Tissue factor expression and fibrin deposition in the lungs of patients with idiopathic pulmonary fibrosis and systemic sclerosis.

Although abnormalities of alveolar fibrin turnover have been reported to play a role in the development of idiopathic pulmonary fibrosis (IPF), the pathophysiological relevance remains unclear. We therefore investigated the localization of tissue factor (TF) and fibrin deposition in patients with IPF using immunohistochemistry and compared the results with those from patients who had interstitial pneumonia associated with systemic sclerosis (IP-SSc) and idiopathic bronchiolitis obliterans with organizing pneumonia (BOOP). Expression of TF-mRNA was also assessed, using in situ hybridization with a digoxigenin-labeled cRNA probe. In patients with IPF, IP-SSc, and idiopathic BOOP, the TF antigen was positively stained in type II pneumocytes and in some alveolar macrophages. The fibrin antigen was stained in the type II pneumocytes and the adjacent area. Tissue factor-mRNA was expressed in the type II pneumocytes and in some alveolar macrophages. Neither TF antigens nor TF-mRNA were detected in the normal lung. These results indicate that type II pneumocytes are a major source of TF, suggesting that TF production in these cells is closely related to fibrin deposition in the lungs of people with these diseases.

Aldosterone rapidly represses protein kinase C activity in neonatal rat cardiomyocytes in vitro.

Aldosterone lowers protein kinase C (PKC) activity in myocyte-enriched cultures from neonatal Sprague-Dawley rat hearts, with activity measured by the transfer of phosphate to myristolated alanine-rich C-kinase substrate, in the presence of Ca2+, phosphatidylserine, and diolein. The effect is rapid, with a significant effect after 1 min exposure, half maximal at < or = 1 nM aldosterone, with steroids showing a hierarchy of potency aldosterone = 9alpha fluorocortisol > deoxycorticosterone > corticosterone > spironolactone. Both Ca2+-dependent and -independent PKC activity appear equally inhibited by aldosterone, and PMA-stimulated increases in PKC activity appear similarly aldosterone-sensitive. No displaceable binding of [3H]aldosterone to purified PKC can be shown, evidence against a direct effect of aldosterone on PKC; aldosterone does not alter basal or PMA-stimulated PKC activity in cardiac fibroblasts, evidence for a cell-specific mediator of the myocyte effect. Taken with the previous demonstration of the potentiation of aldosterone-specific MR-mediated effects by PKC activation, the present data argue for the existence of a complex cross-talk mechanism between aldosterone and factors affecting PKC activity in the heart.

Production and characterization of monoclonal antibodies against two haptenic derivatives of 1 alpha,25-dihydroxyvitamin D3 conjugated with bovine serum albumin through the C-3 or C-24 position.

To develop the immunochemical methods for determining 1 alpha,25-dihydroxyvitamin D3 [1,25(OH)2D3] in clinical samples, a variety of monoclonal anti-1,25(OH)2D3 antibodies have been generated. Two kinds of hapten-carrier conjugates, 25-hydroxyvitamin D3 3-hemisuccinate (hapten 3-HS) and 1 alpha-hydroxy-25,26,27-trinorvitamin D3 24-oic acid (hapten 24-OA) conjugated with bovine serum albumin, were used for immunization. Spleen cells from SD rats or BALB/c mice, each immunized with the conjugate of hapten 3-HS or 24-OA, were fused with P3/NS1/1-Ag4-1 myeloma cells. After screening by ELISA employing beta-galactosidase-labeled haptens, seven kinds of hybridomas secreting anti-1,25(OH)2D3 antibodies were established. Binding characteristics of these antibodies (Ka 0.73-20 x 10(9) M-1) were investigated by an RIA using tritium-labeled 1,25(OH)2D3. The data suggested that the rat monoclonal antibody 3R-1 derived from the hapten 3-HS and the mouse monoclonal antibody 24M-3 from the hapten 24-OA would be available for developing practical analytical systems.

Assessment of insulin resistance in acromegaly associated with diabetes mellitus before and after transsphenoidal adenomectomy.

With a euglycemic hyperinsulinemic clamp method, whole-body insulin resistance was assessed in 6 cases with acromegaly associated with diabetes mellitus before and after transsphenoidal adenomectomy. The glucose infusion rate (GIR) correlated well with the plasma IGF-I level but poorly with that of GH. Further improvement in insulin sensitivity occurred 3-4 months after operation without substantial changes in plasma levels of both GH and IGF-I or glycemic control. These results indicate that GH excess can induce insulin resistance in association with plasma IGF-I and also through undefined secondary effect.

Pioglitazone (AD-4833) ameliorates insulin resistance in patients with NIDDM. AD-4833 Glucose Clamp Study Group, Japan.

We evaluated the effect of pioglitazone, a thiazolidinedione compound, on insulin-stimulated glucose disposal (Rd) and its efficacy on carbohydrate and lipid metabolism in patients with non-insulin-dependent diabetes mellitus (NIDDM). Twenty NIDDM subjects (mean age 58.2+/-9.4 year, body mass index (BMI) 23.9+/-3.4 kg/ m2 (mean+/-S.D.], three with diet alone, 17 with sulfonylureas [SU]) participated in this trial from five diabetes clinics. Euglycemic (5.3 mmol/liter) hyperinsulinemic (insulin infusion rate 9 micromoles x kg[-1] x min[-1]) clamp studies were performed before and after oral administration of pioglitazone (30 mg/day) for 87+/-10 days. The Rd significantly improved from 5.5+/-2.5 to 8.3+/-3.1 mg x kg(-1) x min(-1). Fasting plasma glucose (FPG) level significantly decreased from 11.0+/-2.0 mmol/liter to 8.9+/-1.1 mmol/liter with a significant improvement in the hemoglobin A1c level from 9.2+/-1.8% to 8.3+/-1.5%. Fasting serum insulin and C peptide levels decreased from 83+/-36 pmol/liter and 0.62+/-0.21 nmol/liter to 66+/-29 pmol/liter and 0.58+/-0.25 nmol/liter, respectively. Fasting serum triglyceride and free fatty acids levels significantly decreased with concomitant increase of fasting serum HDL-cholesterol levels from 1.2+/-0.2 to 1.5+/-0.3 mmol/liter. The change in Rd between before and after pioglitazone administration correlated with baseline values of FPG (rho=0.633), serum insulin (rho=0.653), BMI (rho=0.456), Rd (rho 0.558) and 1,5-AG (rho=-0.522). These data indicate that pioglitazone enhances the insulin action in NIDDM patients on diet alone or SU, and thereby improves both plasma glucose level and lipid profiles.

Stiff-man syndrome associated with antecedent myasthenia gravis and organ-specific autoimmunopathy.

We describe a case of stiff-man syndrome accompanied by diabetes mellitus, Hashimoto's thyroiditis and the antecedent myasthenia gravis. The diagnosis of stiff-man syndrome was made based on not only clinical findings and the characteristic electromyographic pattern but also the presence of antibodies to glutamic acid decarboxylase in the serum and cerebrospinal fluid. Stiff-man syndrome is known to be associated with organ-specific autoimmunopathy including insulin-dependent diabetes mellitus. The present case is the first one that stiff-man syndrome was preceded by myasthenia gravis of organ-specific autoimmunopathy. Stiff-man syndrome in the present case probably represents the one of fully expressed manifestations from the broad spectrum of organ-specific autoimmunopathy caused by the loss of self-tolerance.

Assessment of the health effects of trichloroethylene.

The epidemiological studies performed thus far have presented only limited evidence for the carcinogenicity of trichloroethylene (TRI) to humans. However, these studies had drawbacks such as insufficient size of cohort, short observation period, and inadequate TRI exposure assessment; therefore, no concrete conclusion has been reached concerning TRI carcinogenicity to humans. Despite the limited epidemiological evidence as to the carcinogenicity of TRI, the International Agency for Research on Cancer (IARC) has changed the carcinogenicity classification of TRI from Group 3 (not classifiable as to carcinogenicity to humans) to Group 2A (probably carcinogenic to humans). In regard to the new classification by the IARC, the committee for occupational exposure limits of the Japan Society for Occupational Health has made a proposal that it is too early to classify the carcinogenicity of TRI as Group 2A and that it is proper to promote exposure control with the carcinogenicity being classified as 2B for the moment. There are species differences in TRI carcinogenicity, particularly between rats and mice. Although experimental studies have found no evidence that TRI induces liver cancer in rats, there is ample evidence that TRI promotes the development of liver cancer in mice, particularly in B6C3F1 mice. The carcinogenicity of TRI in this strain of mice may be based on an epigenetic mechanism rather on a genotoxic mechanism and the liver cancer may be induced only after TRI has been inhaled for a long period of time at concentrations high enough to cause cytotoxicity. Conversely, with no reports showing TRI-induced renal tumors in mice, the possibility has been suggested that this chemical induces such tumors in male rats. The species differences are mainly accounted for by differences in the metabolism of TRI between rats and mice. From a general survey of the literature, it can be concluded that TRI itself is not mutagenic. However, the conjugation of TRI with glutathione (GSH), a minor pathway of TRI metabolism, results in mutagenic metabolites in the kidney of rats. The acute toxicity of TRI is neurotoxicity based on its anesthetic action. An exposure to extremely high levels of TRI may cause the liver and kidney disorders. Repeated exposures to high levels of TRI may result in neuro-, hepato-, and/or nephrotoxicity. The main symptoms appearing after chronic exposure at low levels are neurological changes represented by subjective symptoms relating to central and autonomic nervous systems, or by a lowered conduction velocity of the nerves or a prolonged latency of the nerve responses. For the present, it is reasonable to use the neurological findings for establishing the reference values of TRI for both work and general environments. A value of 25 ppm (135 mg/m3) is proposed as a reference value for work environments, and 25-50 ppb (135-270 micrograms/m3) for the general environment (1/1,000 of the value for work environment).

Lisinopril improves endothelial dysfunction in hypertensive NIDDM subjects with diabetic nephropathy.

Endothelial dysfunction is a prevalent phenomenon in non-insulin dependent diabetic (NIDDM) patients with hypertension and albuminuria, and may contribute to the development and progression of cardiovascular disease, which is the main cause of the high morbidity and mortality observed in these patients. Therefore the aim of our study was to evaluate whether inhibition of angiotensin-converting enzyme (with lisinopril 10-20 mg day-1) could ameliorate endothelial dysfunction more than reducing blood pressure with conventional antihypertensive treatment (atenolol 50-100 mg day-1), usually in combination with a diuretic. We performed a 12-month prospective, randomized, double-blind, parallel study in 43 hypertensive NIDDM patients with diabetic nephropathy (21 treated with lisinopril and 22 with atenolol). The following variables were measured: 24-h ambulatory blood pressure (ABP); transcapillary escape rate of albumin (TERalb; i.e. initial disappearance of intravenously injected 125I-labelled human serum albumin); serum concentrations of von Willebrand factor (vWF), using ELISA, and urinary albumin excretion rate (UAE). Data are presented for 32 patients (16 lisinopril and 16 atenolol; age 60 years, SD 8; 25 males) out of 35 who completed the study and had valid measurements of TERalb. At baseline the two groups were comparable; TERalb (8.5 (SEM 0.6) vs. 7.2 (0.4)%); vWF (2.09 (range 0.82-4.34) vs. 1.97 (0.95-3.86) IU ml-1; UAE 916 (x/divided by antilog SEM 1.3) vs. 1444 (1.2), and mean ABP 110 (SEM 3) vs. 113 (2) mmHg, in the lisinopril and atenolol group, respectively. During follow up, the mean ABP was equally reduced in the lisinopril and atenolol group, by 12 (SEM 2) vs. 10 (2) mmHg, respectively, TERalb decreased in the lisinopril group by 0.6 (SEM 0.7)%, whereas it increased in the atenolol group 1.5 (0.5)%; the mean difference was 2.2% (95% CI, 0.5 to 3.9; p = 0.015). UAE was reduced by 45% (95% CI, 25 to 60) in the lisinopril group vs. 10% (-15 to 30) in the atenolol group (p = 0.014). Serum vWF was not changed during follow up in either group. Our study suggests that lisinopril has both reno- and vasculoprotective properties in hypertensive NIDDM patients with diabetic nephropathy.

The cervical subluxation and regional cerebral blood flow.

OBJECTIVE: This article specifically addresses the question of whether the manipulable cervical lesion is likely to cause extrinsic compression of the vertebral arteries sufficient to cause such symptoms of reduced regional cerebral blood flow as might be relieved by spinal manipulation. DATA SOURCES: Literature on normal and abnormal cerebral circulation, including vertebrobasilar insufficiency. DATA SYNTHESIS: Signs and symptoms produced by extrinsic compression of the vertebrobasilar system have been compared with those attributed elsewhere to the manipulable cervical lesion (cervical subluxation). RESULTS: Extrinsic compression of the vertebrobasilar system generally does not produce signs and symptoms consistent with those attributed to the manipulable cervical lesion. CONCLUSION: It has been hypothesized elsewhere that the manipulable cervical lesion may induce localized decreases in regional cerebral blood flow, and so signs and symptoms attributable to "cerebral hibernation." If a causal relationship does exist between the cervical subluxation and reduced regional cerebral blood flow, it is not likely to be caused by mechanical compression of the vertebral arteries.

Malignant cystosarcoma phyllodes tumor of the breast mainly composed of chondrosarcoma: a case report.

Sarcoma of the breast is a very rare occurrence. We experienced a patient with malignant phyllodes tumor, in which a chondrosarcomatous element constituted 90% of the tumor volume. That bone or cartilage may be present in mammary neoplasms has been known for a long time. Most mammary tumors with areas of chondroid metaplasia show a benign histologic appearance. Cystosarcoma phyllodes displaying a chondrosarcomatous element is very rare. The results obtained by echographic and pathologic examinations strongly suggest that the chondrosarcomatous element originates from the fibrous stroma.

[Pulmonary infection with Mycobacterium kansasii presenting as solitary nodule shadow in the left anterior basal segment].

A 71-year-old man was admitted to our hospital for evaluation of a solitary pulmonary nodule in the left lower lung field. He had been treated for Parkinson's disease for two years, and had no respiratory symptoms on admission. No abnormal findings were detected in laboratory tests on admission, and Mantoux's skin test was negative. A postero-anterior chest roentgenogram, a conventional tomogram, and a computed tomogram showed that the nodule was located in the left anterior basal segment (S8). The nodule was not calcified, the contour was clear but irregular, and pleural indentation could be seen, so the nodule was strongly suspected to be a primary malignant lung tumor. Because two attempts at transbronchial lung biopsy and bronchial brushing and washing were of no diagnostic value, a thoracotomy was done. The lesion was found in the subpleural region of the left S8 with pleural indentation, and it was partially resected. The nodule was elastic, soft, and filled with suppurative fluid. Histopathologic examination of the nodule revealed epithelioid cell granuloma. A smear test of the fluid was negative but a culture was positive for mycobacteria; which were identified as Mycobacterium kansasii. Rifampicin and isoniazid were administrated to the patient for 1 year. Two years after the operation, the patient was asymptomatic.

Indication of chemoembolization therapy without gelatin sponge for hepatocellular carcinoma.

This study evaluated the effect of chemoembolization (C-LIP) consisting of ethiodized oil (Lipiodol Ultra Fluid; André Guerbet, Aulnay-sous-Bois, France) and epirubicin, without gelatin sponge on hepatocellular carcinoma (HCC), administered by hepatic arterial infusion. We analyzed the cases from two points of view: the local recurrence rate for hypervascular solitary small HCC (tumor size: < or =3 cm in diameter) and the cumulative survival rate for advanced HCC (stage VI according to the criteria of Liver Cancer Group of Japan) following C-LIP therapy. The C-LIP also was compared with transcather arterial embolization (TAE; C-LIP followed by gelatin sponge) and percutaneous ethanol injection therapy (PEIT). In the small HCC cases, the recurrence rate at 1 year after C-LIP was 77% (10 of 13 patients), while the local recurrence rate was 46% (six of 13 patients) at 6 months and 61% (eight of 13 patients) at 1 year. The local recurrence rate at 1 year was 29% (four of 14 patients) after TAE and 20% (three of 15 patients) after PEIT. These results showed that the effect of local anticancer therapy by C-LIP was not as potent as that of TAE or PEIT. In advanced HCC cases, the cumulative survival rate for 13 patients treated by C-LIP was 72% at 6 months, 36% at 1 year, and 14% at 2 years. However, the survival rates for 13 patients at 6 months, 1 year, and 2 years after TAE were 46%, 23%, and 8%, respectively. There was no difference between the C-LIP patients and TAE patients with regard to the pretreatment liver function. Three patients died within 2 months after the initial TAE. These deaths were mainly due to damage to the noncancerous liver parenchyma. Therapy with C-LIP alone was not appropriate for hypervascular solitary small HCCs, and additional treatment was necessary. We think C-LIP therapy should be selected instead of TAE for advanced HCCs to avoid severe parenchymal damage.

[Clinicopathological features of interstitial pneumonia associated with amyopathic dermatomyositis].

We studied clinicopathological characteristics of interstitial pneumonia associated with amyopathic dermatomyositis. The subjects comprised two men and three women, and their mean age was 58.2 years. All subjects had cruptions specific for dermatomyositis, but had no signs of myositis. They all presented with acutely or subacutely developed coughing and dyspnea. Results of tests for anti-Jo-1 antibody were negative in all cases. Chest X-ray films showed infiltrations or streaky shadows, or both in the middle and lower lung fields. Analysis of bronchoalveolar lavage fluid revealed abnormally high percentages of lymphocytes and neutrophils. In one patients a specimen obtained by open lung biopsy showed homogeneous cell infiltrations in alveolar septa and regional alveolar damage. That patient was successfully treated with cyclosporin and corticosteroids in early phase of the disease. The other four patients received immunosuppressive agents after respiratory failure developed. All four died despite having received high-dose corticosteroid and immunosuppressive therapy. Examination of autopsy specimens showed diffuse alveolar damage.

[Incidence and clinical features of lung cancer in patients with idiopathic interstitial pneumonia].

We studied the incidence of lung cancer in the 599 patients with idiopathic interstitial pneumonia (IIP) who had visited to 37 institutions during 1991. In addition to 89 patients with lung cancer registered initially 29 were found during more than 4 years of follow-up, which indicates that annual incidence of lung cancer in these patients with IIP was a few percent. A total of 118 lung cancers accounted for 19.7% of all the IIP patients registered and also for 27.2% of the 435 IIP patients in whom a definite outcome was certified by this study. Most of the patients with lung cancer were older men who were heavy smokers. The distribution of histological types was not noteworthy. On the chest rentgenogrms, the tumors were found predominantly in the peripheral lung fields even in the patients with squamous cell cancer, but no apparent association with IIP lesion was observed. Surgical resection was done in only 52% of the patients with the disease at stage I or II; chemotherapy and/or radiotherapy were given to 52.4% of the patients with advanced disease, which may have been due to considerations of the adverse effects of anticancer treatment on the prognoses. The mortality in IIP patients with lung cancer was higher than in those without lung cancer, mainly due to progression of the lung cancer, not the IIP. This indicates the importance of lung cancer as a prognostic factor in patients with IIP.

[Systemic sarcoidosis with involvement of the spermatic cord].

A 49-year-old man presented with an enlarging, painful, right inguinal mass and small red nodules around his nose and upper lip. He had undergone an operation for a right inguinal hernia in childhood. Because malignancy was suspected, the right spermatic cord, testis, and epididymis were resected. Histological examination of the lesion in the spermatic cord revealed non-caseating epithelioid-cell granuloma and no acid-fast bacilli, which was compatible with sarcoidosis. A chest X-ray film and computed tomograms showed enlarged hilar lymph nodes and reticular infiltrates. Histological examination of specimens obtained by transbronchial lung biopsy and by skin biopsy also showed non-caseating epithelioid-cell granulomas. After resection of the spermatic cord, the activity of angiotensin-converting enzume was 29.9 IU/L, which was above the reference range. Systemic sarcoidosis with involvement of the spermatic cord was diagnosed. Because he had no respiratory symptoms and his skin lesions resolved without treatment, he is currently under observation and is not receiving corticosteroid therapy.
