Ultrastructural observations of the so-called strumal carcinoid of the ovary.

A strumal carcinoid arose in a benign cystic teratoma of the ovary. Ultrastructural studies demonstrated that both the solid acinotubular areas as well as the "follicular" components were comprised of light and dark cells that contained numerous secretory granules. These findings indicate that this neoplasm is a pure carcinoid tumor with a follicular pattern. The present study raises the question of whether all strumal carcinoids are indeed pure carcinoid neoplasms.

Effect of intrauterine estriol on reproductive function in the rabbit.

The effect of intrauterine estriol on spontaneous ovulation, ovum fertilization, blastocyst development, and blastocyst implantation in rabbits has been investigated. Estriolreleasing intrauterine capsules constructed of biocompatible polymer were implanted in one uterine horn of adult New Zealand White female rabbits, while placebo-containing capsules were implanted into the contralateral horn. The animals were artificially inseminated and ovulated or mated to fertile bucks. The does were killed and their reproductive tracts were examined 54 hours, 6 days, and 10 days after ovulation-inducing injection or after coitus. The results indicate that intrauterine estriol released at a steady state rate of 1.25 mug/day effectively inhibits blastocyst development and implantation. This contraceptive effect was clearly local, since implantations in the contralateral,placebo-bearing horn were not inhibited. Furthermore, the same dose of estriol, when released systemically from a subcutaneously placed capsule, had no effect on implantation. Intrauterine estriol appeared also to have no effect on spontaneous ovulation or ovum fertilization.

Catamenial pneumothorax. Bilateral occurrence while on suppressive therapy.

A case of recurrent bilateral catamenial pneumothorax is presented with pathologic, cytologic, and radiologic documentation. Unlike previously reported cases where the pneumothorax is unilateral and the symptoms relieved while ovulation is suppressed with cyclic oral contraceptives, in the present case the pneumothorax occurred despite oral contraceptive therapy concurrent with anovulatory withdrawal or breakthrough bleeding.

Intrauterine contraception.

Newer medicated IUDs, such as the copper or progesterone-releasing devices, are more effective, better tolerated and have fewer side effects than nonmedicated IUDs. Major complications are rare if adequate precautions are taken. The IUD is an ideal contraceptive for patients with contraindications to oral contraception and for those who have difficulty remembering to utilize other methods.

Heterotopic pregnancy following clomid treatment.

A case of concomitant intra- and extrauterine pregnancy after clomiphene treatment is reported. The incidence of multiple ovulations and multiple pregnancies is higher after induced ovulation. The increased likelihood of combined intra- and extrauterine pregnancy in patients with a higher rate of fraternal twinning is discussed.

Uterine contractility data processing system.

A versatile system has been developed for off-line computerized quantitation of uterine contractility. It includes tabulation and display of intensity, frequency, resting tone, uterine activity, cumulative area below the pressure curve, and average intrauterine pressure. Its advantages and drawbacks are compared with the conventional method and with an automated on-line labor surveillance system.
