Indications for coronary arteriography: risks vs. benefits.

Coronary arteriography has become a diagnostic procedure which is no longer limited to major medical centers but is being performed in many community hospitals. The procedure carries an appreciable risk, which is only justifiable by specific benefits to the patient. The benefits are related to the potential availability of newer surgical procedures of bypassing obstructive coronary arterial lesions. A specific set of criteria for the performance of coronary arteriography has been developed from a critical analysis of the ratio of risk to benefit. A survey of the risk of coronary arteriography indicates that mortality increases at least twentyfold and morbidity sixfold when this procedure is performed in laboratories with a low caseload as compared with high caseload institutions. Thus there appears to be no justification for performing coronary arteriography in the absence of proper team facilities.

Production, recognition and treatment of digitalis intoxication.

Recent studies dealing with absorption, distribution in the body and fate of digitalis, have provided a basis for better understanding the great variability of individual responses to this drug. When given to patients in sinus rhythm as a cardiotonic agent, digitalis action cannot be accurately assessed nor monitored by appropriate tests; consequently, blind administration of the drug on average dosages inevitably produces digitalis intoxication in a proportion of patients. Clinical signs of digitalis toxicity, factors enhancing its development, its recognition and treatment are summarized here. It is emphasized that digitalis should always be administered as a calculated risk, when benefits are clearly defined, as in overt cardiac failure and in atrial fibrillation associated with excessive ventricular rates. Use for other indications, particularly prophylactic, should be considered experimental and is to be discouraged.

A retraining program for inactive physicians.

During the past two years a pilot project was conducted in which 19 inactive physicians were retrained in preparation for resumption of active practice. The initial program consisted of a flexible training program of six months to one year patterned after conventional internship-residency concepts. During the second year the program was modified by providing an initial condensed indoctrination period of two months' duration especially designed for this purpose, followed by a preceptorship type of training. The project was considered successful in permitting trainees to enter some form of active medical work, or to enroll in formal specialty training. The observations made by the faculty of the program and its accomplishments are discussed in the light of the effort expended and the cost of the project.
