Symptomatic residual radicular cysts.

The clinical, radiological and histological characteristics of 31 symptomatic residual radicular cysts are compared and contrasted with 39 asymptomatic residual cysts. Differences in size and site incidence are demonstrated along with an inversely proportional relationship between radiographic cortication and degree of acute inflammation in the cyst wall. A number of possible aetiological factors, to account for initiation of active inflammation in deeply placed residual cysts have been examined and excluded, but explanation, in most cases, remains paradoxical.

DNA-ploidy studies in a keratocyst undergoing subsequent malignant transformation.

Using techniques that allow flow cytometry to be performed on tissue stored in paraffin wax, we have examined the DNA content of cells from an odontogenic keratocyst which underwent malignant transformation. On histological examination, the primary lesion was an odontogenic keratocyst showing dysplastic changes within the squamous epithelium. Flow cytometry revealed a prominent, abnormal DNA stemline, which was also present in the subsequent squamous cell carcinoma.

Age changes in residual radicular cysts.

Histological and radiographic study of 33 uninfected residual radicular cysts has shown that there is a slow increase in mineralised deposits within the cyst lumen with time. This becomes prominent histochemically in those cysts more than 8 years old and radiographically 6 years later. It would appear that mineralisation in these elderly cysts is due to dystrophic calcification of degenerate cellular material within the cyst lumen. Inflammation in all cysts, apart from mild granulomatous reactions related to cholesterol within cyst walls, was minimal. Overall reduction of cyst size with increasing epithelial atrophy and loss of hyaline bodies and mucous cells, has been shown to occur with increasing cyst age. It is suggested that the majority of radicular cysts heal spontaneously following extraction of the associated tooth. Furthermore, lesions that persist as residual radicular cysts will also undergo slow resolution if inflammation is not a prominent feature.

A morphometric study of submandibular salivary gland changes in streptozotocin-induced diabetic rats.

The morphology of the glands in 9 male rats was studied after 3 months of streptozotocin-induced diabetes mellitus and compared with that of 9 control rats and 6 streptozotocin-treated, non-diabetic rats which served as controls for any effect of streptozotocin not due to diabetes mellitus. In the diabetic group, mean salivary-gland weight and volume was reduced. There was an increase in the ratio of mean volume to total gland volume for seromucous acini (p less than 0.01), intra-lobular striated ducts (p less than 0.001), fat (p less than 0.001) and blood vessels (p less than 0.01), and a decrease in both granular duct (p less than 0.001) and intercalated duct (p less than 0.01) volume ratios. There was a marked reduction in the total length of granular ducts (p less than 0.001) and an increase in the length of striated ducts (p less than 0.001). Measurements from the streptozotocin-treated, non-diabetic group did not differ significantly from those of controls. These effects may be attributable to failure of growth and chronic sodium depletion.

Abscesses of the frontal lobe of the brain secondary to covert dental sepsis.

The bacterial species found in pus aspirated from brain abscesses in two patients were typical of those found in dental sepsis. Subsequently apical-root abscesses were demonstrated in the upper jaws of both patients. This evidence strongly suggests that these cerebral abscesses were secondary to dental sepsis which could have spread from the teeth to the frontal lobes by several possible antaomical pathways.

Periosteal fasciitis of the mandible.

BACKGROUND: Periosteal fasciitis is rare in the head and neck. We believe only two cases of fasciitis-induced erosion through the mandibular cortical plate have previously been reported. RESULTS: Management of a case of periosteal fasciitis with bone erosion at the mandibular angle is reported in a 38-year-old man. CONCLUSIONS: Misdiagnosis of fasciitis in the past has led to inappropriate management and unnecessary morbidity. The principles of diagnosis and management of this unusual and confusing disorder are highlighted.
