An experimental trial of malaria control was carried out in the village of Karangasso in the south-west Burkina Faso. It was based on the use by the whole population of deltamethrin impregnated bed nets at 25 mg/m2. During the first year pretreatment data on entomology, parasitology and pathological incidence of malaria were collected in the whole village. During the second year a quarter of the village with a population of 1,200 was chosen for the experiment and impregnated bed nets were given to everybody while the other quarter of the same population size was kept as a control area. Malaria transmission was reduced by 82% due to the decrease of both vector populations and sporozoitic indexes. It should be pointed out that this reduction in transmission was evaluated on non-protected catchers and consequently was underestimated for the villagers sleeping under nets. Parasitic index remained about the same but the mean parasitic load decreased significantly. Pathological incidence, based on the number of clinical malaria cases confirmed by blood examination, decreased by 59%. This trial shows that mass use of deltamethrin impregnated bed nets should be considered as a valuable tool for malaria control. The purchase of bed net is expensive but could be reduced sharply. The cost of the impregnation is very low regarding the residual effect which remains one year. The acceptance by the population was good.

Experimental data are presented here proving the accumulation of glycine in serum after transurethral prostatectomy and increased production of glycine metabolites: serine, alanine, glyoxylic acid, and glycolic acid. The presence of the metabolites glyoxylic acid and glycolic acid was demonstrated by gas-liquid chromatography and mass spectrometry. Glycine, glyoxylic acid, and glycolic acid possess neurological activity, so we examined the pathophysiology of the transurethral prostatectomy syndrome in view of the transient accumulation of these compounds in serum.
The effect of treatment with fenofibrate was investigated in nine patients (seven men) presenting with type III hyperlipoproteinemia. This therapy produced a statistically significant decrease in serum triglyceride, cholesterol, apolipoproteins B, CII, CIII, and E levels. Particular attention was focused on lipoproteins containing apolipoprotein B. The concentration of particles recognized by monoclonal antibodies (BL3, BL5, and BL7), associated with atherosclerotic disease, was lowered by the treatment. The most dramatic decrease was observed for lipoproteins LpE:B and LpCIII:B, which are particularly enhanced in type III hyperlipoproteinemia.
In pregnant women rest is essential to the prevention of some accidents at birth (e.g. prematurity). A survey of 357 women examined and delivered in the maternity department of hospital A. Béclère in 1984 shows that abstention from work is general and prolonged, irrespective of the degree of risk inherent in pregnancy: 50% of women have stopped work 7 weeks before their legal maternity leave. This wide prescription of rest seems to have been requested by the women themselves. Its financial cost to the Social security is negligible, but prescriptions for rest from work have probably not yet ceased to increase.
The retrospective homogeneous study of 247 patients with follow-up from 12 months to 384 months (mean 88 months) and managed with or without intra-vesical instillations of thiotepa as prophylactic chemotherapy after surgical conservative treatment is compared with some literature's data principally multicentric randomized series. Statistics contradictions remain with no significative results for possible progression towards infiltration, better "free interval" between recurrences, and better recurrence rate for 100 patients/months. In this series the number of tumors recurrences in each bladder was evaluated: this mean rate of tumors recurrences/year is 1,7 without prophylactic thiotepa and only but always 1,08 with thiotepa. Prophylactic instillations chemoprophylaxy seems to be justified but must be emphasized: mitomycin C, BCG therapy would be better in the future?

In vitro tests, deltamethrin reduced the parasitemic index of Plasmodium falciparum on human red blood corpuscles. In vivo tests, at sublethal doses, deltamethrin limited the development of the sporogonic cycle of Plasmodium yoelii yoelii in Anopheles stephensi. This novel type of antiplasmodic activity involve an intravectorial route and gives an explanation about surprising antimalaria effects observed several months after treatment.
Administration of 25 mg of captopril to 10 patients with heart failure (NYHA II, III) produces at rest after 60 min bradycardia (-7 per cent, p less than 0.01), hypotension (-8 per cent) and improvement of the preload (-30 per cent, p less than 0.01). This effect is still apparent after two months of treatment where PAP is decreased by 20 per cent (p less than 0.05). A prolonged effect of captopril is present in these 10 patients. The capacity to perform a muscular exercise on an ergometric bicycle is prolonged by 10 per cent (p less than 0.01) on the first day and by 18 per cent (p less than 0.01) after two months of treatment. This improvement seems to be related to improvement of the hemodynamic parameters, especially of preload indices. In 5 patients, lactacidemia, the arterio-venous difference and the extraction coefficient of O2 were studied at rest and during exercise. No significant difference are noted after the treatment with the inhibitors of conversion enzymes.
About a case of Von Hippel-Lindau syndrome with association of retinal angioma, bilateral pheochromocytomas, bilateral renal cysts, the authors had many difficulties to confirm associated unilateral renal cell carcinoma. Three solid left renal tumors were not seen neither with abdominal ultrasound, nor with CT scan, but only with hypervascularity on renal angiography. Even intra-operative histologic examination fail to confirm the malignity. After the complete histologic examination of the tumorectomy piece, the unilateral renal cell carcinoma was secondary managed by radical nephrectomy, although the risk of bilateral carcinoma is known in this disease.
About a series of 238 surgically managed renal cell carcinoma with 64 venous invasion's cases (without no lost of follow-up) the prognosis is bad: post-operative mortality: 12,5%. Overall uncorrected five-years actuarial survival: 25%. But 42% of these patients with venous invasion had associated lymph node invasion and-or visceral metastasis. About 37 cases with only venous extension five-years actuarial survival reaches 45,9%. The inferior vena cava (I.V.C.) extension is the worst. Five american data reports some cases (5 to 18 cases) with near half and half surviving patients. In this series about 14 isolated I.V.C. invasion, only 4 patients are surviving with no evidence of disease but 2 have more than six years and fourteen years follow-up.
This paper reports the methods and results of an innovative program of prenatal care, designed to prevent preterm delivery in Clamart, France, during 1976-1981. Rates of preterm delivery among women who entered the prenatal care program early in pregnancy (and, thus, could derive full benefit of the program) were compared with those among two comparison groups: (1) women who entered the same program later in pregnancy; and (2) women who delivered in Paris. In the first comparison, women who entered the program early had significantly lower rates of preterm delivery than did women who entered later, even when stratified by various risk factors, and when high-risk women were eliminated altogether. In the second comparison, the early care group in Clamart did not exhibit the usual inverse relationship between socioeconomic status and rates of preterm delivery. The usual significant inverse relationship was found in the Clamart population as a whole, and in the Paris series.
A specific prenatal care program is proposed to prevent preterm deliveries in twin pregnancies, with ultrasound scanning, early work leave and home visits by midwives. A four-year study has been conducted on 197 pregnancies (160 early followed and booked and 37 late referred). It can be shown that early preterm births and very low birth weights are less frequent in the followed group than in published data, as well as in the late referred pregnancies. The total perinatal mortality rate is 25.6 per 1000 total births.
The authors report the case of a 72 year old patient with ECG changes of anterior myocardial infarction complicated by left ventricular failure and shock which has a favourable outcome with regression of the pathological Q waves on the 7th day. Radionuclide investigation and coronary angiography showed no myocardial sequellae and the coronary arteries appeared normal. This case, an example of stunned myocardium, confirms that severe myocardial ischaemia, even of short duration, may induce reversible but prolonged metabolic disturbances. The practical implications of this concept are discussed.
This study analyses selected characteristics of 13,676 women, all single pregnancies, attending the Department of Obstetrics and Gynaeocology of the A. Béclère Maternity Hospital in Clamart (in the greater Paris region of France). The characteristics analysed for each women were: sociocultural level, occupation, place of residence (i.e. distance from the hospital), frequency and time of prenatal visits, and selected aspects of obstetric history (treatment for infertility, previous preterm birth, perinatal mortality). A comparison was made between the patients living near the institution and the patients living far from the hospital; this revealed four separate groups: Women living near by the hospital followed-up early by the department, and whose social class distribution is similar to that of the parisian population as a whole. Women living near the hospital, followed-up late or not at all by the department, most have a low sociocultural level, and are referred to the hospital by local homes for single mothers. Women living far from the hospital, followed-up early and often having a high socio-cultural level. In this group, a higher level of information about pregnancy, obstetrics, and medical services are observed. However this group also shows a high frequency of previous obstetric incidents: previous contact with a medical institution appears to be an important factor in the choice made by these women, independent of their sociocultural level. Women living far from the hospital and followed-up late by the department. A method is developed for the simultaneous determination of neopynamine and piperonyl butoxide by chromatography on a quartz capillary column using flame ionization detection. Benzyl mandelate is used as an internal standard. Calibration graphs are linear down to at least 3.75 mg% and 3 mg% for neopynamine and piperonyl butoxide, respectively.
