[Histamine liberation and specific IgE against Dermatophagoides pteronyssinus in parasitized patients].

We studied 98 patients with different parasitosis, without clinical symptoms of mite sensitization, most of them coming from Guinea. Histamine release to Dermatophagoides pteronyssinus was performed using whole blood. Specific IgE to the same antigen was measured by EAST, as well as by an immunodot with the same antigen extract employed for the histamine release test. Finally, the EAST positive sera were studied by immunoblotting. The presence of specific IgE by EAST could be proved in 31 patients, but these antibodies were nor detected by dot, blot and histamine release. On the other hand, only two patients showed a positive histamine release test to D. pteronyssinus and in these two cases the EAST to mites was negative. There was no relation between total IgE levels and specific IgE to mites. The presence of mite-specific IgE showed a significative association to the parasite Trichuris trichiura (odds ratio 3.09). This fact suggest that the specific IgE values found in this population can reflect some cross-reaction between parasites and allergens. It is the author's opinion that the same study should be performed in european patients in order to test the relationship between mite-specific antibodies and the presence of parasites, specially of Trichuris trichiura.

[Inverted papillomas of the nasal sinuses].

After reviewing the Arnau de Vilanova Hospital's records, a total of ten cases of inverted papillomas were found. Nasal obstruction was present in 100% of the cases. External surgery was performed in 70% of these patients, finding that in 50% of them, the real size of the tumor was smaller than that show in the radiological study. After surgical treatment, only one of the patients developed a malignant tumor. Incidence of papilloma recurrence was 30%.

[Peritonsillar abscess: treatment guidelines].

A prospective study was carried out on 25 patients showing symptoms suggestive of peritonsillar abscess. Twelve patients were treated with i.v. antibiotics and three months later underwent tonsillectomy. Thirteen patients underwent acute tonsillectomy within 24-48 hours. In order to evaluate the benefits of acute tonsillectomy we compared three parameters; operation length, intraoperative bleeding and overall length of hospitalization.

[Frequency of intestinal protozoosis in the Mexican Republic].

The reports about the frequency of intestinal protozooses found in humans who live in different localities of the Mexican Republic, are in general uncertain and not trustworthy, possibly because very few and poor epidemiological surveys have been undertaken in the country. However, with the few trustful studies carried out (1981 to 1991), it is possible in Mexico, to verify that amibiasis, giardiasis and criptosporidiosis are present with significant percentages of infection: 30.6, 22.3 and 39.3% respectively. With the summary of the researches analyzed in this article, one can conclude that human infections by intestinal protozoa in the Mexican Republic, at the present time, are almost as frequent as in past decades. This occurs because still remain the factors that contribute to the persistence and spreading of intestinal parasites, such as fecalism, poor hygienic and alimentary habits, within a deficient environmental sanitation.

Serum sickness due to bovine serum albumin sensitization during in vitro fertilization.

Of 32 women included in the in vitro fertilization (IVF) program in our hospital in 1987, in whom a medium containing bovine serum albumin (BSA) (Menezo's medium) was employed for rinsing follicles, 5 (15%) developed a symptom complex compatible with serum sickness within 8-12 days after oocyte retrieval by echographic puncture. All the patients had specific IgG antibodies against BSA, and intradermal skin testing with BSA and Menezo's medium were positive. We could not demonstrate the presence of specific IgE against BSA in serum by RAST, probably due to the presence of high levels of specific IgG antibodies, which can interfere in the RAST procedure. Statistical analysis showed that the volume of Menezo's medium was significantly greater (p < 0.05) in patients developing the disease. The risk of disease development is directly related to the amount of heterologous protein administrated.

[Continuous monitoring of hemoglobin oxygen saturation in mixed venous blood during thoracic anesthesia with selective single-lung ventilation].

OBJECTIVES: The lateral decubitus position, the thoracotomy and selective single-lung ventilation all cause serious changes in oxygenation. The aim of this study is to examine the usefulness of continuous monitoring of mixed venous saturation (SvO2) under these conditions. MATERIAL AND METHODS: Twenty-three patients scheduled for pulmonary resection by thoracotomy in lateral decubitus position with single-lung ventilation were monitored continuously for SvO2 by reflection oximetry and a fiber-optic catheter in the pulmonary artery (Abbot Oximetrix 3). Cardiorespiratory profiles were obtained 15 minutes after induction (M1), after 15 minutes in lateral decubitus position (M2), 5 min after start of pleurotomy (M3), 5 min (M4) and 20 min (M5) after lung collapse, 5 min after closure of the thoracic wall (M6), and after 5 min in supine decubitus position (M7). RESULTS: The decrease in SvO2 was not statistically significant. It reached its lowest level at M5 and rose significantly in M6 (p < 0.05). For all SvO2 measurements there was a significant correlation between Oximetrix 3 and Co-oximeter readings. C(a-v)O2 and O2 uptake correlated significantly at all measurements, as did those of PvO2 and Qva/Qt except at M6. VO2I correlated inversely from M1 to M5, and CI an DO2I only at M6 and M7. PaO2 and SaO2 were significantly correlated during single-lung ventilation (M4 and M5) and later (M6). The combined changes in SaO2(a-v) and CI explain the changes in SvO2 only during and after selective ventilation, and the changes are more likely to be reflected by SaO2 than by CI. CONCLUSIONS: Under the conditions studied, continuous monitoring of SvO2 is a reliable method, indicating primarily oxygenation, and cardiac output secondarily, particularly during single-lung ventilation, and also reflecting the balance of oxygen supply and demand in tissues. Given the lack of specificity, however, hemodynamic and gas measurements must be taken to diagnose the cause of any change in the supply/demand balance whenever there is a sudden drop in SvO2.
