New aspects in the treatment of childhood rhabdomyosarcoma: results of the German Cooperative Soft-Tissue Sarcoma Study (CWS-81).

The results of the German Co-operative Soft-Tissue Sarcoma Study (CWS-81) of the treatment of rhabdomyosarcoma are presented. Prior to the introduction of chemotherapy only 10%-20% of the children were successfully treated. Combined multi-agent cytostatic treatment improved the results dramatically. In patients with primary stage III rhabdomyosarcoma, local tumour control by surgery or radiotherapy should be undertaken earlier than week 16, if complete remission has not been achieved by 7-9 weeks. Patients with complete remission or partial tumour regression should be treated with the same combination of chemotherapy, while only partial responders need radiotherapy in addition. Patients with tumours which are primarily resectable without mutilation have a 90% chance of cure; this also applies to patients with primarily unresectable tumours who achieve complete remission after 7-9 weeks of chemotherapy. Total disease-free survival rate for stage III rhabdomyosarcoma patients was 53%. The role of surgery includes primary removal of the tumour or assessment of remission by means of histological spot checks.

Leiomyoma of the fetal membranes: report of a case.

A leiomyoma of the fetal membranes was incidentally discovered on examination of a spontaneously expulsed placenta following an uneventful pregnancy and delivery of a healthy neonate. Perusal of the literature uncovered only a single report of a placental leiomyoma.

Antimyosin imaging in acute transmural myocardial infarctions: results of a multicenter clinical trial.

Murine monoclonal antimyosin antibody has been shown experimentally to bind selectively to irreversibly damaged myocytes. To evaluate the safety and efficacy of monoclonal antimyosin for identifying acute transmural infarction, 50 patients with acute Q wave myocardial infarction were entered into a phase I/II multicenter trial involving three clinical sites. Indium-111 antimyosin was prepared from an instant kit formulation containing 0.5 mg of diethylene triamine pentaacetic acid (DTPA)-coupled Fab fragment (R11D10) and 1.2 to 2.4 mCi of indium-111. Average labeling efficiency was 92%. Antimyosin was injected 27 +/- 16 h after the onset of chest pain. Planar or tomographic imaging was performed 27 +/- 9 h after injection in all patients, and repeat imaging was done 24 h later in 39 patients. Of the 50 patients entered, 46 showed myocardial uptake of antimyosin (sensitivity 92%). Thirty-one of 39 planar scans performed at 24 h were diagnostic; 8 showed persistent blood pool activity that cleared by 48 h. Focal myocardial uptake of antimyosin corresponded to electrocardiographic infarct localization. No patient had an adverse reaction to antimyosin. In addition, 125 serum samples, including 21 collected greater than 42 days after injection, were tested for human antimouse antibodies, and all samples were assessed as having undetectable titers. Intensity of antimyosin uptake was correlated with infarct location and the presence or absence of collateral vessels. There was a significant correlation between faint uptake and inferoposterior infarct location. In 21 patients who had coronary angiography close to the time of antimyosin injection, there was a significant correlation between faint tracer uptake and closed infarct-related vessel with absent collateral flow.(ABSTRACT TRUNCATED AT 250 WORDS)

Ultrasound assessment of the bladder volume after anterior colporrhaphy.

In 40 gynecological patients 44 different determinations of the bladder volume were made using ultrasonic methods. The product of bladder depth, height, and width, as determined from transverse and sagittal scans, showed the best correlation with the bladder volume measured by urethral catheter (r = 0.981). For 73% of the measurements the error was under 20% when the true bladder volume was greater than 100 cm3. This method gives a reasonable assessment of the bladder residual volume. It is quick, safe, and repeatable and, therefore, useful in postoperative clinical practice.

Bacterial vaginosis: a double-blind randomized trial of the effect of treatment of the sexual partner.

In a double-blind randomized controlled trial we assessed the effect of metronidazole treatment of the male partner on the recurrence rate of bacterial vaginosis. Women who fulfilled the diagnostic criteria for bacterial vaginosis were treated with metronidazole given in single doses of 2 g on days 1 and 3. The sexual partners were randomized to receive either the same dosage of metronidazole or a placebo. A total of 107 pairs completed the study. One week after the start of treatment 89% of the women considered themselves improved or cured and 93% no longer had the diagnostic criteria for bacterial vaginosis. At assessment 5 weeks after the treatment, 75% reported that they were cured or improved and the diagnostic criteria were not present in 73%. Treatment of the male partner did not affect subjective symptoms, clinical signs and isolation rates of Gardnerella vaginalis at 1 and 5 weeks after treatment.

Patterns of growth of uterine leiomyomas during pregnancy. A prospective longitudinal study.

In a prospective study 32 leiomyomas (fibroids) in 29 pregnant women were examined with ultrasound every 3-8 weeks. Each patient had between 3 and 6 scans (mean 4.4) during the course of pregnancy, and 13 patients had a final scan at 6 weeks postpartum. An individual growth curve was established for each tumour and the patterns of growth were analysed. No increase in size during the pregnancy was observed in 25 fibroids (78%). Only 7 (22%) increased in size but by no more than 25% of the initial volume. At 6 weeks postpartum the size of the fibroids did not differ significantly from the size during pregnancy.

Evolution of tumor cytogenetic aberrations and N-myc oncogene amplification in a case of disseminated neuroblastoma.

Chromosome analyses including in situ hybridization with the protooncogene N-myc were carried out at diagnosis and several times at relapse in the bone marrow cells of a girl with disseminated neuroblastoma (stage IV) at ages 9, 23, 24, and 26 months, respectively. The tumor karyotype was pseudodiploid exhibiting partial monosomy for the short arm of chromosome #1, an aberration of the short arm of chromosome #2, and double minutes at diagnosis. Further structural aberrations of chromosomes #4, #6, and #17, and homogeneously staining regions could be demonstrated at relapse. In particular, the following structural aberrations were encountered: t(1;?)(p22;?),t(2;?)(p24;?), t(4;6)(q31;q25), and a der(17q). All tumor metaphases without homogeneously staining regions contained double minutes. N-myc, normally positioned within bands 2p23-24, was found to be amplified in a homogeneously staining region on the short arm of chromosome #15 by in situ hybridization. It is speculated that the translocation t(2;?)(p24;?) might be related to N-myc oncogene activation and subsequent amplification.

The usefulness of Doppler echocardiography in the management of rhabdomyoma diagnosed prenatally.

Cardiac rhabdomyoma diagnosed prenatally by cross-sectional echocardiography was confirmed after birth by Doppler echocardiography and cardiac catheterization in a baby with tuberous sclerosis. Doppler echocardiographic follow-up after 2 years revealed regression of tumour masses and concomitant disappearance of pulmonary regurgitation which was caused by the large tumour mass in the right ventricular outflow tract. In spite of a sizeable mass protruding into the left ventricular outflow tract, repeated Doppler examination failed to demonstrate any significant obstruction, thus avoiding repeated catheterizations and unnecessary surgery. Doppler examination of ventricular inflow revealed normal filling characteristics. Doppler and cross-sectional echocardiography was of great value in the management of this patient.

[Induction of sarcomas by the tissue-binding substance Histoacryl-blau in the rat].

The substance Butyl-2-Cyanoacrylate (Firm Braun-Dexon) which is known by the trade name Histoacryl-blau was implanted in rats for the purpose of testing its cancer-producing properties. Of 44 animals evaluated eleven cases of sarcomas developed at the implantation site, that were apparently directly caused by the tissue binding substance. Therefore the use of this preparation in humans should be strongly restrained and employed under strict regulation especially in patients under the age of thirty.

Pulsed Doppler fetal echocardiography.

Pulsed Doppler echocardiography (PDE) has proven value in the examination of the infant or child with known or suspected congenital heart disease. In order to assess how PDE might be useful in the evaluation of the infant in utero, we reviewed our experience with this technique used in combination with both cross-sectional and M-mode echocardiography. Where previous examinations had suggested some abnormality or where there was a risk factor for congenital heart disease, we performed PDE examinations on the cardiovascular system of 36 infants in utero ranging in gestational age from 20-39 weeks (mean 32 weeks). PDE of the fetal heart was found to be valuable in three areas: (1) quantitation of hemodynamics, (2) characterization of cardiac rhythm, and (3) identification of certain congenital cardiovascular structural anomalies.

[Ernst L. Kirchner - psychoanalytic perspectives of the psychoeconomy of his creative achievements].

Looking back on Ernst L., Kirchner's life and work, the close interdependence of both becomes apparent. The artist and his art form a community of fate. In the light of psychoanalytical narcissistic research this interdependence presents itself as a symbiotic relationship of 'real' and 'ideal ego' (= art), the purpose of which it is to regulate the self-esteem of the narcissistically disturbed. Kirchner's example demonstrates well the creative and therapeutic forces set freeby this kind of regulation. It also shows, however, the instability of this state and the risk of the 'narcissistic catastrophe' to which an unfavourable course may lead.

Oral contraceptive pills and clinical otosclerosis.

Clinical otosclerosis is a familial disease which is more frequent among women in their reproductive years. The condition usuallly is aggravated by pregnancy. Endocrinologic variables may influence the time of onset and the course of the disease. It is suspected that oral contraceptives (OCs) might stimulate the onset of the disease. Six hundred nulliparous women between the ages of 16 and 30, who used a variety of OCs for 12-36 months, were examined. The hearing of these women was thoroughly investigated. The first audiometric examination of the 600 women revealed three cases (0.5%) of clinical otosclerosis. This incidence is equal to that of the population as a whole, but lower than the incidence found in previously parous women. Audiometric examinations were normal in the remaining 597 women, and repeated examinations revealed no new cases of clinical otosclerosis, despite continuous OC use.

Antibody-dependent cell-mediated cytotoxicity in treated and nontreated cancer patients.

Antibody-dependent cell-mediated cytotoxicity (ADCC) exhibited by peripheral blood leukocytes was used as a criterion for assessing immune competence of groups of noncancerous individuals and of treated and untreated cancer patients. The results show that, as a group, leukocytes of nontreated cancer patients exhibited significantly lower ADCC than that exhibited by leukocytes of noncancerous individuals. However, the ADCC of leukocytes from cancer patients under treatment approximated that of normals. Thus, using ADCC as criterion, the results indicate that as a group, cancer patients under treatment tend to exhibit restored immune competence.

[Heidegger and modern research].

The unity of thought content and its corresponding object in reality - an unsolved problem till now - is explicated in a gestaltqualitative-neurobiological way. This gives the opportunity of analyzing Heidegger's concepts of truth as "lichtendes Bergen", of "Gestimmtheit" and "Angst", of life and death and "Dasein" in an actualgenetic framework. In opposition to Heidegger's postulation of the non-thinking modern science it results that Heidegger's conceptualization of thought contents is emphasizing primarily the aspects of unity and constancy versus division of unity and complexity by the modern science.
