[Recovery of movement and habits of patients with post-stroke paralysis and altered higher mental function status].

The results of studies concerning higher mental functions, the degree and rate of restitution of disturbed movements and habits in 117 patients with residual symptoms of strokes are reported. It was established that a drop in the rate and degree of restitution of motor functions and especially of complex motor acts (walking skill, working and domestic skill) in patients with psychopathological syndromes is accompanied by decrease activity.

[Morphology of the Shy-Drager syndrome].

The changes found in the brain and spinal cord and vegetative ganglias in one observation of nerogenic orthostatic hypotension or Shy-Drager syndrome are described. The morphological picture corresponds to a variant of Shy-Drager syndrome with a wide involvement of the nervous system at its various levels. Its is concluded that a thorough examination of the nervous system (including the spinal cord and vegetative ganglia) should be done in all cases of Shy-Drager syndrome and clinically similar diseases accompanied by vegetative disorders, for instance in shaking palsy.

[The effectiveness of amantadine (midantan) in the treatment of the parkinsonian syndrome].

The author treated 83 patients with parkinsonism by analouges of amantadin--the Soviet preparation midantan and Swiss preparation simmetril. Twenty patients received only Amantadin and 63 Amantadin in combination with other antiparkinsonic drugs. Optimal doses of other antiparkinsonic drugs were determined prior to amantadin treatment. The preparation exerted a higher influence on such symptoms of parkinsonism as akinesis and rigidity and less on tremor. In 1/3 of the cases a positive effect of the treatment was temporary and disappeared altogether after 2--4 weeks despite the continuation of therapy. The preparation was well tolerated. Side effects observed in 32 of the 83 patients were usually expressed very midly and disappeared without special treatment. The author compares such preparations al 1-Dopa and Amantadin (Midantan).

[A comparative assessment of the results of surgical and conservative treatments in parkinsonism].

Late outcomes of surgical and medical treatment of parkinsonism were studied in 582 (including 321 medical and 261 surgical) patients. The follow-up ranged from 6 to 30 years. Three hundred and fifty eight surgeries were performed. In immediate postoperative period, the following results were observed: significant (64%), moderate (20.3%) improvements, no changes (8%), deterioration (3.1%). Late outcomes were as follows: significant (34.9%), moderate (20.3%) improvements, no changes (28.7%), deterioration (16.1%). In surgical patients the complication rate was 14.8%, mortality 0.8%. Analysis of outcomes of surgical and medical treatment showed a statistically significant predominance of good outcomes in surgical patients (34%) as compared with medical ones (14%).
