Evidence for epidermal growth factor (EGF)-induced intermolecular autophosphorylation of the EGF receptors in living cells.

In response to epidermal growth factor (EGF) stimulation, the intrinsic protein tyrosine kinase of EGF receptor is activated, leading to tyrosine phosphorylation of several cellular substrate proteins, including the EGF receptor molecule itself. To test the mechanism of EGF receptor autophosphorylation in living cells, we established transfected cell lines coexpressing a kinase-negative point mutant of EGF receptor (K721A) with an active EGF receptor mutant lacking 63 amino acids from its carboxy terminus. The addition of EGF to these cells caused tyrosine phosphorylation of the kinase-negative mutant by the active receptor molecule, demonstrating EGF receptor cross-phosphorylation in living cells. After internalization the kinase-negative mutant and CD63 have separate trafficking pathways. This limits their association and the extent of cross-phosphorylation of K721A by CD63. The coexpression of the kinase-negative mutant together with active EGF receptors in the same cells suppressed the mitogenic response toward EGF as compared with that in cells that express active receptors alone. The presence of the kinase-negative mutant functions as a negative dominant mutation suppressing the response of active EGF receptors, probably by interfering with EGF-induced signal transduction. It appears, therefore, that crucial events of signal transduction occur before K721A and active EGF receptors are separated by their different endocytic itineraries.

Oxytocin receptors in rat kidney during development.

The development and characteristics of oxytocin (OT) receptors in the rat kidney were studied by light-microscopic autoradiography and on membrane preparations using the iodinated OT antagonist 125I-d(CH2)5[Tyr(Me)2,Thr4,Orn8,Tyr(NH2)9]OT. Specific binding was first detected by autoradiography at embryonic day 17 (E17) in both the cortex and the medulla. Cortical labeling was found thereafter at all ages examined including in the adult (postnatal day 90, PN90). It was localized on the distal tubule at the level of the juxtaglomerular apparatus. Medullary binding was detected only transiently during two stages of development, first, before PN6, and second, at the approximate time of weaning (PN20-PN30). Binding studies on crude membranes prepared from whole kidneys of animals aged between PN1 and PN15 showed a single class of high-affinity binding sites, with a dissociation constant of 0.13 +/- 0.08 nM. Thus transient OT binding sites expressed in the medulla do not differ from cortical OT binding sites; moreover, the ligand selectivity of kidney OT receptors regardless of location and age appears similar to that of previously characterized OT receptors. Our results suggest that OT may play a role in both renal development and renal function.

[Clinical, electromyography and pathologic-anatomic studies in myopathy--a contribution to rational diagnostic strategy].

A total of 189 patients underwent clinical, electromyographical and patho-anatomical examination (by muscle biopsy). 102 of the patients suffered from myopathy, 45 patients from neurogenic muscle diseases. 31 patients were suffering from chronic renal insufficiency and 11 patients from chronic alcoholism. The statistic elaboration of the findings was done by the multivariate analysis of variance (MANOVA) and discriminant analysis, in order to determine the diagnostic value of single parameters of investigation and to demonstrate diagnostically effective parameter combinations for diagnosing myopathies. Employing the discriminant function for the differentiation of myopathies and neurogenic muscle diseases yielded in patients with chronic renal insufficiency predominantly (30 cases) references to a neurogenic lesion of the proximal muscles and in the patients with chronic alcoholism relatively often (6 cases) references to a myogenic lesion of the proximal muscles.

[Quality of life as a target variable in oncologic clinical research].

The change of target variables in palliative cancer treatment led to a new evaluation of quality of life. Its use as a target variable to discriminate different therapies is problematic. Investigations with a patient-questionnaire assessing quality of life showed that the validity of this term is difficult to judge. At present it should be used in clinical research as an accompanying variable only.

Changes in heparan sulfate structure during transition from the proliferating to the non-dividing state of cultured arterial smooth muscle cells.

Cultured arterial smooth muscle cells synthesize a cell-associated heparan sulfate proteoglycan which consists of a 92 kDa core protein with 3 to 4 heparan sulfate side chains covalently attached. Biosynthesis of the cell-associated heparan sulfate proteoglycan was compared in proliferating and in non-dividing vascular smooth muscle cells which are preincubated in the presence of [35]sulfate or a combination of [35S]methionine and [3H]glucosamine. The Mr of the core protein was identical in either growth state, but changes in the structure of the heparan sulfate side chains were observed. Non-dividing (postconfluent) arterial smooth muscle cells form longer heparan sulfate chains with a higher proportion of hydrophobic (N-acetyl) groups than proliferating (preconfluent) cells as judged from gel filtration experiments, hydrophobic interaction chromatography and heparitinase degradation. An enzyme preparation from proliferating cells catalyzes deacetylation and N-sulfation of heparan sulfate at a 5-fold higher activity than from non-dividing cells. Cell density-dependent structural differences of heparan sulfate are related to the finding that heparan sulfate isolated from non-dividing cells has a 10-fold higher antiproliferative potency than heparan sulfate from proliferating (preconfluent) cells.

Central olfactory and vomeronasal pathways in salamanders.

Central olfactory and vomeronasal pathways were studied in salamanders of the families Salamandridae and Plethodontidae by means of the HRP method. HRP was injected into the olfactory and accessory olfactory bulb as well as into the lateral/dorsal pallium, the main termination areas of secondary olfactory projections. Fibers leaving the olfactory bulb constitute two main tracts, the lateral olfactory tract (LOT), which is mostly restricted to the ipsilateral telencephalon, and the anterior olfactory habenular tract (AOHT), which represents the main contralateral connection. Fibers of the LOT project to the lateral pallium, dorsal striatum and the habenula. Efferent connections of the accessory olfactory bulb (AOT) terminate within the amygdala pars lateralis. No interspecific differences concerning the targets of central olfactory and vomeronasal projections were observed, but the number of olfactory tracts and their separation from each other varies.

[Immunohistochemical detection of chromogranin A and morphometric findings in pancreatic nesidioblastosis in children].

The histologic features and morphometric data of the endocrine pancreas (surgical specimens) of 12 infants suffering from persistent hyperinsulinemic hypoglycemia are reported. In 3 cases, a focal nesidioblastosis, in another 3 cases, a diffuse nesidioblastosis were observed. 2 infants demonstrate B-cell adenomata and 4 adenomata combined with a diffuse nesidioblastosis. In respect to volume density of the endocrine pancreas and diameter of the islets, there are no differences between cases with diffuse nesidioblastosis and age-matched controls. Structural changes of the endocrine pancreas in cases with nesidioblastosis are detectable by their chromogranin-A-immunoreactivity. Further investigations are necessary to clarify if there are disturbances in the relationships between chromogranin A and peptide hormones additionally to structural changes in the pancreas.

ESWL for ureteral calculi. Using the Dornier HM 3, HM 3+ and Wolf Piezolith 2,200.

In a one year period from July 1985 to July 1986 226 upper and 62 distal ureteric calculi were treated. In situ ESWL represents the therapy of first choice for upper and distal ureteric calculi with a success rate of 81% and 76% respectively. Retrograde mobilization of the calculus was used only in cases where in situ ESWL was impossible because of localization difficulties (obesity, stone close to the spine, skeleton deformation). Although ESWL after successful mobilization succeeded in 95%, retrograde mobilization was possible only in 80%. Antegrade ureterorenoscopy via percutaneous nephrostomy was performed to avoid open surgery after impossible retrograde mobilization and succeeded in 90%. Two second generation lithotripters suitable for treatments without invasive forms of the anesthesia, the modified Dornier HM 3- and the Wolf Piezolith 2,200 were compared in terms of efficacy for ureteric calculi. In situ ESWL was successful with the Piezolith 2,200. In situ ESWL of middle ureteric calculi was successful 81.8% with modified HM 3+, while in situ treatment of middle ureteric calculi was impossible with the Piezolith 2,200 due to insufficient localization with ultrasound of middle ureteric calculi were treated successfully in 71.4% with the modified Dornier HM 3+ and in 64% with the Piezolith 2,200. First clinical experience of ESWL in prone position for iliac ureteric calculi was reported. 8 of 10 cases were treated successfully in situ.

[The effect of injection speed and needle gauge on the spread of sensory blockade in spinal anesthesia].

Unanimity has not yet been reached on the influence of injection speed and needle size on the spread of sensory blockade in spinal anesthesia. While McClure et al. [6] proved that a change in injection speed had no effect on the spread of the blockade, Lanz et al. [4] showed in their investigation that increasing injection speed increases the spread of the blockade. The influence of needle size has hardly been investigated so far. Moore et al. [7] report that the needle size alone has no effect on the spread of the blockade. In this study the influence of injection speed and that of needle size on spread of sensory blockade were considered separately. MATERIALS AND METHODS. Spinal puncture was performed via the midline approach at the L3-4 interspace with the patient in a sitting position. Immediately after the induction of anesthesia the patients were placed in the supine position again. As local anesthetic 15 mg (3 ml) bupivacaine 0.5% with epinephrine 1:200000 was administered. The aim of the study was to find out how far injection speed and size of the spinal needle influenced the sensory spread in isobaric spinal anesthesia. The height of sensory blockade was assessed by means of the pin-prick method in the midline, and the onset of analgesia was determined as height of spread. The injection speeds for the local anesthetic solution were 0.25, 0.5 and 1 ml per second. The needle sizes were 22, 25, and 29 gauge. Each group consisted of 15 patients. RESULTS. It turned out that slow (0.25 ml/s) and the fast (1 ml s) injection was associated with a significantly higher level of analgesia than the medium one (0.5 ml/s). Cephalad spread was to T7 in the first two groups, while the medium injection speed only achieved a level of T9. The results are statistically significant (P less than 0.05). The comparison of different needle sizes (22, 25, and 29 gauge) used for spinal anesthesia showed a higher spread of the sensory blockade with increased diameter of the spinal needle given a constant injection speed. With the 22-gauge needles (n = 15) sensory blockade extended on average to T7, with 25-gauge needles (n = 15) to T9, and with 29-gauge needles (n = 15) to T10. It has to be mentioned, however, that with the 29-gauge needle the standard injection speed of 0.5 ml/s could not be achieved because of the small inner diameter. The differences between 22- and 25-gauge needles are statistically significant (P less than 0.05). DISCUSSION. No direct relation could be proved between the different injection speeds and the spread of the blockade. Our results are hardly comparable with those of other investigators, since other groups have used different local anesthetics or performed investigations in vitro. The use of large spinal needles is associated with spread of the spinal block to a significantly higher level than is achieved with thinner needles.(ABSTRACT TRUNCATED AT 400 WORDS)

[The effect of volume and dosage of isobaric bupivacaine on the sensory spread of spinal anesthesia].

There is some controversy about the relationship of volume, concentration and total dose of bupivacaine in the sensory spread of spinal anesthesia. In this study the effects of volume and dose were investigated. MATERIAL AND METHODS. In this study 120 patients undergoing lower extremity, inguinal or transurethral surgery were randomly divided into six groups. Bupivacaine 15 mg with the addition of epinephrine 1:200,000 was administered in 2 ml (0.75%), 3 ml (0.5%), 6 ml (0.25%) and 9 ml (0.166%) solutions. In addition 3 ml isobaric bupivacaine in doses of 7.5 mg (0.25%), 15 mg (0.5%) and 22.5 mg (0.75%). The spinal puncture was performed via the midline approach at the L3-4 interspace, with the patient in the sitting position. The injection speed was 0.5 ml per second. Immediately after the injection the patients were placed in the supine position. The spread of sensory blockade was assessed by means of the pin-prick method in the midline. Motor blockade was assessed on the Bromage scale 0-3. RESULTS. There were no statistically significant differences in motor blockade or cardiovascular changes. The maximum cephalad spread of analgesia (30 min) between the 15 mg groups with different volumes and concentration was: group I (9 ml): T7.7, group II (6 ml): T7.8, group III (3 ml): T8.5 and group IV (2 ml): T10.1. The differences between group IV 2 ml and the groups receiving 3, 6 and 9 ml were statistically significant (P less than 0.05). There were no statistically significant differences in maximum cephalad spread between the 7.5 mg (3 ml), 15 mg (3 ml) and the 22.5 mg (3 ml) groups. The regression after 180 min was significantly shorter in the 7.5 mg group than in the 15 mg and 22.5 mg groups (P less than 0.05). DISCUSSION. Earlier published results indicate that the dose of isobaric bupivacaine is more important in spinal anesthesia than the concentration or the volume of the solution. The comparison between 3 ml:6 ml and 3 ml:9 ml bupivacaine showed no statistically significant differences in cephalad spread. A volume-dependent increase in segmental spread was between the 2 ml (0.75%) and 3 ml (0.5%) bupivacaine. The same statistically significant differences were between the 2 ml and 6 ml groups and the 2 ml and 9 ml groups. No statistically significant difference in cephalad spread resulted from increasing the dose of bupivacaine from 7.5 mg to 22.5 mg. Earlier studies on the effects of changes in volume, concentration and dose of bupivacaine showed similar "jumps of blockade" between 2 ml and 3 ml injected volume. Assembling the results the relation between volume and total dose does not suggest a no linear dependence. The anatomic configuration of the spinal cord at the conus medullaris may affect the distribution of the solution.

[The effect of barbotage on the sensory spread in spinal anesthesia using isobaric and hyperbaric 0.5% bupivacaine].

The effect of spinal anesthesia with barbotage versus without barbotage on the spread of analgesia was investigated. For comparison, hyper- and isobaric bupivacaine 0.5% with adrenaline 1:200,000 was used. MATERIAL AND METHODS. Barbotage was accomplished as follows: after lumbar puncture 0.5 ml CSF was aspirated into the local anesthetic solution, followed by reinjection of 1.0 ml of the solution. This process was repeated six times. Sixty patients who were scheduled for urological or lower limb surgery under spinal anesthesia were selected for this study. Patients were each arbitrarily assigned to one of four groups (isobaric and hyperbaric, without and with barbotage). RESULTS. There was no statistically significant difference in the maximum level of sensory analgesia. The mean maximum level of sensory analgesia reached T9 (group 1), T8 (group 2), T9 (group 3) and T8 (group 4). Time to highest dermatome was significantly shorter with barbotage (groups 1-4: 19.0 min, 13.0 min, 18.7 min, 12.3 min). Times for regression of analgesia to T12 (mean maximum duration) were 142 (+/- 54.9) min, 164 (+/- 29.7) min, 130 (+/- 40.4) min and 144 (+/- 36.2) min (groups 1-4). Motor block grade 3 (Bromage) was achieved in significantly shorter times with barbotage than without. The shortest onset time was recorded with isobaric bupivacaine. The onset time of a complete motor block was 12.5 (+/- 5.5) min in group 1, 6.1 (+/- 2.9) min in group 2, 15.8 (+/- 4.7) min in group 3, and 11.7 (+/- 5.1) min in group 4. CONCLUSIONS. The results showed no significant differences between the maximum segmental sensory levels or duration of anesthesia observed with isobaric and with hyperbaric bupivacaine (with and without barbotage). Sufficient analgesia was obtained with barbotage and without barbotage. Uncontrolled cephalad spread of spinal anesthesia was not observed. Barbotage has the advantage of shortening time for spread to highest dermatome and the time to onset of complete motor block.

[The content of Hb in blood and the protein, Fe, Fe-binding capacity, Cu and Zn in blood plasma of low parity sows before and after oral iron administration].

In investigations of 51 pregnant sows, blood Hb levels above 7.45 mmol/l (mean value of 8.94 +/- 0.88) were recorded from 43 animals (Group A), while levels below that margin were recorded from eight (Group B) (mean value of 6.73 +/- 0.31). Iron levels in blood plasma of Group A were 36.9 +/- 8.07 and were thus higher (p less than 0.01) than those in Group B (28.9 +/- 5.19 mumol/l). No difference was found to exist between the two groups with regard to iron fixation capacity as well as copper and zinc in blood plasma. Oral uptake of 50, 500 or 2,000 mg of iron in the form of FeSO4.7H2O with cereal shred as morning ration by 17 or 15 sows failed to cause any increase in blood plasma iron levels, within 360 minutes from uptake. Literature data as well as the results of these analyses have supported the recommendation that Hb variations between 4.96 and 9.93 mmol/l (8-16 g/dl) should be rated as physiological in the blood of pregnant sows. The GDR Standard TGL 35,423 on "Monitoring of Metabolism on Pig Breeding and Fattening Units" should be accordingly amended.

[Experiences with the Meuli total endoprosthesis of the wrist joint in patients with chronic polyarthritis].

Artificial joint replacement of the wrist requires careful consideration of anatomic and mechanical factors as well as appreciation of the implant fixation techniques. This study presents experience with nine total wrist arthroplasties by the Meuli method in exclusively female patients after an average follow-up period of 7 8/12 years. On the basis of this evaluation and comparison to the results described by other authors cemented wrist prosthesis have shown an unacceptably high rate of failure especially because of loosening problems.

Minimal invasive therapy of ureteral calculi using modern techniques.

Two second generation lithotripters suitable for treatments without invasive forms of anesthesia, the modified Dornier HM3+ and the Wolf Piezolith 2200, were compared in terms of efficacy for ureteric calculi. In situ ESWL was successful for upper ureter calculi in 70.7% with the HM3+ and 37.9% with the Piezolith 2200. In situ ESWL of middle ureteric calculi was successful in 81.8% with the modified HM3+, while in situ treatment of middle ureteric calculi was impossible with the piezolith 2200 due to insufficient localization of middle ureteric calculi with ultrasound. Distal ureteric calculi were treated successfully in 71.4% with the modified Dornier HM3+ and in 64% with the Piezolith 2200. Our initial clinical experience with ESWL in the prone position for iliac ureteric calculi is reported. Eight of 10 cases were treated successfully in situ.

Interleukin-1 costimulatory activity on the interleukin-2 promoter via AP-1.

Interleukin-1 (IL-1) is a major regulator of inflammation and immunity. IL-1 induces T lymphocyte growth by acting as a second signal (together with antigen) in enhancing the production of interleukin-2 (IL-2). An IL-1-responsive element in the promoter region of the human IL-2 gene was similar to the binding site for the transcription factor AP-1. IL-1 enhanced expression of c-jun messenger RNA, whereas the antigenic signal enhanced messenger RNA expression of c-fos. Thus, the two components of the AP-1 factor are independently regulated and the AP-1 factor may serve as a nuclear mediator for the many actions of IL-1 on cells.

Distribution of substance P-like, leucine-enkephalin-like, and bombesine-like immunoreactivity and acetylcholinesterase activity in the visual system of salamanders.

In the urodele species Salamandra salamandra and Batrachoseps attenuatus, the distribution of the neuropeptides substance P (SP), leucine-enkephalin (LENK), and bombesine (BOMB) was investigated by means of immunohistochemistry in brain areas containing retinofugal projection sites (tectum mesencephali, praetectum, thalamus) as well as in brain regions postsynaptic to the tectum. The activity of acetylcholinesterase (AChE) was studied histochemically. Despite its simplified, two-layered morphology, the salamander tectum shows a high degree of neurochemical differentiation, characterized by a laminar organization of neuropeptide-like immunoreactivity and AChE-activity comparable to that found in the anuran tectum, which has a multi-layered morphology. SP-like immunoreactivity constituted four tectal laminae, two of them occurring in the stratum opticum. LENK-like immunoreactivity formed three laminae, one in the stratum opticum. BOMB-like immunoreactivity formed one lamina within the stratum opticum and one in the tectal efferent layers. Layers 1 and 2 of the stratum opticum revealed high AChE-activity, whereas low activity was found in deep fiber layers containing tectal efferents. The outer cellular layer also revealed AChE-activity. After enucleation of one eye, the contralateral tectum lacked neuropeptide-like immunoreactivity and AChE-activity in the layers containing retinofugal projection sites. No reduction of immunoreactivity was found in nuclei postsynaptic to the tectum. Our experiments suggest that the secondary simplification that has taken place in salamanders with respect to tectal morphology did not affect the neurochemical differentiation of the tectum.

Immunological detection of phytoene desaturase in algae and higher plants using an antiserum raised against a bacterial fusion-gene construct.

Immunological characterization of phytoene desaturase, a key enzyme of carotenoid biosynthesis, is reported. For this purpose, a phytoene-desaturase fusion protein has been employed. For its construction 921 base pairs of the crtI gene were fused to the N-terminal region of the Escherichia coli lacZ gene. Plasmid pGABX2 resulted from insertion of a BglI - XhoI fragment from the Rhodobacter capsulatus carotenoid biosynthesis gene cluster, carrying the crtI, crtA and crtB genes, into pBR322. A 968-base-pair SalI-fragment from pGABX2 was cloned into pUR288 at the 3' end of the lacZ gene. Isopropyl-beta-D-thio-galactopyranoside-dependent activation of the lacZ fusion gene resulted in expression of a stable 150-kDa protein. After electroelution from SDS/polyacrylamide slab gels, the protein was used for antibody production. The heterogenic antiserum obtained was tested by Western blotting against proteins from Rhodobacter capsulatus and several different photoautotrophic organisms including higher plants. Apparent molecular masses of immunoreactive proteins from Rhodobacter, Aphanocapsa, rape and spinach were around 64 kDa. In Bumilleriopsis a 55-kDa protein was found instead. The antibody also inhibited in vitro desaturation of phytoene when detergent-solubilized membranes were employed.
