Effect of androgenic gland ablation on morphotypic differentiation and sexual characteristics of male freshwater prawns, Macrobrachium rosenbergii.

Mature males of the freshwater prawn, Macrobrachium rosenbergii (de Man), may change from one to another morphotype, according to a set sequence. Small males may develop into orange-claw males and orange-claw males into dominant blue-claw males. Each of the three morphotypes demonstrates distinctive reproductive behavior and secondary sexual characteristics. The role of the androgenic gland in this morphotypic transformation was examined experimentally by bilateral androgenic gland ablation (andrectomy) of small males and orange-claw males. For andrectomy initiated in the small male morphotype, transformation to the next morphotype was permitted (orange-claw), but subsequent transformation to the blue-claw morphotype was blocked. Andrectomy of orange-claw males did not prevent transformation into the blue-claw. Andrectomy on both small and orange-claw males caused disappearance of the genital papillae and atrophy of the sperm ducts and testes. The growth rates of the andrectomized small and orange-claw males were significantly lower than those of the unoperated and sham-operated controls. We conclude that androgenic gland factors control not only the differentiation of male secondary sexual characteristics but also morphotypic differentiation. Bioassays based on the results of this study will be instrumental in the characterization of such a factor(s).

Jellyfish envenomation: a summer epidemic.

During the summer of 1987 the shores of Israel were infested by waves of jellyfish. Thirty patients, mainly children, suffering various degrees of painful injuries to different parts of their bodies were treated in our emergency ward. A typical case is reported and the appropriate treatment recommended.

Suicide attempts by self-immolation--our experience.

Of 5934 patients admitted to our Burns' Unit at the Soroka University Hospital, Beer-Sheva, Israel between the years 1965 and 1986, 22 had attempted suicide by fire (0.37 per cent). Seventeen of them (77 per cent) died from their burns. The use of flammable liquids was the most common method of immolation. Fifty per cent of the victims were known to have previous mental disorders.

Radiator scald burns: a preventable hazard.

During the last 13 years 80 patients have been admitted to our department suffering from burns caused by a vehicle's radiator. Ten of them were deeply burned and had to be treated surgically. The preventive aspect of this injury is emphasized.

pH-balanced solutions with superoxide dismutase (SOD): an attempt to increase island groin flap survival in rats.

Balanced salt solution (BSS) and balanced salt solution plus (BSS+), alone and in combination with heparin or superoxide dismutase (SOD), were used as irrigation solutions for rat groin flaps after prolonged ischemia. SOD always improved flap survival when it was present in solution, although when combined with BSS and BSS+, it was less effective than when combined with Ringer's lactate. The physiological pH solutions BSS and BSS+ had no advantages over Ringer's lactate in any of the tested combinations.

An example of burn prevention: the 'Urim' factory fire.

The blanket factory building at Kibbutz Urim in Israel was completely destroyed by a devastating fire. Forty-five of the 62 workers were injured, most of them very mildly, by inhalation of smoke. No skin burns were recorded. The preplanned fire evacuation programme that was routinely rehearsed and precisely carried out during the event prevented the grave results that could have followed such a disaster.

'Sabbath' electric plate burn: a ritual hazard.

This report describes the burns caused by an electric hot plate which is used by orthodox Jews for keeping food and liquids warm during the Sabbath (Saturday). An illustrative case is presented and the preventable aspects of this particular burn are discussed.

Burns during pregnancy: our experience.

Although few women sustain thermal injuries during pregnancy, when pregnancy is complicated by thermal injury a clear management scheme plus a team approach is needed to provide optimal care both to the foetus and mother. The clinical course of eight pregnant women treated in our burn unit is presented. The relationship between the burn area, gestational age at treatment and prognosis is analysed. Active surgical treatment is advocated.

Burn hazard from cupping--an ancient universal medication still in practice.

The ancient universal medication of cupping is still practised by devoted believers and people who, for various reasons, have lost faith in other more modern modalities of medical treatment. Faulty technique in producing the vacuum in the cups was the aetiology of the burns in several patients. The history, theories and indications for cupping are reviewed.

Skin injury following contact with a complex amine.

Eight patients with skin damage due to contact with an amine are described. The material is used in the potash industry under the commercial name of Armine to prevent bosselation. Although it is considered to be a strong base, no deep burns were encountered. The reaction to this material initially resembled a dermatitis; later the wound developed the typical appearance of a burn. In addition to the common conservative treatment, antihistamine drugs were used. When a true allergic reaction was suspected steroids were added to the regimen.

Omiderm: a synthetic cover for exposed areas in epidermolysis bullosa.

Omiderm, a synthetic temporary skin substitute, was used for the treatment of skin defects in a newborn who suffered from epidermolysis bullosa. Omiderm served as a biological barrier, and its transparency allowed close observation of the wound healing process and early detection of local infections when they occurred.

Dermodress: a temporary skin substitute for coverage of exposed vital structures in the hand.

A chemically treated bovine skin, Dermodress, is presented as a new type of biological dressing for exposed tendons, bones, and nerves following extensive thermal or severe soft-tissue injuries to the hand. Its use enables the surgeon to postpone closure of defects with grafts or flaps until the presence of healthy, viable tissue is secured. The vital structures underneath the covering are kept moist, thus preventing necrosis and desiccation. Because of its adherence properties, Dermodress use also allows early mobilization and bathing, thereby accelerating rapid return of function.

Use of Van der Meulen principles in repair of hypospadias cripples without chordee.

The principle of Van der Meulen procedure was used for correction of 14 hypospadias cripples. The advantages and drawbacks are discussed. Follow-up ranged from one to five years. None of our patients needed a second intervention. Only one complication (urine leakage) occurred, which could be treated by application of Histoacryl glue at regular intervals and slight compression during voiding.

Maqua (therapeutic burn) as an indicator of underlying disease.

The origin and nature of the maqua (the Arabic therapeutic burn) is presented together with our clinical experience of patients previously treated by this traditional method. Maquas are small deep burns inflicted in areas either in proximity to a diseased organ or in points related traditionally to the original basic problem. These relationships may be rooted in historical ties between old Arab medicine and traditional Oriental, antique Egyptian, and Greco-Roman medicines. Maquas alone only rarely present a threat to the patient, but in many cases they may serve as an indicator of the original underlying disease. This and other folklore treatment modalities, together with the healers themselves, should be acknowledged by us, as markers for health problems or maybe for potential healing methods and doctor-patient relationships.
