[Preliminary experience with the Storz Modulith SL 20 lithotriptor in reno-ureteral lithiasis].

The authors report their initial results in ESWL of urinary calculi with the Storz Modulith SL 20 lithotriptor, a multipurpose device for both urinary and biliary stone treatment. During 176 sessions, a total of 144 cases (81 renal and 63 ureteral) were treated and then, followed up. The over-all rate free of stones was 86.8% (90.1% for kidney and for ureteral stones). The treatment rate was 1.23 per patient. Auxiliary procedures before ESWL were performed in 7.9% of the sessions: insertion of "double-J" stent or nephrostomy tube; all ureteral stones received "in situ" treatment. Mean number of shock waves per session was 1836.1 (range 300-3020). The maximum voltage on the average reached was 17.1 kv. Sedo-analgesia by Diazepam and Ketoprofen was given in 75% of the cases. No serious complications were observed. The experience with this machine is discussed in regard to performance; the advantages of its fluoroscopy and ultrasound guided system of focusing are stressed.

[Defecography and manometry in the postoperative follow-up of patients treated with colo-anal anastomosis by the Parks technique for neoplasm stenosis of the rectum].

The Authors report 15 cases of patients with adenocarcinoma of the rectum who were treated using Park's resection. Morphofunctional parameters are evaluated 3 months after surgery using defecography and manometry before the closure of the neostoma and the reactivation of the ano-rectal canal. Opaque defecography enables the morphology and dimensions of the anal canal to be examined together with modifications to the area between the rectum and the anal canal, and the impression of the pubo-rectal sling. In conclusion these data confirm the importance of surgery, whereas the correlation between defecographic and manometric results allows the functional recovery "ad integrum" of the area to be checked.

Delayed hypersensitivity to diltiazem in two patients.

We report two cases of cutaneous adverse reactions during treatment with diltiazem. Both patients developed wide-spread, pruriginous, erythematous, maculopapular eruptions with fever and facial angioedema, 48 to 72 hours after starting diltiazem. Delayed hypersensitivity to diltiazem was suspected and the diagnosis was confirmed by skin patch tests followed by cutaneous biopsy.

Nonhabituation processes affect stimulus specificity of response habituation in the crab Chasmagnathus granulatus.

The escape response of the crab (Chasmagnathus granulatus) habituates to repeated electrical shock (SH) or to the presentation of a screen moving horizontally (H) or vertically (V) overhead. Stimulus specificity of the habituated response is shown 24 hr after training but not after either 90 or 45 min. This disruption at short intervals is explained by a transient effect that arises from training: SH training and H training attenuate and V training enhances reactivity. The attenuating effect of H training is reduced by naloxone when administered before but not after training. The results support the hypothesis that during the crab's training two types of processes takes place: a stimulus-specific, long-lasting habituation process that promotes between-sessions response habituation and a nonhabituation process that promotes nonstimulus-specific, less persistent changes of behavior that involve either response habituation or response sensitization. The effect of H training seems to be mediated by an opioid mechanism through secondary activation.

Subacute thyroiditis presenting as Cushing's syndrome. Case report.

Subacute thyroiditis is a benign, self-limited disorder characterized by hyperthyroidism that resolves spontaneously. Although cortisol metabolism is accelerated in hyperthyroidism, cortisol levels are usually normal. We describe a child with subacute thyroiditis whose presenting complaints and preliminary evaluation suggested a diagnosis of Cushing's syndrome. There was spontaneous improvement in the signs and symptoms with concomitant normalization of laboratory values. We conclude that hypercortisolism may be a manifestation of hyperthyroidism, and the evaluation of Cushing's syndrome should include thyroid function tests.

Total red cell calcium content, cytosolic red cell free calcium and erythrocyte membrane dynamic properties in diabetes mellitus.

In a group of diabetics subdivided for type (12 of type 1 and 12 of type 2), we evaluated the total red cell Ca content, red cell cytosolic free calcium, erythrocyte membrane fluidity and erythrocyte membrane protein lateral mobility. From the results obtained, it is evident that the total red cell Ca content does not discriminate normals from type 1 and 2 diabetics, whereas the red cell cytosolic free calcium does differentiate between these diabetic types. Erythrocyte membrane fluidity and erythrocyte protein lateral mobility discriminate normals from type 1 and 2 diabetics. In normals and in diabetics of type 1 and 2, no relationship is evident between total red cell Ca content, membrane fluidity and membrane protein lateral mobility. A slight, but significant negative correlation between red cell cytosolic free calcium values and parameters reflecting the red cell dynamic properties is present in type 2 diabetics only.

[Ergospirometry as a method for predicting the clinical course of ischemic, Chagas and idiopathic myocardiopathy].

PURPOSE: To compare and evaluate the modifications variables obtained through spyroergometry in groups of patients with ischemic cardiomyopathy (IS), Chagas' disease (CH), and idiopathic cardiomyopathy (ID). To study tolerance to anaerobic conditions of patients from the above groups who died. METHODS: A study was carried with 50 male patients, 10 of which were found in normal conditions whereas 40 presented cardiomyopathy with compensated heart failure. We observed that from the 40 patients from the cardiomyopathy groups, nine died, subgroups OB--during the study period of 1.005 days. These were compared to subgroups NOB--the remaining 31 patients. These patients were evaluated under 4 conditions--Rest (Rep), anaerobic threshold (LA), power peak of the exercise (P) and in the fourth minute recovery (REC). The investigation was based on data collected by means of spyroergometry: oxygen consumption (MET), production of CO2-(VCO2), respiratory equivalent of oxygen (VE/VO2), oxygen pulse (VO2/FC) and the time elapsed between LA and P. RESULTS: There were significant differences when the three pathological groups (IS, CH, ID) were compared with the control groups (N). There were no significant differences when the three pathological groups compared among themselves. In relation to tolerance to anaerobic conditions significant differences were found among the patients of subgroup OB--1.33 min--who died when compared to subgroup NOB--3.26 min (time elapsed between LA and P). CONCLUSION: The authors conclude that the variables studied, obtained through spyroergometry may indicate important data towards the prognosis with heart failure.

The role of vasopressin and atrial natriuretic factor in postoperative fluid retention after the Fontan procedure.

The Fontan procedure results in right atrial distention and is complicated by fluid retention. Since systemic fluid balance may be hormonally mediated in part and related to right atrium size, we measured plasma atrial natriuretic factor and plasma arginine vasopressin levels in 19 patients undergoing the Fontan procedure and in 12 control patients undergoing other types of heart operations. Preoperative plasma atrial natriuretic factor levels were higher in patients undergoing the Fontan procedure than in control patients (95 +/- 16 pg/ml preoperatively versus 50 +/- 8 pg/ml; p less than 0.05) and increased in patients undergoing the Fontan procedure to 330 +/- 48 pg/ml by postoperative day 2 (p less than 0.05) but not in control patients. Increased plasma atrial natriuretic factor levels could enhance capillary transudation, but elevated plasma atrial natriuretic factor levels should also enhance diuresis and prevent fluid retention. Vasopressin levels, however, were also increased in patients undergoing the Fontan procedure (from 9 +/- 2 pg/ml preoperatively to 144 +/- 37 pg/ml at end of operation) and were higher and remained elevated longer than in control patients undergoing heart operations (37 +/- 7, 20 +/- 4, 16 +/- 6 pg/ml on postoperative days 1, 2, and 3 to 10 for the Fontan group compared with 15 +/- 4, 4 +/- 1, 4 +/- 2 pg/ml for control patients). Vasopressin levels were highest in the Fontan group with the most severe fluid retention and effusions (for example, 51 +/- 10 pg/ml versus 23 +/- 4 pg/ml, on postoperative day 1). Increased vasopressin and atrial natriuretic factor could act synergistically to result in the development of effusions after the Fontan procedure when atrial natriuretic factor-induced capillary transudation is combined with vasopressin-induced antidiuresis.

Unusual reaction to chlorambucil: a case report.

In this brief report we describe a case of cutaneous reaction during treatment of chronic lymphatic leukemia (CLL) with chlorambucil. The patient developed a confluent maculopapular erythema and large flaccid bullae of trunk, legs, feet and mucous membranes, with fever up to 38 degrees C. Toxic epidermal necrolisis (TEN) was supposed and the diagnosis was confirmed by a skin patch test followed by cutaneous biopsy. TEN by chlorambucil is a rare syndrome that may be considered in presence of cutaneous reaction to this drug.

Cell cycle progression and 3-hydroxy-3-methylglutaryl coenzyme A reductase are regulated by thyrotropin in FRTL-5 rat thyroid cells.

The incorporation of [14C]acetate into cholesterol shows that FRTL-5 cells possess an active cholesterol biosynthetic pathway. When these cells were made quiescent, and synchronized by thyrotropin (TSH) starvation, in the presence of low serum (0.2%), addition of this hormone increased acetate conversion into cholesterol up to a maximum of 8-fold. Feedback inhibition of sterol synthesis by exogenous cholesterol occurs in FRTL-5 cells since, in the presence of higher serum concentration (5%), acetate conversion into cholesterol was significantly depressed. Even in high serum TSH increased sterol synthesis, albeit to a lesser extent. The time course of the TSH effect on cholesterol synthesis, strongly suggests that this process is necessary for quiescent FRTL-5 cells to enter the cell cycle. Thus, the rate of cholesterol synthesis was maximal 12-16 h after TSH challenge and declined thereafter, returning to levels slightly above the basal at 48 h. Thymidine incorporation into DNA, measured under identical conditions of TSH starvation/challenge, increased after 20 h, was maximal at 36 h, and returned to pre-TSH level at 70 h. The effect of TSH on cholesterol synthesis is not a general feature of lipid synthesis in FRTL-5 since [14C]acetate incorporation into triglycerides after TSH treatment has a different magnitude and time course. TSH increases cholesterol synthesis through the induction of the enzyme 3-hydroxy-3-methylglutaryl coenzyme A reductase. This is due to an increase in the level of 3-hydroxy-3-methylglutaryl-CoA reductase messenger RNA up to 8-fold caused by a proportional increase in the rate of gene transcription, as assessed by nuclear "run on" experiments. The effect of TSH on cholesterol synthesis and reductase gene expression is likely to be mediated by cAMP since 8-bromo-cAMP mimicked the effect of the hormone. The data presented suggest that an active cholesterol biosynthetic pathway is required for DNA synthesis to occur.

Long-term habituation to a danger stimulus in the crab Chasmagnathus granulatus.

A shadow moving overhead acts as a danger stimulus and elicits an escape response in the crab Chasmagnathus granulatus, that habituates after 15 stimulus presentations at 3-min intervals. Here, the habituation was induced by 3 diverse shadow stimuli and the habituated response persisted after 24 hr in every case. This long-lasting effect failed to appear if the testing stimulus did not keep the same spatial specificity as that of the training one. A partial stimulus generalization was detected, when training and testing stimuli differed in size. The adaptive value of a high danger stimulus specificity in long-term habituation is discussed. Extinction of the long-term habituated response failed to occur although crabs were exposed to the context in the absence of danger stimulation.

Effect of naloxone pretreatment on habituation in the crab Chasmagnathus granulatus.

On sudden presentation of a passing shadow, the crab Chasmagnathus granulatus reacts with an escape response that habituates after repeating the same stimulus. In a first series of experiments, a range of naloxone (NX) doses (0.8, 2.4, 3.2, and 6.2 micrograms/g) was injected into crabs 15 min before one 15-trial habituation session. An enhancing effect of 3.2 micrograms NX/g on responsiveness appeared over trials, that cannot be explained either by a ceiling effect or by a delay in peak drug action. Two doses below 3.2 microgram/g and one dose above had no significant effect. Results from a second series of experiments showed that the 3.2 microgram NX/g effect vanishes after 15 trials (1 h after injection). The hypothesis that crab's habituation involves the action of an endogenous opioid mechanism is put forward to account for the naloxone pretreatment effect.

Vascular atherosclerotic disease: evaluation of the whole-blood filterability and red cell membrane microrheological pattern after intravenous administration of a single dose of pentoxifylline.

A study was carried out in 10 patients with vascular atherosclerotic disease to investigate the effects of a single dose of 300 mg pentoxifylline, given by intravenous infusion over 30 minutes, on various macrorheological and microrheological parameters. Measurements were made at baseline, at the end of the intravenous infusion and again 30 minutes later, of whole-blood filterability, erythrocyte membrane fluidity (using pyrene as probe) and transverse fluidity gradient (using fluorescent fatty acids as probes). The results showed that there was a significant variation in whole-blood filterability and the red cell membrane fluidity gradient after pentoxifylline. No difference, however, was noted for the membrane fluidity. These data suggest, therefore, that pentoxifylline plays a role in affecting the red cell membrane microrheological pattern.

To assess the efficacy of prophylaxis in digestive tract surgery using a predictive model of complication risk.

One hundred and ninety-one patients, all candidates for major clean-contaminated surgery, were classified according to the risk of developing postoperative wound infection, using clinical and nutritional factors. A widely validated nutritional assessment model, the Prognostic Nutritional Index (PNI), was used preoperatively to evaluate the risk and probability of post-operative septic complications. Based on the predicted outcome, patients were assigned to either a poor or a good risk group for statistical comparison. A prospective, double-blind, randomized trial was performed to compare the effectiveness of the monocyclic beta-lactam, aztreonam, with gentamicin for short-term perioperative prophylaxis. In the poor risk group (PNI greater than 40), aztreonam prophylaxis significantly reduced postoperative septic complications (p less than 0.05). The use of an objective scoring system to assess the risk of postoperative sepsis is recommended for future studies on antibiotic prophylaxis as it offers a realistic assessment basis.

Seroprevalence of HBV and HCV markers among intravenous drug abusers and subjects affected by chronic active hepatitis.

In order to investigate the seroprevalence of HBV and HCV in neapolitan area, we studied blood specimens of 180 IVDAs, 115 CAH patients and 72 healthy subjects using hepatitis B core (anti-HBs) and hepatitis C antibody (anti-HCV). High frequency of anti-HBs (80.9%) and strong association with an unexpected seroprevalence of anti-HCV (67%) was found in CAH patients. Our study does not explain these results, but suggest either a possible inference between both viruses or that serum from CAH subjects contains a component that gives false positivity.
