[Needle biopsy of pancreatic masses].

We report the results of 66 ultrasound or CT guided needle aspirations in 59 patients with pancreatic masses, from 1983 to 1987. Cytologic and histologic samples were obtained in 45 cases. Histologic samples, formed by minute tissue fragments, were paraffin embedded for standard sections (minibiopsy). Sensitivity, specificity and overall accuracy of the series as a whole were 88, 100 and 89.8%, respectively. We discuss the clinical usefulness of the technique; isolated cytologic and histologic results are compared and the minibiopsy contribution to the cytologic diagnosis is discussed.

Histopathological classification of childhood rhabdomyosarcoma: a report from the International Society of Pediatric Oncology pathology panel.

Five hundred thirteen soft tissue tumours of childhood referred to the International Society of Pediatric Oncology (SIOP) Rhabdomyosarcoma study have been reviewed. The period covered was from January 1975 to December 1983. Three hundred thirty-nine neoplasms were regarded as embryonal rhabdomyosarcoma. The histological diagnosis of all referred tumours is given, and a classification of rhabdomyosarcomas developed during the course of the study is described. The relationship between histological subtypes and aspects of clinical behaviour is presented. The classification is thought to be helpful in diagnosis and also in assessment of the likelihood of recurrence and/or metastasis of childhood rhabdomyosarcoma.

Rhabdomyosarcoma in infants under one year of age: experience of the Institut Gustave-Roussy.

Forty-three infants under 1 year of age (median 4 months) with rhabdomyosarcoma (RMS), representing 11% of all childhood RMS seen over the same period, were treated and followed between 1955 and 1984 at our institute. Compared to the 340 older children (1-16 years of age), there were no significant differences in male/female ratio, primary site, or clinical stage, but there was a higher frequency of alveolar and poorly differentiated histologic subtypes in infants aged under 1 year (P = 0.05 and P less than 0.001, respectively) and increased frequency of botryoid RMS in older children (P less than 0.02). Although children older than 1 year of age had a significantly higher complete remission rate (83% vs. 51%, P less than 0.001), their 5 year overall survival (43%) was no different from that of infants under 1 year of age (35%). In the older group there was no survival difference between alveolar (38%) and nonalveolar (44%) histologic subtypes, but in infants younger than 1 year, alveolar subtype had a significantly poorer prognosis (13% vs. 47%, P = 0.01). Management of patients at this age group is difficult and major advances are needed to increase survival rates with a minimum of morbidity.

Spinal dural arteriovenous malformations with perimedullary drainage. Indications and results of surgery in 30 cases.

70 patients were treated for spinal dural arteriovenous fistula in the same centre, during a period of 10 years. Conus medullaris and cauda equina syndromes were observed in all patients as the clinical stereotyped presentation. Diagnosis was based on myelography in the first instance and then on angiography. 40 patients were treated by intravascular neuroradiological embolization, whereas the other 30 were operated on. Surgery was proposed because embolization was contraindicated (7 cases of hazardous catheterisation) of inefficient (23 cases = 38%). The results of the operative series are presented, and compared with those of embolization. Improvement was observed in 50% of the 20 patients with follow up, but a complete recovery to an asymptomatic state was never obtained. For the other patients (47%) complete stabilization of the disease could be obtained, whereas in one of the patients (3%), who was operated upon because of failure of embolization, surgery was also completely ineffectual. The long-term results of patients treated surgically are comparable with those patients efficiently embolized. 5 patients of the operative series were submitted to MRI before and after surgery: the results and the place of MRI are discussed.

[Clinical tolerance of CY 216 (Fraxiparin) in the prevention of thromboembolic accidents after neurosurgery].

Clinical tolerance of CY 216 was assessed on 97 patients in two neurosurgical departments (Hopital Lariboisière and Hopital Beaujon, Paris). All the 97 patients were operated on by different cranial approaches, but always including bone flap. No thrombo-embolic event could be detected in this series. Post-operative intracranial hemorrhage was observed in 19 cases on the basis of CT scan performed on the first and seventh post-operative day. Among these 19 patients, 8 presented an increase of the hemorrhagic aspect between the 2 CT scan. However, clinical state of these patients improved and no reoperation was necessary to treat these hemorrhages. From this study, we consider that CY 216 is an effective mean of thrombo-embolism prevention with little hemorrhagic side-effect. Therefore, it can be recommended in the aftermath of intracranial surgical procedures.

Prognostic factors in advanced nonseminomatous testicular cancer. A multivariate logistic regression analysis.

In order to define prognostic factors for advanced stage of nonseminomatous germ cell tumors (NSGCT) of the testis, the authors reviewed 84 patients treated from 1978 through 1985. The survival rate was 51% at 3 years. Patients with elevated seric levels of human chorionic gonadotropin (HCG) and/or alpha-fetoprotein (AFP), or the presence of an abdominal mass had significantly worse survival. Only HCG and AFP levels retained their significance when multivariate Cox analysis was performed. The probability that a patient achieves a complete remission (CR) was assessed by a function of certain patient characteristics using a multivariate logistic regression analysis. The significant variables were a function of HCG and AFP values. Since both variables are related to the CR rate and survival the authors define the obtention of a CR as a unique outcome of interest. The probability of a CR greater than 70% adequately separates the patients into two prognostic subgroups. This model currently is being used to enrole NSGCT patients in a prospective modulated clinical trial according to these prognostic factors.

Epidemiological features of Wilms' tumor: results of studies by the International Society of Paediatric Oncology (SIOP).

This descriptive epidemiology study of 1,040 children with Wilms' tumor (WT) registered in the International Society of Paediatric Oncology (SIOP) clinical trials confirms the findings reported by the National Wilms' Tumor Study. The male:female rate was 0.89:1. The mean age at diagnosis of the 43 bilateral cases was significantly younger than children with unilateral renal involvement (32.4 vs 45 months). However, the mean ages of diagnosis for unilateral multicentric and for unicentric WT were very similar. On the other hand, the mean age at diagnosis of children with sporadic aniridia and hypospadias was younger than the mean age of patients with or without other congenital malformations. Thus aniridia as well as hypospadias could be indices of the first mutation, according to the Knudson and Stron hypothesis. WT was reported in two members of each of five families. However, these familial cases were comparable in terms of demographic and clinical features to the nonfamilial ones. These data suggest that the heritable fraction of WT is relatively small and that genetic and environmental factors interact in the development of WT.

Prognostic factors in 281 children with nonmetastatic rhabdomyosarcoma (RMS) at diagnosis.

Pretreatment characteristics of 281 children with nonmetastatic rhabdomyosarcoma, included in the registry of the International Society of Pediatric Oncology (SIOP) between January 1975 and December 1983, were examined to study the children's prognosis. The multivariate statistical method (Cox regression model) was used for each of two endpoints: survival time and disease-free time. The three most important predictors for survival time were primary site (p less than .001), clinical stage (p = .009), and sex (p = .020). The best results involved paratesticular and orbital primary sites, regardless of the clinical stage; males fared slightly better than females. These same three factors were also significant predictors for disease-free time.

Prognostic factors of adult metastatic renal carcinoma: a multivariate analysis.

In order to define the prognostic factors for metastatic renal carcinoma, we reviewed 134 patients who were treated from 1971 through 1986. Survival rates were 72, 45, and 25% at 6, 12, and 18 months, respectively. Seventeen variables were tested using the logrank test. Improved survival was correlated with normal performance status, and an absence of fever, weight loss, hepatic metastasis, and lung metastasis (or, if lung metastasis was present, less than 2 cm in diameter and limited to one site), a disease-free interval, sedimentation rate less than 100, and renal surgery. Four variables retained significant value in the multivariate analysis: hepatic metastasis, lung metastasis, disease-free interval, and a variable combining the sedimentation rate and the weight loss (SWRL). Predictive survival rates based on these variables were calculated from the Cox model. Six subgroups of patients were identified. The estimation of survival is clinically of value for future phase II trials of chemotherapy in patients with adult metastatic renal carcinoma.

Accuracy of rotavirus diagnosis: modified genome electrophoresis versus electron microscopy.

530 human faecal samples were examined for the presence of rotavirus by electron microscopy (EM) and by a modified electrophoretic analysis of viral genome (PAGE). 516 stools gave identical results by both methods (97.4% agreement). The proportion of EM+, PAGE- samples (1/530) was significantly lower than the proportion of EM-, PAGE + samples (13/530) (P less than 0.01 with McNemar test). Relative sensitivity was 99.6% for PAGE and 95% for EM. False positive results could be excluded for each method because of the characteristic morphology of rotavirus particles (EM) and specificity of rotavirus electrophoretypes (PAGE). Discrepant cases were reexamined whenever possible and the causes of misdiagnosis are discussed.

[Prognostic factors in rhabdomyosarcomas in childhood. Study carried out with 253 children registered by the International Society of Pediatric Oncology].

Two hundred and fifty three children with newly diagnosed nonmetastatic rhabdomyosarcoma, from 30 European centers, were registered by the International Society of Pediatric Oncology (SIOP) between 1975 and 1982, and received a VAC-VAD chemotherapy. Multivariate statistical methods were used to study prognosis. The most important predictors for survival time were as follows: primary site (P less than 0.001), clinical stage (P = 0.02), and sex (P = 0.03). Three primary sites had a favourable prognosis (vagina, paratestis, orbit) as opposed to the most unfavorable ones (parameningeal head and neck sites). Clinical state I had better survival curves than stages II and III. Males had a better prognosis than females, regardless of stage and primary site. The analysis of the evolutive modes of the tumour showed that the lack of local control (no complete remission, local recurrence) was the first reason of failure.

Vascular malformations of the spinal cord: intrathecal perimedullary arteriovenous fistulas fed by medullary arteries.

We studied eleven patients with intrathecal extramedullary arteriovenous fistulas identified by selective angiography and myelography. The fistulas were located on the surface of the cord, fed by medullary arteries. Angiography distinguished three separate types that gave identical clinical disorders. Therapeutic results of surgery and embolization allowed specific indications to be defined for treatment of each type of fistula.

[Malignant melanoma of the penis associated with other types of melanosis. Apropos of an isolated case].

A patient with malignant melanoma of penis also presented a blue nevus, urethral melanosis and zones of malignant lentigo. The patient had a history of melanic lesions since infancy and initial symptoms had developed two months before attending for consultation. This isolated case involved fairly atypical melanic lesions in the urogenital tract. Strict surveillance of patients with mucosal melanic lesions is essential when confronted with any suspicious variation noted clinically.
