[Encysted renal tumor in adults. Apropos of an unusual ultrasonographic and anatomo-pathological case].

Ultrasound imaging in a 45 years old man with a one year history of bladder stone colic treated medically detected a partly encysted tumoral mass in the corticomedullary zone of upper pole of right kidney. Extemporaneous examination, requested because of the original macroscopic appearance of this tumor, prevented excision of right kidney. The atypical microscopic appearances, not found during a literature review, justify reporting of this case.

[Association of Ewing's sarcoma with fibrous dysplasia of the tibia. Apropos of a case report].

The authors have observed a child of 14 presenting fibrous dysplasia localised to the right femur and right tibia and complicated by an Ewing's sarcoma of the right tibia. The complication was marked only by pain and the radiographs were not typical. It was diagnosed only by microscopic examination after biopsy. Scintigraphy did not reveal any other site of fibrous dysplasia. The tibia was treated by a reinforcing nail followed by radiotherapy and chemotherapy. After a 4 year follow-up there was neither recurrence nor metastasis. After a review of the literature it is concluded that this combination was purely accidental and that there is no known relationship between Ewing's sarcoma and Jaffe-Lichtenstein type of fibrous dysplasia. It is remarkable that the sarcoma was not typical clinically and that the satisfactory result was probably related to very early treatment.

Effects of dantrolene sodium on occlusion and reperfusion arrhythmias in the canine heart.

The effects of dantrolene sodium on occlusion and reperfusion ventricular arrhythmias (VA) were studied in a canine model. Transient ischemia was induced by a 10 min occlusion period of the left anterior descending coronary artery in 27 control dogs and 32 dogs pretreated with dantrolene sodium (2.5 mg/kg). In the control group, 14 (52%) dogs experienced VA during occlusion. Five of these had ventricular fibrillation, 3 of which were fatal. Eight of 24 control dogs which reached reperfusion experienced ventricular fibrillation during the first 30 sec of reperfusion. Dantrolene sodium did not lower the frequency of VA during occlusion 22/32 (69%). Moreover, the incidence of fatal ventricular fibrillation during occlusion was significantly (p less than 0.025) higher in the drug treated-group (12/32; 37.5%) than in the control group (3/27; 11%). Dantrolene sodium also significantly reduced the time that lasted from the commencement of occlusion to the appearance of ventricular fibrillation (240 +/- 1 sec versus 166 +/- 9 seconds, control and drug group, respectively p less than 0.05) without significantly altering either heart rate or blood pressure (169 +/- 4 b.p.m., and 170 +/- b.p.m.; 101 +/- 10 mmHg and 104 +/- 9 mmHg, before and after dantrolene sodium, respectively). These results indicate that dantrolene sodium might have an arrhythmogenic effect in the ischemic canine heart.

Lagophthalmos in acquired oculomotor synkinesis.

Two consecutive patients who exhibited classic signs of unilateral acquired oculomotor synkinesis demonstrated an inability to appose eyelid margins on the affected side, when attempting gentle closure of the eyes. In one patient, this occurred in conjunction with a unilateral upper eyelid crease on the affected side, and in the other patient it was associated with accentuation of the crease in comparison with that of the opposite side. Bilateral eyelid closure normally induces a contraction of the superior recti muscles. The lagophthalmos and accentuated upper eyelid crease observed in our patients when they attempted gentle eyelid closure presumably resulted from inappropriate coactivation of the levator palpebrae superioris muscle by impulses destined for the superior rectus muscle.

[Acquired immunodeficiency syndrome, chronic coccidiosis and Salmonella typhimurium septicemia in a couple from Zaire. Immunological functions and attempt at immunostimulation by thymopentin].

A clinical review of a young couple from Zaïre with acquired immunodeficiency syndrome (AIDS) is reported. Both had opportunistic infections such as Salmonella typhimurium septicemia, genital herpes and digestive candidiasis. The husband was hospitalized for diarrhea from Isospora belli infection and for hemoptysis from an aspergilloma. His wife was admitted for a thoracic herpes zoster infection and left hemiplegia preceding subacute encephalitis. In both patients the biological immune functions were compatible with AIDS: presence of antibody against lymphadenopathy virus, decreased in OKT4+/OKT8+ ratio, and lack of lymphocyte response to mitogens which was partially restored in vitro after addition of interleukin 2 and thymopentin (TP-5) in the husband. In vitro monocyte cultures showed increased production of prostaglandins E2 (PGE2) and a slight decrease in interleukin 1 production. The symptomatic treatments and an attempt at immunostimulation with TP-5 in the husband are described.

The value of two combined chemoradiotherapy approaches in the treatment of inoperable esophageal cancer.

The antitumor activity of 5-fluorouracil (5-FU), combined either with bleomycin or adriamycin plus radiation, was studied in a controlled randomized clinical trial. Sixty-one previously untreated inoperable esophageal cancer patients entered the study and 56 have been evaluated: 58 male and 3 female patients with a mean age of 57 years (range 37-74). Concerning localization of the tumors in the esophagus, 2 were in the upper third, 36 in the middle third and 18 in the lower third. The length of the filling defects in the esophagogram (X-ray) was in 9 patients less than 5 cm, in 31 5-8 cm, and in 16 patients greater than 8 cm. Squamous cell carcinoma was found in 51 patients, adenocarcinoma in 3, and anaplastic (squamous cell) carcinoma in 2 patients. Modality A consisted of a combination of 5-FU (10 mg/kg i.v. 2 X weekly, 4 weeks) and bleomycin (10 mg/m2 i.v., 2 X 4 weeks) which was given concurrently with radiation (3600-4000 rad - 1000 rad weekly). In modality B the combination of 5-FU (same dose) and adriamycin (30 mg/m2 i.v. day 1, 2, 23 and 24) was applied with the same schedule and dosage of radiation. Seventy-five percent of the patients (21/28) have responded to treatment (CR + PR) in modality A, with 11 complete and 10 partial responses. In arm B, response was recorded in 64% of patients (18/28), with 2 complete and 16 partial responses. The difference in complete responses (39% vs 8%) was statistically significant (P less than 0.05). The median remission duration in complete responders was 12 months in modality A (range 6-18 months), and 6.8 months in modality B (range 3-10 months). All the responses occurred in patients with squamous cell carcinoma, except one partial response in a case of adenocarcinoma. As far as the age is concerned (less than 55 vs greater than 55 years), no significant difference in response rate was found (67% vs 71%). More favorable results were observed in the group of patients with less than 10% weight loss (79% vs 63%). Toxicity was moderate (myelosuppression, cardiotoxicity), but one treatment-related death (pulmonary fibrosis, cardiac failure) was recorded in arm A, as well as one death (rupture of aorta) in group B. Approximately 60% of patients in both modalities suffered from severe mucositis and retrosternal pain. The results of the study showed that the combination of 5-FU with adriamycin and particularly with bleomycin, given concurrently with lower radiation dosage, is an effective palliative treatment for inoperable esophageal cancer.

CAP (cyclophosphamide, adriamycin, platinum) vs CMFVP (cyclophosphamide, methotrexate, 5-fluorouracil, vincristine, prednisolone) combination chemotherapy in untreated metastatic breast cancer. A preliminary report of a controlled clinical study.

The prospective controlled phase III clinical trial compared the therapeutic value of the cis-platinum - adriamycin - cyclophosphamide combination (CAP) and that of the combination of cyclophosphamide, methotrexate, 5-fluorouracil, vincristine, and prednisolone (CMFVP) in untreated metastatic breast cancer. Seventy-two patients (greater than 2 cycles) were evaluated: 36 had received CAP and 36, CMFVP. An objective response (CR + PR) to CAP combination chemotherapy was achieved in 75% of patients (27 of 36), with a high rate (42%) of complete remissions. In terms of metastatic site, the response rate appeared to be particularly high in soft tissue and visceral organ (lung, liver) metastases. In the CMFVP group, an objective response was noted in 16 of 36 patients (44%) with 19% complete remissions. Overall therapeutic response and the complete remission rate were better with CAP regimen (statistically significant; P less than 0.01). The duration of remissions was 4-16+ months (M = 12) for CAP and 2-12+ months (M = 8) for CMFVP. Toxic side-effects were more pronounced in the CAP group, particularly myelosuppression, and anemia was prevalent. Side-effects of CMFVP treatment were mild. In 11 CMFVP-resistant cases CAP was administered as second-line treatment, and an objective response was observed in 45% of cases (5 of 11). The preliminary results of this controlled trial show the advantage of the CAP combination in the treatment of metastatic breast cancer.

Multiorgan aluminium deposits in a chronic haemodialysis patient. Electron microscope and microprobe studies.

The study reports an aluminium-intoxicated haemodialysis patient who had encephalopathy, osteomalacia and congestive cardiomyopathy prior to his death. Detailed light and electron microscope examination revealed the presence of aluminium deposits within lysosomes of cells from many organs, including the kidney, liver, brain and heart. The heavy aluminium deposits in myocardial lysosomes favor a possible role of the trace element in the patient's congestive cardiomyopathy.

[Ocular venous thrombosis and general vascular pathology].

A retrospective study was carried out covering 100 patients hospitalized at the Geneva Ophthalmological Clinic between 1973 and 1977, in whom an occlusion of the central retinal vein or one of its branches had been diagnosed. An attempt was made to assess the significance of the occurrence of venous occlusion and the relation between the angiographic fundus aspect, the general pathology, and the survival prognosis. The patients still alive were examined between 1982 and 1983, and the occurrence of general vascular pathology was compared with a control group. The results of this study indicate the important role played by arterial pathology (hypertension, arteriosclerosis, diabetes) in the pathogenesis of retinal vein occlusion. It was found that patients presenting an ischemic type of venous occlusion were in a higher risk category as regards the development of cardiac or cerebral ischemia.

Blood group antigen expression in nephroblastoma.

The histological distribution of A, B, H, Lewis, Pr, i and I blood group antigens (BGA) was studied in 19 nephroblastomas and one mesoblastic nephroma by means of the indirect immunofluorescence technique. In addition to general conclusions about the relatively limited BGA expression in the tumour cells, their histological distribution was found to be related to the differentiation level of these renal embryonic tumours. By comparison with normal embryonic, fetal and adult kidney it was shown that the nephroblastoma differentiation level is not higher than that of a 3-month fetal kidney. Furthermore, i antigen appears to be a non-specific marker of the histiocytic macrophage cells of the nephroblastoma stroma.

[Accumulation of carbon dioxide in the operative field in ophthalmic interventions under local anesthesia].

In eye surgery under local anaesthesia, a micro-climate develops under the operative field. Respiratory CO2 concentration, measured by an infrared capnograph, increases rapidly and becomes stable at a certain level. A three-dimensional relation has been established (CO2 concentration X weight of the patient X free space under the field). The effects of the accumulated CO2 are complex and partially depending on premedication. The air inhaled by the patient should be renewed by elimination of the excess CO2.

[An unusual renal tumor, associating nephroblastoma, nephroblastomatosis, teratoma, and cystic dysplasia with arterial hypertension and secretion of alpha-fetoprotein].

The authors report the case of a very unusual kidney tumour discovered in a newborn infant known to have abnormal dense and cystic structures in both kidneys already during the intra-uterine period as a result of antenatal echotomography. Severe hypertension developed on the 10th day of life, accompanied by the hypersecretion of angiotensin. Right nephrectomy of the multicystic and poorly functioning kidney resulted in the normalisation of blood pressure. A left lower polar nephrectomy for tumour finally gave a good result, with three years' follow-up. Histology of the right kidney revealed a nephroblastoma with cystic dysplasia, areas of nephroblastomatosis and the secretion of alpha-foetoprotein, in the nephroblastomatous tubular structures. On the left, there was a nephroblastoma associated with areas of nephroblastomatosis.
