[Allergy to beta-lactam antibiotics in childhood. Diagnostic problems].

This study reports about 126 children with an age ranging from 12 months to 12 years having RA to beta-lactamase. More children with an age ranging from 7-12 years (59.5%), and more male (65.9%) than female subjects were studied. 76 patients (60.3%) showed a familial anamnesis of allergic diseases or similar diseases and a personal anamnesis of allergic diseases (45.2%). Among the RA responsible substances the more frequent were semisynthetic penicillin (44.6%) followed by cephalosporin and penicillin. Most clinical manifestations (87.3%) were cutaneous reactions and in smaller number of cases it was possible to observe gastroenteric or respiratory reactions. In 121 patients cutaneous tests were undertaken (Prick tests and intradermoreactions), patch tests, employing: PPL, MDM, Penicillin, Ampicillin, Cephaloridine. 5.8% of the studied subjects showed an allergy. Particularly 2.4% showed a "early" positivity. RAST was carried out in 92 patients and 3 of them showed a positive result (3.2%). This study stresses the opportunity to carry out allergologic tests within 6 months since the beginning of RA. In fact especially in children this test result can soon become negative. Furthermore the reduced percentage of positive allergologic tests can be due to the inclusion in other studies of patients with "coincidental reactions". A certain number of RA can be caused by additive to "per os" products. Finally it will be possible to use "tests dose" even in selectioned cases. These tests can frequently exclude the etiopathogenetic responsibility of beta-lactamic substances.

EEG findings in West syndrome a follow-up of 20 patients.

Clinical and EEG findings of 36 patients with West syndrome (WS) were reported. Twenty patients (7 males, 13 females; mean age 7.4 months at the first EEG examination) were followed for a mean period of 19.3 months. Fifty per cent of the 8 cases with cryptogenic SW showed a full recovery, 2 cases showed persistence of psychomotor retardation or seizures and 2 of both these features. At the end of follow-up period, 92% of the 12 patients with symptomatic WS showed a psychomotor retardation and 75% a persistence of seizures. EEG pattern was a typical hypsarrhythmic one in 11 patients and atypical in the remaining cases. The typical pattern correlated with a better outcome with full recovery in 27.3% compared with 11.1% in the group with atypical EEG. Moreover EEG monitoring was useful for the evaluation of therapeutic response and in the follow-up of the disease, showing a correlation with clinical course, incidence of spasms and outcome.

CEOP/PEB alternating chemotherapy in advanced intermediate and high-grade non-Hodgkin's lymphomas.

BACKGROUND AND METHOD: From February, 1987 to July, 1990, 28 patients (M/F = 16/12; median age = 60.5 yrs) affected by intermediate (22) or high-grade (6) advanced stage (III = 8; IV = 20) NHL were given a median (range 4-10) of 8 cycles of CEOP (Cyclophosphamide, Epirubicin, Vincristine and Prednisone), alternated every 21 days with PEB (Cisplatin, Etoposide, Bleomycin). RESULTS: Nineteen (68%) pts. achieved a CR, 5 (18%) a PR, and 4 (14%) experienced progressive disease (PD); 11/19 CRs subsequently relapsed within a median (range 3-15) time of 8 mos.. After a follow-up ranging from 6 to 42+ (median 18) mos., the 3-year actuarial overall survival (OS) was 51% and shifted to 18% at 42 mos. After 7-42+ (median 19) mos., 14/19 (73%) Crs were still alive with 63% of them predicted to survive at 3 years. The projected 3-year disease-free survival (DFS) for these pts. after 1-36+ (median 8) mos. was 28%. DISCUSSION: CEOP/PEB alternating chemotherapy failed to improve the therapeutic results we obtained in a previous study with CEOP alone. Toxicity was moderate, but higher than expected.

Nasal secretion specific IgE in rhinitis patients.

Forty-five subjects with rhinitis of suspected allergic origin underwent immune and allergy testing including skin-tests and immuno-enzymatic assay of serum and nasal secretion specific IgE (RAST). Nasal secretion was collected using a cotton tampon inserted in both middle meati. A statistical analysis comparing the results of skin-tests and serum RAST was performed: the results obtained with these two techniques were very homogeneous (86.7% accuracy). An overall concordance (82.2%) was also observed by comparing skin-tests and nasal secretion RAST. Finally, the results of serum and nasal secretion RAST were compared: in this case totally superimposed data were reported (overall accuracy: 93.3%). In all cases the comparison between RAST classes in serum and nasal secretion was carried out: the difference between the semiquantitative evaluation of reaginic concentration classes using these two methods is not statistically significant (chi-square test). This technique of nasal secretion collection is simple and with no risk involved.

[Maxillary sinusitis in children: clinical and diagnostic aspects].

Acute or chronic sinusitis is often overlooked and poorly understood in the pediatric population as many children have frequent episodes of upper respiratory infections that confuse accurate diagnosis. The authors present a prospective study in 89 children with clinical and radiographic presence of maxillary sinusitis in order to underline the most frequent clinical symptoms and signs and the most significant diagnostic procedures that help confirm precise definition of this common disease. The authors also discuss the relationship with allergic factors, clinical causes of poor sinus drainage, and environmental factors that may increase the possibility of sinus infection.

Status of patch and other skin tests in diagnosis of systemic penicillin allergy.

It is generally accepted that cell-mediated immunity is responsible for contact dermatitis, but controversy still exists about the involvement of Type IV immune-reactions in other clinical manifestations following inhalant, oral and parenteral administration of penicillin and semi-synthetic penicillin or cephalosporins. The aim of the present study was to contribute to a better understanding of the pathogenetic mechanism responsible for these reactions. Detailed history taking, clinical and immuno-allergological investigation on 161 patients with a history of an allergic reaction to penicillin and/or semisynthetic penicillins and/or cephalosporins was carried out with the following tests: skin tests, patch tests, RAST, PRIST and lymphocyte transformation test. The statistical evaluation of data was performed by X2 test. Comparative studies were focused on groups of patients who presented a positive reaction to patch test (37) skin test (scratch and intradermal) (42), simultaneous skin and patch test (5) and negative skin test (87). No significant difference was observed among the groups studied, although in positive and skin test, a higher frequency is observed to be associated with anaphylactic shock and eczematous type of reaction. It is not possible however not only, to establish whether this is due to a combined immune-reaction (Type I and IV), orto a causal coexistence of the two mechanisms but even to determine either of allergic or pseudoallergic phenomenon without any accurate immunoallergological test. It is suggested that patch tests be carried out as routine investigations on patients with a systematic reaction to penicillins and/or cephalosporins, regardless of the route of administration.

[Umbilical cord IgE: a test for sensitivity, specificity and predictive value of atopy?].

Total IgE cord serum levels were measured in 200 newborns in the Hospital of Viadana-Bozzolo and familial allergic conditions were investigated in all the cases. In a 6 month follow up study attention was focused on the development of atopic symptoms. The sensibility, specificity and predictive value of the total IgE cord serum levels are discussed and the introducing of preventive measures in the at risk newborns is also questioned.

[Hemifacial microsomia. Description of a case with associated rare multiple visceral anomalies].

A case of hemifacial microsomia is reported in association with rare cardiac, lung and laryngeal abnormalities. Our case presents both atrial and ventricular septal defects, hypoplasia of the right lung and of the larynx. This variety of malformations is rarely present in other reported cases. The literature of hemifacial microsomia is reviewed and differential diagnosis is discussed.
