A comparative trial of timolol and flutonidin as antihypertensive agents.

In a double-blind cross-over clinical trial, flutonidin (2-4 mg daily) was compared with timolol (20-40 mg daily) in the treatment of mildly or moderately hypertensive patients who concomitantly received fixed diuretic treatment. Each drug was administered for 1 month, with an interval of 2 weeks between the two monthly periods. During administration of flutonidin, blood pressure initially fell but returned to baseline values at the end of treatment. Heart rate was not affected by the drug. Timolol significantly reduced both blood pressure and heart rate. Its effect becomes evident during the 1st week and increases throughout the treatment period. Side effects were more frequent during flutonidin administration than during timolol administration. No significant modifications of the laboratory findings were observed during either flutonidin or timolol treatment.

Comparative study of oxdralazine and dihydralazine in essential hypertension.

Thirty-one patients with moderate and severe essential hypertension completed a comparative study of oxdralazine and dihydralazine. Treatments were administered for a 6-month period. The therapeutic regimen also included diuretic and a beta-blocking agent given at a constant daily dose. After a 4-week run-in period on diuretic plus beta blocker, which caused a mean blood pressure reduction of 17/7 mmHg, the administration of the vasodilators produced a further, more marked decrease of pressure values. After 1-month treatment mean blood pressure reductions were 26/18 mmHg on oxdralazine and 16/11 mmHg on dihydralazine. After 6 months, 10/15 patients on oxdralazine and 7/16 patients on dihydralazine had achieved a stabile diastolic pressure below 95 mmHg. These results were confirmed by an additional phase of the study, in which 21 patients had the vasodilator cross-over and were observed for a second 6-mont period. Oxdralazine and dihydralazine therapy were well tolerated: only a few patients complained of side effects that were of mild intensity and never required any change in treatment. At the first visit after the cross-over, those patients changing from oxdralazine to dihydralazine complained of side effects with a higher frequency than the patients changing from dihydralazine to oxdralazine. In one patient from the former group it was necessary to stop dihydralazine treatment.

Long-term study of oxdralazine in hypertensive patients.

56 moderate and severe hypertensive patients entered an open long-term trial aimed at evaluating the efficacy and tolerability of a combined treatment consisting of 3-hydrazino-6-[N,N-bis-(2-hydroxyethyl)amino]pyridazine (oxdralazine, L 6150), propranolol and chlorthalidone. The mean basal blood pressure was 186.9/111.8 mmHg: after one month of treatment, the mean value was 149.7/95.5 mmHg (p less than 0.01). The heart rate was practically unaffected by treatment, the mean value changing from 75.4 b.p.m. (basal) to 73.3 b.p.m. (one month). The significant reduction in the blood pressure observed at the end of the first month remained unchanged in the following months of therapy, and only minor variations occurred. The combined treatment was well tolerated. Five patients were withdrawn from the trial during the first month; three of them because of side-effects and two of them for personal reasons.

Evaluation of separate renal function by means of 99mTc-aprotinin uptake test.

The possibility that relative kidney uptake of technetium-99m aprotinin (TcA) might be indicative of separate renal function was investigated in 89 patients who underwent both effective renal plasma flow (ERPFs) and glomerular filtration rate (GFR) determination. A reference group consisted of 27 healthy volunteers, studied only with TcA. The correlation with ERPFs (r = .73) was similar to that previously reported and confirmed. The correlation with GFR (r = .68) was better if a subgroup of renal units with TcA uptake lower than 16% (lower normal limit) was considered. Most likely, glomerular filtration is a limiting factor of the tubular uptake of TcA, and when GFR is reduced, both parameters decrease in the same manner, while if GFR is normal the two parameters are relatively independent. The correlation between TcA and GFR in 32 children was very similar to that found in adults. TcA uptake test seems to be a useful indicator of separate renal function, providing morphological information at, the same time.

Immunochemical characteristics of a particular cryoglobulin. A new cryoglobulin subgroup?

In a patient (BAR) affected by chronic active hepatitis we isolated a cryoglobulin constituted of polyclonal IgM (k and lambda) and a monoclonal IgG3 lambda. The IgM represented the antibody of the cryoglobulin complex. This type of cryoglobulin, where the monoclonal component is the antigen and not the antibody, cannot be correctly classified using the nomenclature of Brouet et al. (1) currently in use. In this patient a two-year follow-up excluded any clinical signs of cryoglobulin toxicity. The immunochemical and clinical characterization of other cryoglobulins similar to BAR could establish a new homogeneous group.

[Determination of aluminum in the blood: extra-analytical factors].

Values of Al in serum or plasma of normal subjects reported in the literature vary greatly. The wide range reflects the analytical difficulties and large contamination problems. In order to obtain accurate results we report a direct method for determination of Al-S(P) by using flameless atomic absorption spectrophotometry. (ETA-ASS). Special attention was paid to the precautions to be taken in order to minimize external aluminum contamination. Procedures are described to reduce contamination during sample collection, storage and preparation during sample collection, storage and preparation of sample. Reference values for a healthy population were 7.4 + 2.7 (S.D.) micrograms of Al per liter. (n = 78).

[Functional state and indications for a rehabilitation program of patients in a dialysis center: a multidisciplinary study with ergonomic, physiatrical and psychological evaluation].

The following study describes the functional status of the entire chronic hemodialysis ambulatory population in treatment at our center. The patients were evaluated from 3 points of view: ergonomic, phyiatric and psychological, thus going a snapshot picture in time of their status. Based on these three evaluations, the need for rehabilitation intervention was determined and rehabilitation treatment program was elaborated. The ergonomic evaluation determined the maximal sustained workload (MSW) from O2 consumption, minute ventilation, heart rate, subjective perception of fatigue and dyspnea. The phyiatric examination included a thorough musculoskeletal examination, application of a functional ability instruments (Karnofsky + FIM) and then integrated the results with the ergonomic + psychologic testing in order to generate phyiatric prescription. The psychologist had a conversation with all patients and applied 2 instruments for evaluation of health-related quality of life (CBA-H + SIP). The results of these studies indicated a markedly compromised aerobic capacity, which worsened with age in years rather than with years in dialysis. Phyiatric prescription were considered necessary in 78% of the patients with major goals being re-conditioning and joint mobilization in order to diminish the fragility of independence in this population. The psychological arm of the study revealed good adaptation to chronic dialysis in 75% of the population, with however absorption of a passive lifestyle. The remaining 25% of the population showed severe psychological distress and social withdrawal. Psychological support for this population is necessary to identify families at risk for stress and poor adaptability as well as to help motivate patients who require rehabilitation to participate in therapy.
