Virus antibody levels in serum specimens from 77 patients with optic neuritis (ON) were compared with those in healthy controls which had been matched with regard to sex, age and place of residence. A group of 58 patients with various neurological diseases other than multiple sclerosis (MS) or infectious diseases of the central nervous system (CNS) served as a second control group. The ON patients had significantly higher measles antibody titers in serum than the two control groups in both measles HI and HLI tests. Tests of cerebrospinal fluid (CSF) specimens revealed similar differences between ON patients and controls. Tests for antibodies to other viruses showed no statistically significant differences between ON patients and controls. There were several ON patients with normal serum/CSF albumin ratios but low serum/CSF IgG and measles antibody ratios. An increase in measles antibody CSF titers was observed during the study time in two ON patients. The results support the hypothesis that local production of measles antibodies takes place in the CNS of some patients with ON as has been earlier suggested to occur in patients with MS.

The possible immunosuppressive properties of the combination sulphamethoxazole-trimethoprim or either of the agents alone were studied in healthy human volunteers. The effect of the 4-day drug treatment on the antibody synthesis after vaccination was measured by titrating tetanus antitoxin, mumps antibody and Salmonella Hb antibody levels 4 and 5 weeks after vaccination. No significant differences were found in mumps or Salmonella antibody titres in the drug-treated group compared to controls. Tetanus antitoxin levels, however, were significantly lower than in controls after the combined drug. When a fourfold or greater increase in tetanus antitoxin titre was obtained in 75% of controls, the respective figures were 38% in the sulphamethoxazole-trimethoprim-treated group (p less than 0.05), 45% in the trimethoprim group (p greater than 0.05), and 50% in the sulphamethoxazole group (p greater than 0.05). The implications of these findings are discussed.

Rubella and influenza A (H3N2) haemagglutination inhibition (HI) antibody titres and measles complement-fixing (CF), haemagglutination inhibition (HI), haemolysis inhibition (HLI), and ribonucleoprotein gel precipitation (RNP-GP) antibody titres were studied in the serum and synovial fluid of twenty patients with rheumatoid arthritis (RA), two patients with ankylosing spondylitis, and two patients with Reiter's syndrome. Antibody titres were also studied in the serum and CSF of four patients with systemic lupus erythematosus (SLE), one patient with dermatomyositis, and in the synovial fluid only of five patients with osteoarthritic knee effusions. Antibodies were found with each serological technique used in the synovial fluid of RA patients and the antibody titres were usually at about the same level as in the serum. The mean measles (HI, HLI, and RNP-GP) antibody titres were 4 to 6 times higher in the synovial fluid of RA patients than in synovial fluid of patients with osteoarthritic knee effusions, but a corresponding difference was not found in rubella and influenza A antibody titres. The mean measles antibody titres (CF, HI, HKI, and RNP-GP) were consistently higher in the synovial fluid of RA patients without rheumatoid factor than in the synovial fluid of RA patients with rheumatoid factor. In serum this difference was observed only with measles CF titres. The mean measles, antibody titres were consistently lower in the serum and synovial fluid of the RA patients without the synovial fluid haemolytic complement than in the RA patients with this haemolytic complement. No similar differences were found in the rubella and influenza antibody titres. No significant measles antibody titres were found in the CSF of patients with SLE or dermatomyositis.

The occurrence of a local production in the central nervous system (CNS) of antibodies against different selected viruses was analyzed by comparison of titers in serum and cerebrospinal fluid samples from groups of 50 patients with multiple sclerosis from Finland, Norway, and Sweden. Measles antibodies were determined in hemagglutination inhibition, hemolysis inhibition, and nucleocapsid complement fixation tests; mumps, parainfluenza virus type 1, and rubella virus antibodies were determined in hemagglutination inhibition tests; and herpes simplex virus type 1 antibodies were determined in passive hemagglutination tests. For reference purposes tests were also made for adenovirus antibodies in penton hemagglutination enhancement tests and poliovirus antibodies in neutralization enhancement tests. Among the 150 multiple sclerosis patients, a local production of antibodies against measles virus was found in the CNS in 57%, against rubella virus in 19%, mumps virus in 15%, herpes simplex virus type 1 in 11%, and parainfluenza virus type 1 (Sendai) in 3%. A local production in the CNS of antibodies against any of the viruses studied was found in 71% of multiple sclerosis patients. These included 48, 16, and 7% that produced antibodies to one, two, and three or more viruses, respectively.
