Alterations in tyrosine and protein kinetics produced by injury and branched chain amino acid administration in rats.

1. To determine whether the alterations in amino acid metabolism after injury were a result of changes in protein synthesis, whole body tyrosine and individual tissue protein kinetics were estimated 24 h after three different forms of stress in rats. 2. In addition, injured rats were either starved or infused with 180 mg of nitrogen/day (as branched chain amino acids or L-alanine) to ascertain whether the mechanism and degree of nitrogen sparing were unique to branched chain amino acid administration or whether they could be attributed to the infusion of alpha-amino nitrogen. 3. In the more catabolic types of injury, increased nitrogen loss during starvation appeared to be due to both an increased plasma amino acid appearance and a greater percentage being oxidized. Rates of tyrosine incorporation into whole body protein were also enhanced and could be explained in part by increases in the fractional synthesis rate of hepatic non-secretory protein. 4. Both branched chain amino acid and L-alanine administration reduced endogenous tyrosine oxidation and improved nitrogen balance. However, branched chain amino acid administration significantly increased amino acid incorporation into whole body protein and fractional synthetic rates of skeletal muscle, kidney and hepatic non-secretory protein. 5. It is concluded that the catabolic response to severe injury is consistent with increased rates of plasma amino acid appearance and branched chain amino acid administration spares body protein by improving amino acid utilization for whole body protein synthesis.

Submucosal hematoma producing mechanical obstruction in the sigmoid colon, complicated by systemic amyloidosis.

A case of systemic amyloidosis involving the gastrointestinal tract is presented. The initial manifestation of this case was mechanical obstruction. On laparotomy, a submucosal hematoma of the sigmoid colon which completely obliterated the lumen, was found. With intense medical treatment, the obstructed lumen became patent, but segmental ischemic colitis ensued. The terminal course of this case was complicated by chronic renal failure, upper gastrointestinal bleeding and coagulopathy. Pathological examination of the stomal specimen revealed massive amyloid deposits in the wall of the large intestine as well as other vital organs.

[Nodal psammoma body in thyroid cancer as an indicator of cancer metastasis to the lymph node].

A 64-year-old woman underwent subtotal thyroidectomy and cervical lymphadenectomy for thyroid carcinoma. In 5 of 15 resected nodes, histologic examination revealed metastasis. The other 10 nodes showed no metastasis, though psammoma bodies were seen in 3 nodes. To clarify the relationship between psammoma bodies and metastasis, 10 cancer-negative nodes were serially sectioned (4 microns in thickness). This disclosed cancer cells and psammoma bodies in 2 to 3 psammoma body-positive nodes. We suggest that the presence of psammoma bodies in the lymph node may indicate metastasis.

Carcinoma, Papillary↗

[Multiple primary malignancies associated with thyroid cancer].

The characteristics of multiple primary carcinomas associated with thyroid carcinoma were studied in 35 patients (35/362; 9.7%) who met the clinicopathological criteria of Warren and Gate. The frequency of the association was similar in papillary and follicular thyroid carcinomas. The average patient age at the time of initial carcinoma detection was 60.3 years for males and 51.5 years for females. The most frequent sites of the associated cancers were breast, uterine cervix and uterine body in females, and stomach and larynx in males. Patients should be followed closely for recurrence and second cancer detection in the early stage.

Adenocarcinoma↗

[Sequential histological studies of prostatic adenocarcinoma after treatment].

Serially obtained needle biopsy specimens of 30 prostatic adenocarcinoma patients were examined to clarify the therapeutic effect on histology. Histology of the tumours after radiotherapy (Linac, 6,440 rad on average) revealed gradual degeneration without the formation of massive necrosis. The rates of cancer-free cases were 22.2% and 41.2% less and more than 1 year after radiation therapy, respectively. The 5-year survival rate was 53.3%. Of 10 patients who survived 5 years, all died of recurrence within 10 years. This indicates the necessity for longterm follow up in prostatic cancer patients.

[Malignant transformation of thyroid cancer; poorly differentiated cancer and anaplastic cancer transformation].

The prognosis of thyroid cancer depends largely on histological differentiation and clinical stage. The prognosis in anaplastic cancer is worst, in well differentiated cancer it is best; poorly differentiated cancer takes a middle position. The prognosis of patients with differentiated cancer is generally good, however, there are cases with malignant transformation such as poorly differentiated, anaplastic and squamous-cell cancer transformation. We studied the frequency and the inducement factors of malignant transformation in patients with thyroid cancer. The frequency of poorly differentiated, anaplastic and squamous-cell cancer transformation was 13.6%, 6.4% and 0.7%, respectively. In anaplastic cancer transformation, irradiation is an important factor.

[Clinical application of futraful suppositories in malignant head and neck cancer--effects of the therapy combined with radiation].

Futraful suppository 1.5 g per day was administered to 33 patients with head and neck malignancies. Among 33 cases which entered into the study, 16 cases were evaluated. Out of 16 evaluable cases, complete responses (CRs) were obtained in 4 cases and partial responses (PRs) in 7 cases, an overall response rate including 4 CRs and 7 PRs was 68.8%. Side effects due to FT-207 suppository were developed in 14 cases out of 33 patients (42.4%). Side effects such as anal pain, anorexia, diarrhea, pigmentation, exanthema and nail change were observed. No severe side effect was encountered.

Significance of abnormal gastrointestinal gas in systemic amyloidosis.

Nine cases of amyloidosis involving the gastrointestinal tract were reviewed for the presence of abnormal gastrointestinal gas by taking plain roentgenograms of the abdomen (PRA) as a diagnostic adjunct. Findings were compared to data obtained by conventional barium study or endoscopic examination. PRA revealed marked gaseous dilatation of the gastrointestinal tract simulating intestinal obstruction in three cases and a moderate or mild increase of gas in two cases each. Abnormality was thus found in seven of the nine cases by PRA. Conventional barium study by means of upper gastrointestinal series, small bowel series and barium enema showed abnormal findings in five of seven cases. Endoscopic examination of the stomach and distal colon disclosed mucosal abnormality in three of seven cases. The rate of abnormal findings detected by the three methods cannot easily be compared to each other because of the differences in symptoms and treatment of individual patients. Nevertheless, PRA seem to be valuable in demonstrating a high rate of abnormalities in amyloidosis.
