[A randomized prospective study of antibiotic prophylaxis compared to lavage of the surgical wound in nonperforating appendicitis].

BACKGROUND: There are many doubts as to the efficacy of systemic antibiotic prophylaxis versus the methods of local treatment in the prevention of infection of the contaminated surgical wound. A controlled prospective study was designed to compare the effectiveness of a combination of parenteral antibiotics with lavage with physiologic serum of the surgical wound to prevent infection of the postappendectomy wound. METHODS: The patients in group A (antibiotic, n = 70) received a sole preoperative dose of methronidazol and gentamicin while in those in group I (irrigation, n = 71) the wounds were irrigated with physiologic serum prior to and following closure of aponeurosis. The patients were controlled at one week and one month after the intervention. RESULTS: The global rate of infection was 9.3%. Six patients of group A and five of group I developed wound infection (p = 0.06), The age and length of the intervention were significantly higher in the infected patients (41 vs 23 years, p = 0.0001 and 53 vs 41 minutes, p = 0.03, respectively). Intraperitoneal culture was positive in 70% of the patients who posteriorly developed wound infection, being positive in only 9.4% of the uninfected patients (p = 0.0001). Eight of the infections (73%) were detected following discharge from hospital. The cost of prophylaxis in group A was seven-fold higher than that of group I. CONCLUSIONS: Lavage of the surgical wound with physiologic serum may be an effective, safe and inexpensive method to prevent infection of the wound following appendicectomy for unperforated appendicitis.

The use of ultrasonography as the initial diagnostic exploration in blunt renal trauma.

Fifty-four patients with blunt renal trauma underwent abdominal ultrasound as their initial diagnostic exploration if their hemodynamic status and associated injuries did not contraindicate unnecessary delays. Renal ultrasound allowed us to diagnose the injuries and to identify which patients required a more aggressive radiological exploration to obtain a diagnosis of certainty. At the same time we were able to certify that this technique is highly accurate in the diagnosis of lesions associated with gross hematuria. The noninvasiveness and low cost of this procedure makes it, in our experience, the technique of choice in the initial workup of blunt renal trauma.

Value of electrical dispersion as a cause of urethral stenosis after endoscopic surgery.

We have evaluated the incidence of iatrogenic urethral strictures in a prospective study of 125 patients with a diagnosis of initial, superficial bladder tumors (without prior urethral instrumentation) and who were treated with endoscopic surgery. A global incidence of 12% was found after 24 months of follow-up with the strictures detected a mean of 9.5 months after the endoscopic procedure. These patients were divided into two groups. Group 1 was treated using standard transurethral resection (TUR) and group 2 underwent Nd:YAG laser photocoagulation. Both populations of patients underwent cystoscopic controls every 4 months as part of our standard follow-up protocol in bladder tumors. In the group of patients who underwent diathermy using a 24-F sheath (85 patients), the percentage of stenosis was 16.4%. In the patients treated with laser via a 21-F panendoscope (40 patients), the incidence of postoperative urethral stenosis was only 2.5%. We believe that the electrical dispersion generated by the unipolar electric current as well as the caliber of the operating instruments are factors which may influence the development of iatrogenic endoscopic urethral strictures.

The influence of environment and parasitism on the prevalence of asthma in two Venezuelan regions.

The point prevalence of bronchial asthma and the influence of environmental conditions among 100 individuals chosen from two Venezuelan regions, i.e., rural and urban, were investigated by history, routine laboratory tests, determination of total and specific IgE antibody to common allergens and spirometric tests. The point prevalence of bronchial asthma was 3.0% in rural subjects who were also highly parasitized by helminths and 3.6% in subjects from the urban region. A significative difference in skin responses to Dermatophagoides pteronnysinus was observed in urban asthmatics when compared with rural asthmatics. A similar response to selected allergens was found among both populations. The prevalence of infestation by Ascaris in rural asthmatics was high compared with that of rural controls. Total IgE levels were elevated for both populations, but significatively higher (p less than 0.05) for rural individuals. Results suggest a significative point prevalence of bronchial asthma in both regions, despite the helminth infestation of rural subjects. The effects of environmental factors, their concentration and time of exposure are considered as main factors responsible for allergic reactivity observed in the Venezuelan regions studied.

Characterization of antibodies to androgen-dependent secretory proteins of the mouse dorsolateral prostate.

The mouse prostate is an attractive model for studying the relationship between epithelial-mesenchymal interactions and the mechanism of androgen action because of the volume of information on tissue interactions in the development of the prostate of this species and the existence of a mutant mouse lacking functional androgen receptors (Tfm mouse). In this paper the major proteins of the mouse dorsolateral prostate (DLP) have been described, and antibodies to these proteins have been characterized. The two most abundant secreted proteins were of 110,000-115,000 (Mj1) and 55,000-62,000 (Mj2) mol wt. They were glycosylated, androgen dependent, and appeared to exist in an oligomeric complex. Antibodies raised against mouse DLP secretion reacted mainly with Mj1, Mj2, and a minor protein of 140,000 mol wt (Mn1). The antibodies were of a high titer and recognized these three mouse DLP proteins by Western blotting, immunoprecipitation, and immunocytochemical techniques. Mj1 and Mj2 were antigenically similar to proteins in the mouse coagulating gland and in the rat DLP, but were not found in other organs. Immunocytochemical staining of the DLP from intact mice revealed many ducts that were lined by a tall columnar epithelium whose cells stained intensely. However, ducts that were distended with luminal secretion had a low columnar epithelium that rarely showed intracellular staining. These marker proteins and the antibodies to them will be useful for detecting androgen-dependent functional activity in tissue recombinant studies with a variety of experimental tissues.

Eosinophilic cystitis and allergy to mitomycin-C.

Eosinophilic cystitis appears to have an allergic origin to different substances causing an immunological reaction. Up to now it was unknown that Mitomycin-C could cause this reaction. We present 6 cases of eosinophilic cystitis induced by Mitomycin-C analysing their clinical and pathological characteristics.

[Results of long-term follow-up of patients with ureterosigmoidostomy].

The late results of ureterosigmoidostomy (US) have been known for many years; however, the incidence rates of second neoplasms that have been reported for the foregoing procedure are in disagreement with our own findings. A comprehensive review we had undertaken demonstrates that although it is a risk factor, its true incidence is much lower than that widely-accepted and published. Its very long latency period make it a currently valid surgical procedure in some adult conditions.

Orbital blood cyst.

A blood cyst of the orbit is an unusual cause of proptosis and most likely arises in a pre-existing haemangioma. If the cyst occurs at the apex of the orbit the blood should be aspirated. This is followed by excision of the cyst wall, the dissection being done with the aid of an operating microscope. If there is evidence of intracranial connection of the blood cyst, namely, splayed superior orbital fissure or cerebrospinal fluid leak, then the patient should be treated with an antibiotic to prevent intracranial infection.

Ozone and UV radiation over southern South America: climatology and anomalies.

Ozone and UV radiation were analyzed at eight stations from tropical to sub-Antarctic regions in South America. Ground UV irradiances were measured by multichannel radiometers as part of the Inter American Institute for Global Change Radiation network. The irradiance channels used for this study were centered at 305 nm (for UV-B measurements) and 340 nm (for UV-A measurements). Results were presented as daily maximum irradiances, as monthly averaged, daily integrated irradiances and as the ratio of 305 nm to 340 nm. These findings are the first to be based on a long time series of semispectral data from the southern region of South America. As expected, the UV-B channel and total column ozone varied with latitude. The pattern of the UV-A channel was more complex because of local atmospheric conditions. Total column ozone levels of < 220 Dobson Units were observed at all sites. Analysis of autocorrelations showed a larger persistence of total column ozone level than irradiance. A decreasing cross-correlation coefficient between 305 and 340 nm and an increasing cross-correlation coefficient between 305 nm and ozone were observed at higher latitudes, indicating that factors such as cloud cover tend to dominate at northern sites and that ozone levels tend to dominate at southern sites. These results highlight the value of long-term monitoring of radiation with multichannel radiometers to determine climatological data and evaluate the combination of factors affecting ground UV radiation.

[Risk of occupational exposure to hepatitis B virus in Venezuelan health workers. Multicenter study].

In a multicenter study, 970 serum samples were collected from venezuelan health workers. Presence of HBV serological markers (HBsAg, antiHBs and total anticore) were screened by microELISA. Two-hundred and forty-seven samples (27%) showed at least one positive marker, being the most frequently found the antisurface antibody which was present in different working areas, including those considered at the low exposure level (medicine 24.7%). The proportion of antisurface positive samples also was significantly higher in the group with more than 10 years of service compared to the prevalence showed by the group with less than 5 years (19.1 vs 12.8%). Groups considered to be at low risk, contrary to the expected results, showed a similar or higher prevalence compared to the groups classified as continues or frequent exposure (17.7 vs 8.6%). Our findings suggest a permanent HBV load circulation at the venezuelan hospitals environment.

Effects of neonatal undernutrition on the electrocortical development of the association areas in the rat.

The electrocortical effects provoked by neonatal undernutrition and the environmental sensorial stimuli were studied in the cortical association areas of developing Wistar rats. When the interaction between these two factors was interfered (Experiment 1), the average frequency of the ECoG in the early starved rats was significantly increased than controls. Moreover, if these two factors were combined (Experiment 2) not significant differences in the ECoG average frequencies were observed. The data suggest that the maturation of cells underlying the ECoG in the association areas of the rat, requires not only an adequate supply of nutrients, but also the influence of sensory cues arising from the mother, littermates and the environmental surrounding.
