A new regimen of cisplatin, epirubicin and methotrexate (PEM-3) as primary chemotherapy for locally advanced bladder cancer.
In this phase II study, 41 patients with locally advanced urothelial bladder cancer (T2-4, N0, M0) were treated with primary chemotherapy (cisplatin, epirubicin, methotrexate: PEM-3). All the patients were assessable for response and toxicity. Clinical monitoring was performed with computerized tomography and cystoscopy. Nineteen clinical complete remissions (46%) and 10 partial remissions (24.5%) were obtained (CR + PR, 70.5%; 95% confidence interval, 57%-85%). Ten patients were considered to have clinically stable disease (24.5%), and 2 patients progressed (5%). Surgery after chemotherapy was performed in 22 cases: in 6 patients (27%) a pathologic complete response was achieved. The pathologic stage was lower than the initial clinical stage in 13 patients (59%). After a median follow-up of 3 years (range, 1-4), the median time to progression was 104 weeks. At this writing, 20 patients, 12 of which were submitted to surgery and 8 were not operated, are disease-free. The 3-year survival rate is 52%. No one had to interrupt the treatment because of toxicity. In conclusion, the PEM-3 regimen is a very active and well-tolerated regimen in locally advanced bladder cancer.
In vitro MRS and HPLC studies on human renal cell carcinomas.
Samples from ten patients with renal cell carcinomas (RCC) were examined by 1H MRS and high performance liquid chromatography (HPLC) measurements. Twenty samples, ten from the tumor and ten from the surrounding healthy renal region were obtained. MRS yielded information on renal osmolytes which can be considered markers of physiological kidney function. The marked decrease of these osmolytes is a hallmark of cancer. Moreover, HPLC measurements disclosed the amino acid pattern of both healthy and neoplastic tissues. The Glu and Ea content had statistical significance. GSH was present in tumor tissues only.
[Therapy of chronic hepatitis C with alpha-interferon in 182 patients. Evaluation of results, response predictive factors and side effects].
BACKGROUND: The aim of this study was to assess the results, predictors of response and side effects of therapy with alpha-interferon (IFN) in chronic C virus (HCV) infection. METHODS: A group of 182 patients (150 chronic hepatitis and 32 cirrhosis) was treated with alpha-interferon--recombinant IFN in 120 cases (66%), and lymphoblastoid IFN in 62 cases (34%)--at a dosage of 3 MU three times weekly for 12 months. All our patients were prospectively followed for at least 6 months, and 133 patients for more than 2 years. RESULTS: A short-term positive response was achieved in 65% of our patients, a sustained response in 34%, and a long-term response in 35%, the responses being similar both for hepatitis and cirrhosis. Liver histology, after 2 years, improved in 73% of long-term responders, whereas it improved only in 34% of relapsers. The predictors of response were: age, duration of disease, baseline levels of gamma-glutamyltranspeptidase (GGT) and serum ferritin. Both types of IFN proved to have the same efficacy. Side effects were observed in 52% of our patients, which were correlated with age and female sex. CONCLUSIONS: Interferon therapy yields good results, if administered for 12 months, in young patients with disease of short duration and low baseline levels of GGT and serum ferritin, even in the presence of cirrhosis, if at early-stage. Patients with normal alanine aminotransferase (ALT) levels after 3 years may be considered fully recovered. The dosage employed was well tolerated.
[The mortality among the makers of vinyl chloride in Italy].
A mortality study was performed on 5,946 vinyl chloride (VC)-polyvinyl chloride (PVC) workers of the nine Italian plants. The aims of the study were to investigate the cause-specific mortality of workers exposed to VC in production and polymerization stages and to ascertain whether exposure is associated with a higher risk of cancers other than liver angiosarcoma. An incident cohort was enumerated in five plants, a mixed cohort in two and a cross sectional one in the remaining two facilities. Cause-specific Standardized Mortality Ratios (SMR) were calculated for the six incidence cohorts with a sufficiently long follow-up period. Both Italian and regional rates were used for the comparison. A pooled analysis of six cohorts was performed. For deceased persons, information from death certificates was used in the mortality analysis. Additional clinical and pathological data were sought for all decreased individuals (Best Evidence = BE). The results of the study confirm the carcinogenic action of vinyl chloride on the liver but not on the other suggested target organs (i.e., lung, lymphopoietic tissues, brain). A deficit for all causes of death and cardiovascular disease was evident in most cohorts and in the pooled cohort. Analysis of BE data showed a broad spectrum of carcinogenic action of VCM on the liver as demonstrated by the occurrence of both angiosarcomas and other histotypes of tumours.
[Safety forms for industrial lubricating oils: a working proposal at the national level].
The health and safety data sheets for lubricating oils currently in use present considerable differences regarding the information they provide. Often, the information is either too generalized or incomplete--particularly on the chemical aspects of oils--and therefore the sheets are inadequate for the prevention and control of the occupational risks. For the purposes of harmonization, a data sheet has been prepared which takes into account the specific features of the products, and particularly the handling of confidential data. The most important oil manufacturers have been consulted, and they have agreed to use the data sheet in order to provide information on their products to the local health authorities. The widespread use of this data sheet should enable an easier comparison of the health and safety data of oils to be made, provide a better understanding of the information received and, consequently, evaluate the risks involved.
[Echographic diagnosis of Peyronie's disease].
Thirty-six patients with Peyronie's disease were evaluated with diagnostic sonography. Radiographic and/or xeroradiographic examinations were also performed on 23 patients. Sonography could demonstrate the presence of the plaques characteristic of the disease in the majority of cases (94.5%), whereas only calcified lesions were depicted by conventional X-rays. Sonography is therefore recommended for its well-known advantages over other techniques, not only as the diagnostic methodology of choice, but also in the follow-up of the disease.
