[The effect of representation of a person on the impression rating].

The relationship between representation of a person and evaluative impression of that person was investigated by presenting two stimulus persons, each by six trait adjectives, to subjects of the impression group, memory group, and category group. The representation was measured by two kinds of RTs and the amount of recall, and the evaluative impression by ratings on S-D items. When subjects' representation of a stimulus person contained less information for impression ratings, the resulting impression was stable and consistent with the implicit personality theory. As the amount of the information increased in the order of the impression, category, and memory groups, however, the stability of the impression ratings declined even for the same stimulus person.

[Dubin-Johnson syndrome associated with hepatocellular carcinoma--report of an autopsy case].

An autopsy case of Dubin-Johnson syndrome (DJS), associated with hepatocellular carcinoma (HCC), the third such case in Japanese literature, is reported. A 64-year-old man, known to have had jaundice from his childhood, was admitted because of weight loss. Close examination revealed that the patient had DJS with HCC and a right lobectomy of the liver was performed. He died, however, of metastasis of HCC about 36 months later. At autopsy, the liver was found to be brownish-black in color. Microscopic findings of the liver were those common to HCC and chronic active hepatitis, and the brown pigment seen in the hepatocytes was mainly confined to the centrilobular ares. After histochemical and electronmicroscopic study, a close relation between this pigment and lysosome is suspected.

[Metastatic brain tumor after chemotherapy of ovarian cancer].

A case of brain metastasis in ovarian cancer is reported. Recently cis platin-containing combination chemotherapeutic regimens have improved the prognosis of patients with advanced ovarian carcinoma. Metastasis to the brain in ovarian cancer is very rare. However, the incidence of cerebral metastasis after chemotherapy for ovarian carcinoma seems to be increasing, and this may be related to prolonged survival due to chemotherapy. Thus, neurological examination after chemotherapy with cis platin is necessary to detect metastases to the brain.

[Clinical study of primary carcinoma of the fallopian tube: experience with 14 cases].

Between 1950 and 1986, 14 cases of primary carcinoma of the fallopian tube were treated and diagnosed at the Cancer Institute Hospital. These cases constituted 0.13% of the total number of gynecologic malignancies at the hospital during the period. The clinical/pathological findings and prognoses were described. Of the 14 cases, the average age was 56.0 years. The most frequent symptom was atypical genital bleeding, seen in 11 cases (79%). Massive watery discharge was seen in four cases (29%). In preoperative cytologic examination of vaginal smears, six cases (43%) were positive for cancer. All cases underwent operation as therapy. Postoperative irradiation, adjuvant chemotherapy, and/or second-or third- look operation was also used. Histopathologically, all materials were found to be adenocarcinoma. Four cases were well differentiated, seven were moderately differentiated, and three were poorly differentiated. Two patients with stage III and IV cancers died of the disease. Nine patients were still alive at the end of this study. The five-year survival rate was 57% (4/7). In stage I cancers, the five-year survival rate was 80% (4/5). The prognosis of stage I cancer patients was estimated as rather good.

[Histology and prognosis of endometrial carcinoma].

The relationship between the histology and prognosis has been analyzed, using 608 surgically removed uterus with endometrial carcinoma. Among the uterus studied, 506 cases (92%) were determined as being an adenocarcinoma, endometrial type, including adenoacanthomas. This type of carcinoma showed a favourable postoperative survival rate (83.3%), when compared to serous adenocarcinomas (65.8%), clear cell (mesonephroid) adenocarcinomas (32.4%), and adenosquamous carcinomas (63.7%). Among adenocarcinomas of the endometrial type, these with a well differentiated variant showed a better prognosis than others. The squamous component of the adenoacanthoma was found to play no significant role in the prognosis.

[Clinico-pathological study of atypical adenomas of the thyroid].

A total of five cases of atypical adenomas of the thyroid were clinicopathologically examined to clarify their malignant potential and incidence. These adenomas showed both structural and cellular atypia when compared with follicular adenoma of the usual type, A follow-up study after surgery revealed neither metastasis nor postoperative recurrence. Therefore it has been concluded that an atypical adenoma of the thyroid can be regarded as a benign neoplasm.

Immunoreactive neuromedin B and neuromedin C: distribution and molecular heterogeneity in rat and human tissue extracts.

Immunoreactive neuromedin B and neuromedin C were characterized and measured in rat and human tissue extracts, using radioimmunoassay combined with gel filtration and high performance liquid chromatography in order to clarify tissue distribution and molecular structure. There are two distinct systems of bombesin-like peptide found in rat and human tissue. The neuromedin B family, including neuromedin B and big neuromedin B, is a major bombesin-like peptide in the brain; the neuromedin C gastrin-releasing peptide family is found mainly in the alimentary tract. Neuromedin B is the major and big neuromedin B the minor constituent of central nervous neuromedin B immunoreactivity. Gastrointestinal neuromedin C immunoreactivity is composed chiefly of neuromedin C in rats, and two N-terminally extended forms of neuromedin C including gastrin-releasing peptide in man. It is probable that molecular heterogeneity reflects species variation.

Quantification of disease activity in patients with ulcerative colitis.

To develop an efficient index for evaluating ulcerative colitis, we quantitatively assessed disease activity by multiple regression analysis. Thirty-two consecutive cases with ulcerative colitis were studied. These cases were divided by degree of severity into three groups: mild, moderate, and severe; this tripartite grouping was used as a dependent variable. As independent variables, 11 predictors were selected from among physical findings and laboratory data. By multiple regression analysis with stepwise deletion, six factors indispensable to the disease activity were selected: temperature, pulse rate, erythrocyte sedimentation rate, red blood cell count, alpha 2-globulin, and number of bowel movements per day. Disease activity is predicted by using the following equation: Y = -12.168 + 0.333X1 + 0.022X2 + 0.018X3 - 0.337X4 + 0.056X5 + 0.033X6, where X1, X2, X3, X4, X5, and X6 denote temperature, pulse rate, erythrocyte sedimentation rate, red blood cell count, alpha 2-globulin, and number of bowel movements per day, respectively. By applying this equation to each case, the range of activity indices is expressed as follows: mild, less than 1.52; moderate, between 1.52 and 2.44; and severe, more than 2.44.

Clinical evaluation of bisoprolol in the treatment of extrasystoles and sinus tachycardia: an interim report.

The effectiveness and safety of bisoprolol, a newly developed beta-adrenergic blocking drug, in the treatment of arrhythmias were evaluated in 17 patients with premature ventricular contraction (PVC), 8 with premature atrial contraction (PAC) and 7 with sinus tachycardia. More than 50% reduction of the PVC frequency was observed in 7 out of 16 patients. PVC was reduced in 2 out of 5 patients at a daily dose of 2.5 mg. The PAC frequency was decreased in 50% of the patients, and sinus tachycardia was improved in all 7 patients. Adverse reactions were observed in 8 of 32 patients. This preliminary study suggests that the antiarrhythmic effects of bisoprolol could be assessed starting at a daily dose of 2.5 mg to determine its optimal effective dose in Japanese patients.

[Postoperative recurrence and death rates of poorly differentiated carcinoma of the thyroid].

Postoperative recurrence and death rates of 54 cases of poorly differentiated thyroid carcinoma were analyzed. The rates of recurrence of 18 widely resected cases during 1979 and 1981 and 36 patients undergoing conventional surgery during 1965 and 1978 were 22.2% and 50%, respectively. The difference between the figures was statistically significant (p less than 0.05). The death rates of the two groups were 5.6% and 41.7%, respectively. These results indicate that wide resection is appropriate in the surgical treatment of the thyroid.

[Squamous cell carcinoma of the renal pelvis associated with giant hydronephrosis--report of an autopsy case].

An autopsy case of squamous cell carcinoma of the renal pelvis associated with giant hydronephrosis, which is the fourth case in the Japanese literature, is reported. A 37-year-old man, who had been found to have a left abdominal mass was admitted because of dyspnea and lumbar pain. At autopsy the left kidney contained 2,550 ml of turbid yellowish fluid in the pelvis, with a tumor protruding from the cystic wall. Histologically, the tumor was mostly composed of squamous cell carcinoma, which showed keratinization and pearl formation, with focal transitional cell carcinoma and squamous metaplasia. The ureteropelvic junction was slightly obstructed by muscular hypertrophy and fibrosis.

[Vascular invasion, lymph node metastasis and extraglandular extension of well-differentiated carcinoma of the thyroid].

Forty-five surgically removed thyroid glands with well-differentiated carcinoma were histologically analyzed to clarify the relationships among vascular invasion, lymph node metastasis and extraglandular extension. Using Victoria blue-HE stain, vascular invasion can be detected as the evident finding of circular elastic fibers surrounding the cancer cluster. In our series, 60% of the subjects were positive for vascular invasion. Patients with vascular invasion tend to show lymph node metastasis and/or extraglandular extension. This correlation was statistically significant. Therefore, in a patient with lymph node metastasis and/or extraglandular extension, there is a greater possibility of vascular invasion being present as well.

The effects of volume loading during epidural analgesia.

We have determined the effects of volume loading on the cardiovascular changes during epidural analgesia in 37 patients, who underwent various kinds of surgery. The patients were placed in 4 groups, depending upon the level of analgesia and utilization of volume loading with colloidal solutions. If the analgesia extended above Th4 we grouped them as "high epidural" and lower than Th5 level they were grouped as "low epidural". The cardiac output was measured through a Swan-Ganz catheter with thermo-dilution methods and cardiovascular variables were calculated by standard formulas. Under epidural block the most significant changes were a fall in blood pressure with decrease in cardiac output which were more pronounced during high epidural analgesia. Volume loading during the induction period with colloidal solutions would prevent the marked fall of blood pressure in half of cases studied, but in the other half the infusion was not effective for the prevention of fall in blood pressure. On every occasion over-loading effects on the right side of the heart were observed with the infusion of colloidal solutions. Also, a marked fall in systemic vascular resistance was observed with the infusion. In consequence the volume loading did not prevent the fall in arterial pressure. To manage the latter which was observed during epidural block, some sympathomimetic agents would be necessary with the volume loading. This approach would be much more important in patients with dehydration and high level of epidural analgesia.
