Peripheral neuropathy in patients treated with almitrine dimesylate.

Sensory peripheral neuropathy developed in 5 patients treated with almitrine dimesylate, 60-100 mg/day. Onset was insidious, beginning symmetrically in the legs with stocking sensory loss and loss of ankle-jerks. Cerebrospinal fluid protein levels were slightly increased and there was electrophysiological and histological evidence of distal axonopathy. The interval between the first dose of almitrine dimesylate and onset of symptoms ranged from 2 to 4 months in 4 patients. All patients had noted a recent weight loss of 4-15 kg which may have resulted in release of previously bound drug.

Left innominate vein stenosis as a late complication of central vein catheterization.

A patient was seen for acute exercise-induced left superior limb swelling. Phlebography disclosed left innominate vein stenosis and the lack of subclavian vein thrombosis. Ten months earlier, the patient underwent repeated and prolonged central vein catheterization procedures. No other cause of central vein stenosis was evidenced. Emphasis is placed on the symptomatic presenting event, the very late discovery, and the site of stenosis.

Age-related changes in biological parameters in Zucker rats.

Changes in a number of morphological and biochemical parameters were observed in genetically obese Zucker rats and in lean controls between 3 and 58 weeks of age. By 3 weeks, the genetically obese rats had higher proportional (wt/100 g body wt) and absolute amounts of adipose tissue, hyperlipemia affecting all the lipid fractions, and hyperproteinemia compared to lean controls. Obesity, hepatomegaly, high concentrations of hepatic lipids and hyperinsulinemia did not appear until the fifth week. In obese animals, liver lipid concentration reached a maximum at 17 weeks of age and then declined. During this time, the triacylglycerol concentration in the serum remained stable, whereas the cholesterol and phospholipid concentrations continued to increase. The glycogen concentration in obese animals increased, both absolutely and compared to lean controls, between the 12th and 43rd weeks of age. From weaning, the Zucker rats, compared to lean controls, exhibited characteristics of obesity (accumulation of adipose tissue, hyperlipemia and hyperproteinemia), which persisted to the age of 58 weeks.

[Idiopathic peliosis hepatis: case report and review of the literature (author's transl)].

Peliosis hepatis is a very uncommon disease defined by pathological lesions located in the liver and in the spleen characterized by blood filled lakes irregularly disseminated throughout parenchyma, without abnormalities of the hepatic veins or the vena cava system. A new case, regularly followed 3 years after the initial diagnosis is reported. Recent reports of the literature have pointed out etiologic factors, particularly iatrogenic factors. None of the reported factors could be found out in the present case. This emphasizes the need of epidemiological studies of such a rare disease (case-control study in a multicentric basis). Finally a successful diagnosis approach is proposed: transvenous (transjugular) liver biopsy. This procedure prevents hemorrhagic risk which could be a complication of routine transcutaneous liver biopsy in this disease.

[Paroxystic hypertension after ingestion of gruyere cheese during isoniazide treatment: a report on two cases (author's transl)].

Paroxystic hypertension was observed in two patients receiving isoniazide treatment, after they had ingested gruyere cheese. It is highly probable that isoniazide was responsible, as hypertension did not reoccur under the same conditions after interrupting treatment. One patient developed a vasomotor reaction identical to that which sometimes occurs after the eating of foods rich in tyramine by patients taking monoamine oxydase inhibitors. It could be that in our patients this phenomenon was consecutive to the inhibition of plasma monoamine oxydase by the isoniazide. It was not due to an overdose of isoniazide. This unusual reaction suggest that patients taking isoniazide should be questioned whether they have had post-prandial vasomotor flushes, and if so, advised to avoid foods rich in tyramine.

Salicylate intoxication and influenza in ferrets.

A model of salicylate intoxication was developed in ferrets to permit the evaluation of the interaction with viruses isolated from patients with Reye's syndrome. Salicylate intoxication produced a mild elevation of the serum glutamic oxaloacetic transaminase and fatty changes in the liver, but these changes differed from those seen in Reye's syndrome on light and electron microscopy. Salicylates were associated with decreased activity of hepatic phosphorylase and a slight depression of activity or ornithine transcarbamylase, a mitochondrial urea cycle enzyme. Infection with influenza viruses produced mild fatty changes in the liver, but did not significantly potentiate the effects of salicylate intoxication on the over-all mortality, the degree of fatty changes, or the hepatic enzymes. Influenza infection alone was not associated with decreased hepatic phosphorylase activity, but was associated with decreased activity of ornithine transcarbamylase. Influenza A was isolated from the livers of two of four animals cultured in embryonated eggs.

Biological parameters of the blood in the genetically obese Zucker rat.

A study of the various biological parameters of the blood in the genetically obese Zucker rat, the nonobese Zucker rat, and the Wistar rat has revealed great similarity between the two latter types of animals. On the other hand, in genetically obese Zucker rats as compared with the nonobese ones, (1) the blood mass per unit of weight was lower; (2) the level of nitrogenous degradation compounds was the same; (3) the lipase activity was lower; (4) the levels of substances for which liver plays a crucial role--all lipid and protein fractions, glucose, and the enzyme GPT--were higher; (5) the levels of Ca, Zn, Fe, Cu and Pi were high; (6) the blood and bone-marrow cells were unremarkable.
