Abel inversion applied to a small set of emission data from a microwave plasma.

In this work we propose a criterion to apply the Abel inversion in the case of a small set of experimental data to be used in laboratory plasmas. The Nestor-Olsen method, spline interpolation, and Fourier transform Abel inversion have been compared in order to study the influence of statistical noise and the number of sampled data. The application of this criterion permits us to obtain a radial distribution of the plasma parameters (densities and temperatures) from the spectral line profiles emitted by the discharge. The proposed criterion has been tested using the lateral intensities of several lines emitted by a microwave helium plasma column generated at atmospheric pressure.

[Evaluation of a pharmaceutical care program to improve adherence to antiretroviral therapy].

OBJECTIVE: To establish the impact of a pharmaceutical care program on the improvement of adherence to antiretroviral therapy, and on patient immunologic and virologic outcome. MATERIALS AND METHODS: A multicenter, observational, prospective study in a HIV-infected patient cohort under treatment with antiretrovirals selected by random sampling in 19 Spanish hospitals. The study lasted 12 months, in which the program was applied through a baseline preprocedural visit and 4 quarterly visits. Adherence estimation was based on pill counting. An adherence > or = 90, or > or = 95% was considered adequate (in two time points). RESULTS: 541 patients were included, most of them were males (68.8%) between 20 and 78 years of age. Major risk groups included injecting drug users (43.4%) and heterosexuals (29.4%). Sixty percent had already received treatment for more than 3 years. Mean baseline viral load and CD4 count values were 32,866 copies/ml and 485 cells/mm3, respectively. Throughout the study a slight increase in the percentage of adherent patients was seen; however, statistical significance was not reached (64.3 and 79.2% of patients showed an adherence > 95 and > 90%, respectively, during the fourth quarter, versus 59.8 and 75.5% at baseline). A statistically significant decrease in viral load and increase in CD4 cells was seen following program application. The percentage of patients with a viral load < 200 copies/ml was 72.2, 76.7, and 75.0% at the 2nd, 3rd, and 4th quarters, respectively, versus 64.2% at baseline. CD4 cell counts increased by 50 cells/mm3 on average from the start to the end of follow-up. CONCLUSIONS: Patients included in the program had a good immunologic and virologic outcome, and a trend towards an increased percentage of patients with good adherence was also seen. These results confirm the need to implement follow-up programs for patients receiving antiretrovirals in order to ensure maximum therapeutic benefits.

Application of cluster analysis to the geochemistry zonation of the estuary waters in the Tinto and Odiel rivers (Huelva, Spain).

The combination of acid water from mines, industrial effluents and sea water plays a determining role in the evolutionary process of the chemical makeup of the water in the estuary of the Tinto and Odiel rivers. This estuary is in the southwest of the Iberian Peninsula and is one of the estuarine systems on the northwest coast of the Gulf of Cádiz. From the statistical treatment of data obtained by analyzing samples of water from this system, which is affected by industrial and mining pollution processes, we can see how the sampling points studied form two large groups depending on whether they receive tidal or fluvial influences. Fluvial input contributes acid water with high concentrations of heavy metal, whereas industrial effluents are responsible for the presence of phosphates, silica and other nutrients. The estuarine system of the Tinto and Odiel Rivers can be divided into three areas--the Tinto estuary, the Odiel estuary and the area of confluence--based on the physical--chemical characteristics of the water.

Characterisation of sequential leachate discharges of mining waste rock dumps in the Tinto and Odiel rivers.

Mining waste rock dumps (WRDs) are potential sources of pollution, which after rainfall produce leachate discharges, loaded with acid mining drainage (AMD). The discharges generally occur in two phases: initial rapid leaching over a period of 1-7 days, followed by a period of variable duration during which leaching decelerates. The relative preponderance of each phase depends on the characteristics of the WRD, including its antecedent hydrological condition, and the temporal pattern and amount of rainfall.

Increased gastric acid secretion after massive small bowel resection is related to a decrease in enterogastrones.

The reported increase of gastric secretion after small bowel (SB) resection is controversial. To determine the effect of SB resection on gastric acid secretion we studied basal and dose step pentagastrin-stimulated gastric acid secretion as well as basal serum gastrin, secretin, neurotensin and postprandial gastrin levels in 12 dogs, before and after resection of 60% of the intestine representing both proximal (n = 6) and distal (n = 6) SB. Rat bioassay was also performed to rule out the presence of unknown gastric secretagogues in the blood. Proximal SB resection produced a significant increase in basal and low dose (100 ng/kg/h) pentagastrin-stimulated gastric acid secretion (ED50 = 1,110 vs. 720 ng/kg/h after resection). However, no significant changes in gastric secretion were observed after distal SB resections. Neither proximal nor distal SB resection altered basal or postprandial serum gastrin levels. Proximal SB resection reduced serum secretin levels (229 +/- 38 vs. 134 +/- 16 pg/ml, p < 0.05) but did not alter neurotensin levels. Rat bioassay failed to reveal a circulating secretagogue after SB resections. We conclude that proximal but not distal SB resection increases basal and submaximally stimulated gastric acid secretion. Such an effect may be due to the observed decrease in circulating secretin levels.

[Myocardial contusion with and without coronary artery involvement].

We present the first case of myocardial contusion secondary to blunt chest trauma with injury in two different areas, with and without coronary lesions. The akinesis of the anterolateral wall was secondary to the obstruction of the left anterior descending coronary artery and the severe hypokinesis of the posterior wall, without lesion of the right coronary artery, was probably related with the compression between the sternum and vertebral column.

[Severe digestive hemorrhage of an unusual etiology. Visible isolated and bleeding esophageal vessels].

We describe two patients with massive upper gastrointestinal bleeding. The upper gastrointestinal endoscopy revealed jet haemorrhage in the distal portion of the esophagus, from a visible vessel, without esophageal mucosal damage. In both cases arteriography was performed, and did not show vascular malformations or fistulous points. The patients were successfully treated, one with endoscopic sclerosis and balloon tamponade, and the other with endoscopic sclerosis and transcatheter embolization through left gastric artery. For the time being we do not know the true importance of these findings, but we hope that other reports help us to understand the clinical and pathologic features of this vascular lesion, and the best therapeutic approach.
