[Effect of halogen aerosol therapy on resistance parameters of the respiratory tract].

AIM: Assessment of the efficacy of dry high-dispersive aerosol of sodium chloride--the main acting factor of haloaerosol therapy--on defense system of the respiratory tract. MATERIAL AND METHODS: 188 patients with respiratory disease and at risk of pulmonary pathology received course of haloaerosol therapy. 49 matched patients were given placebo. The effect of the treatment was assessed by clinicoendoscopic picture, cytomorphological and bacteriological characteristics of the bronchoalveolar lavage, contamination activity of the microflora, activity of local humoral immunity in pharyngeal brush-biopsies and saliva, rheological indices of the sputum. RESULTS: Dry aerosol of sodium chloride demonstrated antiinflammatory activity in the respiratory tract, mucoregulating action. It enhances drainage of the bronchi, activates alveolar macrophages, improves biocenosis and local humoral immunity. CONCLUSION: Haloaerosol therapy has positive effect on the defense system, improves function of the respiratory tracts.

[Diagnostic value of Diamed AG latex gel kit for detection of diphtheria toxin].

ID-PaGIA diphtheria toxin polymer particle diagnostic agent manufactured by DiaMed AG, Switzerland, was tried at bacteriological laboratory of Institute of Childhood Infections in St. Petersburg. The trials were carried out using two methods, direct and capture, which differ by the duration of incubation of the studied C. diphtheriae cultures. Ouchterloney's immunoprecipitation test in agar was the control method. Ninety-seven toxigenic strains were tested by direct test and 76 by capture test; in addition, 19 nontoxigenic strains were tested. The results coincided with control tests in 100% cases. The advantages of each method are defined and the possibility of their utilization at bacteriological laboratories of infectious hospitals and State Sanitary and Epidemiological Surveillance is evaluated.

[Viral and bacterial associations and their role in the development of bronchopulmonary diseases in children].

Clinical and laboratory criteria for estimating the role of viruses and bacteria that determine bronchopulmonary diseases have been derived. The clinical importance of the detectable microflora, part of which permanently invade the nasopharynx (pneumococcus, adenoviruses) is under critical review. Pneumonias that may develop within the first days of acute respiratory viral infection are characterized by monoviral influenzal or RS-infection; later pneumonias are marked by viral infection with the predominance of adenoviruses. Attempt has been made to reveal the role of geno- and phenotypic factors (N-acetylation, lipid peroxidation, synthesis of alpha-interferon). The data obtained support an assumption about self-regulation of the child's immune system and the adaptation character of responses in mixed infections.

[Diagnosis of pneumococcal otitis in children].

The pneumococcal origin of purulent otitis was diagnosed in 74 (35 percent) of 211 children, in whom acute respiratory viral diseases or purulent otitis were previously diagnosed, by a combination of indirect immunofluorescence, cytobacterioscopic and culture methods. Use of indirect immunofluorescence and of special medium for isolation of atypical forms of the agent (L-forms and unbalanced growth forms) helped diagnose pneumococcal otitis in 47 (22.3 percent) more patients.

[Biological characteristics of Streptococcus pneumoniae strains isolated from children with pneumococcal diseases].

The biological properties of 97 S. pneumoniae strains isolated from 92 children with purulent meningitides, meningoencephalitides, pneumonia and otitis, hospitalized at the Leningrad Research Institute of Childhood Infections, were studied. The data obtained in this investigation were indicative of the formation of atypical forms of pneumococci (R-forms, unbalanced growth forms and L-forms) in all clinical forms of pneumococcal infection in children. In purulent meningitides and meningoencephalitides serovars 1 and 6, in pneumonia serovars 1, 3 and 6 and in otitis serovars 6 and 19 played the leading role. The determination of sensitivity to antibiotics showed that the forms under study retained high sensitivity to a number of antibiotics. The appearance of strains resistant to benzylopenicillin was registered. (10%). The isolated strains either possessed low virulence or were avirulent in bioassay on white mice.

[Problems in dissociating strains of Streptococcus pneumoniae].

The processes of dissociation occurring under the influence of soluble starch and some substances with surface activity (Tween-80, cattle bile and sodium desoxycholate) in 4 pneumococcal strains (Nos. 204, 205, 1225, 1317) isolated from patients with acute and chronic inflammatory lung diseases have been studied in vitro. Stable pneumococcal R-forms have been obtained by treatment with 0.1% sodium desoxycholate and 0. 5% bile and subsequent selection. The colonies of pneumococcal R-forms are characterized by a large size, a rough surface, uneven edges and pronounced alpha-hemolysis. The features typical of the population of these cultures are the polymorphism of cell element, the formation of long diplococcal chains, the almost complete disappearance of the capsule, as well as the negative results of the slide agglutination test and Neufeld s test with "omni" and typing pneumococcal antisera. Besides, the pneumococcal R-forms thus obtained have proved to be resistant to optochine (6 micrograms/ml) and bile (10-20%) and to possess low virulence.

[Adaptation mechanisms in the formation of the Corynebacterium diphtheriae carrier state].

The complex clinico-laboratory examination of 92 children aged 3-14 years with the localized form of stomatopharyngeal diphtheria and toxigenic C. diphtheriae (TCD) carrier state was carried out. The children were hospitalized in the clinic of drop infections during the period of 1996-1998. The hydrophobic properties, adhesive, lysozyme, antilysozyme and DNAase activities of 92 TCD strains, as well as the levels of the specific and nonspecific immunity of the macroorganism, were studied. The study revealed that the character of the infectious process in TCD carrier state was determined by the degree of the colonization activity of the infective agent. Direct correlation between the time of the persistence of TCD in the body and their hydrophobic properties, adhesive and antilysozyme activities (with r = 0.89-0.93) was established. The duration of TCD carrier state was found to be inversely related to the electrophysiological state of epithelial cells of the mucosa, the level of humoral specific antibacterial immunity and the content of local antidiphtheria IgA.

[The role of hyaluronidase in the occurrence of a generalized pneumococcal infection].

The hyaluronidase activity of pneumococcal strains isolated from 39 patients with purulent meningitis, 26 patients with acute internal otitis and 130 healthy carriers was studied. All strains isolated from patients with purulent meningitis and meningoencephalitis and 84.6% of strains isolated in cases of otitis were found to have hyaluronidase, while in healthy children hyaluronidase-synthesizing strains were detected only in 11.5% of cases. Hyaluronidase titers in pneumococcal strains isolated from patients with meningitis and meningoencephalitis were significantly higher than in strains causing purulent otitis. At the same time strains with high hyaluronidase titers were also isolated from 7% of healthy carriers. Noncapsular pneumococcal strains had no hyaluronidase. The average value of the reverse correlation between hyaluronidase activity and the virulence of strains for mice was established. Hyaluronidase activity did not correlate with different serotypes of pneumococcal strains. The intranasal administration of hyaluronidase in high titers (> or = 1:8) to mice and rats led to the penetration of the infective agent through the hematoencephalic barrier of the animals and to their death from generalized pneumococcal infection. The role of hyaluronidase as the invasiveness factor of pneumococci was confirmed in experiments on the intranasal infection of mice with low active strains mixed with the preparation of exogenous hyaluronidase.

[Respiratory chlamydial infection in children with a complicated course of acute respiratory viral infections].

The role and influence of Chlamydia trachomatis on the development and course of acute respiratory virus infections (ARVI) in 130 children admitted to the hospital of the Research Institute of Children's Infections (St. Petersburg) was studied. The occurrence of respiratory Chlamydia infections in ARVI patients with an unfavorable premorbid background was 35-36%. The disease took its course simultaneously with the mixed infection of the respiratory tract with viruses, pneumococci and their associations with staphylococci and Haemophilus influenzae. Chlamydia infection in children had no influence on the character of the clinical manifestation of ARVI with the exception of the obstruction syndrome which was constantly observed in children with ARVI (p < 0.001).

[Streptococcus pneumoniae adhesion].

The adhesive capacity of 425 S.pneumoniae strains (S- and R-forms) isolated from children with various clinical forms of pneumococcal infection was studied with the use of the epithelium of oral cavity as a model. The strains isolated from patients with meningitis and otitis appeared to be less adhesive than the strains isolated from mucous membrane of respiratory tracts. In all clinical forms of infections the adhesive capacity of S.pneumoniae S-forms was significantly lower than that of R-forms. The hyaluronidase activity and the adhesive capacity of S.pneumoniae strains were found to be inversely related.

[The pathogenicity factors of Streptococcus pneumoniae strains causing meningitis].

Pneumococcal meningitis was registered in St. Petersburg in 1985-1991, morbidity rate being 1.7-2.3 cases per 100,000 children, including 7.4-19.8 cases among children under 1 year. Two thirds of the pneumococcal strains isolated in cases of meningitis belonged to serovars 19, 1, 6, 15 and 2. The comparative study of pneumococcal strains revealed that the presence of capsules, pneumolysin, high hyaluronidase activity or belonging to serovars 1 and 2 (irrespective of their hyaluronidase titers) were the most important factors contributing to the development of generalized infection. The role of such a factor as virulence for mice is not yet clear.

[Variants of the immune response in the dynamics of the localized form of stomatopharyngeal diphtheria in children].

4 variants of immune response (IR) have been identified according to the dynamics of the development of nonspecific immunosuppression as determined in the reaction of lymphocyte blast transformation to phytohemagglutinin. These variants, characterized by a rise in the production of tumor necrosis factor alpha, interleukin 1 beta, immunoglobulins of 4 classes, are linked with the increased risk of the development of clinically pronounced forms (CPF) of diphtheria. The antigen-specific profile of each of these variants (types) of IR has been studied. Proofs of the fact that each type of IR has definite critical mechanisms of immune protection, contributing to the prevention of CPF of diphtheria, are presented. Treatment with antitoxic antidiphtheria serum produces different effect in 4 types of IR as it acts on different mechanisms of IR.
