[The PM-prosthesis].

The outcome of 114 PM-hip prostheses is reported, the mean time since operation of which is between 5 and 10.1 years (7.6 years mean). The fate of 23 hip replacements (10.1%) could not be evaluated. The scores of Merle d'Aubigné und Harris were used for evaluation. The mean score according to Merle d'Aubigné rose from 9.15 to 13.95 and that of Harris from 44.13 to 75.60. Subjectively 84 patients (73.7%) were fully satisfied with the outcome and 27 (23.6%) with limitations. In the patients opinion the state of the hip had improved 107 times (93.9%), twice (1.7%) it had deteriorated. 95 times (83.4%) the range of motion of the joint was found improved, 6 times (5.2%) the were regarded as deteriorated. The outcome is analysed in the line with the numerous criteria of the employed scores. Survival analysis suggests a 90.5% (confidence interval 86.3%-94.6%) probability of success of primarily implanted PM-Prostheses after 5 years and of 73.3% (confidence interval 60.3%-86.4%) after 10 years.

[Bone and boundary zone changes following the implantation of double-cup arthroplasties].

21 femoral heads of 20 patients with double cup arthroplasty were examined after an implantation period of 1 to 5 years, in one case after 18 years. In 8 cases the hipe socket and in 10 cases the femoral cup became loose. Therefore an operative removal was necessary. 2 patients showed extensive periarticular ossifications. In connection with the metal cup the resected femoral heads were cut in slices of 1 cm. thickness. After taking an X-ray they were embedded without decalcification in methylmetacrylate. The sections were stained with different methods (Goldner, v. Kossa modification). In the group with fixed cup a local osteoporosis is dominating. The fibrous tissue membrane between cement and bone tissue is varying in thickness. It is thicker at the periphery of the cup. In the group with loosened cups large irreparable bone necrosis with included histiocytic granulomas, could be observed. The clinical problems are discussed in accordance to the long term prognosis on the basis of the morphological results.

[Rotator cuff rupture. Surgical results in relation to the approach].

This paper presents the results of surgery in a total of 43 cases of rupture of rotator muscle sheaths which were followed up. Surgery was carried out from a saber-cut incision (n = 24) and from a transacromial approach (n = 19). The follow-up period for the first group was 42.2 months, for the second 14.3 months. The procedures employed were, almost without exception, the Schuhnestel or continuous suture and transosseous fixation after McLaughlin. In Group I the results were very good and good in 22 patients (91.6%); results in two shoulders were poor. In Group II the results were somewhat poorer, with 76.5% very good and good results; this is attributable principally to the much shorter follow-up period. The poor results are analyzed. The coraco-acromial ligament was severed or resected in 22 cases. Neer acromioplasty is now performed routinely. The surgical approach appears to have no decisive influence on the result. Advantages and disadvantages of the various approaches are described. A particularly important factor is the small therapeutic team (surgeon and physiotherapist).
