glands in the lining of the lacrimal sac. The presence of secretory IgA in the cytoplasm of the apical epithelial cells was demonstrated. The results indicate that the lacrimal sac mucosa possesses certain active defense mechanisms against ascending infections.

A point mutation in the RDS-peripherin gene in a Spanish family with central areolar choroidal dystrophy.

The RDS-peripherin gene encodes a photoreceptor-specific protein that is localized in the outer segment disc membranes of both rods and cones. We screened a Spanish family with central areolar choroidal dystrophy for mutations in candidate genes. A base substitution was identified in the RDS-peripherin gene of one patient and DNA sequencing revealed a C-to-T transition in codon 172, arginine being substituted by tryptophan. The mutation was also detected in two asymptomatic family members who showed irregular pigmentation in the retinal pigment epithelium (RPE). The phenotype is similar to other macular dystrophies caused by mutation in the RDS-peripherin gene.

A system for recording electrophysiological signals in flight.

A special preamplifying system was designed to record electrophysiological signals on fighter aircrew in flight. The system is composed of two elements worn by the pilot in a specially designed jacket. The first element is a preamplifying device for ECG or EMG signals, which includes one or two accelerometer channels, according to the needs of the studies. All recorded signals may thus be correlated with the accelerations actually sustained by the pilot. Accelerometers are new generation equipment. Accelerometer and preamplifier performances have been measured and are reported. The recorder performance has been tested on a centrifuge under high acceleration. The results show that up to +10 G, no frequency shift and no change in the harmonic distortion between acceleration and resting stages have been measured. An example of surface EMG recording on two neck muscles during head movements under high-G loading in flight is given.

Effect of topical antioxidant therapy on experimental infectious keratitis.

To test the effect that the treatment with topical antioxidants may have on corneal infection, we have studied the effect of topically applied antioxidants, such as dimethylthiourea 0.5% (DMTU) and dismutase superoxide 0.2% (SOD), on infectious experimental keratitis caused by Staphylococcus aureus. We have quantified the results of the incubated corneas in ex vivo as well as in in vivo treated with antioxidants by using the luminol amplified chemiluminescence technique (LAC). The evaluation of corneal inflammation was performed calculating the average inflammatory index obtained from the clinical observation of the corneal secretion, corneal edema and ciliary injection. The evolution of the corneal infiltration was evaluated by means of computerized planymetry. The antioxidants used in this study demonstrated a significant reduction of the LAC values when compared with a control group both in the in vivo as well as in ex vivo studies. No significant differences in the clinical evaluation of the average inflammatory index were observed between the study and the control groups. However, a significant increase in the corneal infiltration was registered in the antioxidant treated group (p < 0.001) evaluated by computerized planymetry. Our results indicate that the use of antioxidants as antiinflammatory drugs may have a potential negative influence on the course of infectious keratitis.

[Registry of activities of the Hemodynamics and Interventional Cardiology Section in 1994].

Results of the Spanish Registry for Hemodynamic and Interventional Cardiology are presented. As previous years, has been done by the Chapter of Hemodyamics and Interventional Cardiology of the Spanish Society of Cardiology. Seventy seven cardiac catheterization laboratories have collaborated, which represents 100% of those who had any activity in 1994. Sixty nine performed cardiac catheterization in adults, in 90% of the cases performing coronary intervention, and 22 performed both adult and pediatric studies. Eight laboratories exclusively performed pediatric cardiac catheterizations. Fifty five thousand one hundred sixty nine diagnostic procedures were performed, the majority of them being coronariographies (41,887), representing an increase of 10% from 1993. Ten thousand four hundred thirty three angioplasty procedures were performed in 1994. This represents 274 PTCA per million people (an increase of 21% compared to 1993). Eighty seven per cent of these procedures were done with conventional balloon, 1,361 stents were implanted (724 electivily), and 308 directional and 305 rotational atherectomies were performed. The overall results are similar to those of 1993, except for a marked reduction in complications of stents procedures (mainly in those implanted in a rescue situation). PTCA success was 91% with a complication rate of 3.4% and a mortality of 0.69%. The number of mitral valvuloplasties decreased 16% (694 vs 825 in 1993), and the pediatric interventional procedures were 7% less than those of 1993.

Inhibition of c-myc expression induced by 4-hydroxynonenal, a product of lipid peroxidation, in the HL-60 human leukemic cell line.

4-Hydroxynonenal is a highly reactive aldehyde, produced by cellular lipid peroxidation, able to inhibit cell proliferation "in vitro" and "in vivo". Its concentration in non proliferating cells ranges up to 1 microM, whereas in the highly undifferentiated tumour cells, it is very low or undetectable. We have now demonstrated that micromolar concentrations of 4-hydroxynonenal inhibit c-myc but not N-ras expression in HL-60 human leukemic cells. This inhibitory effect is observed after an incubation of 1 hour with both 1 and 10 microM aldehyde. Moreover, we report that down-regulation of c-myc expression increases when repeated additions of 1 microM 4-hydroxynonenal are performed, to maintain the cells in presence of aldehyde for 7.5 hours. These results indicate that not only the concentration but also the length of exposure to the aldehyde is important in determining the extent of the c-myc expression inhibition and suggest a role of lipid peroxidation products in the control of gene expression.

Organic anion transport and reabsorptive fluid flux in diabetic retinal vessels: a microperfusion study.

Microperfusion techniques were used to characterize in vitro the fluorescein transport and fluid movement across isolated diabetic rabbit retinal vessels. Unbranched segments of arterioles of alloxan-induced diabetic rabbits were isolated and cannulated on a microperfusion apparatus. Movement of 3H-inulin, the reference tracer, and 14C-fluorescein, the test tracer, across the vascular walls was studied using microperfusion techniques adapted by us to study vascular permeability. The diabetic retinal vessels showed a fluid flux from bath to lumen of 3.6 +/- 0.6 nl min-1 mm-1 with perfusion rates of 7.1 +/- 0.3 nl min-1. This value was 43% (P < or = 0.05) lower when compared with normal values obtained in previous work. Bath to lumen fluorescein fluxes were 2.1 +/- 0.5 and 7.6 +/- 1.6 x 10(-12) mol min-1 mm-1, for fluorescein concentrations of 0.87 x 10(-4) and 1.5 x 10(-4) M, respectively. These values are 23% (P < or = 0.05) and 38.7% (P < or = 0.05), respectively, lower when compared to values obtained from normal retinal vessels. Probenecid, a competitive inhibitor of organic anion transport, added to the bath, had no effect. Increasing bath concentrations of fluorescein caused increasing fluid flux from bath to lumen, and again, probenecid added to the bath has no effect. These results show a significant decrease of fluorescein transport and of net fluid flux across diabetic retinal vessel walls from outside into the lumen, suggesting that in experimental alloxan-induced diabetes there is an early, probably functional, involvement of the retinal vessels in the initial alteration of the blood-retinal barrier (BRB).

Highly polymorphic repeat marker within the beta-amyloid precursor protein gene.

We have identified a polymorphic compound dinucleotide repeat sequence in intron 1 of the beta-amyloid precursor protein (APP) gene on chromosome 21. Using polymerase chain reaction (PCR) amplification of the locus, designated APPivs1, we detected 13 alleles in the CEPH family members (heterozygosity = 0.69). Lod score analysis showed complete linkage of the marker to the loci D21S210 and D21221.

Genetic analysis of p53 and RB1 tumor-suppressor genes in blast crisis of chronic myeloid leukemia.

We have investigated the involvement of the p53 and RB1 tumor-suppressor genes in 26 cases of chronic myeloid leukemia (CML) blast crisis, including 17 myeloid, eight lymphoid, and one megakaryoblastic crisis. The presence of p53 mutations in exons 5 through 9 was tested by the PCR-single-strand conformation polymorphism (SSCP) assay, followed by PCR-direct sequencing; in addition, loss of heterozygosity (LOH) at 17p13, the site of the p53 gene, was assayed by Southern blot. Given the variability of the mechanisms of inactivation of the RB1 gene in human tumors, a combination of Southern blot and mutational analysis by PCR-SSCP was used. p53 mutations were restricted to one case of myeloid blast crisis, showing a CGC-->TGC (Arg-->Cys) mutation at codon 283; two additional cases displayed LOH at 17p13 in the absence of p53 mutations. No molecular lesions of the RB1 gene were detected in any of the cases analyzed. These data indicate that inactivation of p53 and RB1 is a rare event in the molecular pathogenesis of CML acute transformation.

The 'Ermenonville' classification of observations at coronary angioscopy--evaluation of intra- and inter-observer agreement. European Working Group on Coronary Angioscopy.

A European coronary angioscopy working group has been established to create and evaluate a classification system for angioscopic observation. The 'Ermenonville' classification features items, graded in 3-5 categories, such as lumen diameter, shape of narrowing, colours of surface, atheroma, dissection, thrombus, etc. Inter- and intra-observer agreement on the interpretation of angioscopic images, using this classification system, was studied within the working group. Kappa values for chance-corrected intra-observer agreement of the diagnostic items were 0.51-0.67. The mean kappa values for inter-observer agreement were very low at 0.13-0.29. The important items, such as red thrombus and dissection were studied after recoding as either present or absent. These items proved to have a good intra-observer agreement, and an acceptable inter-observer agreement after recoding. Other angioscopic diagnoses should be made with caution. Multicentre angioscopy studies should make use of an angioscopy core laboratory. A set of definitions for coronary angioscopy is proposed, and this working group will re-evaluate observer agreements using these definitions.

Susceptibilities to 10 antimicrobial agents of 1,220 Campylobacter strains isolated from 1987 to 1993 from feces of pediatric patients.

We report the in vitro antibiotic susceptibility of 1,220 strains belonging to the thermotolerant Campylobacter species, isolated from the feces of pediatric patients with diarrhea in the period from 1987 to 1993. The strains were identified as 1,148 C. jejuni isolates and 72 C. coli isolates. The overall results show that the strains showed drug resistance as follows: 51.8% to ampicillin, 4.4% to clindamycin, 2.6% to chloramphenicol, 21.2% to tetracycline, and 1% to gentamicin. Twenty-one strains (1.7%) displayed resistance to the combination of amoxicillin-clavulanic acid, and 3.2% of the strains were resistant to erythromycin (MIC of > or = 4 micrograms/ml), with a notable difference according to the species under consideration. While C. jejuni remained stable at 0.9 to 4% resistance to erythromycin, for C. coli the percentages detected ranged from 0 to 33%, with overall rates of 2.5 and 15.2% for the two species, respectively. Resistance to nalidixic acid (MIC of > or = 32 micrograms/ml) was found in 27.2% of the strains (27.8% for C. jejuni and 18% for C. coli), and resistance to ciprofloxacin (MIC of > or = 4 micrograms/ml) was found in 24.2% of the strains for C. jejuni and 15.2% for C. coli). Cross-resistance between nalidixic acid and ciprofloxacin was found in 89.1% of the strains (type 1 mutants), while 10.9% were resistant to nalidixic acid but susceptible to ciprofloxacin (type 2 mutants).

Oxygen free radicals are not the main factor in experimental gentamicin nephrotoxicity.

As a role for oxygen free radicals has been suggested in gentamicin (G) nephrotoxicity, we tested the hypothesis that exogenously administered glutathione (GSH), able to restore intracellular antioxidant potential, could be useful in reducing damage. Adult Sprague-Dawley rats were injected with saline (n = 30), subcutaneous (s.c.) G 100 (n = 23) and 150 mg/kg/day (n = 14), or s.c. G at the same dosages plus intraperitoneal (i.p.) GSH 1200 mg/kg/day (n = 24 and 14, respectively) for 7 days. In the G-100-day protocol, GSH-treated rats showed significantly lower renal G content (2.79 +/- 0.8 vs. 3.61 +/- 1.4 micrograms/mg prot) coupled with lower plasma urea (153 +/- 79 vs. 188 +/- 61 mg/dL) and creatinine levels (1.63 +/- 1 vs. 2.45 +/- 1 mg/dL). As to renal oxidant/antioxidant balance, local GSH was increased (0.32 +/- 0.01 vs. 0.19 +/- 0.01 microgram/mg prot) while lipid peroxidation, determined by production of thiobarbituric acid reactive substances (TBARS), was decreased (0.35 +/- 0.02 vs. 0.52 +/- 0.02 nmol/mg prot). In the G-150-mg protocol, GSH-treated rats showed no differences in renal gentamicin content or in blood urea and creatinine levels, in spite of a significantly lower renal TBARS production and a significantly higher GSH content. Urine enzyme excretion did not significantly change in GSH-treated vs. not-GSH-treated rats in both protocols. We conclude that: (a) GSH interferes with G nephrotoxicity mainly via a reduction in G uptake; (b) the oxidative renal stress is not crucial in inducing renal damage. In fact, when increased G dosages blunt the ability of GSH in reducing G uptake, no substantial protection is demonstrated.
